C-Ring Strength of Advanced Monolithic Ceramics

    Get PDF
Alumina, silicon carbide, silicon nitride, and zirconia are common candidate ceramics for load-bearing tubular components. To help facilitate design and reliability modeling with each ceramic, Weibull strength distributions were determined with each material using a diametrally compressed c-ring specimen in accordance with ASTM C1323. The investigated silicon nitride and zirconia were found to exhibit higher uncensored characteristic strengths than the alumina and silicon carbide. The occurrence of chamfer-located fracture initiation was problematic, and hindered the ability to generate valid design data in some of these ceramics. Fractography and stress modeling results suggest that some aspects of ASTM C1323 should be revised to further minimize the frequency of chamfer-located failure initiation in c-ring test specimens

Investigating Reproductive Coercion and Violence Towards Women with Disabilities

4 Executive summary Background: the issue For centuries, women with disabilities (WWD) have been subject to multiple and intersecting forms of violence (Abbas, 2022). Much of the literature has focused on intimate partner violence towards WWD (i.e. Campbell et al., 2022; Meyer et al., 2022). Reproductive violence towards WWD has received less attention, despite the long history of reproductive injustice within Canadian and international contexts. This particular form of violence towards WWD is a violation of human rights and has multiple and pervasive impacts on social, emotional, and physical health (Björnsdóttir et al., 2017; Grace & Anderson, 2018; McConnell & Phelan, 2022; Powell, 2021). The Senate of Canada Committee on Human Rights (2022), recently published a report indicating the prevalence of reproductive coercion on intersecting and marginalized communities, calling to action the government and health and social service sectors. Despite the recognition of the issues and rights of WWD within their reproductive journey, there is scant literature in the field. Objectives The objective of this project was to conduct a scoping review investigating and synthesizing the research literature related to reproductive violence against WWD within Canadian and international contexts. Results Following a rigorous research strategy, 48 publications were included in this scoping review. Although heterogeneous, this review provides an overview of the meaning and types of reproductive violence towards WWD. The results of this scoping review are centered on legal analysis and most of the publications are related to women with intellectual disabilities. Within the legal disciplines, questions related to the ‘best interests’ of WWD and informed consent were widely examined in the selected international publications. Key messages • Stereotypes within society, such as WWD being identified as asexual, dependent, and unfit for motherhood prevail. Within the legal studies, it was evidenced that these ideologies underpinned many decisions that were taken in court applications for sterilization. • WWD lack access to information and services related to sexual and reproductive health and rights. 5 • WWD are often coerced into long-term and/or irreversible contraception methods. • Managing menstrual issues is one of the main arguments in the application of sterilization towards WWD. The other important issues are related to protecting against sexual abuse and preventing unexpected pregnancies. • There is a gap in research-knowledge related to sterilization that occurs outside institutions. • There is a need for more empirical studies related to reproductive violence including the exploration of the experiences of WWD. Methods (search methods, selection criteria, data collection and analysis) In this project, we conducted a scoping review, following the six steps recommended by Arksey and O’Malley (2005) in their framework for conducting scoping reviews (updated by Levac et al., 2010). The inclusion and exclusion criteria were related to gender (had to be oriented towards women), disabilities (had to be disability focused) and reproductive violence (had to refer specifically to reproductive violence). We excluded all publications related to HIV/AIDS and mental health issues. Another important exclusion criterion was related to the accessibility of the publication

Costs and effects of screening and treating low risk women with a singleton pregnancy for asymptomatic bacteriuria, the ASB study

Abstract

Background

The prevalence of asymptomatic bacteriuria (ASB) in pregnancy is 2-10% and is associated with both maternal and neonatal adverse outcomes as pyelonephritis and preterm delivery. Antibiotic treatment is reported to decrease these adverse outcomes although the existing evidence is of poor quality.

Methods/Design

We plan a combined screen and treat study in women with a singleton pregnancy. We will screen women between 16 and 22 weeks of gestation for ASB using the urine dipslide technique. The dipslide is considered positive when colony concentration ≥105 colony forming units (CFU)/mL of a single microorganism or two different colonies but one ≥105 CFU/mL is found, or when Group B Streptococcus bacteriuria is found in any colony concentration. Women with a positive dipslide will be randomly allocated to receive nitrofurantoin or placebo 100 mg twice a day for 5 consecutive days (double blind). Primary outcomes of this trial are maternal pyelonephritis and/or preterm delivery before 34 weeks. Secondary outcomes are neonatal and maternal morbidity, neonatal weight, time to delivery, preterm delivery rate before 32 and 37 weeks, days of admission in neonatal intensive care unit, maternal admission days and costs.

Discussion

This trial will provide evidence for the benefit and cost-effectiveness of dipslide screening for ASB among low risk women at 16–22 weeks of pregnancy and subsequent nitrofurantoin treatment.

Trial registration

Dutch trial registry: NTR-3068</p

Guidelines on uncomplicated urinary tract infections are difficult to follow: perceived barriers and suggested interventions

Contains fulltext : 88451.pdf (publisher's version ) (Open Access)BACKGROUND: Urinary tract infections (UTI) are among the most common health problems seen in general practice. Evidence-based guidelines on UTI are available, but adherence to these guidelines varies widely among practitioners for reasons not well understood. The aim of this study was to identify the barriers to the implementation of a guideline on UTI perceived by Dutch general practitioners (GPs) and to explore interventions to overcome these barriers. METHODS: A focus group study, including 13 GPs working in general practices in the Netherlands, was conducted. Key recommendations on diagnosis and treatment of uncomplicated UTI were selected from the guideline. Barriers to guideline adherence and possible interventions to address these barriers were discussed. The focus group session was audio-taped and transcribed verbatim. Barriers were classified according to an existing framework. RESULTS: Lack of agreement with the recommendations, unavailable and inconvenient materials (i.e. dipslides), and organisational constraints were perceived as barriers for the diagnostic recommendations. Barriers to implementing the treatment recommendations were lack of applicability and organisational constraints related to the availability of drugs in pharmacies. Suggested interventions were to provide small group education to GPs and practice staff members, to improve organisation and coordination of care in out of hour services, to improve the availability of preferred dosages of drugs, and to pilot-test guidelines regionally. CONCLUSIONS: Despite sufficient knowledge of the recommendations on UTI, attitudinal and external barriers made it difficult to follow them in practice. The care concerning UTI could be optimized if these barriers are adequately addressed in implementation strategies. The feasibility and success of these strategies could be improved by involving the target group of the guideline in selecting useful interventions to address the barriers to implementation
