Methyl iodide poisoning presenting as a mimic of acute stroke: a case report
<p>Abstract</p> <p>Introduction</p> <p>Stroke mimics are usually non-vascular disease processes. These raise the possibility of a stroke and are common in clinical practice. It is necessary to distinguish these mimics in order to provide early and appropriate management, as well as reduce possible harm on our patient.</p> <p>Case presentation</p> <p>We report the case of a 50-year-old Caucasian man who developed symptoms suggestive of posterior circulation stroke after he was exposed to methyl iodide at his workplace. Results of stroke investigations of our patient were negative, and a detailed occupational history clinched the diagnosis. Acute presentation with a stroke-like picture is rare in cases of methyl iodide poisoning. We have attempted to discuss the differential diagnosis of stroke mimics through a review of literature.</p> <p>Conclusion</p> <p>Stroke mimics are difficult to diagnose in an emergency room situation and may be initially treated as stroke. This case report underlines the importance of history taking, especially occupational history, in the differential diagnosis of stroke. We also stress the need to recognize mimics at presentation in order to arrive at an early and appropriate management of patients.</p
Environmental and Climatic Determinants of Molecular Diversity and Genetic Population Structure in a Coenagrionid Damselfly
Identifying environmental factors that structure intraspecific genetic diversity
is of interest for both habitat preservation and biodiversity conservation.
Recent advances in statistical and geographical genetics make it possible to
investigate how environmental factors affect geographic organisation and
population structure of molecular genetic diversity within species. Here we
present a study on a common and wide ranging insect, the blue tailed damselfly
Ischnuraelegans, which has been the target of many
ecological and evolutionary studies. We addressed the following questions: (i)
Is the population structure affected by longitudinal or latitudinal gradients?;
(ii) Do geographic boundaries limit gene flow?; (iii) Does geographic distance
affect connectivity and is there a signature of past bottlenecks?; (iv) Is there
evidence of a recent range expansion and (vi) what is the effect of geography
and climatic factors on population structure? We found low to moderate genetic
sub-structuring between populations (mean
FST = 0.06,
Dest = 0.12), and an effect of longitude, but
not latitude, on genetic diversity. No significant effects of geographic
boundaries (e.g. water bodies) were found. FST-and
Dest-values increased with geographic distance; however, there was no
evidence for recent bottlenecks. Finally, we did not detect any molecular
signatures of range expansions or an effect of geographic suitability, although
local precipitation had a strong effect on genetic differentiation. The
population structure of this small insect has probably been shaped by ecological
factors that are correlated with longitudinal gradients, geographic distances,
and local precipitation. The relatively weak global population structure and
high degree of genetic variation within populations suggest that I.
elegans has high dispersal ability, which is consistent with this
species being an effective and early coloniser of new habitats

In contrast to birds, male mammals rarely help to raise the offspring. Of all mammals, only among rodents, carnivores, and primates, males are sometimes intensively engaged in providing infant care (Kleiman and Malcolm 1981). Male caretaking of infants has long been recognized in nonhuman primates (Itani 1959). Given that infant care behavior can have a positive effect on the infant’s development, growth, well-being, or survival, why are male mammals not more frequently involved in “building babies”? We begin the chapter defining a few relevant terms and introducing the theory and hypotheses that have historically addressed the evolution of paternal care. We then review empirical findings on male care among primate taxa, before focusing, in the final section, on our own work on paternal care in South American owl monkeys (Aotus spp.). A program of nurse algorithm-guided care for adult patients with acute minor illnesses in primary care
Background: Attention to patients with acute minor-illnesses requesting same-day consultation represents a major burden in primary care. The workload is assumed by general practitioners in many countries. A number of reports suggest that care to these patients may be provided, at in least in part, by nurses. However, there is scarce information with respect to the applicability of a program of nurse management for adult patients with acute minor-illnesses in large areas. The aim of this study is to assess the effectiveness of a program of nurse algorithm-guided care for adult patients with acute minor illnesses requesting same-day consultation in primary care in a largely populated area. Methods: A cross-sectional study of all adult patients seeking same day consultation for 16 common acute minor illnesses in a large geographical area with 284 primary care practices. Patients were included in a program of nurse case management using management algorithms. The main outcome measure was case resolution, defined as completion of the algorithm by the nurse without need of referral of the patient to the general practitioner. The secondary outcome measure was return to consultation, defined as requirement of new consultation for the same reason as the first one, in primary care within a 7-day period. Results: During a two year period (April 2009-April 2011), a total of 1,209,669 consultations were performed in the program. Case resolution was achieved by nurses in 62.5% of consultations. The remaining cases were referred to a general practitioner. Resolution rates ranged from 94.2% in patients with burns to 42% in patients with upper respiratory symptoms. None of the 16 minor illnesses had a resolution rate below 40%. Return to consultation during a 7-day period was low, only 4.6%. Conclusions: A program of algorithms-guided care is effective for nurse case management of patients requesting same day consultation for minor illnesses in primary care
Unraveling the forcings controlling the vegetation and climate of the best orbital analogues for the present interglacial in SW Europe
The suitability of MIS 11c and MIS 19c as analogues of our present interglacial and its natural evolution is still debated. Here we examine the regional expression of the Holocene and its orbital analogues over SW Iberia using a model-data comparison approach. Regional tree fraction and climate based on snapshot and transient experiments using the LOVECLIM model are evaluated against the terrestrial-marine profiles from Site U1385 documenting the regional vegetation and climatic changes. The pollen-based reconstructions show a larger forest optimum during the Holocene compared to MIS 11c and MIS 19c, putting into question their analogy in SW Europe. Pollen-based and model results indicate reduced MIS 11c forest cover compared to the Holocene primarily driven by lower winter precipitation, which is critical for Mediterranean forest development. Decreased precipitation was possibly induced by the amplified MIS 11c latitudinal insolation and temperature gradient that shifted the westerlies northwards. In contrast, the reconstructed lower forest optimum at MIS 19c is not reproduced by the simulations probably due to the lack of Eurasian ice sheets and its related feedbacks in the model. Transient experiments with time-varying insolation and CO2 reveal that the SW Iberian forest dynamics over the interglacials are mostly coupled to changes in winter precipitation mainly controlled by precession, CO2 playing a negligible role. ALMaQUEST. IV. The ALMA-MaNGA QUEnching and STar Formation (ALMaQUEST) Survey
The ALMaQUEST (ALMA-MaNGA QUEnching and STar formation) survey is a program
with spatially-resolved CO(1-0) measurements obtained with the Atacama
Large Millimeter Array (ALMA) for 46 galaxies selected from the Mapping Nearby
Galaxies at Apache Point Observatory (MaNGA) DR15 optical integral-field
spectroscopic survey. The aim of the ALMaQUEST survey is to investigate the
dependence of star formation activity on the cold molecular gas content at kpc
scales in nearby galaxies. The sample consists of galaxies spanning a wide
range in specific star formation rate (sSFR), including starburst (SB),
main-sequence (MS), and green valley (GV) galaxies. In this paper, we present
the sample selection and characteristics of the ALMA observations, and showcase
some of the key results enabled by the combination of spatially-matched stellar
populations and gas measurements. Considering the global (aperture-matched)
stellar mass, molecular gas mass, and star formation rate of the sample, we
find that the sSFR depends on both the star formation efficiency (SFE) and the
molecular gas fraction (), although the correlation with the
latter is slightly weaker. Furthermore, the dependence of sSFR on the molecular
gas content (SFE or ) is stronger than that on either the atomic
gas fraction or the molecular-to-atomic gas fraction, albeit with the small HI
sample size. On kpc scales, the variations in both SFE and
within individual galaxies can be as large as 1-2 dex thereby demonstrating
that the availability of spatially-resolved observations is essential to
understand the details of both star formation and quenching processes.STFC

The ALMaQUEST Survey. VII. Star Formation Scaling Relations of Green Valley Galaxies
We utilize the ALMA-MaNGA QUEnch and STar formation (ALMaQUEST) survey to
investigate the kpc-scale scaling relations, presented as the resolved star
forming main sequence (rSFMS: vs. ), the
resolved Schmidt-Kennicutt relation (rSK: vs. ), and the resolved molecular gas main sequence (rMGMS: vs. ), for 11478 star-forming and 1414 retired spaxels
(oversampled by a factor of ) located in 22 green valley (GV) and 12
main sequence (MS) galaxies. For a given galaxy type (MS or GV), the retired
spaxels are found to be offset from the sequences formed by the star-forming
spaxels on the rSFMS, rSK, and rMGMS planes, toward lower absolute values of
sSFR, SFE, and by 1.1, 0.6, and 0.5 dex. The scaling
relations for GV galaxies are found to be distinct from that of the MS
galaxies, even if the analyses are restricted to the star-forming spaxels only.
It is found that for star-forming spaxels, sSFR, SFE, and in GV
galaxies are reduced by 0.36, 0.14, and 0.21 dex, respectively, compared
to those in MS galaxies. Therefore, the suppressed sSFR/SFE/ in
GV galaxies are associated with not only an increased proportion of retired
regions in GV galaxies but also a depletion of these quantities in star-forming
regions. Finally, the reduction of SFE and in GV galaxies
relative to MS galaxies is seen in both bulge and disk regions (albeit with
larger uncertainties), suggesting that statistically, quenching in the GV
population may persist from the inner to the outer regions
West Nile virus: characterization and diagnostic applications of monoclonal antibodies
<p>Abstract</p> <p>Background</p> <p>Diagnosis of West Nile virus (WNV) infections is often difficult due to the extensive antigenic cross-reactivity among flaviviruses, especially in geographic regions where two or more of these viruses are present causing sequential infections. The purpose of this study was to characterize a panel of monoclonal antibodies (MAbs) produced against WNV to verify their applicability in WNV diagnosis and in mapping epitope targets of neutralizing MAbs.</p> <p>Methods</p> <p>Six MAbs were produced and characterized by isotyping, virus-neutralization, western blotting and MAb-epitope competition. The MAb reactivity against various WNVs belonging to lineage 1 and 2 and other related flaviviruses was also evaluated. The molecular basis of epitopes recognized by neutralizing MAbs was defined through the selection and sequencing of MAb escape mutants. Competitive binding assays between MAbs and experimental equine and chicken sera were designed to identify specific MAb reaction to epitopes with high immunogenicity.</p> <p>Results</p> <p>All MAbs showed stronger reactivity with all WNVs tested and good competition for antigen binding in ELISA tests with WNV-positive equine and chicken sera. Four MAbs (3B2, 3D6, 4D3, 1C3) resulted specific for WNV, while two MAbs (2A8, 4G9) showed cross-reaction with Usutu virus. Three MAbs (3B2, 3D6, 4D3) showed neutralizing activity. Sequence analysis of 3B2 and 3D6 escape mutants showed an amino acid change at E307 (Lys → Glu) in the E protein gene, whereas 4D3 variants identified mutations encoding amino acid changed at E276 (Ser → Ile) or E278 (Thr → Ile). 3B2 and 3D6 mapped to a region on the lateral surface of domain III of E protein, which is known to be a specific and strong neutralizing epitope for WNV, while MAb 4D3 recognized a novel specific neutralizing epitope on domain II of E protein that has not previously been described with WNV MAbs.</p> <p>Conclusions</p> <p>MAbs generated in this study can be applied to various analytical methods for virological and serological WNV diagnosis. A novel WNV-specific and neutralizing MAb (4D3) directed against the unknown epitope on domain II of E protein can be useful to better understand the role of E protein epitopes involved in the mechanism of WNV neutralization.</p
Investigation Outcomes of Tuberculosis Suspects in the Health Centers of Addis Ababa, Ethiopia
BACKGROUND: Little is known about the prevalence of tuberculosis (TB) and HIV among TB suspects in primary health care units in Ethiopia. METHODS: In the period of February to March, 2009, a cross sectional survey was done in 27 health centers of Addis Ababa to assess the prevalence of TB and HIV among TB suspects who have > = 2 weeks symptoms of TB such as cough, fever and weight loss. Diagnosis of TB and HIV was based on the national guidelines. Information concerning socio-demographic variables and knowledge of the respondents about TB was collected using pretested questionnaire. RESULTS: Of the 545 TB suspects, 506 (92.7%) of them participated in the study. The prevalence of both pulmonary and extra pulmonary TB was 46.0% (233/506). The smear positivity rate among pulmonary TB suspect was 21.3%. Of the TB suspects, 298 (58.9%) of them were tested for HIV and 27.2% (81/298) were HIV seropositive. Fifty percent of the HIV positive TB suspects had TB. TB suspects who had a contact history with a TB patient in the family were 9 times more likely to have TB than those who did not have a contact history, [OR = 9.1, (95%CI:4.0, 20.5)]. Individuals who had poor [OR = 5.2, (95%CI: 2.3, 11.2)] and fair knowledge [OR = 3.7, (95%CI: 1.3, 10.4)] about TB were more likely to have TB than individuals who had good knowledge. CONCLUSION: In conclusion, the prevalence of TB among TB suspects with duration of 2 or more weeks is high. Fifty percent of the HIV positive TB suspects had TB. Case finding among TB suspects with duration of 2 or more weeks should be intensified particularly among those who have a contact history with a TB patient
