On the Management of Drug Interactions in the Course of Concomitant Treatments for COVID-19 and Antineoplastic Agents
The unprecedented global pandemic caused by coronavirus 2 (SARS-Cov2) drewmedical attention
toward patients with co-morbid disorders, who are more exposed to prognostically unfavorable
outcomes (1). A peculiar clinical scenario is represented by oncologic patients, who appear more
susceptible to COVID-19 infection, as they may develop more severe symptoms than those
observed in individuals with different comorbidity profiles (2)
Eradication of isolated para-aortic nodal recurrence in a patient with an advanced high grade sorous ovarian carcinoma: our experience and review of literature
Abstract: We report a case report regarding the eradication of isolated lymph-nodal para-aortic recurrence
in the aortic region down the left renal vein (LRV) in a patient treated two years earlier in
another hospital for a FIGO stage IC2 high-grade serous ovarian carcinoma with a video showing
the para-aortic space after eradication of the metastatic tissue. A 66 year-old woman was admitted
24 months after the initial surgical procedure for an increased Ca 125 level and CT scan that revealed
a 3 cm para-aortic infrarenal lymph-nodal recurrence that was confirmed by PET/CT scan. A secondary
cytoreductive surgery (SCS) with a para-aortic lymph-nodal dissection of the tissue down
the LRV and radical omentectomy were performed: during the cytoreduction, the right hemicolon
was mobilized. The anterior surface of the inferior vena cava (IVC), aorta and LRV were exposed.
The metastatic lymph nodes were detected in the para-ortic space down the proximal part of the
LRV and eradicated; an en bloc infrarenal lymph-node dissection from the aortocaval region was
performed. The operative time during the surgical procedure was 212 min with a blood loss of 120
mL. No intra- and postoperative complications, including ureteral or vascular injury or renal dysfunction,
occurred. At histological examination, three dissected lymph nodes were positive for metastasis,
and the patient was discharged five days after laparotomy without side effects and underwent
chemotherapy 3 weeks later; after a follow-up of 42 months, no recurrence was detected. In
conclusion, secondary debulking surgery can be considered a safe and effective therapeutic option
for the management of recurrences, although long-term follow-ups are necessary to evaluate the
overall oncologic outcomes of this procedure
Distributed Computing Grid Experiences in CMS
The CMS experiment is currently developing a computing system capable of serving, processing and archiving the large number of events that will be generated when the CMS detector starts taking data. During 2004 CMS undertook a large scale data challenge to demonstrate the ability of the CMS computing system to cope with a sustained data-taking rate equivalent to 25% of startup rate. Its goals were: to run CMS event reconstruction at CERN for a sustained period at 25 Hz input rate; to distribute the data to several regional centers; and enable data access at those centers for analysis. Grid middleware was utilized to help complete all aspects of the challenge. To continue to provide scalable access from anywhere in the world to the data, CMS is developing a layer of software that uses Grid tools to gain access to data and resources, and that aims to provide physicists with a user friendly interface for submitting their analysis jobs. This paper describes the data challenge experience with Grid infrastructure and the current development of the CMS analysis system
The role of ultrasound guided sampling procedures in the diagnosis of pelvic masses: A narrative review of the literature
Ultrasound-guided sampling methods are usually minimally invasive techniques applied to obtain cytological specimens or tissue samples, mainly used for the diagnosis of different types of tumors. The main benefits of ultrasound guidance is its availability. It offers high flexibility in the choice of sampling approach (transabdominal, transvaginal, and transrectal) and short duration of procedure. Ultrasound guided sampling of pelvic masses represents the diagnostic method of choice in selected patients. Reproductive outcomes and fertility preservation strategies in women with malignant ovarian germ cell tumors after fertility sparing surgery
Malignant ovarian germ cell tumors are rare tumors that mainly affect patients of reproductive age. The aim of this study was to investigate the reproductive outcomes and fertility preservation strategies in malignant ovarian germ cell tumors after fertility-sparing surgery. Data in literature support that fertility-sparing surgery is associated with an excellent oncological outcome not only in early stages malignant ovarian germ cell tumors but also in advanced stages. Moreover, the possibility of performing conservative treatment should be considered even in case of relapse or advanced disease, given the high chemosensitivity. Indeed, available data have shown that menstrual function is maintained after platinum-based regimens in over 85–95% of patients with malignant ovarian germ cell tumors and rate of premature menopause reported in literature ranges between 3% and 7.4%, while premature ovarian failure rates are between 3.4% and 5%. Moreover, reproductive outcomes are about 80% with no increase in the risk of teratogenicity compared to general population. Therefore, conservative surgery for malignant ovarian germ cell tumors currently may represent a therapeutic option in patients who wish to preserve fertility but must be available for extended follow-up and after subscribing to informed consent
Grape seed extracts modify the outcome of oxaliplatin in colon cancer cells by interfering with cellular mechanisms of drug cytotoxicity
Grape seed extracts are commonly utilized as dietary supplements for their antioxidant properties, even from cancer patients. However, whether these natural extracts interfere with chemotherapeutics utilized in colon cancer treatment is still poorly investigated. The cytotoxicity of extracts from Italia and Palieri cultivars either alone or in combination with oxaliplatin was evaluated in colon cancer cells. Grape seed extracts displayed anti-proliferative activity depending on the concentration utilized through apoptosis induction. In combination, they affected the activation of Erk1/2 and counteracted the intrinsic and the extrinsic pathway of apoptosis, the DNA damage and the generation of ROS induced by oxaliplatin. Noteworthy grape seed extracts strongly enhanced the uptake of oxaliplatin into all cells, by affecting the cell transport system of platinum. The addition of these natural extracts to oxaliplatin strongly reduced the cellular response to oxaliplatin and allowed a huge accumulation of platinum into cells. Here, we shed light on the chemical biology underlying the combination of grape seed extracts and oxaliplatin, demonstrating that they might be detrimental to oxaliplatin effectiveness in colon cancer therap
Basics and frontiers on pancreatic cancer for radiation oncology: Target delineation, SBRT, SIB technique, MRgRT, particle therapy, immunotherapy and clinical guidelines
Pancreatic cancer represents a modern oncological urgency. Its management is aimed to both distal and local disease control. Resectability is the cornerstone of treatment aim. It influences the clinical presentation\u2019s definitions as up-front resectable, borderline resectable and locally advanced (unresectable). The main treatment categories are neoadjuvant (preoperative), definitive and adjuvant (postoperative). This review will focus on i) the current indications by the available national and international guidelines; ii) the current standard indications for target volume delineation in radiotherapy (RT); iii) the emerging modern technologies (including particle therapy and Magnetic Resonance [MR]-guided-RT); iv) stereotactic body radiotherapy (SBRT), as the most promising technical delivery application of RT in this framework; v) a particularly promising dose delivery technique called simultaneous integrated boost (SIB); and vi) a multimodal integration opportunity: the combination of RT with immunotherapy
