    A health in all policies approach to promote active, healthy lifestyle in Israel

    In December 2011, Israel launched the National Program to Promote Active, Healthy Lifestyle, an inter-ministerial, intersectoral effort to address obesity and its contribution to the country’s burden of chronic disease. This paper explores the National Program according to the “Health in All Policies” (HiAP) strategy for health governance, designed to engage social determinants of health and curb health challenges at the causal level. Our objective is twofold: to identify where Israel’s National Program both echoes and falls short of Health in All Policies, and to assess how the National Program can be utilized to enrich the Health in All Policies research-base. We review Health in All Policies’ evolution, why it developed and how it is diverges from other approaches to intersectoriality in health. We describe why obesity and related chronic diseases necessitate an intersectoral response, cite obstacles and gaps to implementation and list examples of HiAP-type initiatives from around the world. We then analyze Israel’s National Program as it relates to Health in All Policies, and propose directions through which the initiative may constitute a useful case study. We contend that joint planning, implementation and to a limited extent, budgeting, between the Ministries of Health, Education and Culture and Sport reflect an HiAP-approach, as does integrating health into the policymaking of other ministries. To further incorporate health in all Israeli policies, we suggest leveraging the Health Ministry’s presence on governmental and non-governmental committees in areas like building, land-use and urban planning, institutional food policy and environmental health, and focusing on knowledge translation according to the policy needs, strengths and limitations of other sectors. Finally, we suggest studying the National Program’s financing, decision-making and evaluation mechanisms in order to complement existing research on the implementation of Health in All Policies and intersectoral action for health

    Paricalcitol reduces oxidative stress and inflammation in hemodialysis patients

    Background: Treatment with selective vitamin D receptor activators such as paricalcitol have been shown to exert an anti-inflammatory effect in patients on hemodialysis, in addition to their action on mineral metabolism and independently of parathyroid hormone (PTH) levels. The objective of this study was to evaluate the additional antioxidant capacity of paricalcitol in a clinical setting. Methods: The study included 19 patients with renal disease on hemodialysis, of whom peripheral blood was obtained for analysis at baseline and three months after starting intravenous paricalcitol treatment. The following oxidizing and inflammatory markers were quantified: malondialdehyde (MDA), nitrites and carbonyl groups, indoleamine 2,3-dioxygenase (IDO), tumor necrosis factor alfa (TNF-α), interleukin-6 (IL-6), interleukin-18 (IL-18) and C-reactive protein (CRP). Of the antioxidants and anti-inflammatory markers, superoxide dismutase (SOD), catalase, reduced glutathione (GSH), thioredoxin, and interleukin-10 (IL-10) levels were obtained. Results: Baseline levels of oxidation markers MDA, nitric oxide and protein carbonyl groups significantly decreased after three months on paricalcitol treatment, while levels of GSH, thioredoxin, catalase and SOD activity significantly increased. After paricalcitol treatment, levels of the inflammatory markers CRP, TNF-α, IL-6 and IL-18 were significantly reduced in serum and the level of anti-inflammatory cytokine IL-10 was increased.     Production of bacterial cellulose nanocrystals via enzymatic hydrolysis and evaluation of their coating on alginate particles formed by ionotropic gelation

    This study aimed to obtain the bacterial cellulose nanocrystals (CNC) by enzymatic hydrolysis and verify the CNC application as coating material in alginate particles. Therefore, the production of CNC was carried out through two enzymatic hydrolysis methods involving a time period of 48 and 72 h. 0.35 mg of dry mass of cellulose was produced approximately 1.6 × 1011 CNC/mL. The CNC obtained by enzymatic hydrolysis at 72 h (Method II) was applied to cover the alginate particles, obtained by ionotropic gelation. The CNC Zeta potential value was about +15 mV and for alginate particles -26.46 ± 1.48 mV. These results confirmed the application of CNC as coating material for alginate particles.     Pathogenic <em>PDE12 </em>variants impair mitochondrial RNA processing causing neonatal mitochondrial disease

    \ua9 The Author(s) 2024.Pathogenic variants in either the mitochondrial or nuclear genomes are associated with a diverse group of human disorders characterized by impaired mitochondrial function. Within this group, an increasing number of families have been identified, where Mendelian genetic disorders implicate defective mitochondrial RNA biology. The PDE12 gene encodes the poly(A)-specific exoribonuclease, involved in the quality control of mitochondrial non-coding RNAs. Here, we report that disease-causing PDE12 variants in three unrelated families are associated with mitochondrial respiratory chain deficiencies and wide-ranging clinical presentations in utero and within the neonatal period, with muscle and brain involvement leading to marked cytochrome c oxidase (COX) deficiency in muscle and severe lactic acidosis. Whole exome sequencing of affected probands revealed novel, segregating bi-allelic missense PDE12 variants affecting conserved residues. Patient-derived primary fibroblasts demonstrate diminished steady-state levels of PDE12 protein, whilst mitochondrial poly(A)-tail RNA sequencing (MPAT-Seq) revealed an accumulation of spuriously polyadenylated mitochondrial RNA, consistent with perturbed function of PDE12 protein. Our data suggest that PDE12 regulates mitochondrial RNA processing and its loss results in neurological and muscular phenotypes

    Characterization of SARS-CoV-2 variants associated with reinfection cases in Botucatu, Brazil: a comparative genomic study

    Abstract The global spread of SARS-CoV-2 triggered the COVID-19 pandemic, resulting in a significant number of deaths. Variants such as B.1.1.7, B.1.351, P.1, and BA.1 distinguished by spike protein mutations arose due to immune pressure and lack of repair mechanisms. These changes amplified infectivity, increasing the chances of reinfection. We retrospectively included patients who have two positive tests for SARS-CoV-2 detected by real-time reverse transcription polymerase chain reaction with an interval of 90 days, or more, between 01 March 2020 and 30 April 2022. SARS-CoV-2 whole genome sequencing was performed for both infections, and the variants were identified using the Phylogenetic Assignment of Named Global Outbreak Lineages (Pangolin) database. This study identified a total of 254 cases of reinfections, 50.8% of the cases caused by the P.1 and BA.1 variants. Compared to the state scenario, a lower detection of the B.1.617.2 variant was identified, highlighting the importance of mass vaccination in the city of Botucatu, Brazil. However, most second infections (98.8%) were identified during the BA.1 wave, showing the effect of viral adaptative mechanisms on infection dynamics even in a vaccinated population. Taken together, our results emphasize the importance of constant monitoring and the maintenance of COVID-19 prevention and control strategies

    Outbreak of acute gastroenteritis in an air force base in Western Greece

    BACKGROUND: On the 20(th )September 2005, soldiers and staff at the Air Force base in Western Greece experienced an outbreak of acute gastroenteritis. The purpose of this study was to identify the agent and the source of the outbreak in order to develop control measures and to avoid similar outbreaks in the future. METHODS: A case-control analytical approach was employed with 100 randomly selected cases and 66 controls. Patients completed standardized questionnaires, odds ratios were calculated and statistical significance was determined using χ(2 )test. In addition, to identify the source of the infection, we performed bacteriological examination of food samples (included raw beef, cooked minced meat, grated cheese and grated cheese in sealed package) collected from the cuisine of the military unit. RESULTS: More than 600 out of the 1,050 individuals who ate lunch that day, became ill. The overall attack rate, as the military doctor of the unit estimated it, was at least 60%. The overall odds ratio of gastroenteritis among those who had lunch was 370 (95% CI: 48–7700) as compared to those who didn't eat lunch. Among the symptoms the most prominent were watery diarrhoea (96%) and abdominal pain (73%). The mean incubation period was 9 h and the median duration of the symptoms was 21 h. In the bacteriological examination, Staphylococcus aureus was detected in a sample of raw beef (2,000 cfu per g) and in two samples of grated cheese; leftover cheese from lunch (7,800 cfu per g) and an unopened package purchased from the market (3,000 cfu per g). CONCLUSION: The findings of this study suggest that the aetiological agent of this outbreak was S. aureus. The food vehicle was the grated cheese, which was mixed with the beef and served for lunch in the military unit. This outbreak highlights the capacity of enterotoxin-producing bacteria to cause short term, moderately-severe illness in a young and healthy population. It underscores the need for proper food handling practices and reinforces the public health importance of timely notification of such outbreaks
