    Developing a Back Series of Monthly and Quarterly National Income Estimates for India: 1983Q1 – 1999Q4 (1993-94 = 100)

    In this study we present estimates of monthly and quarterly GDP for India starting 1983-84. Following the recommendations of the CSO (KK,2000) we interpolate annual GDP by economic activity using appropriate physical indications as the basis. Results are compared against the estimates provided by the CSO for the years 1996-97 onwards.

    Presentation and management of bilateral fracture shaft humerus in a multiple myeloma patient: An extremely rare case report and review of literature

    Multiple myeloma (MM) is a malignancy of plasma cells and the second most common hematologic malignancy (13%); these cellsaccumulate in bone marrow and overproduce a monoclonal protein. The bone disease develops in 80–90% of patients with MMincludes bone pain, pathologic fractures (40%), spinal cord compression (5%), and hypercalcemia. Pathological fractures can occurin extremities, but bilateral involvement is extremely rare. We report a case of 65-year-old male presented with a chief complaint ofspontaneous bilateral humerus shaft fractures. On subsequent workup, MM was confirmed. The patient was managed with internalfixation bilaterally and showed favorable union at 1-year follow-up

    Metabolic effects of Olanzapine versus Iloperidone: A 24 weeks randomized, prospective, interventional study

    Atypical antipsychotics have become the mainstay of therapy for psychosis. Though extrapyramidal side effects have been reduced with atypical antipsychotics, yet there are increased concerns over metabolic effects. The present study is aimed to comparatively evaluate the metabolic profile of olanzapine and iloperidone in cases of psychosis. A prospective, randomized, open label, observational study of 6 months duration was conducted in the Department of Pharmacology and Department of Psychiatry, Rohilkhand Medical College and Hospital, Bareilly. A total of 62 patients of both sexes newly diagnosed with psychosis (ICD-10, F20- F29) were included in the study, 31 each in olanzapine and iloperidone groups. Demographic parameters were recorded, following which the patient’s body weight, BMI, fasting blood sugar and lipid profile were estimated at baseline. Follow-up of the patients was done periodically after one month, three months and six months. Olanzapine treated patients showed markedly significant rise in body weight up to 7 kg at the endpoint (p<0.0001) at each follow-up, with a significant increase in BMI. Rise in fasting blood sugar (FBS), total cholesterol (TC), triglycerides (TG) and low-density lipoprotein (LDL) levels werealso statistically significant. At the same time, significant decrease in HDL levels was also observed. Iloperidone treated patients showed statistically significant less rise in body weight (upto 1kg, p<0.05) and BMI. No significant changes in fasting blood sugar, total cholesterol, LDL and HDL levels were noted, while TG levels were significantly reduced. Iloperidone caused numerically less rise in bodyweight and BMI, and fewer metabolic adverse effects as compared to olanzapine, and hence should be preferred.Keywords: Atypical antipsychotics; Weight gain; Blood sugar level; Dyslipidemi

    An inquiry into the productivity of the Indian pharmaceutical industry: application of data envelopment analysis

    Pharmaceutical industry is an important and significant industry in India. The common practice by experts, would-be investors and stakeholders of a pharmaceutical firm is to observe the year-end or quarterly financial figures of a firm and then use them to assess the firm’s future growth and competitive standing against rivals. However, over the past few years, there is a strange environment in which paid consultants and scamsters are making false and conflicting claims in terms of the performance and future growth of the companies. The researchers, however, have attempted, by means of Data Envelopment Analysis (DEA), to get past all this by benchmarking the companies based on the conversion from input to outputs. The advantage of using DEA is that, by simply using the figures from the financial reports, it brings a more rigorous quantitative analysis to make a comparison of the peers with the best virtual firm in their neighborhood. The technique itself may suggest measures for improvement. It is illustrated in the analysis by observing the slacks and targets about various companies of the pharmaceutical sector, i.e., decision making units (DMU).

    Is there a relationship between wound infections and laceration closure times?

    BACKGROUND: Lacerations account for a large number of ED visits. Is there a “golden period” beyond which lacerations should not be repaired primarily? What type of relationship exists between time of repair and wound infection rates? Is it linear or exponential? Currently, the influence of laceration age on the risk of infection in simple lacerations repaired is not clearly defined. We conducted this study to determine the influence of time of primary wound closure on the infection rate. METHODS: This is a prospective observational study of patients who presented to the Emergency Department (ED) with a laceration requiring closure from April 2009 to November 2010. The wound closure time was defined as the time interval from when the patient reported laceration occurred until the time of the start of the wound repair procedure. Univariate analysis was performed to determine the factors predictive of infection. A non-parametric Wilcoxon rank-sum test was performed to compare the median differences of time of laceration repair. Chi-square (Fisher's exact) tests were performed to test for infection differences with regard to gender, race, location of laceration, mechanism of injury, co-morbidities, type of anesthesia and type of suture material used. RESULTS: Over the study period, 297 participants met the inclusion criteria and were followed. Of the included participants, 224 (75.4%) were male and 73 (24.6%) were female. Ten patients (3.4%) developed a wound infection. Of these infections, five occurred on hands, four on extremities (not hands) and one on the face. One of these patients was African American, seven were Hispanic and two were Caucasian (p = 0.0005). Median wound closure time in the infection group was 867 min and in the non-infection group 330 min (p = 0.03). CONCLUSIONS: Without controlling various confounding factors, the median wound closure time for the lacerations in the wound infection group was statistically significantly longer than in the non-infection group

    Hepatoblastoma in an Adult with Biliary Obstruction and Associated Portal Venous Thrombosis

    We present a case of adult hepatoblastoma. This young female presented with severe acute cholangitis. Preoperative diagnosis was common bile duct (CBD) obstruction with portal vein thrombosis. On exploration she had a tumor mass in the CBD. The unusual features of this case are discussed in this report

    Molecular surveillance of Dengue Virus (DENV) and its co-infection with Chikungunya Virus (CHIKV) among febrile patients: A comparative study from South Delhi, India

    Dengue and Chikungunya are two major arboviral infections transmitted worldwide by the mosquitoes, Aedes aegypti and Ae. albopictus. India suffers enormously with both Dengue and Chikungunya as they pose a great public health challenge. The present study aims to evaluate the prevalence of Dengue Virus (DENV), Chikungunya Virus (CHIKV) and DENV/CHIKV co-infection (by Reverse-Transcriptase Polymerase Chain Reaction (RT-PCR)/Enzyme Linked Immunosorbent Assay (ELISA), their clinical features, DENV serotypes and CHIKV specific Immunoglobulin G (IgG) within a 7 years gap in the Delhi population. The study sample included clinically suspected febrile patients (?7 days) sera collected during 2017-2018 (n=87) and during 2008-2010 (n=623) from Delhi. Captured ELISA was performed for CHIKV IgG screening and nested PCR was done for DENV serotyping. The percentage prevalence for DENV was significantly higher than CHIKV with 41.38% (n=87) and 16.1% (n=87), respectively; interestingly, DENV/CHIKV co-infection was detected in 10.34% (n=9/87) cases during 2017-2018. Similarly, a high DENV prevalence was observed during 2008-2010 with the prevalence rate of 38.3% (69/180),  34.65% (35/101) and 47.07% (161/342), respectively. DENV 1 and DENV 3 were dominant serotype during 2008-2010 and 2017-2018 respectively. We have noticed a high prevalence (36.67%, 22/60) of the CHIKV IgG antibody in the 2017-2018 samples. Joint pain was more preferential to CHIKV mono-infection and DENV/CHIKV co-infection compared to DENV mono-infection. The present study highlights the need for active surveillance simultaneously for both DENV and CHIKV and to evaluate the role of CHIKV/DENV co-infections in disease severity in the endemic regions.

    Role of Serine/Threonine Kinase 11 (STK11) or liver kinase B1 (LKB1) Gene in Peutz-Jeghers Syndrome

    Peutz-Jeghers syndrome (PJS) is a well-described inherited syndrome, characterized by the development of gastrointestinal polyps and characteristic mucocutaneous freckling. PJS is an autosomal prevailing disease, due to genetic mutation on chromosome 19p, manifested by restricted mucocutaneous melanosis in association with gastrointestinal (GI) polyposis. The gene for PJS has recently been shown to be a serine/threonine kinase, known as LKB1 or STK11, which maps to chromosome subband 19p13.3. This gene has a putative coding region of 1302 bp, divided into nine exons, and acts as a tumor suppressor in the hamartomatous polyps of PJS patients and in the other neoplasms that develop in PJS patients. It is probable that these neoplasms develop from hamartomas, but it remains possible that the LKB1 or STK11 locus plays a role in a different genetic pathway of tumor growth in the cancers of PJS patients.
