    THE VUCA ERA CREATES COVID-19 PANDEMIC IN INDONESIA BEING COMPLICATED

    This article aims to analyze the confusion of information, uncertainty in the situation, and human puzzlement have overshadowed the actions of Indonesian citizens throughout 2020 until the end of January 2021 in line with the pandemic outbreak which have not yet ended and still cannot be predicted when it will end. Volatility, uncertainty, problems of complexity and ambiguity of the choices are conditions that have been described by the VUCA concept. Covid-19, which has infected more than one million people in the country, has caused a lot of excitement in various domains: economic, political, social, cultural, and various other aspects of life. By conducting a literature review from March 2020 to January 2021 and analyzing secondary data from Twitter and distributing short questionnaires through Google Form on August 2020, as 969 respondents in Java, Bali, Sumatera, Kalimantan, Sulawesi, Nusa Tenggara dan Papua Islands the authors found that the VUCA era facilitates various complexities in dealing with the Covid-19 pandemic. The clumsy policies create uncertainty way in the process of social transformation towards society who cares about health protocols. Some of the solutions offered by Bennet and Lemoine (2014) or Aura Codreanu (2016) are still insufficient to manage the changes that occur towards a better life. This article contributes ideas related to social action about new social care, togetherness, and responsibility for realizing a new normal life

    The Dynamic Role of Moslem in Building Indonesia as a Nation-State

    This study figures out the historical journey of the Indonesian nation which is full of its own dynamics is marked by the prominent role of Moslem figures in fighting for and maintaining national unity. The socio-cultural diversity is vital capital as well as a big challenge for Indonesia in building a nation-state. For more than seventy years this pluralistic society has been united by one state ideology, namely "Pancasila" which is expressly stated in the constitution. Optimism was reflected in the economic progress achieved in the mid-1990s, but after the 1997-1998 economic crisis which was followed by a multidimensional crisis, social integration experienced major problems. This article explains the role of the Moslem community in establishing the Indonesian nation, seizing, and maintaining independence, as well as the potential threats to national integration. This article emphasizes again that the long journey of the Indonesian nation and the role of the Islamic community in its long history is an important capital that should not be forgotten in efforts to increase Indonesia's glory for the future. From independence until two-thirds of the century of it, the representation of Islamic groups is very calculated in managing government, however, in elections, they only have a significant enough vote but have never won. With a historical approach through a literature review, the author argues that there are many things that need to be done by the Indonesian people, especially the Moslem community to build Indonesia as a strong nation-state

    A pathogenic long non-coding RNA redesigns the epigenetic landscape of the infected cells by subverting host Histone Deacetylase 6 activity

    [EN] Viroids - ancient plant-pathogenic long noncoding RNAs - have developed a singular evolutionary strategy based on reprogramming specific phases of host-metabolism to ensure that their infection cycle can be completed in infected cells. However, the molecular aspects governing this transregulatory phenomenon remain elusive. Here, we use immunoprecipitation assays and bisulfite sequencing of rDNA to shown that, in infected cucumber and Nicotiana benthamina plants, Hop stunt viroid (HSVd) recruits and functionally subverts Histone Deacetylase 6 (HDA6) to promote host-epigenetic alterations that trigger the transcriptional alterations observed during viroid pathogenesis. This notion is supported by the demonstration that, during infection, the HSVd-HDA6 complex occurs invivo and that endogenous HDA6 expression is increased in HSVd-infected cells. Moreover, transient overexpression of recombinant HDA6 reverts the hypomethylation status of rDNA observed in HSVd-infected plants and reduces viroid accumulation. We hypothesize that the host-transcriptional alterations induced as a consequence of viroid-mediated HDA6 recruitment favor spurious recognition of HSVd-RNA as an RNA Pol II template, thereby improving viroid replication. Our results constitute the first description of a physical and functional interaction between a pathogenic RNA and a component of the host RNA silencing mechanism, providing novel evidence of the potential of these pathogenic lncRNAs to physically redesign the host-cell environment and reprogram their regulatory mechanisms.The authors thank Dr M.     The lack of availability of antidotes in emergency services is a worldwide concern. The aim of the present study was to evaluate the availability of antidotes used for treating poisoning in Campinas (SP). DESIGN AND SETTING: This was a cross-sectional study of emergency services in Campinas, conducted in 2010-2012. METHODS: The availability, amount in stock, place of storage and access time for 26 antidotal treatments was investigated. In the hospitals, the availability of at least one complete treatment for a 70 kg adult over the first 24 hours of admission was evaluated based on stock and access recommendations contained in two international guidelines. RESULTS: 14 out of 17 functioning emergency services participated in the study, comprising pre-hospital services such as the public emergency ambulance service (SAMU; n = 1) and public emergency rooms for admissions lasting ≤ 24 hours (UPAs; n = 3), and 10 hospitals with emergency services. Six antidotes (atropine, sodium bicarbonate, diazepam, phytomenadione, flumazenil and calcium gluconate) were stocked in all the services, followed by 13 units that also stocked activated charcoal, naloxone and diphenhydramine or biperiden. No service stocked all of the recommended antidotes; only the regional Poison Control Center had stocks close to recommended (22/26 antidotal treatments). The 10 hospitals had almost half of the antidotes for starting treatments, but only one quarter of the antidotes was present with stocks sufficient for providing treatment for 24 hours. CONCLUSION: The stock of antidotes for attending poisoning emergencies in the municipality of Campinas is incomplete and needs to be improved. © 2017, Associacao Paulista de Medicina.     Radiographic parameters of the digit in a cohort population of amiata donkeys

    Background: The most common musculoskeletal conditions reported in donkeys are related to the foot. Radiographic examinations are clinically important in the diagnosis of foot abnormalities and are commonly used. However, few studies have been conducted to establish the normal radiographic appearance of a donkey’s foot. Aim: To determine the radiographic features of the front digit in healthy Amiata donkeys. Methods: Radiographic examinations were performed on 56 forefeet of 28 Amiata donkeys. Three radiographic views of each front foot were taken: lateromedial, dorsopalmar and dorso-65°proximal/palmarodistal oblique. Seventeen angular and linear radiographic parameters and the crena solearis were evaluated in all forefeet, and 18 morphometric parameters were evaluated in 16 out of 56 forefeet. Statistical analysis was carried out on all the measures assessed. Results: The radiographic appearance of the forefoot was ascertained, and data were reported as median ± standard error, minimum and maximum values. No statistical differences were obtained between the right and left forefeet. Conclusion: The normal baseline parameters of the forefeet of Amiata donkeys were recorded and described and compared with other donkey breeds and horses.     Predictors of Bone Metastases at68Ga-PSMA-11 PET/CT in Hormone-Sensitive Prostate Cancer (HSPC) Patients with Early Biochemical Recurrence or Persistence

    Prostate-specific-membrane-antigen/positron-emission-tomography (PSMA-PET) can accurately detect disease localizations in prostate cancer (PCa) patients with early biochemical recurrence/persistence (BCR/BCP), allowing for more personalized image-guided treatments in oligometastatic patients with major impact in the case of bone metastases (BM). Therefore, this study aimed to identify predictors of BM at PSMA-PET in early-BCR/BCP hormone-sensitive PCa (HSPC) patients, previously treated with radical intent (radiotherapy or radical prostatectomy ± salvage-radiotherapy (SRT)). A retrospective analysis was performed on 443 (68)Ga-PSMA-11-PET/CT scans. The cohort median PSA at PET-scan was 0.60 (IQR: 0.38–1.04) ng/mL. PSMA-PET detection rate was 42.0% (186/443), and distant lesions (M1a/b/c) were found in 17.6% (78/443) of cases. BM (M1b) were present in 9.9% (44/443) of cases, with 70.5% (31/44) showing oligometastatic spread (≤3 PSMA-positive lesions). In the multivariate binary logistic regression model (accuracy: 71.2%, Nagelkerke-R(2): 13%), T stage ≥ 3a (OR: 2.52; 95% CI: 1.13–5.60; p = 0.024), clinical setting (previous SRT vs. first-time BCR OR: 2.90; 95% CI: 1.32–6.35; p = 0.008), and PSAdt (OR: 0.93; 95% CI: 0.88–0.99; p = 0.026) were proven to be significant predictors of bone metastases, with a 7% risk increment for each single-unit decrement of PSAdt. These predictors could be used to further refine the indication for PSMA-PET in early BCR/BCP HSPC patients, leading to higher detection rates of bone disease and more personalized treatments

    Radiation therapy for oligometastatic oropharyngeal cancer

    At presentation, isolated metastasis from oropharyngeal squamous cell carcinoma is rare. Liver is a relatively uncommon first site of failure, especially in the absence of other distant metastases, particularly without diagnosis of lung metastases. We report on a case of HPV-related oropharyngeal squamous cell carcinoma with synchronous liver metastasis treated with radiation therapy. This condition, defined as "oligometastatic state," describes a subset of patients with limited volume metastatic disease in whom favorable outcomes were reported with the use of local ablative therapies on both the primary tumor and metastatic sites. As a definitive treatment, we offered the patient, ineligible for other therapeutic approaches, exclusive radiation treatment on the head and neck region and a stereotactic ablative approach targeted to the liver metastasis
