    Intraobserver reproducibility of parameters of standard and 2D speckle tracking echocardiography, dynamics of global longitudinal strain I in patients with acute primary anterior STEMI

    The aim of this study was to assess the intraobserver reproducibility of parameters of standard and 2 dimensional speckle tracking echocardiography, dynamics of global longitudinal strain in patients with acute primary anterior STEMI. The study included 24 patients, mean age 58.46±10.2. Echocardiography with 2D speckle tracking imaging was performed on the 1st (T1), 7th (T2), 14th days (T3) after STEMI («Vivid E9»). Analysis of echocardiographic images was performed offline at the different periods by the two independent observers (EchoPac) – experienced and inexperienced. In order to assess the agreement between standard and 2D speckle tracking echocardiography, a correlation analysis (Pearson correlation, Spearman’s rank correlation coefficient) and Bland-Altman analysis were undertaken. The 23 patients had urgent reperfusion therapy, 6 patients underwent primary PCI, 16 patients – PCI after successful fibrinolysis (68%). GLS and WMSI had the best intraobsever reproducibility. Dynamics of EDV LV, ESV LV, EF LV was without significant differences. Nevertheless, it was found positive dynamic of GLS: – 12.65±3.53 (T1), -13.61±3.81 (T2), –14.27±4.1 (T3), p<0.05. GLS reduced 11.35% (p=0.0048) from T1 to T3. The best intraobserver reproducibility of parameters of 2 D speckle-tracking and standard echocardiography was revealed in GLS and WMSI. The modern management of STEMI patients limits adverse postinfarction remodeling and preserves of global left ventricular contractility detected by the EF LV. However, GLS had the positive dynamics and improved to the 14th day

    In recent years, examines the role of viral infection in the development of IgA-nephropathy. In our study, an analysis of renal biopsy tissue from 17 patients IgA-nephropathy in the presence of adenoviral antigen. Shown the presence of adenovirus infection in kidney tissue in patients with IgA-nephropathy, regardless of age: 52% of patients 60 years and 33% of patients older than 60 years. Revealed that the presence of adenovirus in the glomerular zone of influence on the severity of global sclerosis in both age groups (p &lt;0.05) and in patients younger than 60 years the presence of adenovirus in the interstitium correlated with the severity of degeneration of the epithelium of tubules (p = 0.014). It also shows that the presence of adenovirus in the interstitium correlated with cell phenotype CD19 / λ (p = 0,004) and CD19 / κ (p = 0,002), intenstivnostyu expression of IL-10 (p = 0,029) and membranoatakuyuschego complex S5b-C9 (p = 0.011) in the glomerular zone in patients younger than 60 years. In patients older than 60 years, the influence of adenoviral infection of renal tissue to a local immune response have been identified. Based on these results, it is concluded that the presence of adenovirus infection in the kidney plays a role in the pathogenesis of IgA-nephropath

    Abstract. In the article is considered the modern approach to the assessment of climate parameters on the basis of the thermal comfort indices in accordance with the requirements of GOST R ISO 7730-2009. For the first time is introduced the concept of isocomfortable values of microclimate parameters, representing a set providing a predetermined thermal comfort level. Are calculated and presented the examples of three-dimensional arrays of given parameters as the convection temperature function from relative humidity and mobility of air. Microclimate parameters ensuring a specified level of thermal comfort were calculated by the method of successive approximations.Proposed in this article matrixes of isocomfortable values of microclimate parameters can be formulated for different combinations of source parameters

    Short ORF-Dependent Ribosome Shunting Operates in an RNA Picorna-Like Virus and a DNA Pararetrovirus that Cause Rice Tungro Disease

    Rice tungro disease is caused by synergistic interaction of an RNA picorna-like virus Rice tungro spherical virus (RTSV) and a DNA pararetrovirus Rice tungro bacilliform virus (RTBV). It is spread by insects owing to an RTSV-encoded transmission factor. RTBV has evolved a ribosome shunt mechanism to initiate translation of its pregenomic RNA having a long and highly structured leader. We found that a long leader of RTSV genomic RNA remarkably resembles the RTBV leader: both contain several short ORFs (sORFs) and potentially fold into a large stem-loop structure with the first sORF terminating in front of the stem basal helix. Using translation assays in rice protoplasts and wheat germ extracts, we show that, like in RTBV, both initiation and proper termination of the first sORF translation in front of the stem are required for shunt-mediated translation of a reporter ORF placed downstream of the RTSV leader. The base pairing that forms the basal helix is required for shunting, but its sequence can be varied. Shunt efficiency in RTSV is lower than in RTBV. But in addition to shunting the RTSV leader sequence allows relatively efficient linear ribosome migration, which also contributes to translation initiation downstream of the leader. We conclude that RTSV and RTBV have developed a similar, sORF-dependent shunt mechanism possibly to adapt to the host translation system and/or coordinate their life cycles. Given that sORF-dependent shunting also operates in a pararetrovirus Cauliflower mosaic virus and likely in other pararetroviruses that possess a conserved shunt configuration in their leaders it is tempting to propose that RTSV may have acquired shunt cis-elements from RTBV during their co-existence

    РОЛЬ CHLAMYDIA TRACHOMATIS ПОЧЕЧНОЙ ТКАНИ В ПАТОГЕНЕЗЕ IGA-НЕФРОПАТИИ С УЧЕТОМ ВОЗРАСТА БОЛЬНОГО

    IgA-nephropathy is the most common form of primary glomerulonephritis in the world and therefore the mechanisms of this isease are actively explored. In our study, an analysis of renal biopsy issue from 117 patients IgA-nephropathy in the presence of Chlamydia trachomatis antigen related to age (before and after 60 years). It was shown that the presence of C. trachomatis in the glomerular zone influence on the severity of segmental sclerosis (p &lt;0.05), and its presence in the interstitium affect on the size of the glomeruli (p &lt;0.02) and severity of degeneration of epithelial tubules (p &lt;0.02) regardless of patient age. It was shown the effect of C. trachomatis on the expression of local immune response of kidney tissue. In patients under 60 years: C. trachomatis in the glomeruli affects the number of cells of the phenotype CD25 (p = 0,04) and CD19 / κ (p = 0,034) in the glomerular infiltration and the presence of antigen in the interstitium affect the expression of CD95 (APO-1/Fas) (p = 0,038) by mononuclear cells infiltration and formation of deposits S5b-C9 (p = 0,042) in the interstitial space. In patients older than 60 years of presence C. trachomatis in the glomerular zone impacts on the expression of TNF-α (p = 0,039) in the glomeruli, the presence of antigen in interstitium affect the number of cells CD71 (p = 0,025) in the interstitial infiltrate. Based on these results, we concluded that the presence of Chlamydia trachomatis antigen has an impact on the development and course of the disease and is the etiologic agent in patients with IgA-nephropathy, regardless of age.IgA-нефропатия является наиболее часто встречающейся формой первичного гломерулонефрита в мире и поэтому сегодня активно изучаются механизмы развития этого заболевания. В нашем исследовании проведен анализ биопсии почек у 117 больных IgA-нефропатией на присутствие антигена Chlamydia trachomatis до и после 60 лет. В работе показано, что присутствие C. trachomatis в гломерулярной зоне влияет на выраженность сегментарного склероза (р&lt;0,05), а присутствие ее в интерстиции – на размер клубочка (р&lt;0,02) и выраженность дистрофии эпителия канальцев (р&lt;0,02) независимо от возраста больного. Также выявлено влияние этого антигена на выраженность местного иммунного ответа почечной ткани. У больных до 60 лет C. trachomatis в клубочке влияет на количество клеток фенотипа CD25 (p=0,04) и CD19/κ (p=0,034) в составе гломерулярного инфильтрата; наличие антигена в интерстиции влияет на экспрессию CD95(APO-1/Fas) (p=0,038) мононуклеарами инфильтрата и формирование депозитов С5b-C9 (p=0,042) в интерстициальном пространстве. У больных старше 60 лет присутствие C. trachomatis в гломерулярной зоне влияет на экспрессию TNF-α (p=0,039) в клубочке; наличие C. trachomatis-антигена в интерстиции оказывает влияние на количество клеток CD71 (p=0,025) в составе интерстициального инфильтрата. Показано присутствие аденовирусной инфекции в почечной ткани у больных IgA-нефропатией независимо от возраста: у 52% больных до 60 лет и у 33% больных старше 60 лет. Выявлено, что присутствие аденовируса в гломерулярной зоне влияет на выраженность глобального склероза в обеих возрастных группах (р&lt;0,05), а у лиц моложе 60 лет присутствие аденовируса в интерстиции коррелирует с выраженностью дистрофии эпителия канальцев (р=0,014).
