    Family support for stroke: one year follow up of a randomised controlled trial

    Background: There is evidence that family support can benefit carers of stroke patients, but not the patients themselves. Objective: To extend the follow up of a single blind randomised controlled trial of family support for stroke patients and carers to one year to ascertain whether there were any late effects of the intervention. Methods: The study was a randomised controlled trial. Patients admitted to hospital with acute stroke who had a close carer were assigned to receive family support or normal care. Families were visited at home by a researcher 12 months after the stroke, and a series of questionnaires was administered to patient and carer. Results: The benefits to carers mostly persisted, though they were no longer statistically significant because some patients were lost to follow up. There was no evidence of any effects on patients. Conclusion: Family support is effective for carers, but different approaches need to be considered to alleviate the psychosocial problems of stroke patients. Abbreviations: FSO, family support organiser; SF-36, short form 36 item health assessment questionnaire Keywords: caregiver; family support; stroke In recognition of the impact that stroke has on carers as well as patients,1 services such as Stroke Association family support have been developed in the United Kingdom which provide information, emotional support, and liaison with other services. The service maintains contact through a combination of home and hospital visits and telephone calls. In the Oxford family support trial, we found that this service was associated with significantly improved quality of life of carers at follow up six months after the stroke, but had no effects on patients.2 Other randomised controlled trials of the service in other areas have also found no evidence of benefit to patients with follow up varying from four to nine months after recruitment.3,4 The lack of benefit to patients may be attributable to the short duration of follow up in these trials. The service usually maintains contact with a family for a year, and some patients spend a significant proportion of the first six months in hospital, during which time family support might be anticipated to have less impact.     Philip II of Macedon: a consideration of books VII – IX of Justin’s epitome of Pompeius Trogus

    The aim of this dissertation is two-fold: firstly to examine the career and character of Philip II of Macedon as portrayed in Books VII - IX of Justin's epitome of the Historiae Philippicae of Pompeius Trogus, and to consider to what extent Justin-Trogus (a composite name for the author of the views in the text of Justin) furnishes accurate historical fact, and to what extent he paints a one-sided interpretation of the events, and secondly to identify as far as possible Justin's principles of selection and compression as evidenced in Books VII - IX. Apart from this account of Justin-Trogus, there is only one other continuous account of the reign of Philip II, namely that found in Diodorus Siculus XVI. A comparison between Justin-Trogus’ material and that of Diodorus, together with evidence from other ancient sources and also modern scholars, has provided a large quantity of historical matter which has been used to compile a historical commentary. From ah examination of some aspects covered by this commentary the following conclusions have been reached: The factual information in Books VII - IX seems to be as reliable as that given by Diodorus, but there is in Justin-Trogus' account some considerable rhetorical padding which must be treated with extreme caution by modem researchers of the reign of Philip II. Justin's principles of selection seem to have been dominated by an Interest in the more anecdotal aspects of the Macedonian monarchy, the loss of Greek freedom and a lack of Interest in military matters. His methods of compression are closely linked to the latter, and can be identified to some extent by examining the links between different topics

    Thermodynamic modelling of synthetic communities predicts minimum free energy requirements for sulfate reduction and methanogenesis

    Microbial communities are complex dynamical systems harbouring many species interacting together to implement higher-level functions. Among these higher-level functions, conversion of organic matter into simpler building blocks by microbial communities underpins biogeochemical cycles and animal and plant nutrition, and is exploited in biotechnology. A prerequisite to predicting the dynamics and stability of community-mediated metabolic conversions is the development and calibration of appropriate mathematical models. Here, we present a generic, extendable thermodynamic model for community dynamics and calibrate a key parameter of this thermodynamic model, the minimum energy requirement associated with growth-supporting metabolic pathways, using experimental population dynamics data from synthetic communities composed of a sulfate reducer and two methanogens. Our findings show that accounting for thermodynamics is necessary in capturing the experimental population dynamics of these synthetic communities that feature relevant species using low energy growth pathways. Furthermore, they provide the first estimates for minimum energy requirements of methanogenesis (in the range of −30 kJ mol−1) and elaborate on previous estimates of lactate fermentation by sulfate reducers (in the range of −30 to −17 kJ mol−1 depending on the culture conditions). The open-source nature of the developed model and demonstration of its use for estimating a key thermodynamic parameter should facilitate further thermodynamic modelling of microbial communities

    On three-dimensional reconstruction of optically thin solar emission sources

    Calculations are given for constructing the three dimensional distribution of optically thin EUV emission sources associated with solar active regions, from two dimensional observations (projections) recorded by the spectroheliograph on the OSO 7 satellite. The relation of the method to other image reconstruction methods is briefly discussed as well as the special requirements imposed in the solar case such as a knowledge of the true solar rotation function. A useful correlation criterion for establishing the physical validity of solutions is given

    Maternal Expression Relaxes Constraint on Innovation of the Anterior Determinant, bicoid

    The origin of evolutionary novelty is believed to involve both positive selection and relaxed developmental constraint. In flies, the redesign of anterior patterning during embryogenesis is a major developmental innovation and the rapidly evolving Hox gene, bicoid (bcd), plays a critical role. We report evidence for relaxation of selective constraint acting on bicoid as a result of its maternal pattern of gene expression. Evolutionary theory predicts 2-fold greater sequence diversity for maternal effect genes than for zygotically expressed genes, because natural selection is only half as effective acting on autosomal genes expressed in one sex as it is on genes expressed in both sexes. We sample an individual from ten populations of Drosophila melanogaster and nine populations of D. simulans for polymorphism in the tandem gene duplicates bcd, which is maternally expressed, and zerknĂŒllt (zen), which is zygotically expressed. In both species, we find the ratio of bcd to zen nucleotide diversity to be two or more in the coding regions but one in the noncoding regions, providing the first quantitative support for the theoretical prediction of relaxed selective constraint on maternal-effect genes resulting from sex-limited expression. Our results suggest that the accelerated rate of evolution observed for bcd is owing, at least partly, to variation generated by relaxed selective constraint

    Searching for links between magnetic fields and stellar evolution. I. A survey of magnetic fields in open cluster A- and B-type stars with FORS1

    About 5% of upper main sequence stars are permeated by a strong magnetic field, the origin of which is still matter of debate. With this work we provide observational material to study how magnetic fields change with the evolution of stars on the main sequence, and to constrain theory explaining the presence of magnetic fields in A and B-type stars. Using FORS1 in spectropolarimetric mode at the ESO VLT, we have carried out a survey of magnetic fields in early-type stars belonging to open clusters and associations of various ages. We have measured the magnetic field of 235 early-type stars with a typical uncertainty of about 100 G. In our sample, 97 stars are Ap or Bp stars. For these targets, the median error bar of our field measurements was about 80 G. A field has been detected in about 41 of these stars, 37 of which were not previously known as magnetic stars. For the 138 normal A and B-type stars, the median error bar was 136 G, and no field was detected in any of them.Comment: Accepted by A&A; 15 pages (article)+15 pages (tables), 8 figure

    A search for strong, ordered magnetic fields in Herbig Ae/Be stars

    The origin of magnetic fields in intermediate-mass and high-mass stars is fundamentally a mystery. Clues toward solving this basic astrophysical problem can likely be found at the pre-main sequence (PMS) evolutionary stage. With this work, we perform the largest and most sensitive search for magnetic fields in pre-main sequence Herbig Ae/Be (HAeBe) stars. Sixty-eight observations of 50 HAeBe stars have been obtained in circularly polarised light using the FORS1 spectropolarimeter at the ESO VLT. An analysis of both Balmer and metallic lines reveals the possible presence of weak longitudinal magnetic fields in photospheric lines of two HAeBe stars, HD 101412 and BF Ori. The intensity of the longitudinal fields detected in HD 101412 and BF Ori suggest that they correspond to globally-ordered magnetic fields with surface intensities of order 1 kG. Monte Carlo simulations of the longitudinal field measurements of the undetected stars allow us to place an upper limits of about 300 G on the general presence of aligned magnetic dipole magnetic fields, and of about 500 G on perpendicular dipole fields. We find that the observed bulk incidence of magnetic HAeBe stars in our sample is 8-12%, in good agreement with that of magnetic main sequence stars of similar masses. We also find that the rms longitudinal field intensity of magnetically-detected HAeBe stars is similar to that of Ap stars and consistent with magnetic flux conservation during stellar evolution. These results are all in agreement with the hypothesis that the magnetic fields of main sequence Ap/Bp stars are fossils, which already exist within the stars at the pre-main sequence stage. Finally, we explore the ability of our new magnetic data to constrain magnetospheric accretion in Herbig Ae/Be stars.Comment: Accepted by Monthly Notices of the Royal Astronomical Society, 2007 January 11.
