    Week 48 resistance analyses of the once-daily, single-tablet regimen darunavir/cobicistat/emtricitabine/tenofovir alafenamide (D/C/F/TAF) in adults living with HIV-1 from the Phase III Randomized AMBER and EMERALD Trials

    Darunavir/cobicistat/emtricitabine/tenofovir alafenamide (D/C/F/TAF) 800/150/200/10 mg is being investigated in two Phase III trials, AMBER (NCT02431247; treatment-naive adults) and EMERALD (NCT02269917; treatment-experienced, virologically suppressed adults). Week 48 AMBER and EMERALD resistance analyses are presented. Postbaseline samples for genotyping/phenotyping were analyzed from protocol-defined virologic failures (PDVFs) with viral load (VL) >= 400 copies/mL at failure/later time points. Post hoc analyses were deep sequencing in AMBER, and HIV-1 proviral DNA from baseline samples (VL = 3 thymidine analog-associated mutations (24% not fully susceptible to tenofovir) detected at screening. All achieved VL <50 copies/mL at week 48 or prior discontinuation. D/C/F/TAF has a high genetic barrier to resistance; no darunavir, primary PI, or tenofovir RAMs were observed through 48 weeks in AMBER and EMERALD. Only one postbaseline M184I/V RAM was observed in HIV-1 of an AMBER participant. In EMERALD, baseline archived RAMs to darunavir, emtricitabine, and tenofovir in participants with prior VF did not preclude virologic response

    The research is devoted to the necessity of maintaining the elderly people's vitality in changing social and economic situation resulted in changing their social status, life style and activity structure, and affecting their communication style, behavior and value orientation. The research objective involves the development of supporting techniques for elderly people in the fitness club training environment. The techniques in question are directed at developing the elderly people's active ability of participation in changing society. The methodology is based on the interrelation of gerontological, axiological and systematic activity approaches. The authors identified the most common physical and moral problems of the elderly people, affecting their inherent values, personal attitudes, communicative competence and health condition, and impeding the perspective modeling of personal development.Тема описываемого исследования обусловлена необходимостью сохранения активной жизненной позиции пожилых людей ввиду вынужденных изменений их социального статуса, образа жизни, структуры жизнедеятельности, невольно отражающихся на их стиле общения, поведении, ценностных ориентациях. Целью научной работы стало создание методики поддержки жизненной позиции пожилого человека в образовательном пространстве фитнес-клуба, направленной на формирование способности быть активным участником развивающегося социума.     The relevance of the chosen topic is due to the fact that the accident that occurred at the production association "Mayak" brought negative consequences. When the radiation was released, the territories located within the radius of the enterprise were contaminated, that is, water bodies, pastures, fields, forests were contaminated - everything that a person needs to live on the territory, conduct economic activities, raise livestock and engage in agriculture. From all this, we can conclude that the accident caused environmental, socio-economic damage, in addition, this circumstance negatively affected the health of people living in settlements contaminated with radiation, because they were exposed to radiation, as a result of which at the life stage of an individual, radiation infertility, radiation cataract, radiation burns, radiation sickness, metabolic disorders, infectious complications and other diseases. At the proposed stage in the life of an individual with a diagnosed disease, a certain set of physical culture and rehabilitation works is required, which will allow maintaining the harmonious work of the body and stabilizing the pace of the lifestylе.Актуальность выбранной темы обусловлена тем, что авария, произошедшая на производственном объединении «Маяк», принесла негативные последствия. При выбросе радиации произошло загрязнение территорий, расположенных в радиусе предприятия, то есть были загрязнены водоёмы, пастбища, поля, леса- все то, что необходимо человеку для проживания на территории, ведения хозяйственной деятельности, выращивания скота и занятия сельским хозяйством. Из всего этого можно сделать вывод, что авария нанесла экологический, социально-экономический ущерб, кроме того, данное обстоятельство негативно отразилось на здоровье людей, проживающих в населенных пунктах, загрязненных радиацией, ведь они подверглись облучению, в результате чего на жизненном этапе индивида, могут проявиться лучевое бесплодие, лучевая катаракта, лучевой ожог, лучевая болезнь, нарушения обмена веществ, инфекционные осложнения и другие заболевания.     Background: Effective two-drug regimens could decrease long-term drug exposure and toxicity with HIV-1 antiretroviral therapy (ART). We therefore aimed to evaluate the efficacy and safety of a two-drug regimen compared with a three-drug regimen for the treatment of HIV-1 infection in ART-naive adults. Methods: We conducted two identically designed, multicentre, double-blind, randomised, non-inferiority, phase 3 trials: GEMINI-1 and GEMINI-2. Both studies were done at 192 centres in 21 countries. We included participants ( 6518 years) with HIV-1 infection and a screening HIV-1 RNA of 500 000 copies per mL or less, and who were naive to ART. We randomly assigned participants (1:1) to receive a once-daily two-drug regimen of dolutegravir (50 mg) plus lamivudine (300 mg) or a once-daily three-drug regimen of dolutegravir (50 mg) plus tenofovir disoproxil fumarate (300 mg) and emtricitabine (200 mg). Both drug regimens were administered orally. We masked participants and investigators to treatment assignment: dolutegravir was administered as single-entity tablets (similar to its commercial formulation, except with a different film colour), and lamivudine tablets and tenofovir disoproxil fumarate and emtricitabine tablets were over-encapsulated to visually match each other. Primary endpoint was the proportion of participants with HIV-1 RNA of less than 50 copies per mL at week 48 in the intention-to-treat-exposed population, using the Snapshot algorithm and a non-inferiority margin of 1210%. Safety analyses were done on the safety population. GEMINI-1 and GEMINI-2 are registered with ClinicalTrials.gov, numbers NCT02831673 and NCT02831764, respectively. Findings: Between July 18, 2016, and March 31, 2017, 1441 participants across both studies were randomly assigned to receive either the two-drug regimen (n=719) or three-drug regimen (n=722). At week 48 in the GEMINI-1 intention-to-treat-exposed population, 320 (90%) of 356 participants receiving the two-drug regimen and 332 (93%) of 358 receiving the three-drug regimen achieved plasma HIV-1 RNA of less than 50 copies per mL (adjusted treatment difference 122\ub76%, 95% CI 126\ub77 to 1\ub75); in GEMINI-2, 335 (93%) of 360 in the two-drug regimen and 337 (94%) of 359 in the three-drug regimen achieved HIV-1 RNA of less than 50 copies per mL (adjusted treatment difference 120\ub77%, 95% CI 124\ub73 to 2\ub79), showing non-inferiority at a 1210% margin in both studies (pooled analysis: 655 [91%] of 716 in the two-drug regimen vs 669 [93%] of 717 in the three-drug regimen; adjusted treatment difference 121\ub77%, 95% CI 124\ub74 to 1\ub71). Numerically, more drug-related adverse events occurred with the three-drug regimen than with the two-drug regimen (169 [24%] of 717 vs 126 [18%] of 716); few participants discontinued because of adverse events (16 [2%] in the three-drug regimen and 15 [2%] in the two-drug regimen). Two deaths were reported in the two-drug regimen group of GEMINI-2, but neither was considered to be related to the study medication. Interpretation: The non-inferior efficacy and similar tolerability profile of dolutegravir plus lamivudine to a guideline-recommended three-drug regimen at 48 weeks in ART-naive adults supports its use as initial therapy for patients with HIV-1 infection.
