    Skyrmions, Skyrme stars and black holes with Skyrme hair in five spacetime dimension

    We consider a class of generalizations of the Skyrme model to five spacetime dimensions (d = 5), which is de fined in terms of an O (5) sigma model. A special ansatz for the Skyrme field allows angular momentum to be present and equations of motion with a radial dependence only. Using it, we obtain: 1) everywhere regular solutions describing localised energy lumps (Skyrmions); 2) Self-gravitating, asymptotically flat, everywhere non-singular solitonic solutions (Skyrme stars), upon minimally coupling the model to Einstein's gravity; 3) both static and spinning black holes with Skyrme hair, the latter with rotation in two orthogonal planes, with both angular momenta of equal magnitude. In the absence of gravity we present an analytic solution that satisfies a BPS-type bound and explore numerically some of the non-BPS solutions. In the presence of gravity, we contrast the solutions to this model with solutions to a complex scalar field model, namely boson stars and black holes with synchronised hair. Remarkably, even though the two models present key differences, and in particular the Skyrme model allows static hairy black holes, when introducing rotation, the synchronisation condition becomes mandatory, providing further evidence for its generality in obtaining rotating hairy black holes

    Clinical and economic ramifications of switching antipsychotics in the treatment of schizophrenia

    <p>Abstract</p> <p>Background</p> <p>Switching between antipsychotic medications is common in the treatment of schizophrenia. However, data on clinical and economic outcomes from antipsychotic switching, in particular acute care service use, is fairly limited. The goal of this research was to assess the clinical and economic ramifications of switching antipsychotics during outpatient management of schizophrenia.</p> <p>Methods</p> <p>Data from a 1-year randomized, open-label cost-effectiveness study involving typical and atypical antipsychotics were assessed. The study protocol permitted switching of antipsychotics when clinically warranted. The risk of crisis-related events, use of acute-care services, and the time to the initial use of such services were determined in outpatients who switched antipsychotics compared with those who continued with their initial medications. Health care resource utilization data were abstracted from medical records and other sources (e.g., patient self-report), and direct costs were estimated using previously published benchmarks.</p> <p>Results</p> <p>Almost one-third of patients (29.3%) underwent a switch from their initial antipsychotic agent, with an average duration of 100 days before such treatment alterations. Compared with their counterparts who remained on their initial therapies, individuals who switched antipsychotics experienced a significantly higher risk of acute-care services, including hospitalization (p = .013) and crisis services (p = .011). Patients undergoing medication switches also used acute-care services significantly sooner (p = .004) and accrued an additional $3,000 (a 25% increase) in annual total health care costs per patient, most of which was due to acute-care expenditures.</p> <p>Conclusion</p> <p>Switching antipsychotic medications was found to be associated with considerably poorer clinical and economic outcomes, as reflected by, more frequent and more rapid use of acute-care services compared with persons remaining on their initial treatments.</p> <p>Trial Registration</p> <p>Trial ID 2325 in LillyTrials.com (also accessible via ClinicalStudyResults.org).</p

    Development of cognitive enhancers based on inhibition of insulin-regulated aminopeptidase

    The peptides angiotensin IV and LVV-hemorphin 7 were found to enhance memory in a number of memory tasks and reverse the performance deficits in animals with experimentally induced memory loss. These peptides bound specifically to the enzyme insulin-regulated aminopeptidase (IRAP), which is proposed to be the site in the brain that mediates the memory effects of these peptides. However, the mechanism of action is still unknown but may involve inhibition of the aminopeptidase activity of IRAP, since both angiotensin IV and LVV-hemorphin 7 are competitive inhibitors of the enzyme. IRAP also has another functional domain that is thought to regulate the trafficking of the insulin-responsive glucose transporter GLUT4, thereby influencing glucose uptake into cells. Although the exact mechanism by which the peptides enhance memory is yet to be elucidated, IRAP still represents a promising target for the development of a new class of cognitive enhancing agents

    <p>Abstract</p> <p>Background</p> <p>Recent data have shown that HTLV-1 is prevalent among HIV positive patients in Mozambique, although the impact of HTLV-1 infection on HIV disease progression remains controversial. Our aim was to determine the phenotypic profile of T lymphocytes subsets among Mozambican patients co-infected by HIV and HTLV-1.</p> <p>Methods</p> <p>We enrolled 29 patients co-infected by HTLV-1 and HIV (co-infected), 59 patients mono-infected by HIV (HIV) and 16 healthy controls (HC), respectively.</p> <p>For phenotypic analysis, cells were stained with the following fluorochrome-labeled anti-human monoclonal antibodies CD4-APC, CD8-PerCP, CD25-PE, CD62L-FITC, CD45RA-FITC. CD45RO-PE, CD38-PE; being analysed by four-colour flow cytometry.</p> <p>Results</p> <p>We initially found that CD4<sup>+ </sup>T cell counts were significantly higher in co-infected, as compared to HIV groups. Moreover, CD4<sup>+ </sup>T Lymphocytes from co-infected patients presented significantly higher levels of CD45RO and CD25, but lower levels of CD45RA and CD62L, strongly indicating that CD4<sup>+ </sup>T cells are more activated under HTLV-1 plus HIV co-infection.</p> <p>Conclusion</p> <p>Our data indicate that HTLV-1/HIV co-infected patients progress with higher CD4<sup>+ </sup>T cell counts and higher levels of activation markers. In this context, it is conceivable that in co-infected individuals, these higher levels of activation may account for a faster progression to AIDS.</p

    Kidney tubule health scores and their associations with incident CKD in women living with HIV

    ObjectivesIndividual kidney tubule biomarkers are associated with chronic kidney disease (CKD) risk in people living with HIV (PLWH). Whether a combination of kidney biomarkers can be integrated into informative summary scores for PLWH is unknown.MethodsWe measured eight urine biomarkers of kidney tubule health at two visits over a 3-year period in 647 women living with HIV in the Women's Interagency Health Study. We integrated biomarkers into factor scores using exploratory factor analysis. We evaluated associations between CKD risk factors and factor scores, and used generalized estimating equations to determine associations between factor scores and risk of incident CKD.ResultsFactor analysis identified two unique factor scores: a tubule reabsorption score comprising alpha-1-microglobulin, beta-2-microglobulin and trefoil factor-3; and a tubule injury score comprising interleukin-18 and kidney injury molecule-1. We modelled the two factor scores in combination with urine epidermal growth factor (EGF) and urine albumin. Predominantly HIV-related CKD risk factors were independently associated with worsening tubule reabsorption scores and tubule injury scores. During a median follow-up of 7&nbsp;years, 9.7% (63/647) developed CKD. In multivariable time-updated models that adjusted for other factor scores and biomarkers simultaneously, higher tubule reabsorption scores [risk ratio (RR)&nbsp;=&nbsp;1.27, 95% confidence interval (CI): 1.01-1.59 per 1&nbsp;SD higher time-updated score], higher tubule injury scores (RR&nbsp;=&nbsp;1.36, 95% CI: 1.05-1.76), lower urine EGF (RR&nbsp;=&nbsp;0.75, 95% CI: 0.64-0.87), and higher urine albumin (RR&nbsp;=&nbsp;1.20, 95% CI: 1.02-1.40) were jointly associated with risk of incident CKD.ConclusionsWe identified two novel and distinct dimensions of kidney tubule health that appear to quantify informative metrics of CKD risk in PLWH
