    Effectiveness of a home-based pulmonary rehabilitation programme in pulmonary function and health related quality of life for patients with pulmonary tuberculosis: a pilot study

    Background: Patients with Pulmonary Tuberculosis (PTB) often develop impairment in pulmonary function due to anatomical changes secondary to the illness. Physiotherapy in the form of pulmonary rehabilitation has been advocated.Objective: The aim of the study was to determine whether adherence to a six-week home-based pulmonary rehabilitation programme (PRP) improved the baseline measurements of lung function, exercise tolerance and health-related quality of life (HRQoL) in patients receiving out-patient treatment for PTB.Method: A single blinded randomized control study design was used to assess the effects of a six-week home- based PRP in patients receiving treatment for PTB at a local clinic in Khayelitsha, Western Cape. We evaluated lung function by spirometry (MINATO AUTOSPIRO-model no. AZ-505), exercise tolerance using the 6-min-walk test (6MWT), the Borg exercise exertion scale and HRQoL using the EQ-5 D questionnaire in an intervention group (n=34) and a control group (n=33). The trend of the effects of the PRP on lung function was towards increases, but there was no statistical difference between the intervention and control groups at the end of the sixth week in the values of FVC (p=0.2; 95% CI -0.9 to 0.51) as well as FEV1 (p=0.1; 95% CI -0.07 to 0.51). Similar trend was observed for exercise tolerance, and there was no significant difference in HRQoL (p=0.789).Conclusion: The outcome of the study provides motivation for further consideration and implementation of a pulmonary rehabilitation programme for patients with PTB.Keywords: Pulmonary rehabilitation, pulmonary tuberculosi

    Non-destructive Three-dimensional Imaging of Artificially Degraded CdS Paints by Pump-probe Microscopy

    Cadmium sulfide (CdS) pigments have degraded in several well-known paintings, but the mechanisms of degradation have yet to be fully understood. Traditional non-destructive analysis techniques primarily focus on macroscopic degradation, whereas microscopic information is typically obtained with invasive techniques that require sample removal. Here, we demonstrate the use of pump-probe microscopy to nondestructively visualize the three-dimensional structure and degradation progress of CdS pigments in oil paints. CdS pigments, reproduced following historical synthesis methods, were artificially aged by exposure to high relative humidity (RH) and ultraviolet (UV) light. Pump-probe microscopy was applied to track the degradation progress in single grains, and volumetric imaging revealed early CdS degradation of small particles and on the surface of large particles. This indicates that the particle dimension influences the extent and evolution of degradation of historical CdS. In addition, the pump-probe signal decrease in degraded CdS is observable before visible changes to the eye, demonstrating that pump-probe microscopy is a promising tool to detect early-stage degradation in artworks.     Memory-assisted Statistically-ranked RF Beam Training Algorithm for Sparse MIMO

    This paper presents a novel radio frequency (RF) beam training algorithm for sparse multiple input multiple output (MIMO) channels using unitary RF beamforming codebooks at transmitter (Tx) and receiver (Rx). The algorithm leverages statistical knowledge from past beam data for expedited beam search with statistically-minimal training overheads. Beams are tested in the order of their ranks based on their probabilities for providing a communication link. For low beam entropy scenarios, statistically-ranked beam search performs excellent in reducing the average number of beam tests per Tx-Rx beam pair identification for a communication link. For high beam entropy cases, a hybrid algorithm involving both memory-assisted statistically-ranked (MarS) beam search and multi-level (ML) beam search is also proposed.     Thinking in action: Need for cognition predicts self-control together with action orientation

    Need for Cognition describes relatively stable interindividual differences in cognitive motivation. Previous research has shown relations of Need for Cognition to Self-Control–a capacity that can be broadly defined as resistance to temptation–yet, the processes underlying this relation remain unclear. One explanation for the prediction of Self-Control by Need for Cognition can be an increased motivation to invest cognitive effort with higher levels of Need for Cognition. Another possible link could be that individual differences in the implementation of Self-Control intentions may play a moderating or mediating role for the predictive value of Need for Cognition. Such individual differences in the self-motivated initiation and maintenance of intentions are described by dispositional Action Orientation. Therefore, in the present study, Action Orientation was examined with regard to its possible role in explaining the relation of Need for Cognition to Self-Control. In a sample of 1209 young adults, Self-Control was assessed with two different self-report instruments and moderation and mediation models of the relationship between Need for Cognition, Action Orientation, and Self-Control were tested. While there was no evidence for a moderating role of Action Orientation in explaining the relation of Need for Cognition and Self-Control, Action Orientation was found to partly mediate this relation with a remaining direct effect of Need for Cognition on Self-Control. These results add to the conceptual understanding of Need for Cognition and demonstrate the relevance of trait variables to predict Self-Control

    Impact of Variations in the Nursing Care Supply-Demand Ratio on Postoperative Outcomes and Costs.

    Improving surgical outcomes is a priority during the last decades because of the rising economic health care burden. The adoption of enhanced recovery programs has been proven to be part of the solution. In this context, the impact of variations in the nursing care supply-demand ratio on postoperative complications and its economic consequences is still not well elucidated. Because patients require different amounts of care, the present study focused on the more accurate relationship between demand and supply of nursing care rather than the nurse-to-patient ratio. Through a 3-year period, 838 patients undergoing elective and emergent colorectal and pancreatic surgery within the institutional enhanced recovery after surgery (ERAS) protocol were retrospectively investigated. Nursing demand and supply estimations were calculated using a validated program called the Projet de Recherche en Nursing (PRN), which assigns points to each patient according to the nursing care they need (estimated PRN) and the actual care they received (real PRN), respectively. The real/estimated PRN ratio was used to create 2 patient groups: one with a PRN ratio higher than the mean (PRN+) and a second with a PRN ratio below the mean (PRN-). These 2 groups were compared regarding their postoperative complication rates and cost-revenue characteristics. The mean PRN ratio was 0.81. A total of 710 patients (84.7%) had a PRN+ ratio, and 128 (15.3%) had a PRN- ratio. Multivariable analysis focusing on overall complications, severe complications, and prolonged length of stay revealed no significant impact of the PRN ratio for all outcomes (P > 0.2). The group PRN- had a mean margin per patient of U.S. dollars 1426 (95% confidence interval, 3 to 2903) compared with a margin of U.S. dollars 676 (95% confidence interval, -2213 to 3550) in the PRN+ group (P = 0.633). A PRN ratio of 0.8 may be sufficient for patients treated following enhanced recovery after surgery guidelines, pending the adoption of an accurate nursing planning system. This may contribute to better allocation of nursing resources and optimization of expenses on the long run

    On the importance of hydrodynamic interactions in polyelectrolyte electrophoresis

    The effect of hydrodynamic interactions on the free-solution electrophoresis of polyelectrolytes is investigated with coarse-grained molecular dynamics simulations. By comparing the results to simulations with switched-off hydrodynamic interactions, we demonstrate their importance in modelling the experimentally observed behaviour. In order to quantify the hydrodynamic interactions between the polyelectrolyte and the solution, we present a novel way to estimate its effective charge. We obtain an effective friction that is different from the hydrodynamic friction obtained from diffusion measurements. This effective friction is used to explain the constant electrophoretic mobility for longer chains. To further emphasize the importance of hydrodynamic interactions, we apply the model to end-labeled free-solution electrophoresis.Comment: 15 pages, 7 figures; accepted for publication in J.     51-57The present assay was performed in order to evaluate the biological effects of polar and non-polar metabolites form the aerial parts of (leaves and inflorescences) Chenopodium album L. (white quinoa) on Oryzaephilus surinamensis L (Coleoptera: Silvanidae) feeding. Previously dried and ground plant material was submitted to sequential continuous extraction with petroleum ether and then with methanol; the remaining plant material was finally extracted in batch with water, giving rise to the corresponding extracts. 2 g of basic diet were impregnated with 66 mg/ml solutions of ether, methanol or aqueous extracts. Petroleum ether, methanol and distilled water were used respectively as control treatments. Six treatments were performed in a completely randomized design with four repetitions, analyzing insect behavior and the evolution of the biological cycle of the resulting larvae. The number of living adults and larvae was determined every seven days. All treatments were evaluated by means of ANOVA, and differences between mean values using Tukey's test. (p less than 0.05).     Focal cerebral ischemia in the TNFalpha-transgenic rat

    <p>Abstract</p> <p>Background</p> <p>To determine if chronic elevation of the inflammatory cytokine, tumor necrosis factor-α (TNFα), will affect infarct volume or cortical perfusion after focal cerebral ischemia.</p> <p>Methods</p> <p>Transgenic (TNFα-Tg) rats overexpressing the murine TNFα gene in brain were prepared by injection of mouse DNA into rat oocytes. Brain levels of TNFα mRNA and protein were measured and compared between TNFα-Tg and non-transgenic (non-Tg) littermates. Mean infarct volume was calculated 24 hours or 7 days after one hour of reversible middle cerebral artery occlusion (MCAO). Cortical perfusion was monitored by laser-Doppler flowmetry (LDF) during MCAO. Cortical vascular density was quantified by stereology. Post-ischemic cell death was assessed by immunohistochemistry and regional measurement of caspase-3 activity or DNA fragmentation. Unpaired <it>t </it>tests or analysis of variance with post hoc tests were used for comparison of group means.</p> <p>Results</p> <p>In TNFα-Tg rat brain, the aggregate mouse and rat TNFα mRNA level was fourfold higher than in non-Tg littermates and the corresponding TNFα protein level was increased fivefold (p ≤ 0.01). Infarct volume was greater in TNFα-Tg rats than in non-Tg controls at 24 hours (p ≤ 0.05) and 7 days (p ≤ 0.01). Within the first 10 minutes of MCAO, cortical perfusion measured by LDF was reduced in TNFα-Tg rats (p ≤ 0.05). However, regional vascular density was equivalent between TNFα-Tg and non-Tg animals (p = NS). Neural cellular apoptosis was increased in transgenic animals as shown by elevated caspase-3 activity (p ≤ 0.05) and DNA fragmentation (p ≤ 0.001) at 24 hours.</p> <p>Conclusion</p> <p>Chronic elevation of TNFα protein in brain increases susceptibility to ischemic injury but has no effect on vascular density. TNFα-Tg animals are more susceptible to apoptotic cell death after MCAO than are non-Tg animals. We conclude that the TNFα-Tg rat is a valuable new tool for the study of cytokine-mediated ischemic brain injury.</p

    Improvement of quality of reporting in randomised controlled trials to prevent hypotension after spinal anaesthesia for caesarean section

    Hypotension is a frequent complication of spinal anaesthesia for caesarean section and can threaten the well-being of the unborn child. Numerous randomised controlled trials (RCTs) dealt with measures to prevent hypotension. The aim of this study was to determine the reporting quality of RCTs using the Consolidated Standards of Reporting Trials (CONSORT) statement since low quality can lend false credibility to a study and overestimate the effect of an intervention. We performed a systematic literature search in PubMed to identify relevant RCTs in a pre-CONSORT period (1990–1994) and a post-CONSORT period (2004–2008). A comparative evaluation was done between the two periods, and the trials were assessed for compliance with each of the 22 CONSORT items. A total of 37 RCTs was identified. The CONSORT score increased significantly (p < 0.05) from 66.7% (±12.5%) in the pre-CONSORT period to 87.4% (±6.9%) in the post-CONSORT period. A statistically significant improvement was found for eight items, including randomization, blinding and intention-to-treat analysis. The CONSORT score in the post-CONSORT era was fairly good, also in comparison to other medical fields. In the post-CONSORT era, reporting of important items improved, in particular in the domains that are crucial to avoid bias and to improve internal validity. Use of CONSORT should be encouraged in order to keep or even improve the reporting quality
