    Optimising the conceptualisation of context; comment on "Stakeholder Perspectives of Attributes and Features of Context Relevant to Knowledge Translation in Health Settings: A Multi-Country Analysis"

    Context matters. Therefore, efforts to develop greater conceptual clarity are important for science and practice. In this commentary, we outline some key issues that were prompted by Squire’s et al.’s contribution. Specifically, we reinforce context as an interactive concept and therefore something that is hard to ‘pin down’, the problematic nature of conceptualising context in implementation and de-implementation, and a requirement for the development of culturally sensitive understandings. Finally, we suggest it is vital that continued investment into providing a more comprehensive list of determinants needs to be accompanied by an equal effort in developing practical methods and tools to support use and application

    Resilience of the Internet of Things (IoT) from an Information Assurance (IA) Perspective

    Internet infrastructure developments and the rise of the IoT Socio-Technical Systems (STS) have frequently generated more unsecure protocols to facilitate the rapid intercommunication between the plethoras of IoT devices. Whereas, current development of the IoT has been mainly focused on enabling and effectively meeting the functionality requirement of digital-enabled enterprises we have seen scant regard to their IA architecture, marginalizing system resilience with blatant afterthoughts to cyber defence. Whilst interconnected IoT devices do facilitate and expand information sharing; they further increase of risk exposure and potential loss of trust to their Socio-Technical Systems. A change in the IoT paradigm is needed to enable a security-first mind-set; if the trusted sharing of information built upon dependable resilient growth of IoT is to be established and maintained. We argue that Information Assurance is paramount to the success of IoT, specifically its resilience and dependability to continue its safe support for our digital economy

    Use of probiotic Lactobacillus preparation to prevent diarrhoea associated with antibiotics: randomised double blind placebo controlled trial

    Objective To determine the efficacy of a probiotic drink containing Lactobacillus for the prevention of any diarrhoea associated with antibiotic use and that caused by Clostridium difficile. Design Randomised double blind placebo controlled study. Participants 135 hospital patients (mean age 74) taking antibiotics. Exclusions included diarrhoea on admission, bowel pathology that could result in diarrhoea, antibiotic use in the previous four weeks, severe illness, immunosuppression, bowel surgery, artificial heart valves, and history of rheumatic heart disease or infective endocarditis. Intervention Consumption of a 100 g (97 ml) drink containing Lactobacillus casei, L bulgaricus, and Streptococcus thermophilus twice a day during a course of antibiotics and for one week after the course finished. The placebo group received a longlife sterile milkshake. Main outcome measures Primary outcome: occurrence of antibiotic associated diarrhoea. Secondary outcome: presence of C difficile toxin and diarrhoea. Results 7/57 (12%) of the probiotic group developed diarrhoea associated with antibiotic use compared with 19/56 (34%) in the placebo group (P=0.007). Logistic regression to control for other factors gave an odds ratio 0.25 (95% confidence interval 0.07 to 0.85) for use of the probiotic, with low albumin and sodium also increasing the risk of diarrhoea. The absolute risk reduction was 21.6% (6.6% to 36.6%), and the number needed to treat was 5 (3 to 15). No one in the probiotic group and 9/53 (17%) in the placebo group had diarrhoea caused by C difficile (P=0.001). The absolute risk reduction was 17% (7% to 27%), and the number needed to treat was 6 (4 to 14). Conclusion Consumption of a probiotic drink containing L casei, L bulgaricus, and S thermophilus reduce the incidence of antibiotic associated diarrhoea and C difficile associated diarrhoea. This has the potential to decrease morbidity, healthcare costs, and mortality if used routinely in patients aged over 50

    The Effect of Walking an Unfamiliar Versus Companion Dog on Mood, Exercise Enjoyment, and Heart Rate: A Pilot Field Study

Background: Walking unfamiliar dogs, such as therapy dogs, has been shown to improve physiological exercise responses and exercise adherence, but whether walking a companion dog results in superior benefits is currently unknown. The purpose of the current pilot field study was to elucidate preliminary evidence of how walking an unfamiliar or companion dog influences mood, exercise enjoyment, and heart rate during a 1.5-mile walk. Methods: Participants (n=8) walked 1.5-miles at their own pace with an unfamiliar or companion dog while mood, exercise enjoyment, and heart rate were measured. Point of application #1: Walking an unfamiliar dog resulted in improved pre- to post- exercise mood changes compared to walking their own companion dog. Point of application #2: Enjoyment of exercise was higher while walking the unfamiliar dog compared to the companion dog. Point of application #3: Mean exercise heart rate was significantly higher while walking the unfamiliar versus companion dog although time to completion of the 1.5-miles was unaffected

    Acute Caffeine Ingestion Increases Velocity and Power in Upper and Lower Body Free-Weight Resistance Exercises

    International Journal of Exercise Science 12(2): 1280-1289, 2019. The purpose of this study was to examine the acute effects of caffeine supplementation on velocity and power output during bench press and back squat exercises. Resistance trained males (n = 12) consuming less than 300 mg of caffeine daily, were recruited for this study. In a blinded crossover study design, participants supplemented with 6 mg· kg-1 caffeine or placebo (placebo, gluten-free cornstarch) 60 min prior to exercise. Participants completed 3 × 1 repetition with maximum explosive intent at 80% of their 1-RM for bench and squat exercises with two minute rest periods between each repetition. A linear position transducer was used to measure power and velocity of barbell movement. Each trial was separated by a 72 h washout period. Results indicated that mean velocity (p = 0.027; ES = 1.04) and mean power (p = 0.008; ES = 0.24) were higher during bench press exercise with caffeine versus placebo. Furthermore, mean velocity (p = 0.005; ES=1.06) and mean power (p = 0.020; ES = 0.71) values were higher for back squat exercise with caffeine versus placebo. This study suggests that caffeine ingestion imposes ergogenic benefits by increasing velocity and power in both upper and lower body resistance exercises. However, caffeine had a larger effect on lower body power output versus upper body exercise. Results may hold important implications for using caffeine during training
