    Mothers’ experience of having children with diabetes

    Abstract Background: Diabetes is a major health problem, which has a wide prevalence in the world. There is no sign of its stopping, but it is increasing. Diabetes in children is three to four times more common than other childhood diseases. Diagnosis of diabetes for children causes emotional responses in parents and family members. Interventions for children with diabetes involve the family, child, and professionals including physicians, nurses, and nutritionist. Self-care is difficult without direct parents’ participation. According to studies, burden of diabetes for mothers is more than for fathers. This study aimed to explore mothers’ experience of children with diabetes. Materials and Methods: This is a qualitative content analysis. Study population was recruited through purposeful sampling. Eleven mothers who have a child with diabetes and referring to the “Glands and Metabolism Research Center” and “Al-Zahra Hospital” were selected. Participants were aged 28 to 42 years. Data gathering was done through deep interviews with participants in 2007 that was tape-recorded. Mean average of interviews was 45 minutes. Data analysis was done using conventional qualitative content analysis. Results: Participants’ experience was classified in the two main concepts including reaction at the time of diagnosis and disease consequences for mothers. Conclusion: Mothers of children with diabetes expressed some reactions at the time of diagnosis, which was mainly due to lack of information, and lack of attention to their needs at the time of diagnosis, especially it was due to the sudden diagnosis and lack of enough opportunities for mothers to accept the disease. The disease causes some consequences for mothers that affect their lives. Therefore, it is necessary to consider the needs of families of children with diabetes and to provide support and sufficient information about their child’s illness for them

    زمینه و هدف: درد و خستگی از مشکلات شایع بعد از عمل بیماران تحت جراحی پیوند عروق کرونر می باشد. هدف از این مطالعه تعیین تأثیر ماساژ درمانی بر شدت درد و خستگی بعد از عمل بیماران تحت جراحی پیوند عروق کرونر بوده است. روش بررسی: در این مطالعه کارآزمایی بالینی تصادفی سازی شده، تعداد 72 نفر بیمار تحت عمل جراحی قرارگرفته عروق کرونر در بیمارستان شهید چمران شهر اصفهان، انتخاب شدند و بطور تصادفی در دو گروه مداخله و شاهد قرار گرفتند. بیماران گروه مداخله (36 نفر)، ماساژ استروک سطحی 20 دقیقه ای به مدت 4 جلسه در 4 روز پیاپی در روزهای 3 تا 6 بعد از عمل جراحی دریافت کردند و بیماران گروه شاهد تنها مراقبت های روتین را دریافت نمودند. متغیرهای درد و خستگی قبل و بعد از مداخله با استفاده از مقیاس قیاسی دیداری ثبت شدند. یافته ها: نتایج نشان داد که اختلاف معنی داری بین شدت درد و خستگی در دو گروه مداخله و شاهد بعد از انجام مداخله وجود داشته است (0/001>P). بطوریکه کاهش میانگین شدت درد و خستگی از روز اول تا چهارم بعد از عمل در گروه مداخله بیشتر از گروه شاهد بود. نتیجه گیری: استفاده از ماساژ استروک سطحی می تواند باعث کاهش درد و خستگی بعد از عمل بیماران تحت جراحی پیوند عروق کرونر شود و با توجه به سادگی و کم هزینه بودن این روش، شاید بتواند به عنوان مکمل مناسبی برای دارودرمانی و مداخلات بعد از عمل در این بیماران مورد استفاده قرار گیرد

    A biodistribution study of Hemiscorpius lepturus scorpion venom and available polyclonal antivenom in rats

    The purpose of the present study was to investigate the biodistribution profile of the venom of Hemiscorpius lepturus, the most dangerous scorpion in Iran. Blood and tissue samples were taken at various predetermined intervals during a 400-minute period for the venom and a 360-minute period for the antivenom in rats. The radio-iodination was carried out using the chloramine-T method. The results showed that the descending order of venom uptake was skin, kidneys and intestine, respectively. The descending order of polyclonal antivenom uptake was kidneys, intestine, heart and lungs. The calculated pharmacokinetic parameters of the venom were Telimination half-life = 521.5 ± 12.6 minutes; Vd/F (apparent volume of distribution) = 14.9 ± 3.3 mL; clearance (CL/F, apparent total clearance of the drug from plasma) 0.02 ± 0.005 mL/minute and for the antivenom Telimination half-life = 113.7 ± 7.4 minutes; Vd/F = 13 ± 1.2 mL and CL/F 0.08 ± 0.01 mL/minute. The pharmacokinetics profile comparison of the venom with that of the antivenom shows that serotherapy may be more effective if administered within 2-4 hours following envenomation by H. lepturus

    Topological modes bound to dislocations in mechanical metamaterials

    Mechanical metamaterials are artificial structures with unusual properties, such as negative Poisson ratio, bistability or tunable vibrational properties, that originate in the geometry of their unit cell. At the heart of such unusual behaviour is often a soft mode: a motion that does not significantly stretch or compress the links between constituent elements. When activated by motors or external fields, soft modes become the building blocks of robots and smart materials. Here, we demonstrate the existence of topological soft modes that can be positioned at desired locations in a metamaterial while being robust against a wide range of structural deformations or changes in material parameters. These protected modes, localized at dislocations, are the mechanical analogue of topological states bound to defects in electronic systems. We create physical realizations of the topological modes in prototypes of kagome lattices built out of rigid triangular plates. We show mathematically that they originate from the interplay between two Berry phases: the Burgers vector of the dislocation and the topological polarization of the lattice.     Suprasternal innominate artery cannulation for reoperative aortic surgery: A technical note

    Suprasternal cannulation of the innominate artery in aortic reoperations may be useful in specific situations. Over a period of 3.5 years, 9 patients (6 males, average age = 49.2 ± 16.1 years) underwent suprasternal cannulation prior to resternotomy. Cannulation was performed using a side graft. All operations were successfully completed. Two patients died after surgery because of coagulopathy and multiorgan failure. There were no complications related to access or technique, and no site complications were detected during follow-up. Suprasternal cannulation of the innominate artery may play a role in selected reoperations. © 2016 The Author 2016.     Electrical tuning of elastic wave propagation in nanomechanical lattices at MHz frequencies

    Nanoelectromechanical systems (NEMS) that operate in the megahertz (MHz) regime allow energy transducibility between different physical domains. For example, they convert optical or electrical signals into mechanical motions and vice versa. This coupling of different physical quantities leads to frequency-tunable NEMS resonators via electromechanical non-linearities. NEMS platforms with single- or low-degrees of freedom have been employed to demonstrate quantum-like effects, such as mode cooling, mechanically induced transparency, Rabi oscillation, two-mode squeezing and phonon lasing. Periodic arrays of NEMS resonators with architected unit cells enable fundamental studies of lattice-based solid-state phenomena, such as bandgaps, energy transport, non-linear dynamics and localization, and topological properties, directly transferrable to on-chip devices. Here we describe one-dimensional, non-linear, nanoelectromechanical lattices (NEML) with active control of the frequency band dispersion in the radio-frequency domain (10–30 MHz). The design of our systems is inspired by NEMS-based phonon waveguides and includes the voltage-induced frequency tuning of the individual resonators. Our NEMLs consist of a periodic arrangement of mechanically coupled, free-standing nanomembranes with circular clamped boundaries. This design forms a flexural phononic crystal with a well-defined bandgap, 1.8 MHz wide. The application of a d.c. gate voltage creates voltage-dependent on-site potentials, which can significantly shift the frequency bands of the device. Additionally, a dynamic modulation of the voltage triggers non-linear effects, which induce the formation of a phononic bandgap in the acoustic branch, analogous to Peierls transition in condensed matter. The gating approach employed here makes the devices more compact than recently proposed systems, whose tunability mostly relies on materials’ compliance and mechanical non-linearities

    Right ventricular dysfunction in patients with Brugada-like electrocardiography: a two dimensional strain imaging study

    <p>Abstract</p> <p>Background</p> <p>Sodium channel blockers augment ST-segment elevation in the right precordial leads in patients undergoing Brugada-type electrocardiography (ECG). However, their effect on echocardiographic features is not known. We address this by assessing global and regional ventricular function using conventional Doppler and two- dimensional (2D) speckle tracking techniques.</p> <p>Methods</p> <p>Thirty-one patients with Brugada-type ECG were studied. A pure sodium channel blocker, pilsicainide, was used to provoke an ECG response. The percentage longitudinal systolic myocardial strain at the base of both the right ventricular (RV) free wall and the interventricular septum wall was measured using 2D speckle tracking. Left ventricular (LV) and RV myocardial performance (TEI) indices were also measured.</p> <p>Results</p> <p>The pilsicainide challenge provoked a positive ECG response in 13 patients (inducible group). In the inducible group, longitudinal strain was significantly reduced only at the RV (-27.3 ± 5.4% vs -22.1 ± 3.6%, <it>P </it>< 0.01), and both RV and LV TEI indices increased (RV: 0.19 ± 0.09 vs 0.27 ± 0.11, <it>P </it>< 0.05; LV: 0.30 ± 0.10 vs 0.45 ± 0.10, <it>P </it>< 0.01) after pilsicainide administration.</p> <p>Conclusions</p> <p>Temporal and spatial analysis using the TEI index and 2D strain imaging revealed the deterioration of global ventricular function associated with conduction disturbance and RV regional function in patients with Brugada-type ECG and coved type ST elevation due to administration of a sodium channel blocker.</p
