How do paediatricians use and monitor antithyroid drugs in the UK? A clinician survey
Objective
We aimed to document current practice in the medical management of paediatric hyperthyroidism in the UK and compare to international recommendations.
Design
A 27âquestion online survey distributed via an electronic newsletter in August 2018.
Participants
Responses from 48 members (11%) of the British Society for Paediatric Endocrinology and Diabetes.
Measurements
Information about antithyroid drug (ATD) preference, treatment duration, monitoring of full blood count (FBC), management of neutropaenia, agranulocytosis screening and patient education.
Results
Carbimazole is favoured by 98% of respondents and a âdose titrationâ regimen preferred over âblock and replaceâ (65% vs 29%). TRAbs (thyroidâstimulating hormone receptor antibodies) are used for diagnostic purposes by 85% and by 33% to look for evidence of disease remission. The majority (81%) treat for a minimum of 2 years before considering a trial off ATD. All respondents reported that they âalways/usuallyâ warn their patients about the risk of agranulocytosis before starting ATD, but written information is ârarely/neverâ provided by 63%. Sore throat (98%) and fever (92%) are the most commonly cited symptoms used to alert a patient to possible agranulocytosis. FBC is measured prior to treatment by 65% and measured periodically during treatment by 70%.
Conclusions
The management of paediatric hyperthyroidism with ATDs in the UK is not consistent with all international recommendations because a block and replace ATD regimen remains widely used. TRAbs are utilized at presentation, but underused for detecting disease remission. National consensus guidelines and written patient information may refine the management of paediatric patients on ATDs
Analysis of Parametric Oscillatory Instability in Power Recycled LIGO Interferometer
We present the analysis of a nonlinear effect of parametric oscillatory
instability in power recycled LIGO interferometer with the Fabry-Perot (FP)
cavities in the arms. The basis for this effect is the excitation of the
additional (Stokes) optical mode and the mirror elastic mode, when the optical
energy stored in the main FP cavity main mode exceeds the certain threshold and
the frequencies are related so that sum of frequencies of Stokes and elastic
modes are approximately equal to frequencyof main mode. The presence of
anti-Stokes modes (with frequency approximately equal to sum of frequencies of
main and elastic modes) can depress parametric instability. However, it is very
likely that the anti-Stokes modes will not compensate the parametric

This study investigated student attitude and behaviours when encountering learning about health services in host and home students in the context of problem based learning. Materials and Methods First year graduate entry students' health systems interest and exposure and their perceptions of the dynamics of learning in PBL were investigated via a questionnaire comprising open and closed questions. Results and Conclusions Results showed a difference between home and host students in the ways they learned about home health systems and their attitudes to the value of learning about home and international health systems. There was no difference in the quantity of health service related learning objectives generated. Both groups reported noticing differences between the PBL cases and clinical practice, however, perceptions of the reasons for the differences varied between home and host students. We are interested in the way in which this perception of difference was reported as either a stimulus or a barrier to learning
Eosinophils Are Important for Protection, Immunoregulation and Pathology during Infection with Nematode Microfilariae
Eosinophil responses typify both allergic and parasitic helminth disease. In helminthic disease, the role of eosinophils can be both protective in immune responses and destructive in pathological responses. To investigate whether eosinophils are involved in both protection and pathology during filarial nematode infection, we explored the role of eosinophils and their granule proteins, eosinophil peroxidase (EPO) and major basic protein-1 (MBP-1), during infection with Brugia malayi microfilariae. Using eosinophil-deficient mice (PHIL), we further clarify the role of eosinophils in clearance of microfilariae during primary, but not challenge infection in vivo. Deletion of EPO or MBP-1 alone was insufficient to abrogate parasite clearance suggesting that either these molecules are redundant or eosinophils act indirectly in parasite clearance via augmentation of other protective responses. Absence of eosinophils increased mast cell recruitment, but not other cell types, into the broncho-alveolar lavage fluid during challenge infection. In addition absence of eosinophils or EPO alone, augmented parasite-induced IgE responses, as measured by ELISA, demonstrating that eosinophils are involved in regulation of IgE. Whole body plethysmography indicated that nematode-induced changes in airway physiology were reduced in challenge infection in the absence of eosinophils and also during primary infection in the absence of EPO alone. However lack of eosinophils or MBP-1 actually increased goblet cell mucus production. We did not find any major differences in cytokine responses in the absence of eosinophils, EPO or MBP-1. These results reveal that eosinophils actively participate in regulation of IgE and goblet cell mucus production via granule secretion during nematode-induced pathology and highlight their importance both as effector cells, as damage-inducing cells and as supervisory cells that shape both innate and adaptive immunity
Negative emotions in informal feedback: The benefits of disappointment and drawbacks of anger
Using the emotions as social information (EASI) model, this study investigated the emotional, attitudinal and behavioral reactions to failure feedback by manipulating negative emotional displays (angry, disappointed or none) and the position level and relational distance of the feedback source. Undergraduate students (N = 260) responded to an organizational failure feedback vignette and completed a subsequent performance task. Results demonstrated that guilt was the complementary emotional experience following displays of disappointment, while reciprocal anger followed displays of anger. These emotional reactions served as important mediators between the emotional displays paired with the feedback message and participant responses of social behaviors, creative task performance and perceptions of the feedback source. In addition, our findings indicated that negative emotions can have positive organizational and interpersonal outcomes. Guilt in response to disappointed displays resulted in beneficial behaviors and attitudes, while anger in response to angry displays was socially detrimental. The emotion displayed during feedback provision also served as a consistent contextual factor that did not interact with the position level or relational distance of the feedback source to impact behavioral and attitudinal reactions. Mindfulness based interventions in multiple sclerosis: a systematic review
<b>Background</b> Multiple sclerosis (MS) is a stressful condition; depression, anxiety, pain and fatigue are all common problems. Mindfulness based interventions (MBIs) mitigate stress and prevent relapse in depression and are increasingly being used in healthcare. However, there are currently no systematic reviews of MBIs in people with MS. This review aims to evaluate the effectiveness of MBIs in people with MS.<p></p>
<b>Methods</b> Systematic searches were carried out in seven major databases, using both subject headings and key words. Papers were screened, data extracted, quality appraised, and analysed by two reviewers independently, using predefined criteria. Study quality was assessed using the Cochrane Collaboration risk of bias tool. Perceived stress was the primary outcome. Secondary outcomes include mental health, physical health, quality of life, and health service utilisation. Statistical meta-analysis was not possible. Disagreements were adjudicated by a third party reviewer.<p></p>
<b>Results</b> Three studies (nâ=â183 participants) were included in the final analysis. The studies were undertaken in Wales (nâ=â16, randomised controlled trial - (RCT)), Switzerland (nâ=â150, RCT), and the United States (nâ=â17, controlled trial). 146 (80%) participants were female; mean age (SD) was 48.6 (9.4) years. Relapsing remitting MS was the main diagnostic category (nâ=â123, 67%); 43 (26%) had secondary progressive disease; and the remainder were unspecified. MBIs lasted 6â8Â weeks; attrition rates were variable (5-43%); all employed pre- post- measures; two had longer follow up; one at 3, and one at 6 months. Socio-economic status of participants was not made explicit; health service utilisation and costs were not reported. No study reported on perceived stress. All studies reported quality of life (QOL), mental health (anxiety and depression), physical (fatigue, standing balance, pain), and psychosocial measures. Statistically significant beneficial effects relating to QOL, mental health, and selected physical health measures were sustained at 3- and 6- month follow up.<p></p>
<b>Conclusion</b> From the limited data available, MBIs may benefit some MS patients in terms of QOL, mental health, and some physical health measures. Further studies are needed to clarify how MBIs might best serve the MS population.<p></p>
Mindfulness-based interventions in epilepsy: a systematic review
Mindfulness based interventions (MBIs) are increasingly used to help patients cope with physical and mental long-term conditions (LTCs). Epilepsy is associated with a range of mental and physical comorbidities that have a detrimental effect on quality of life (QOL), but it is not clear whether MBIs can help. We systematically reviewed the literature to determine the effectiveness of MBIs in people with epilepsy. Medline, Cochrane Central Register of Controlled Trials, EMBASE, CINAHL, Allied and Complimentary Medicine Database, and PsychInfo were searched in March 2016. These databases were searched using a combination of subject headings where available and keywords in the title and abstracts. We also searched the reference lists of related reviews. Study quality was assessed using the Cochrane Collaboration risk of bias tool. Three randomised controlled trials (RCTs) with a total of 231 participants were included. The interventions were tested in the USA (nâ=â171) and China (Hong Kong) (nâ=â60). Significant improvements were reported in depression symptoms, quality of life, anxiety, and depression knowledge and skills. Two of the included studies were assessed as being at unclear/high risk of bias - with randomisation and allocation procedures, as well as adverse events and reasons for drop-outs poorly reported. There was no reporting on intervention costs/benefits or how they affected health service utilisation. This systematic review found limited evidence for the effectiveness of MBIs in epilepsy, however preliminary evidence suggests it may lead to some improvement in anxiety, depression and quality of life. Further trials with larger sample sizes, active control groups and longer follow-ups are needed before the evidence for MBIs in epilepsy can be conclusively determined
