Enteropathic arthritis in Brazil: data from the brazilian registry of spondyloarthritis
Inflammatory bowel diseases (Crohn's disease and ulcerative rectocolitis) have extraintestinal manifestations 25% of the patients, with the most common one being the enteropathic arthritis. METHODS: Prospective, observational, multicenter study with patients from 29 reference centers participating in the Brazilian Registry of Spondyloarthritis (RBE), which incorporates the RESPONDIA (Ibero-American Registry of Spondyloarthritis) group. Demographic and clinical data were collected from 1472 patients and standardized questionnaires for the assessment of axial mobility, quality of life, enthesitic involvement, disease activity and functional capacity were applied. Laboratory and radiographic examinations were performed. The aim of this study is to compare the clinical, epidemiological, genetic, imaging, treatment and prognosis characteristics of patients with enteropathic arthritis with other types of spondyloarthritis in a large Brazilian cohort. RESULTS: A total of 3.2% of patients were classified as having enteroarthritis, 2.5% had spondylitis and 0.7%, arthritis (peripheral predominance). The subgroup of individuals with enteroarthritis had a higher prevalence in women (P < 0.001), lower incidence of inflammatory axial pain (P < 0.001) and enthesitis (P = 0.004). HLA-B27 was less frequent in the group with enteroarthritis (P = 0.001), even when considering only those with the pure axial form. There was a lower prevalence of radiographic sacroiliitis (P = 0.009) and lower radiographic score (BASRI) (P = 0.006) when compared to patients with other types of spondyloarthritis. They also used more corticosteroids (P < 0.001) and sulfasalazine (P < 0.001) and less non-steroidal anti-inflammatory drugs (P < 0.001) and methotrexate (P = 0.001). CONCLUSION: There were differences between patients with enteroarthritis and other types of spondyloarthritis, especially higher prevalence of females, lower frequency of HLA-B27, associated with less severe axial involvement.As doenças inflamatórias intestinais (doença de Crohn e retocolite ulcerativa) apresentam manifestações extraintestinais em um quarto dos pacientes, sendo a mais comum a artrite enteropática. MÉTODOS: Estudo prospectivo, observacional e multicêntrico, realizado com pacientes de 29 centros de referência participantes do Registro Brasileiro de Espondiloartrites (RBE), que se incorpora ao grupo RESPONDIA (Registro Ibero-americano de Espondiloartrites). Dados demográficos e clÃnicos de 1472 pacientes foram colhidos, e aplicaram-se questionários padronizados de avaliação de mobilidade axial, de qualidade de vida, de envolvimento entesÃtico, de atividade de doença e de capacidade funcional. Exames laboratoriais e radiográficos foram realizados. Objetivamos, neste presente artigo, comparar as caracterÃsticas clÃnicas, epidemiológicas, genéticas, imagenológicas, de tratamento e prognóstico de enteroartrÃticos com os outros espondiloartrÃticos nesta grande coorte brasileira. RESULTADOS: Foram classificados como enteroartrite 3,2% dos pacientes, sendo que 2,5% tinham espondilite e 0,7%, artrite (predomÃnio periférico). O subgrupo de indivÃduos com enteroartrite apresentava maior prevalência de mulheres (P < 0,001), menor incidência de dor axial inflamatória (P < 0,001) e de entesite (P = 0,004). O HLA-B27 foi menos frequente no grupo de enteroartrÃticos (P = 0,001), mesmo se considerado apenas aqueles com a forma axial pura. Houve menor prevalência de sacroiliÃte radiológica (P = 0,009) e também menor escore radiográfico (BASRI) (P = 0,006) quando comparado aos pacientes com as demais espondiloartrites. Também fizeram mais uso de corticosteroides (P < 0,001) e sulfassalasina (P < 0,001) e menor uso de anti-inflamatórios não hormonais (P < 0,001) e metotrexato (P = 0,001). Atopic dermatitis : a cutaneous or systemic disease? The search for answers in the history of Dermatology
A dermatite atópica é doença inflamatória cutânea associada à atopia, predisposição a produzir resposta IgE a alérgenos ambientais, constituindo uma das manifestações das doenças atópicas, junto com a asma e a rinite alérgica. A dermatite atópica é caracterizada por episódios recorrentes de eczema associado a prurido, acometendo superfÃcie cutânea geneticamente alterada, induzindo, por fenômenos imunológicos, a presença de inflamação. Trata-se de doença multifatorial, com enfoque nas alterações sistêmicas e alérgicas ou nas manifestações cutâneas, de acordo com diferentes visões da doença. A conceituação da dermatite atópica é importante, porque a conduta terapêutica pode variar segundo essas duas formas diferentes de analisá-la. Autores modernos discutem extensivamente esses aspectos sem, contudo, alcançar uma conclusão sobre a dermatite atópica como doença sistêmica ou cutânea. A procura dos conceitos sobre a doença, desde os primeiros relatos, associada à evolução do pensamento na dermatologia, poderia esclarecer a origem dessas dúvidas. Uma análise histórica demonstra que a dermatite atópica tem seus conceitos atuais oriundos dos estudos de diversos pensadores, que, em diferentes momentos históricos, descreveram a doença, e que muito do que acreditamos atualmente tem, nesses escritos, seus fundamentos.Atopic dermatitis is an inflammatory disease associated to atopy, which is a predisposition to produce an IgE response to environmental allergens and considered one of the manifestations of the atopic diseases, including asthma and allergic rhinitis. Atopic dermatitis is characterized by recurrent eczema flares, associated to pruritus, affecting a genetically disrupted skin surface, inducing, by immunological phenomena, the onset of inflammation. It is a multifactorial disease, with an emphasis on systemic and allergic alterations or skin manifestations, according to different concepts. The definition of atopic dermatitis is important, since its management may vary according to these two different points of view. Modern authors have extensively discussed these concepts, though with no conclusion as to its nature - systemic or cutaneous disease. The search for concepts about the disease, since its first descriptions, associated to the evolution of the dermatology rationale through history, may help understand the origin of these doubts. A historical analysis demonstrates that the currently accepted concepts of atopic dermatitis have their background from different researchers, who, at different historical moments, described the disease, and a great part of our beliefs about atopic dermatitis are related to these ancient writings
Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial
Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Viabilidade de o aleitamento exclusivo prevenir o desenvolvimento da dermatite atópica em lactentes até um ano de idade

bastante pruriginosa, em geral benigna, contudo com prognóstico incerto. Trata-se de
uma doença multifatorial, manifestando-se devido a fatores genéticos e ambientais.
Estudos relacionando o papel do aleitamento materno no desenvolvimento da dermatite
atópica apresentam resultados conflitantes. O objetivo da presente pesquisa foi avaliar
o papel do aleitamento materno exclusivo como fator associado ao surgimento da
dermatite atópica em lactentes até um ano de idade. Para atingir os objetivos propostos
foi realizado um estudo prospectivo de coorte acompanhando 177 crianças, sendo 87
com história familiar de atopia e 90 sem história familiar de atopia, da maternidade até
um ano de idade. Dentre os pacientes avaliados, 37(21%) apresentaram o diagnóstico
de dermatite atópica realizado por exame fÃsico até um ano de idade. Destes,
24(27,6%) com história e 13(14,4%) sem história familiar de atopia.
O tempo médio de aleitamento materno exclusivo foi de 3,8 meses. O risco do
desenvolvimento de dermatite atópica com menos de 4 meses de aleitamento materno
exclusivo foi de 1,97 (OR 1,97; IC 95%: 1,26-3,09; p<0,0001). Quando o lactente
apresentava o tempo de aleitamento materno exclusivo de pelo menos 4 meses, a
razão de chance (odds ratio) para o desenvolvimento de DA encontrada foi de 0,47 ( IC
95%: 0,34-0,67).
No grupo que apresentava risco de atopia, os lactentes com aleitamento
materno exclusivo por menos que 4 meses apresentaram risco de desenvolver
dermatite atópica de 2,34(OR 2,34;IC 95%:1,22-4,46;p<0,001). Quando estes lactentes
apresentavam tempo de aleitamento materno de pelo menos 4 meses, o risco de
apresentarem dermatite atópica com 1 ano foi de 0,44 (OR 0,44; IC 95%: 0,28-0,69),
enquanto no grupo sem história familiar de atopia, houve uma tendência estatÃstica não
significante para um menor desenvolvimento de dermatite atópica com pelo menos 4
meses de aleitamento materno exclusivo (p=0,08).
O tempo de aleitamento materno exclusivo por pelo menos 4 meses preveniu o
surgimento de dermatite atópica, enquanto o desmame precoce aumentou a chance de
desenvolvimento de dermatite atópica em lactentes até um ano de idade, sendo a
proteção maior em lactentes com história familiar de atopia, mesmo quando os dados
foram considerados em relação a possÃveis confundidores ambientais. _________________________________________________________________________________________ ABSTRACTAtopic dermatitis is a chronic and recurrent skin disease, very itchy, with a benign
course, but with an uncertain outcome. It is a multifatorial disease where genetic and
environmental factors interact. Many clinical studies show that the role of breastfeeding
on the development of atopic dermatitis is controversial. The aim of this study was to
evaluate the relation between exclusive breastfeeding and the onset of atopic dermatitis
in infants within one year of age. In order to achieve the proposed results, a prospective
cohort study was conducted with 177 patients (87 with parental history and 90 without
parental history of atopy). 37(21%) of the evaluated infants developed atopic dermatitis
evaluated by physical examination within one year of age, 24(27.6%) with and
13(14,4%) without parental atopy history.
Exclusive breastfeeding mean period was 3.8 months. Exclusive breastfeeding
for less than 4 months increased the risk for atopic dermatitis development among
infants until one year of age (OR 1.97; CI 95%: 1.26-3.09; p<0.0001). When
breastfeeding period was at least 4 months, calculated odds ratio for atopic dermatitis
development was 0.47 (CI 95%: 0.34-0.67). Among infants with atopy risk,
breastfeeding for less than 4 months was even strongly associated with atopic dermatitis
risk (OR 2.34;CI 95%: 1.22-4.46;p<0,001). The protective effect of exclusive
breastfeeding for at least 4 months among infants with atopy risk until 1 year of age was
0.44(OR 0.44; CI 95%: 0.28-0.69). There was a non-significant statistical tendency for
exclusive breastfeeding protection against the development of atopic dermatitis in the
group of patients without atopic parental history (p=0.08).
Exclusive breastfeeding for at least 4 months had a protective effect against the
development of atopic dermatitis until 1 year of age, while early weaning before 4
months of age increased the risk for atopic dermatitis development within 1 year of age,
mainly in infants with atopy risk (parental history of atopy) even when environmental
confound factors were evaluated
