The clinical pharmacology of intranasal l-methamphetamine.
BackgroundWe studied the pharmacology of l-methamphetamine, the less abused isomer, when used as a nasal decongestant.Methods12 subjects self-administered l-methamphetamine from a nonprescription inhaler at the recommended dose (16 inhalations over 6 hours) then at 2 and 4 (32 and 64 inhalations) times this dose. In a separate session intravenous phenylephrine (200 microg) and l-methamphetamine (5 mg) were given to define alpha agonist pharmacology and bioavailability. Physiological, cardiovascular, pharmacokinetic, and subjective effects were measured.ResultsPlasma l-methamphetamine levels were often below the level of quantification so bioavailability was estimated by comparing urinary excretion of the intravenous and inhaled doses, yielding delivered dose estimates of 74.0 +/- 56.1, 124.7 +/- 106.6, and 268.1 +/- 220.5 microg for ascending exposures (mean 4.2 +/- 3.3 microg/inhalation). Physiological changes were minimal and not dose-dependent. Small decreases in stroke volume and cardiac output suggesting mild cardiodepression were seen.ConclusionInhaled l-methamphetamine delivered from a non-prescription product produced minimal effects but may be a cardiodepressant

Opportunities for system level improvement in antibiotic use across the surgical pathway
Optimizing antibiotic prescribing across the surgical pathway (before, during, and after surgery) is a key aspect of tackling important drivers of antimicrobial resistance and simultaneously decreasing the burden of infection at the global level. In the UK alone, 10 million patients undergo surgery every year, which is equivalent to 60% of the annual hospital admissions having a surgical intervention. The overwhelming majority of surgical procedures require effectively limited delivery of antibiotic prophylaxis to prevent infections. Evidence from around the world indicates that antibiotics for surgical prophylaxis are administered ineffectively, or are extended for an inappropriate duration of time postoperatively. Ineffective antibiotic prophylaxis can contribute to the development of surgical site infections (SSIs), which represent a significant global burden of disease. The World Health Organization estimates SSI rates of up to 50% in postoperative surgical patients (depending on the type of surgery), with a particular problem in low- and middle-income countries, where SSIs are the most frequently reported healthcare-associated infections. Across European hospitals, SSIs alone comprise 19.6% of all healthcare-acquired infections. Much of the scientific research in infection management in surgery is related to infection prevention and control in the operating room, surgical prophylaxis, and the management of SSIs, with many studies focusing on infection within the 30-day postoperative period. However it is important to note that SSIs represent only one of the many types of infection that can occur postoperatively. This article provides an overview of the surgical pathway and considers infection management and antibiotic prescribing at each step of the pathway. South African medical students’ perceptions and knowledge about antibiotic resistance and appropriate prescribing: Are we providing adequate training to future prescribers?
Background. Education of medical students has been identified by the World Health Organization as an important aspect of antibiotic resistance (ABR) containment. Surveys from high-income countries consistently reveal that medical students recognise the importance of antibiotic prescribing knowledge, but feel inadequately prepared and require more education on how to make antibiotic choices. The attitudes and knowledge of South African (SA) medical students regarding ABR and antibiotic prescribing have never been evaluated.Objective. To evaluate SA medical students’ perceptions, attitudes and knowledge about antibiotic use and resistance, and the perceived quality of education relating to antibiotics and infection.Methods. This was a cross-sectional survey of final-year students at three medical schools, using a 26-item self-administered questionnaire. The questionnaires recorded basic demographic information, perceptions about antibiotic use and ABR, sources, quality, and usefulness of current education about antibiotic use, and questions to evaluate knowledge. Hard-copy surveys were administered during whole-class lectures.Results. A total of 289 of 567 (51%) students completed the survey. Ninety-two percent agreed that antibiotics are overused and 87% agreed that resistance is a significant problem in SA – higher proportions than those who thought that antibiotic overuse (63%) and resistance (61%) are problems in the hospitals where they had worked (p<0.001). Most reported that they would appreciate more education on appropriate use of antibiotics (95%). Only 33% felt confident to prescribe antibiotics, with similar proportions across institutions. Overall, prescribing confidence was associated with the use of antibiotic prescribing guidelines (p=0.003), familiarity with antibiotic stewardship (p=0.012), and more frequent contact with infectious diseases specialists (p<0.001). There was an overall mean correct score of 50% on the knowledge questionnaire, with significant differences between institutions. Students who used antibiotic prescribing guidelines and found their education more useful scored higher on knowledge questionnaires.Conclusion. There are low levels of confidence with regard to antibiotic prescribing among final-year medical students in SA, and most students would like more education in this area. Perceptions that ABR is less of a problem in their local setting may contribute to inappropriate prescribing behaviours. Differences exist between medical schools in knowledge about antibiotic use, with suboptimal scores across institutions. The introduction and use of antibiotic prescribing guidelines and greater contact with specialists in antibiotic prescribing may improve prescribing behaviours
Inclusive Masculinity and Facebook Photographs Among Early Emerging Adults at a British University
Central to debates about the construction of masculinity in sociology is the influence of culture and what constitutes acceptable displays of masculinity. This article adopts a novel approach in examining this question. It adopts a summative content analysis, combined with a semiotic analysis, of 1,100 Facebook photographs, in order to explore the underlying meanings within the photos and the performances of masculinity. Facebook photographs from 44, straight, White, male, early emerging adults attending the same university are used as a representation of an individual’s ideal self. These are then analyzed in order to determine the behaviors endorsed by peer culture. It was found that the sample overwhelmingly adopted inclusive behaviors (including homosocial tactility, dancing, and kissing each other), and inclusive masculinity theory was utilized to contextualize participants’ constructions of masculinity. Thus, this research shows that emerging adult males at this university construct their masculine identities away from previous orthodox archetypes. It is argued that the reducing importance of gendered behavior patterns may represent an adoption of what are perceived as wider cultural norms and act as a symbol of adulthood to these early emerging adults
The use of imagery in global health: an analysis of infectious disease documents and a framework to guide practice
We report an empirical analysis of the use of imagery by the key actors in global health who set policy and strategy, and we provide a comprehensive overview, particularly related to images used in reports on vaccination and antimicrobial resistance. The narrative currently depicted in imagery is one of power imbalances, depicting women and children from low-income and middle-income countries (LMICs) with less dignity, respect, and power than those from high-income countries. The absence of any evidence of consent for using intrusive and out-of-context images, particularly of children in LMICs, is concerning. The framework we have developed provides a platform for global health actors to redefine their intentions and recommission appropriate images that are relevant to the topic, respect the integrity of all individuals depicted, are accompanied by evidence of consent, and are equitable in representation. Adhering to these standards will help to avoid inherent biases that lead to insensitive content and misrepresentation, stigmatisation, and racial stereotyping
Signs of low frequency dispersions in disordered binary dielectric mixtures (50-50)
Dielectric relaxation in disordered dielectric mixtures are presented by
emphasizing the interfacial polarization. The obtained results coincide with
and cause confusion with those of the low frequency dispersion behavior. The
considered systems are composed of two phases on two-dimensional square and
triangular topological networks. We use the finite element method to calculate
the effective dielectric permittivities of randomly generated structures. The
dielectric relaxation phenomena together with the dielectric permittivity
values at constant frequencies are investigated, and significant differences of
the square and triangular topologies are observed. The frequency dependent
properties of some of the generated structures are examined. We conclude that
the topological disorder may lead to the normal or anomalous low frequency
dispersion if the electrical properties of the phases are chosen properly, such
that for ``slightly'' {\em reciprocal mixture}--when , and
--normal, and while for ``extreme'' {\em reciprocal
mixture}--when , and --anomalous
low frequency dispersions are obtained. Finally, comparison with experimental
data indicates that one can obtain valuable information from simulations when
the material properties of the constituents are not available and of

Gravitational Microlensing Evidence for a Planet Orbiting a Binary Star System
The study of extra-solar planetary systems has emerged as a new discipline of
observational astronomy in the past few years with the discovery of a number of
extra-solar planets. The properties of most of these extra-solar planets were
not anticipated by theoretical work on the formation of planetary systems. Here
we report observations and light curve modeling of gravitational microlensing
event MACHO-97-BLG-41, which indicates that the lens system consists of a
planet orbiting a binary star system. According to this model, the mass ratio
of the binary star system is 3.8:1 and the stars are most likely to be a late K
dwarf and an M dwarf with a separation of about 1.8 AU. A planet of about 3
Jupiter masses orbits this system at a distance of about 7 AU. If our
interpretation of this light curve is correct, it represents the first
discovery of a planet orbiting a binary star system and the first detection of
a Jovian planet via the gravitational microlensing technique. It suggests that

Labels for non-individuals
Quasi-set theory is a first order theory without identity, which allows us to
cope with non-individuals in a sense. A weaker equivalence relation called
``indistinguishability'' is an extension of identity in the sense that if
is identical to then and are indistinguishable, although the
reciprocal is not always valid. The interesting point is that quasi-set theory
provides us a useful mathematical background for dealing with collections of
indistinguishable elementary quantum particles. In the present paper, however,
we show that even in quasi-set theory it is possible to label objects that are
considered as non-individuals. We intend to prove that individuality has
nothing to do with any labelling process at all, as suggested by some authors.

Direct mechanical thrombectomy with or without bridging thrombolysis in patients with acute ischemic stroke: a systematic review and meta-analysis of randomized trials
Twirling and Whirling: Viscous Dynamics of Rotating Elastica
Motivated by diverse phenomena in cellular biophysics, including bacterial
flagellar motion and DNA transcription and replication, we study the overdamped
nonlinear dynamics of a rotationally forced filament with twist and bend
elasticity. Competition between twist injection, twist diffusion, and writhing
instabilities is described by a novel pair of coupled PDEs for twist and bend
evolution. Analytical and numerical methods elucidate the twist/bend coupling
and reveal two dynamical regimes separated by a Hopf bifurcation: (i)
diffusion-dominated axial rotation, or twirling, and (ii) steady-state
crankshafting motion, or whirling. The consequences of these phenomena for
self-propulsion are investigated, and experimental tests proposed.Comment: To be published in Physical Review Letter
