    Acute Kidney Injury in Coronavirus Disease 2019 Infected Patients: A Meta-Analytic Study

    Introduction: In clinical reports on coronavirus disease 2019 (COVID-19), the incidence of acute kidney injury (AKI) is extremely variable, although AKI is described as an independent risk factor for mortality. A meta-analysis was performed to clarify the incidence and the impact of COVID-19-related AKI on mortality. Methods: All trials reporting the incidence of AKI in COVID-19 patients were searched using MEDLINE, the Cochrane Library, and EMBASE databases (last update April 26, 2020). Results: Ten trials with a sample of 5,166 patients were included. AKI occurred in 947 out of 5,166 (18.3%) patients. AKI incidence was higher in severe cases: 62/305 severe patients developed AKI (20%) versus 27/1,268 nonsevere patients (2%) (p = 0.00001). AKI occurred in 475 out of 915 (52%) deceased patients versus 183 out of 2,678 (7%) survivors (p = 0.00001). Continuous renal replacement therapy was significantly more frequent in severe cases and in dead patients. Conclusion: A significant increase in mortality rate was observed in COVID patients who developed AKI, and AKI incidence was also higher in severe cases. Any supportive strategies to protect kidney could represent valuable intervention to reduce mortality in severe COVID-19 patients

    Endoscopy-assisted tracheal reconstruction of post-traumatic obstruction in a cat: A case report

    A domestic shorthair cat was referred with a history of dyspnoea and lethargy that had arisen gradually within the last few days. The cat had been hit by an automobile 10 days earlier. A thoracic radiograph suggested stenosis of the intrathoracic trachea, proximal to the tracheal bifurcation. Endoscopic examination confirmed a narrowing of the tracheal lumen due to the presence of a fibrotic ring, with remaining patent lumen of about 2 mm. A surgical treatment with a right lateral thoracotomy approach and resection of the narrowed portion of the trachea (a length of about 1 cm) was performed under endoscopic vision. The procedure was unsuccessful because of the size of the tissue removed and the laxity of the remaining tracheal tissue that caused dehiscence of sutures between the tracheal stump and tracheal bifurcation. After consultation with the cat\u2019s owners, an intraoperative euthanasia was performed

    A pilot study on the e-kayak system: A wireless DAQ suited for performance analysis in flatwater sprint kayaks

    Nowadays, in modern elite sport, the identification of the best training strategies which are useful in obtaining improvements during competitions requires an accurate measure of the physiologic and biomechanical parameters that affect performance. The goal of this pilot study was to investigate the capabilities of the e-Kayak system, a multichannel digital acquisition system specifically tailored for flatwater sprint kayaking application. e-Kayak allows the synchronous measure of all the parameters involved in kayak propulsion, both dynamic (including forces acting on the paddle and footrest) and kinematic (including stroke frequency, displacement, velocity, acceleration, roll, yaw, and pitch of the boat). After a detailed description of the system, we investigate its capability in supporting coaches to evaluate the performance of elite athletes\u2019 trough-specific measurements. This approach allows for a better understanding of the paddler\u2019s motion and the relevant effects on kayak behavior. The system allows the coach to carry out a wide study of kayak propulsion highlighting, and, at the same time, the occurrences of specific technical flaws in the paddling technique. In order to evaluate the correctness of the measurement results acquired in this pilot study, these results were compared with others which are available in the literature and which were obtained from subjects with similar characteristics

    Effects of intraarticular treatment with stanozolol on synovial membrane and cartilage in an ovine model of osteoarthritis

    Aim of the study was to verify the clinical and morphological effects of intra-articular stanozolol or placebo treatment, lasting 3 and 9 months, in sheep in which a femoro-tibial osteo-arthritis (OA) were surgically induced (medial bilateral meniscectomy). Twenty healthy sheep divided into four groups and two control animals group, after surgical medial bilateral meniscectomy, were weekly injected in femoral-tibial joint (FTJ) with stanozolol or placebo. Lameness evaluation was performed and synovial fluid was collected from all sheep at each treatment time. Necropsies were performed after 3 or 9 month as described in experimental design. Gross pathologies were described and specimen tissues collected from femoro-tibial articular joints were processed for routine histological examination. The gross anatomy of the FTJ was well-preserved in stanozolol-treated sheep; this also applied to the histological features of articular cartilage. Joint aseptic inflammation and fibrosis were observed in placebo- treated sheep, associated with a different degree of severity of condylar and tibial plate cartilage degeneration. Stanozolol intra-articular treatment reduces osteophytes formation and subchondral bone reaction and promotes articular cartilage regeneration

    Pinch Grip per SE Is Not an Occupational Risk Factor for the Musculoskeletal System: An Experimental Study on Field

    Introduction: Some ergonomic evaluation methods define pinch grip as a risk factor independent of the exerted force. The present experimental study was performed with the main aim of objectively measuring the muscle engagement during the execution of pinch grip. Methods: the participants of the study were healthy workers occupationally involved in a high-intensity repetitive job related to the sorting of letters and small packages. Surface electromyography (sEMG) was used to study the activity of the abductor pollicis brevis and first dorsal interosseous fibers related to the execution of the required working tasks, while the force exerted during voluntary muscle contraction for pinch grip was measured by a portable acquisition system. The subjects were specifically asked to exert the maximum voluntary isometric contraction (MVIC) and further voluntary isometric contractions with a spontaneous force (SF) equal to 10%,20% and 50% of the MVIC; finally, the workers were asked to hold in pinch grip two types of envelopes, weighing 100 g and 500 g, respectively. Results: The force required to pinch 100 and 500 g envelopes by the fifteen subjects of the study corresponded to 4 and 5% MVIC, respectively. The corresponding sEMG average rectified values (ARV) were approximately 6% of that at MVIC for first dorsal interosseus (FDI) fibers and approximately 20-25% of MVIC for abductor pollicis brevis (ABP) fibers. Bivariate correlation analysis showed significant relationships between force at MVIC and FDI ARV at MCV. Conclusions: The obtained results demonstrate that muscle recruitment during pinch grip varies as a function of the SF: not only the position but also the exerted force should be considered when assessing the pinch grip as risk factor for biomechanical overload of the upper limb

    The Portrayal of Complementary and Alternative Medicine in Mass Print Magazines Since 1980

    Objectives: The objectives of this study were to examine and describe the portrayal of complementary and alternative medicine (CAM) in mass print media magazines. Design: The sample included all 37 articles found in magazines with circulation rates of greater than 1 million published in the United States and Canada from 1980 to 2005. The analysis was quantitative and qualitative and included investigation of both manifest and latent magazine story messages. Results: Manifest analysis noted that CAM was largely represented as a treatment for a patient with a medically diagnosed illness or specific symptoms. Discussions used biomedical terms such as patient rather than consumer and disease rather than wellness. Latent analysis revealed three themes: (1) CAMs were described as good but not good enough; (2) individualism and consumerism were venerated; and (3) questions of costs were raised in the context of confusion and ambivalence

    Beach monitoring and morphological response in the presence of coastal defense strategies at riccione (Italy)

    The coastal area at Riccione, in the southern Emilia-Romagna littoral region, is exposed to erosive processes, which are expected to be enhanced by climate change. The beach, mostly composed of fine sand, is maintained through various defense strategies, including frequent nourishment interventions for balancing the sediment deficit and other experimental solutions for reducing coastal erosion. Artificial reshaping of the beach and \u201ccommon practices\u201d in the sediment management redefine the beach morphology and the sediment redistribution almost continuously. These activities overlap each other and with the coastal dynamics, and this makes it very difficult to evaluate their effectiveness, as well as the role of natural processes on the beach morphological evolution. Topo-bathymetric and sedimentological monitoring of the beach has been carried out on a regular basis since 2000 by the Regional Agency for Prevention, Environment and Energy of Emilia-Romagna (Arpae). Further monitoring of the emerged and submerged beach has been carried out in 2019\u20132021 in the framework of the research project STIMARE, focusing on innovative strategies for coastal monitoring in relation with erosion risk. The aim of this study is to assess the coastal behavior at the interannual/seasonal scale in the southern coastal stretch of Riccione, where the adopted coastal defense strategies and management actions mostly control the morphological variations in the emerged and submerged beach besides the wave and current regime. The topo-bathymetric variations and erosion/accretion patterns provided by multitemporal monitoring have been related to natural processes and to anthropogenic activities. The morphological variations have been also assessed in volumetric terms in the different subzones of the beach, with the aim of better understanding the onshore/offshore sediment exchange in relation with nourishments and in the presence of protection structures. The effectiveness of the adopted interventions to combat erosion, and to cope with future climate change-related impacts, appears not fully successful in the presence of an overall sediment deficit at the coast. This demonstrates the need for repeated monitoring of the emerged and submerged beach in such a critical setting

    Proteinase Activated Receptor 4 in the Jejunum of Healthy Horses and of Horses With Epiploic Hernia

    Proteinase activated receptor 4 (PAR4) in the gastrointestinal tract is involved in the regulation of inflammation and pain pathways. The aim of the present study was to evaluate the distribution and expression of PAR4 in the jejunum of healthy horses and in the pathologic tracts from horses undergoing surgery for herniation of the small intestine through the epiploic foramen. Eight healthy horses (Group H) and eight horses with epiploic hernia (Group EH) were included; the jejunum samples were collected at the slaughter or intraoperatively after enterectomy, respectively. To evaluate PAR4 expression in sections of the jejunum, immunofluorescence, western blot and quantitative polymerase chain reaction (qRT-PCR) were performed. Immunohistochemistry of PAR4 in the jejunum of the healthy horses showed that receptors are predominantly expressed in the immune cell population scattered throughout the lamina propria of the mucosa and in the submucosa. Quantitative PCR data demonstrated that PAR4 mRNA was detectable in all of the samples analyzed without any difference between the H and the EH groups, however the PAR4 protein level was significantly lower in the jejunums of the EH horses. In the Group EH horses, PAR4 immunoreactivity was mainly expressed in the mast cells and was extensively distributed in the sierosa. In the lamina propria of mucosa of Group EH, leukocytes were less abundant than in Group H. In this study, the distribution and expression of PAR4 in the jejunums of the healthy horses and in those with spontaneous occurring epiploic hernia was demonstrated
