    To study the short-term outcome of multimodality treatment on morbidity and quality of life in patients with carcinoma oesophagus

    Background: Oesophageal cancer is sixth most common cause of cancer related deaths worldwide. Despite complete resections, overall survival remained low. To improve the existing treatment combination of chemotherapy and radiotherapy in both neoadjuvant and adjuvant settings was introduced. Impact of treatment on quality of life, morbidities and toxicities associated with multimodality treatment in patients of carcinoma oesophagus was studied prospectively and retrospectively.Methods: A total of 40 patients histologically proven carcinoma of the oesophagus were enrolled in the study. Study included assessment of clinical features, risk factors, biopsy, multimodality treatment and associated morbidities and toxicities. Quality of Life (QOL) in patients of multimodality treatment assessed using European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 and Core 18 before, immediate post treatment and 6 months after completion of the planned treatment.Results: In 40 patients, male to female ratio was 1.10:1. Dysphagia was the leading complain. 3 patients were treated using neoadjuvant therapy (chemotherapy and radiotherapy followed by surgery) among them 2 patients expired, 1 patient is under regular follow up. 6 patients underwent upfront surgery followed by adjuvant therapy out of which 3 patients expired,15 patients were treated with curative intent by definitive chemotherapy and radiotherapy out of which only 7 patients survived for >1 year. 16 patients were treated with palliative intent (radiotherapy, chemotherapy, oesophageal stenting and feeding procedures) in which only 1 patient survived for 1 year.Conclusions: The present study demonstrates that patients over all QOL was reduced after treatment and in the follow up period which was due to the treatment related toxicities but in contrast symptom improvement was observed simultaneously

    GPs’ views and perspectives on patient non-adherence to treatment in primary care prior to suicide

    Background: Individuals who do not adhere to their treatment regimens may be at greater risk of suicide but these issues are relatively unexplored in primary care services. Aim: To explore GPs’ views and perspectives on the reasons why patients who were in contact with mental health services in the year prior to suicide was non-adherent to treatment prior to their death. Method: In total, 198 semi-structured face-to-face interviews with GPs of people who had died by suicide. Interviews were transcribed verbatim and analyzed using a thematic approach. Results: The following themes were conceptualized from GP interviews: (i) “Lack of insight or denial?” relates to what GPs perceived as their patients lack of insight into their psychiatric illness; (ii) “Lack of treatment choices” discusses GPs’ understanding of patient treatment choices; (iii) “Services for comorbidity and dual diagnosis” refers to treatment availability for suicidal patients with two or more mental health diagnoses. Conclusions: For suicide prevention, it is crucial for health professionals to assess patients and manage the possible causes of non-adherence. Policies for increasing treatment adherence, improving services for dual diagnosis, or providing alternative treatments to meet individual patient needs may help to achieve the best health outcomes and could potentially prevent suicide

    Assessment and Management of Suicide Risk in Primary Care

    Abstract Background Risk assessment and management of suicidal patients is emphasized as a key component of care in specialist mental health services, but these issues are relatively unexplored in primary care services. Aim To examine risk assessment and management in primary and secondary care in a clinical sample of individuals who were in contact with mental health services and died by suicide. Method Data collection from clinical proformas, case records, and semi-structured face–to-face interviews with general practitioners. Results Primary and secondary care data was available for 198 of the 336 cases (59%). The overall agreement in the rating of risk between services was poor (overall kappa = 0.127; p = 0.10). Depression, care setting (post discharge), suicidal ideation at last contact and a history of self-harm were associated with a rating of higher risk. Suicide prevention policies were available in 25% of primary care practices and 33% of staff received training in suicide risk assessments. Conclusion Risk is difficult to predict, but the variation in risk assessment between professional groups may reflect poor communication. Further research is required to understand this. There appears to be a relative lack of suicide risk assessment training in primary care

    Forensic Examination of Class Characteristics in English Handwriting of the Three States: Andhra Pradesh, Punjab and Kashmir in India

    The present study analyzed the English handwriting of subjects from Andhra Pradesh, Punjab and Jammu and Kashmir to investigate whether the examination of style characteristics helps determine the nationality/ethnicity of the writer or the place where the writer learned writing. The subjects had completed secondary education in their respective regional languages along with English as a second language. Characteristic features such as letter formations, punctuation marks, spacing between letters, and margins were observed. The data were analysed using the chi- square test to determine significant class characteristics in the English handwritings. Significant class characteristics were identified; these occurrences were attributable to the influence of habitually writing in their native language, because the impact of regional language on the subjects’ English handwriting was observed in a few samples. Forensic document examiners will be able to determine class characteristic in the English handwritings of people from different states using the established procedure and possibly also determine the nationality or ethnic origin of writers

    Naphthalimide based hybrid nanoparticles for selective recognition of Hg(II)

    1,8-Naphthalic anhydride and cystamine dihydrochloride have been used as precursors for the synthesis of a dipodal compound (1) which has been further processed into fluorescent organic nanoparticles (N1) by re-precipitation technique. This has resulted in the formation of spherical nanoparticles which have been characterized by TEM and DLS studies. These hybrid nanomaterials composed of organic and inorganic nanoparticles have been used as a sensing platform for determination of mercury(II). The sensing phenomenon shows a ratiometric response for the selective determination with Hg2+ ion among other heavy metal ions with enhancement in fluorescence emission at 340 nm and decrease in emission intensity at 469 nm. Other heavy metal show no interference towards recognition of Hg2+ ion

    A Comparative Study on Class Characteristics of DevanĂŁgri Script Writers from Three Different States of India

    India is a multilingual country. Hindi is the national language of India and Devanãgri script is used to write Hindi language. Various documents with legal value are made in Devanãgri script, which may be questioned for their authenticity and authorship. Sufficient research has been done and reported about different scripts. However, researches based on Devanãgri script are limited. This study focuses on finding the significant class characteristics of writers from three different states of India, namely, Bihar, Madhya Pradesh and Punjab. The data was also statistically analysed using Pearson’s Chi-Square test for its significance. Handwriting samples from 300 subjects were collected from three different states of India to analyze various class characteristics in Devanãgri script. The samples were examined qualitatively and statistically. Various general characteristics were selected for the analysis and characteristic features were tabulated after qualitative examination. Statistical analysis showed that the data was statistically significant. The general characteristics selected for the analysis and comparison of the handwriting samples in Devanãgri script were found to be significant. The impact of regional scripts on the Devanãgri script should be performed, as the influence of regional language could be seen in the samples collected from Punjab

    Analysis of noise pollution level in and around SIDCUL area in District Haridwar (Uttarakhand) India

    The primary objective of this study is to evaluate the issue of noise pollution in the vicinity of the industrial zone of Haridwar city and its correlation with the health of the people in and around the SIDCUL (State Industrial Development Corporation of Uttarakhand Limited). The study revealed that noise pollution levels were above the CPCB Standard for the sound level for industrial zones and residential zones. During the period from January to December 2018, noise levels were observed and monitored in both the industrial zone (Site-I SIDCUL) and the residential zone (Site-II Siwalik Nagar). The average noise levels varied throughout the year. At Site-I (Industrial Zone), during the daytime in July, the maximum average noise level recorded was 89.5 dB, while in April; the minimum average noise level was 81.1 dB. During nighttime, in January, the maximum average noise level reached 84.1 dB, and in May, the minimum average noise level was 76.6 dB. In Site-II Siwalik Nagar (Residential Zone), the daytime noise levels were a maximum average noise level of 61.1 dB in May and a minimum average noise level of 58.8 dB in September. During nighttime, the noise level reached a maximum average of 47.2 dB in October and a minimum average of 44.5 dB in May. Overall, the study revealed that noise levels were generally higher in both zones, except for the average nighttime noise level at Site II (residential zone), which was below the prescribed standard limit for noise

    A Systematic Literature Review on Task Allocation and Performance Management Techniques in Cloud Data Center

    As cloud computing usage grows, cloud data centers play an increasingly important role. To maximize resource utilization, ensure service quality, and enhance system performance, it is crucial to allocate tasks and manage performance effectively. The purpose of this study is to provide an extensive analysis of task allocation and performance management techniques employed in cloud data centers. The aim is to systematically categorize and organize previous research by identifying the cloud computing methodologies, categories, and gaps. A literature review was conducted, which included the analysis of 463 task allocations and 480 performance management papers. The review revealed three task allocation research topics and seven performance management methods. Task allocation research areas are resource allocation, load-Balancing, and scheduling. Performance management includes monitoring and control, power and energy management, resource utilization optimization, quality of service management, fault management, virtual machine management, and network management. The study proposes new techniques to enhance cloud computing work allocation and performance management. Short-comings in each approach can guide future research. The research's findings on cloud data center task allocation and performance management can assist academics, practitioners, and cloud service providers in optimizing their systems for dependability, cost-effectiveness, and scalability. Innovative methodologies can steer future research to fill gaps in the literature

    Primary care contact prior to suicide in individuals with mental illness

    BACKGROUND: Previous studies have reported differing rates of consultation with GPs prior to suicide. Patients with a psychiatric history have higher rates of consultation and consult closer to the time of their death. AIM: To investigate the frequency and nature of general practice consultations in the year before suicide for patients in current, or recent, contact with secondary mental health services. DESIGN OF STUDY: Retrospective case-note study and semi-structured interviews. SETTING: General practices in the northwest of England. METHOD: General practice data were obtained by a retrospective review of medical records (n = 247) and semi-structured interviews with GPs (n = 159). RESULTS: GP records were reviewed in 247 of the 286 cases (86%). Overall, 91% of individuals (n = 224) consulted their GP on at least one occasion in the year before death. The median number of consultations was 7 (interquartile range = 3–10). Interviews were carried out with GPs with regard to 159 patients. GPs reported concerns about their patient's safety in 43 (27%) cases, but only 16% of them thought that the suicide could have been prevented. Agreement between GPs and mental health teams regarding risk of suicide was poor. Both sets of clinicians rated moderate to high levels of risk in only 3% of cases for whom information was available (n = 139) (overall κ = 0.024). CONCLUSION: Consultation prior to suicide is common but suicide prevention in primary care is challenging. Possible strategies might include examining the potential benefits of risk assessment and collaborative working between primary and secondary care
