    Ultrasonic (UT) inspection of aircraft engine parts has traditionally been conducted in an immersion water tank. However, experience has shown that the immersion tank is usually large, awkward, and tedious to work with. An alternative method which does not require immersion would increase the time efficiency of the UT inspection. One such method would be to use a captured water column coupling system, which closely approximates the immersion method and eliminates the need for a large immersion tank. The tank would be replaced by a trough or water collection tray to collect the water produced by the low water flow of the captured water column

    Immunophenotypic characterization of plasma cells in patients with multiple myeloma

    INTRODUCTION AND OBJECTIVE: Multiple myeloma is an incurable malignancy characterized by the proliferation of a single clone of plasma cells in bone marrow. The aim of this study was to evaluate the frequency and prognostic value of the expression of aberrant phenotypes in patients with multiple myeloma by multiparametric flow cytometry. METHODS: The study was carried out at Department of Hematology and Hemotherapy of Federal University of São Paulo and 30 patients with MM were analyzed prospectively. In an attempt to identify myeloma cells by flow cytometry (FACSCalibur, BD), specific monoclonal antibodies anti-CD138, anti-CD38 and anti-CD45 were used for the selection of plasma cells. The control group comprised four healthy bone marrow donors. RESULTS: All myeloma plasma cells expressed at least one aberrant phenotype and CD56+++, CD117++, CD33++, CD13++ and CD28++ markers were more frequently observed in 88% of patients. Lymphoid markers were found in cases with a higher number of aberrant phenotypes. DISCUSSION: CD56+++ and CD28++ antigens showed high levels of β2-microglobulin, which are associated with more aggressive stages of the disease and larger tumor mass. The absence of adhesion molecule CD56 was associated with high levels of β2M and calcium ion, showing that this finding may have prognostic value. CONCLUSION: From this study it was concluded that the aberrant phenotypes are present in most cases of MM, and immunophenotyping by multiparametric flow cytometry is a useful tool to distinguish normal plasma cells from myeloma plasma cells.INTRODUÇÃO E OBJETIVO: O mieloma múltiplo (MM) é uma doença maligna incurável caracterizada pela proliferação de um único clone de plasmócitos na medula óssea. O objetivo deste estudo foi avaliar a frequência e o valor prognóstico da expressão dos fenótipos aberrantes em pacientes com MM por meio de citometria de fluxo multiparamétrica. MÉTODOS: O estudo foi realizado no Hospital São Paulo/Disciplina de Hematologia e Hemoterapia da Universidade Federal de São Paulo (UNIFESP), sendo analisados de maneira prospectiva 30 pacientes portadores de MM ao diagnóstico. Na tentativa de identificar as células mielomatosas por citometria de fluxo (FACScalibur, BD), foram utilizados anticorpos monoclonais anti-CD138, anti-CD38, anti-CD45 específicos para seleção dos plasmócitos. O grupo controle consistiu em quatro doadores saudáveis de medula óssea. RESULTADOS: Todos os plasmócitos mielomatosos expressaram pelo menos um fenótipo aberrante, e CD56+++, CD117++, CD33++, CD13++, CD28++ foram os marcadores mais frequentes, observados em 88% dos pacientes. Os marcadores linfoides foram encontrados nos casos com maior número de fenótipos aberrantes. DISCUSSÃO: Os antígenos CD56+++ e CD28++ apresentaram altos níveis de β2-microglobulina, sendo estes associados a estágios mais agressivos da doença e maior massa tumoral. A ausência da molécula de adesão CD56 foi relacionada com altos níveis de β2M e de cálcio iônico, mostrando que este achado pode ter valor prognóstico.     Long-term outcome with the automatic implantable cardioverter-defibrillator

    AbstractThe automatic implantable cardioverter-defibrillator was implanted in 270 patients because of life-threatening arrhythmias over a 7 year period. There was a history of sustained ventricular tachycardia or fibrillation, or both, in 96% of these patients, 80% had one or more prior cardiac arrests and 78% had coronary artery disease as their underlying diagnosis. The average ejection fraction was 34%, and 96% of these patients had had an average of 3.4 antiarrhythmic drug failures per patient before defibrillator implantation. There were four perioperative deaths and eight patients had generator infection or generator erosion, or both, during the perioperative period or during long-term follow-up. Concomitant antiarrhythmic drug therapy was given to 69% of patients.Shocks from the device were given to 58% of patients, and 20% received “problematic” shocks. The device was removed from 16 patients during long-term follow-up for a variety of reasons. There were 7 sudden cardiac deaths and 30 nonsudden cardiac deaths, 18 of which were secondary to congestive heart failure. The actuarial incidence of sudden death, total cardiac death and total mortality from all causes was 1%, 7% and 8%, respectively, at 1 year, and 4%, 24% and 26% at 5 years.The automatic implantable cardioverter-defibrillator nearly eliminates sudden death over a long-term follow-up period in a high risk group of patients. It has an acceptable rate of complications or problems, or both, and most late deaths in these patients are nonsudden and of cardiovascular origin

    High incidence of chromosomal numerical abnormalities by multicentromeric FISH in multiple myeloma patients

    This study aimed to characterize genetic alterations by interphase multicentromeric FISH focusing on chromosomal numerical abnormalities and using some locus specific probes for the most frequent aberrations found in the disease, in a homogeneous cohort of 34 advanced stage, but recently diagnosed MM patients; 97% had numerical chromosomal abnormalities detected by FISH, being 75% hyperdiploid, 18% hypodiploid and 3% tri/tetraploid. Using locus specific probes, we found 13q deletion in 30% and IGH rearrangement in 25% of cases. Grouping hypodiploid patients together with del13q (unfavorable group) and comparing them to the remaining cases (non unfavorable group) we found a trend towards younger patients presenting more unfavorable abnormalities (p = 0.06) and significant lower hemoglobin level (Hb < 8.5 mg/dl, p = 0.03).Este estudo objetivou detectar as alterações genéticas em pacientes com mieloma múltiplo (MM), usando o método de hibridação in situ por fluorescência em interfases (FISH interfásico). Para detectar as alterações numéricas foram usadas sondas multicentroméricas e para os rearranjos mais freqüentemente observados na doença foram utilizadas as sondas lócus específicas para IGH, P53, ciclina D1 e RB1. Foram estudados 34 pacientes com MM em estágio avançado, ainda que recém-diagnosticados, 97% dos quais apresentaram anormalidades numéricas detectadas por FISH, sendo 75% hiperdiplóides, 18% hipodiplóides e 3% tri/tetraplóides. Em relação às demais anormalidades, a deleção 13q foi encontrada em 30% dos casos e o rearranjo IGH, em 25%. Agrupando os pacientes com hipodiploidia e com deleção 13q14 (grupo desfavorável) e comparando-os com os demais (grupo não-desfavorável), houve tendência a pacientes jovens no grupo desfavorável (p = 0,06) e níveis de hemoglobina (Hb) significativamente mais baixos (< 8,5 g/dl, p = 0,03).Universidade Federal de São Paulo (UNIFESP)UNIFESPSciEL

    A novel screening system improves genetic correction by internal exon replacement

    Trans-splicing is a powerful approach to reprogram the genome. It can be used to replace 5′, 3′ or internal exons. The latter approach has been characterized by low efficiency, as the requirements to promote internal trans-splicing are largely uncharacterized. The trans-splicing process is induced by engineered ‘RNA trans-splicing molecules’ (RTMs), which target a selected pre-mRNA to be reprogrammed via two complementary binding domains. To facilitate the development of more efficient RTMs for therapeutic applications we constructed a novel fluorescence based screening system. We incorporated exon 52 of the COL17A1 gene into a GFP-based cassette system as the target exon. This exon is mutated in many patients with the devastating skin blistering disease epidermolysis bullosa. In a double transfection assay we were able to rapidly identify optimal binding domains targeted to sequences in the surrounding introns 51 and 52. The ability to replace exon 52 was then evaluated in a more endogenous context using a target containing COL17A1 exon 51–intron 51–exon 52–intron 52–exon 53. Two selected RTMs produced significantly higher levels of GFP expression in up to 61% assayed cells.     BEHAVIOUR of "CANINO" APRICOT CULTIVAR GROWN IN THREE DIFFERENT SOILS

    The response of “Canino” apricot trees to different soils were studied in three different regions, in El-Menoufia Governorate (as a sand and clay soil) & Alexandria Governorate (as a calcareous soil) during the two successive seasons of 2010 & 2011. Evaluation included vegetative growth, flowering, fruit set, tree productivity and fruit characteristics. Chilling units as well as growing –degree hours were also estimated through the two studied seasons. Results indicated that the vegetative growth was the best in clay soil for shoot length, leaf area and leaf chlorophyll content. Concerning periods of flowering, fruit set and harvesting dates there were variations among the three types of soils. Sand soil was the earliest followed by clay and calcareous soils. Percentages of nitrogen, phosphorus and potassium were positively affected by soil type. Also, Fe, Mn and Zn were affected by soil type. “Canino” apricot trees produced earlier fruit yield with the better fruit quality in sandy soil, whereas clay soil type gave the greatest yield followed by sand and calcareous soils

    Kidney protection with canagliflozin: A combined analysis of the randomized CANVAS program and CREDENCE trials

    AIM: In the CANVAS Program and CREDENCE trials, the sodium glucose co-transporter 2 inhibitor canagliflozin reduced the risk of cardiovascular and kidney events in patients with type 2 diabetes. The current study analysed a pooled population to ascertain the kidney protection provided by canagliflozin across the full spectrum of kidney parameters. METHODS: This post-hoc pooled analysis of the CANVAS Program (N = 10 142) and CREDENCE trial (N = 4401), assessed the risk of the primary kidney composite (doubling of serum creatinine, end-stage kidney disease, renal death), in all patients and subgroups defined by baseline estimated glomerular filtration rate (300 mg/g (33.9 mg/mmol)] and 2012 Kidney Disease: Improving Global Outcomes (KDIGO) classification of chronic kidney disease (low/moderate, high and very high risk). RESULTS: In the overall population, the risk for the primary kidney composite outcome was 37% lower in the canagliflozin group versus placebo (HR: 0.63; 95% CI: 0.53, 0.77; p  .05). A statistically significant risk reduction of the primary kidney composite outcome was sustained by approximately 18 months after randomization. CONCLUSIONS: These results emphasize a critical role of canagliflozin in kidney protection across a broad spectrum of participants with type 2 diabetes with varying levels of kidney function
