    Oral malodor in Special Care Patients: current knowledge

    Epidemiological studies report that about 50% of the population may have oral malodor with a strong social and psychological impact in their daily life. When intra-oral causes are excluded, referral to an appropriate medical specialist is paramount for management and treatment of extra-oral causes. The intra-oral causes of halitosis are highly common, and the dentist is the central clinician to diagnose and treat them. Pseudohalitosis or halitophobia may occur and an early identification of these conditions by the dentist is important in order to avoid unnecessary dental treatments for patients who need psychological or psychiatric therapy. The organoleptic technique is still considered the most reliable examination method to diagnose genuine halitosis. Special needs patients are more prone than others to have oral malodor because of concurrent systemic or metabolic diseases, and medications. The present report reviews halitosis, its implications, and the management in special care dentistry

    Eigenvalue Distributions for a Class of Covariance Matrices with Applications to Bienenstock-Cooper-Munro Neurons Under Noisy Conditions

    We analyze the effects of noise correlations in the input to, or among, BCM neurons using the Wigner semicircular law to construct random, positive-definite symmetric correlation matrices and compute their eigenvalue distributions. In the finite dimensional case, we compare our analytic results with numerical simulations and show the effects of correlations on the lifetimes of synaptic strengths in various visual environments. These correlations can be due either to correlations in the noise from the input LGN neurons, or correlations in the variability of lateral connections in a network of neurons. In particular, we find that for fixed dimensionality, a large noise variance can give rise to long lifetimes of synaptic strengths.     Analysis of the intraspinal calcium dynamics and its implications on the plasticity of spiking neurons

    The influx of calcium ions into the dendritic spines through the N-metyl-D-aspartate (NMDA) channels is believed to be the primary trigger for various forms of synaptic plasticity. In this paper, the authors calculate analytically the mean values of the calcium transients elicited by a spiking neuron undergoing a simple model of ionic currents and back-propagating action potentials. The relative variability of these transients, due to the stochastic nature of synaptic transmission, is further considered using a simple Markov model of NMDA receptos. One finds that both the mean value and the variability depend on the timing between pre- and postsynaptic action-potentials. These results could have implications on the expected form of synaptic-plasticity curve and can form a basis for a unified theory of spike time-dependent, and rate based plasticity.Comment: 14 pages, 10 figures.     Procjena izloženosti ljudi pesticidima u staklenicima i djelotvornosti osobne zaštitne opreme

    The exposure to pesticides in a group of workers growing vegetables in greenhouses in farms near Rimini in Italy was evaluated. The pesticides used were organophosphorus compounds, org anochlorine compounds, carbamates, pyrethroids. amide and anilide derivatives. Measurements were carried out in seven greenhouses randomly selected. The environment was free of atmospheric agents which could have influenced the pesticide concentrations in the indoor air. Two types of chemical sampling were performed; environmental and personal. The latter was done in the liquid for washing hands and by means ot pads applied directly to me worker\u27s skin and to the clothes. The aim of the determination of airborne pesticide concentrations was to evaluate mean environmental exposure to pesticides in the sprayed areas. The sampling went on from a fixed point during the entire spraying period. The aim of individual sampling was to determine the active ingredients as contaminants. The values obtained showed a risk of exposure for the greenhouse personnel working without using personal protective devices (masks, gloves and waterproof clothes), considering that pesticides could be absorbed through the skin in between the spraying intervals.Istraživana je izloženost pesticidima u skupini radnika zaposlenih u staklenicima za uzgoj povrća u blizini Riminija LI Italiji. Primjenjivani pesticidi bili su organofosforni, organoklorirani, karbamatni spojevi, piretroidi te derivati amida i anilida. Mjerenja su provedena u sedam slučajno odabranih staklenika. Unutrašnjost staklenika bila je izolirana od vanjskih utjecaja kako pesticidi u vanjskoj atmosferi ne bi utjecali na koncentracije u atmosferi staklenika. Provedene su dvije vrste sakupljanja uzoraka tijekom razdoblja raspršavanja pesticida: iz okoliša i sa ispitanika. U uzorcima zraka sakupljenim u atmosferi staklenika izmjerene su prosječne koncentracije pesticida u području raspršivanja pesticida. Također je sakupljena tekućina za ispiranje ruku radnika i pasivni upijači zagađivala s kože radnika i sa odjeće. Svrha individualnog sakupljanja bila je odrediti aktivne sastojke zagađivala. Dobiveni rezultati pokazuju rizik za izloženost radnika u staklenicima kada rade bez zaštitne opreme (maske, rukavice i vodootporna odjeća) imajući na umu da se pesticidi mogu apsorbirati kožom i u razdoblju između raspršivanja pesticida

    Intravenous Fluid Administration May Improve Post-Operative Course of Patients with Chronic Subdural Hematoma: A Retrospective Study

    Background: The treatment of chronic subdural hematoma (cSDH) is still charged of significant risk of hematoma recurrence. Patient-related predictors and the surgical procedures themselves have been addressed in many studies. In contrast, postoperative management has infrequently been subjected to detailed analysis. Moreover variable intravenous fluid administration (IFA) was not reported in literature till now in the context of cSDH treatment. Methodology/Principal Findings: A total of 45 patients with cSDH were operated in our department via two burr hole craniostomy within one calendar year. Downward drainage was routinely left in hematoma cavity for a one day. Independent variables selected for the analysis were related to various aspects of patient management, including IFA. Two dependent variables were chosen as measure of clinical course: the rate of hematoma recurrence (RHR) and neurological status at discharge from hospital expressed in points of Glasgow Outcome Scale (GOS). Univariate and multivariate regression analyses were performed. Hematoma recurrence with subsequent evacuation occurred in 7 (15%) patients. Univariate regression analysis revealed that length of IFA after surgery influenced both dependent variables: RHR (p = 0.045) and GOS (p = 0.023). Multivariate regression performed by backward elimination method confirmed that IFA is a sole independent factor influencing RHR. Post hoc dichotomous division of patients revealed that those receiving at least 2000 ml/day over 3 day period revealed lower RHR than the group with less intensive IFA. (p = 0.031)

    Unresponsive wakefulness syndrome: a new name for the vegetative state or apallic syndrome

    BACKGROUND: Some patients awaken from coma (that is, open the eyes) but remain unresponsive (that is, only showing reflex movements without response to command). This syndrome has been coined vegetative state. We here present a new name for this challenging neurological condition: unresponsive wakefulness syndrome (abbreviated UWS). DISCUSSION: Many clinicians feel uncomfortable when referring to patients as vegetative. Indeed, to most of the lay public and media vegetative state has a pejorative connotation and seems inappropriately to refer to these patients as being vegetable-like. Some political and religious groups have hence felt the need to emphasize these vulnerable patients' rights as human beings. Moreover, since its first description over 35 years ago, an increasing number of functional neuroimaging and cognitive evoked potential studies have shown that physicians should be cautious to make strong claims about awareness in some patients without behavioral responses to command. Given these concerns regarding the negative associations intrinsic to the term vegetative state as well as the diagnostic errors and their potential effect on the treatment and care for these patients (who sometimes never recover behavioral signs of consciousness but often recover to what was recently coined a minimally conscious state) we here propose to replace the name. CONCLUSION: Since after 35 years the medical community has been unsuccessful in changing the pejorative image associated with the words vegetative state, we think it would be better to change the term itself. We here offer physicians the possibility to refer to this condition as unresponsive wakefulness syndrome or UWS. As this neutral descriptive term indicates, it refers to patients showing a number of clinical signs (hence syndrome) of unresponsiveness (that is, without response to commands) in the presence of wakefulness (that is, eye opening)

    Rapamycin Response in Tumorigenic and Non-Tumorigenic Hepatic Cell Lines

    The mTOR inhibitor rapamycin has anti-tumor activity across a variety of human cancers, including hepatocellular carcinoma. However, resistance to its growth inhibitory effects is common. We hypothesized that hepatic cell lines with varying rapamycin responsiveness would show common characteristics accounting for resistance to the drug.We profiled a total of 13 cell lines for rapamycin-induced growth inhibition. The non-tumorigenic rat liver epithelial cell line WB-F344 was highly sensitive while the tumorigenic WB311 cell line, originally derived from the WB-F344 line, was highly resistant. The other 11 cell lines showed a wide range of sensitivities. Rapamycin induced inhibition of cyclin E-dependent kinase activity in some cell lines, but the ability to do so did not correlate with sensitivity. Inhibition of cyclin E-dependent kinase activity was related to incorporation of p27(Kip1) into cyclin E-containing complexes in some but not all cell lines. Similarly, sensitivity of global protein synthesis to rapamycin did not correlate with its anti-proliferative effect. However, rapamycin potently inhibited phosphorylation of two key substrates, ribosomal protein S6 and 4E-BP1, in all cases, indicating that the locus of rapamycin resistance was downstream from inhibition of mTOR Complex 1. Microarray analysis did not disclose a unifying mechanism for rapamycin resistance, although the glycolytic pathway was downregulated in all four cell lines studied.We conclude that the mechanisms of rapamycin resistance in hepatic cells involve alterations of signaling downstream from mTOR and that the mechanisms are highly heterogeneous, thus predicting that maintaining or promoting sensitivity will be highly challenging

    Resonant wave energy harvester based on dielectric elastomer generator

    Dielectric elastomer generators (DEGs) are a class of capacitive solid-state devices that employ highly stretchable dielectrics and conductors to convert mechanical energy into high-voltage direct-current electricity. Their promising performance in terms of convertible energy and power density has been mostly proven in quasi-static experimental tests with prescribed deformation. However, the assessment of their ability in harvesting energy from a dynamic oscillating source of mechanical energy is crucial to demonstrate their effectiveness in practical applications. This paper reports a first demonstration of a DEG system that is able to convert the oscillating energy carried by water waves into electricity. A DEG prototype is built using a commercial polyacrylate film (VHB 4905 by 3M) and an experimental campaign is conducted in a wave-flume facility, i.e. an artificial basin that makes it possible to generate programmed small-scale waves at different frequencies and amplitudes. In resonant conditions, the designed system demonstrates the delivery of a maximum of 0.87 W of electrical power output and 0.64 J energy generated per cycle, with corresponding densities per unit mass of dielectric elastomer of 197 W kg-1 and 145 J kg-1. Additionally, a notable maximum fraction of 18% of the input wave energy is converted into electricity. The presented results provide a promising demonstration of the operation and effectiveness of ocean wave energy converters based on elastic capacitive generators
