    Is physician assessment of alcohol consumption useful in predicting risk of severe liver disease among people with HIV and HIV/HCV co-infection?

    Background: Alcohol consumption is a known risk factor for liver disease in HIV-infected populations. Therefore, knowledge of alcohol consumption behaviour and risk of disease progression associated with hazardous drinking are important in the overall management of HIV disease. We aimed at assessing the usefulness of routine data collected on alcohol consumption in predicting risk of severe liver disease (SLD) among people living with HIV (PLWHIV) with or without hepatitis C infection seen for routine clinical care in Italy. Methods: We included PLWHIV from two observational cohorts in Italy (ICONA and HepaICONA). Alcohol consumption was assessed by physician interview and categorized according to the National Institute for Food and Nutrition Italian guidelines into four categories: abstainer; moderate; hazardous and unknown. SLD was defined as presence of FIB4 > 3.25 or a clinical diagnosis of liver disease or liver-related death. Cox regression analysis was used to evaluate the association between level of alcohol consumption at baseline and risk of SLD. Results: Among 9542 included PLWHIV the distribution of alcohol consumption categories was: abstainers 3422 (36%), moderate drinkers 2279 (23%), hazardous drinkers 637 (7%) and unknown 3204 (34%). Compared to moderate drinkers, hazardous drinking was associated with higher risk of SLD (adjusted hazard ratio, aHR = 1.45; 95% CI: 1.03-2.03). After additionally controlling for mode of HIV transmission, HCV infection and smoking, the association was attenuated (aHR = 1.32; 95% CI: 0.94-1.85). There was no evidence that the association was stronger when restricting to the HIV/HCV co-infected population. Conclusions: Using a brief physician interview, we found evidence for an association between hazardous alcohol consumption and subsequent risk of SLD among PLWHIV, but this was not independent of HIV mode of transmission, HCV-infection and smoking. More efforts should be made to improve quality and validity of data on alcohol consumption in cohorts of HIV/HCV-infected individuals

    In this paper, we reinterpret the most basic exponential smoothing equation, St+1 = (1 − α)St + αXt, as a model of social influence. This equation is typically used to estimate the value of a series at time t + 1, denoted by St+1, as a convex combination of the current estimate St and the actual observation of the time series Xt. In our work, we interpret the variable St as an agent’s tendency to adopt the observed behavior or opinion of another agent, which is represented by a binary variable Xt. We study the dynamics of the resulting system when the agents’ recently adopted behaviors or opinions do not change for a period of time of stochastic duration, called latency. Latency allows us to model real-life situations such as product adoption, or action execution. When different latencies are associated with the two different behaviors or opinions, a bias is produced. This bias makes all the agents in a population adopt one specific behavior or opinion.     Bioprospecting of aerobic endospore-forming bacteria with biotechnological potential for growth promotion of banana plants

    High amounts of agrochemicals are regularly used for increasing yields in cultivation of banana. The use of plant growth-promoting rhizobacteria (PGPR) could represent an environmentally friendly alternative that can improve productivity. In this study, we tested a funnel-like strategy to bioprospect aerobic endospore-forming bacteria (AEFB) that could be useful to develop a biotechnological product to promote the growth of banana plants. First, 837 aerobic endospore-forming bacteria were obtained from the rhizosphere of banana and plantain. Then, the isolates were assessed for both: their capacity to promote growth of maize seedlings (used as a model plant) and to display specific biochemical PGPR-associated traits. Twenty-two of these strains significantly increased the dry weight of maize seedlings, some of them showing in vitro PGPR traits. Based on this screening, four isolates were selected to conduct evaluations on banana plants, from which, the bacterial strain Bacillus subtilis EA-CB0575 was chosen as a promising plant growth -promoting isolate. Further studies with this strain showed that the application of either spores, vegetative cells (both at concentrations 1 x 10(7) and 1 x 10(8) CFU/mL), or the cell-free supernatant (CFS) of its fermentation significantly increased the dry weight of banana plants, compared with the non-treated control. Our results suggest that both cellular structures of B. subtilis EA-CB0575 and the metabolites and/or elements contained in its CFS enhance the growth and development of banana plants.     Establishing the selective phospholipid membrane coordination, permeation and lysis properties for a series of ‘druggable’ supramolecular self-associating antimicrobial amphiphiles

    The rise of antimicrobial resistance remains one of the greatest global health threats facing humanity. Furthermore, the development of novel antibiotics has all but ground to a halt due to a collision of intersectional pressures. Herein we determine the antimicrobial efficacy for 14 structurally related supramolecular self-associating amphiphiles against clinically relevant Gram-positive methicillin resistant Staphylococcus aureus and Gram-negative Escherichia coli. We establish the ability of these agents to selectively target phospholipid membranes of differing compositions, through a combination of computational host:guest complex formation simulations, synthetic vesicle lysis, adhesion and membrane fluidity experiments, alongside our novel 1H NMR CPMG nanodisc coordination assays, to verify a potential mode of action for this class of compounds and enable the production of evermore effective next-generation antimicrobial agents. Finally, we select a 7-compound subset, showing two lead compounds to exhibit ‘druggable’ profiles through completion of a variety of in vivo and in vitro DMPK studies

    Human immunodeficiency virus continuum of care in 11 european union countries at the end of 2016 overall and by key population: Have we made progress?

    Background. High uptake of antiretroviral treatment (ART) is essential to reduce human immunodeficiency virus (HIV) transmission and related mortality; however, gaps in care exist. We aimed to construct the continuum of HIV care (CoC) in 2016 in 11 European Union (EU) countries, overall and by key population and sex. To estimate progress toward the Joint United Nations Programme on HIV/AIDS (UNAIDS) 90-90-90 target, we compared 2016 to 2013 estimates for the same countries, representing 73% of the population in the region. Methods. A CoC with the following 4 stages was constructed: number of people living with HIV (PLHIV); proportion of PLHIV diagnosed; proportion of those diagnosed who ever initiated ART; and proportion of those ever treated who achieved viral suppression at their last visit. Results. We estimated that 87% of PLHIV were diagnosed; 92% of those diagnosed had ever initiated ART; and 91% of those ever on ART, or 73% of all PLHIV, were virally suppressed. Corresponding figures for men having sex with men were: 86%, 93%, 93%, 74%; for people who inject drugs: 94%, 88%, 85%, 70%; and for heterosexuals: 86%, 92%, 91%, 72%. The proportion suppressed of all PLHIV ranged from 59% to 86% across countries. Conclusions. The EU is close to the 90-90-90 target and achieved the UNAIDS target of 73% of all PLHIV virally suppressed, significant progress since 2013 when 60% of all PLHIV were virally suppressed. Strengthening of testing programs and treatment support, along with prevention interventions, are needed to achieve HIV epidemic control
