    Evaluaci?n del efecto analg?sico de la lidoca?na v?a intratesticular e intrafunicular en perros sometidos a orquiectomia electiva bajo un protocolo de anestesia disociativa

Recurso Electr?nicoOBJETIVOS: Evaluar el efecto analg?sico de la lidoca?na administrada v?a intratesticular e intrafunicular en perros sometidos a orquiectom?a electiva bajo un protocolo de anestesia disociativa. M?TODOS: Ensayo cl?nico prospectivo, doble ciego, aleatorizado. Animales: 31 caninos machos cl?nicamente sanos con edades comprendidas entre los 0,5 a 10 a?os y pesos entre los 6 a 30 kg. Los animales fueron incluidos al azar mediante un sistema de sobre cerrado dentro de uno de los siguientes grupos: Grupo 1 lidoca?na intratesticular (n=11); Grupo 2 control (n=10) y Grupo 3 lidoca?na intrafunicular (n=10). Los animales del grupo 1 y 3 recibieron 2 mg/kg de lidoca?na y los animales del grupo control recibieron soluci?n salina v?a intratesticular en un volumen equivalente al empleado de la lidoca?na para cada caso. Lo anterior como parte de un protocolo de anestesia balanceada que tambi?n incluy? acepromacina, meloxicam, tramadol y ketamina. Las variables cardiorrespiratorias: frecuencia cardiaca, frecuencia respiratoria y presi?n arterial media, as? como y los consumos intraoperatorios de ketamina y fentanilo (mg/kg) fueron evaluados mediante un ANOVA como valores porcentuales con respecto al tiempo basal pos inducci?n (5 minutos). RESULTADOS Y CONCLUSIONES: Los resultados del presente estudio permiten sugerir que la infiltraci?n de lidoca?na por cualquiera de las dos v?as evaluadas contribuye a disminuir el consumo de analg?sicos (p=0,02) y anest?sicos (p=0,001) intraoperatorios, disminuir significativamente la respuesta nociceptiva aut?noma del paciente durante los momentos m?s dolorosos del procedimiento (p=0,04) y reducir el n?mero de complicaciones intraoperatorias asociadas. Palabras clave: analgesia, anestesia local, castraci?n, lidoca?na, orquiectom?a, perro.OBJECTIVE: To evaluate the analgesic effect of lidocaine administered via intratesticular and intrafunicular in dogs undergoing elective orchiectomy under dissociative anesthesia protocol. METHODS: A prospective, double-blind, randomized. Animals: 31 male dogs clinically healthy aged 0.5 to 10 years old and weight between 6 to 30 kg. The animals were randomly included with sealed system within one of the following experimental groups: Group 1 intratesticular lidocaine (n = 11); Group 2 Control (n = 10) and Group 3 intrafunicular lidocaine (n = 10). The animals in group 1 and 3 received 2 mg/kg of lidocaine and control animals received saline via intratesticular an equivalent employee of lidocaine for each volume. This as part of a balanced anesthesia protocol also included acepromazine, meloxicam, tramadol and ketamine. Cardiorespiratory variables: heart rate, respiratory rate and blood pressure as well as intraoperative and consumption of ketamine and fentanyl (mg / kg) were evaluated by ANOVA as percentage values relative to baseline after induction time (5 minutes). RESULTS AND CONCLUSIONS: The results from this study suggest that infiltration of lidocaine evaluated either way helps to reduce the consumption of analgesics (p = 0.02) and anesthetic (p = 0.001) intraoperative significantly reduce the response nociceptive independent of the patient during the most painful moments of the procedure (p = 0.04) and reduce the number of intraoperative complications associated.     Long-Term Influence of the Practice of Physical Activity on the Self-Perceived Quality of Life of Women with Breast Cancer: A Randomized Controlled Trial.

    Background: There is still no consensus on the most suitable interventions for exercise practice in breast cancer survivors. Therefore, the aim of this study was to evaluate the effect of a two-year physical activity intervention (strength, aqua fitness and aerobic exercise programs) on the self-perceived quality of life and physical functionality of female breast cancer survivors. Methods: A randomized, controlled, experimental trial with a sample of 316 women (63 +/- 7 years), who had been diagnosed with breast cancer. The evaluations were performed using the Rikli & Jones Senior Fitness Test, and the Short Form 12 Health Survey (SF-12). Results: The participants in the strength program showed statistically significant improvements in all the items of the SF-12. The aqua fitness program obtained significant improvements in Physical Functioning and Limitations, Pain and Emotional Limitations, General Health, Vitality, Social Functioning and the physical and mental components of the SF-12. The participants in the aerobic program showed a progressive deterioration of Vitality and Mental Health. Conclusion: When assigning breast cancer survivors to an exercise program, the preferential or predominant activity should include strength exercises. On the other hand, as the second choice, those patients with particularly low levels of Vitality or Physical Limitations will show greater improvement with an aqua fitness program

    Development of a New Control Algorithm for Automatic Irrigation Scheduling in Soilless Culture

    In soilless culture, water and nutrients must be frequently and precisely applied due to the reduced volume and low water holding capacity of the substrate. For this reason, irrigation scheduling is an important and difficult task. The use of an irrigation control tray with two electrodes (level sensor) is a simple way for controlling irrigation. Irrigation operation is triggered when the water level in the tray decreases below a preset level. This is a simple on-off control but inflexible because it requires periodic manually calibrations. This work aims at developing a more efficient control system for the irrigation management of soilless culture. The control system is based on a Proportional Integral Derivative (PID) algorithm and it allows for fully automatic operation with a minimum set of variables in order to reduce the cost of the equipment. The results obtained demonstrate that the PID control algorithm can efficiently be used to control irrigation in soilless culture. The calculated average daily leaching fractions fit reasonably well to the target values. Nevertheless, some improvements of the control algorithm are required to reduce the variability of the calculated leaching fractions during the initial stage of crop development

    Increased Serum Levels of sCD14 and sCD163 Indicate a Preponderant Role for Monocytes in COVID-19 Immunopathology

    Background: Emerging evidence indicates a potential role for monocytes in COVID-19 immunopathology. We investigated two soluble markers of monocyte activation, sCD14 and sCD163, in COVID-19 patients, with the aim of characterizing their potential role in monocyte-macrophage disease immunopathology. To the best of our knowledge, this is the first study of its kind. Methods: Fifty-nine SARS-Cov-2 positive hospitalized patients, classified according to ICU or non-ICU admission requirement, were prospectively recruited and analyzed by ELISA for levels of sCD14 and sCD163, along with other laboratory parameters, and compared to a healthy control group. Results: sCD14 and sCD163 levels were significantly higher among COVID-19 patients, independently of ICU admission requirement, compared to the control group. We found a significant correlation between sCD14 levels and other inflammatory markers, particularly Interleukin-6, in the non-ICU patients group. sCD163 showed a moderate positive correlation with the time lapsed from admission to sampling, independently of severity group. Treatment with corticoids showed an interference with sCD14 levels, whereas hydroxychloroquine and tocilizumab did not. Conclusions: Monocyte-macrophage activation markers are increased and correlate with other inflammatory markers in SARS-Cov-2 infection, in association to hospital admission. These data suggest a preponderant role for monocyte-macrophage activation in the development of immunopathology of COVID-19 patients

    Psychiatric comorbidities in Asperger syndrome are related with polygenic overlap and differ from other Autism subtypes

    There is great phenotypic heterogeneity within autism spectrum disorders (ASD), which has led to question their classification into a single diagnostic category. The study of the common genetic variation in ASD has suggested a greater contribution of other psychiatric conditions in Asperger syndrome (AS) than in the rest of the DSM-IV ASD subtypes (Non_AS). Here, using available genetic data from previously performed genome-wide association studies (GWAS), we aimed to study the genetic overlap between five of the most related disorders (schizophrenia (SCZ), major depression disorder (MDD), attention deficit hyperactivity disorder (ADHD), obsessive-compulsive disorders (OCD) and anxiety (ANX)), and AS, comparing it with the overlap in Non_AS subtypes. A Spanish cohort of autism trios (N = 371) was exome sequenced as part of the Autism Sequencing Consortium (ASC) and 241 trios were extensively characterized to be diagnosed with AS following DSM-IV and Gillberg's criteria (N = 39) or not (N = 202). Following exome imputation, polygenic risk scores (PRS) were calculated for ASD, SCZ, ADHD, MDD, ANX, and OCD (from available summary data from Psychiatric Genomic Consortium (PGC) repository) in the Spanish trios' cohort. By using polygenic transmission disequilibrium test (pTDT), we reported that risk for SCZ (Pscz = 0.008, corrected-PSCZ = 0.0409), ADHD (PADHD = 0.021, corrected-PADHD = 0.0301), and MDD (PMDD = 0.039, corrected-PMDD = 0.0501) is over-transmitted to children with AS but not to Non_AS. Indeed, agnostic clustering procedure with deviation values from pTDT tests suggested two differentiated clusters of subjects, one of which is significantly enriched in AS (P = 0.025). Subsequent analysis with S-Predixcan, a recently developed software to predict gene expression from genotype data, revealed a clear pattern of correlation between cortical gene expression in ADHD and AS (P < 0.001) and a similar strong correlation pattern between MDD and AS, but also extendable to another non-brain tissue such as lung (P < 0.001). Altogether, these results support the idea of AS being qualitatively distinct from Non_AS autism and consistently evidence the genetic overlap between AS and ADHD, MDD, or SCZ

    This article presents a study of the perceived risk in the construction sector. The study was conducted from the analysis of questionnaires from several samples of construction workers from southern Spain. We report the perceived risk profile obtained from the psychometric paradigm through qualitative attributes and then we analyze the results. Stands out the attribute regarding how work affects our own long-term health. This represents something new with respect to previous studies. Also analyzed by analysis of variance (ANOVA) the effects of socio-demographic variable craft with respect to the different attributes of the perceived risk of the questionnaire. It presents the differences obtained between the group of masons and framers.Este art?culo presenta un estudio acerca del riesgo percibido en el sector de la construcci?n. El estudio se realiz? a partir del an?lisis de cuestionarios provenientes de una muestra de trabajadores de la construcci?n del sur de Espa?a. Se presenta el perfil del riesgo percibido obtenido seg?n los enfoques del llamado paradigma psicom?trico a trav?s de atributos cualitativos y se analizan los resultados.     NS5806 Induces Electromechanically Discordant Alternans and Arrhythmogenic Voltage-Calcium Dynamics in the Isolated Intact Rabbit Heart

    Background: NS5806 activates the transient outward potassium current I to, and has been claimed to reproduce Brugada Syndrome (BrS) in ventricular wedge preparations. I to modulates excitation-contraction coupling, which is critical in alternans dynamics. We explored NS5806-arrhythmogenic effects in the intact whole heart and its impact on alternans. Methods: Langendorff-perfused rabbit hearts (n = 20) underwent optical AP and Ca mapping during pacing at decremental cycle lengths (CL). Spontaneous arrhythmias and pacing-induced alternans was characterized at baseline (BL), after perfusing with NS5806, before and after adding verapamil (VP), and SEA0400 (SEA, n = 5 each), to modulate Ca-current and Na-Ca exchange, the main AP-Ca coupling mechanisms. Results: NS5806 induced BrS-like ECG features in 6 out of 20 hearts. NS5806 prolonged steady-state (3 Hz) action potential duration (APD) by 16.8%, Ca decay constant by 34%, and decreased conduction velocity (CV) by 52.6%. After NS5806 infusion, spontaneous ventricular ectopy (VE) and AP/Ca alternans occurred. Pacing-induced alternans during NS5806 infusion occurred at longer CL and were AP/Ca discordant from its onset. Spatially discordant alternans after NS5806 infusion had non-propagation-driven nodal line distribution. No spontaneous phase-2 reentry occurred. Under NS5806 + VP, alternans became AP/Ca concordant and only induced in two out of five; NS5806 + SEA did not affect alternans but suppressed spontaneous ectopy. Conclusions: NS5806 disrupts AP-Ca coupling and leads to Ca-driven, AP/Ca-discordant alternans and VE. Despite BrS-like ECG features, no spontaneous sustained arrhythmias or phase-2 reentry occurred. NS5806 does not fully reproduce BrS in the intact rabbit heart

    New Recurrent Structural Aberrations in the Genome of Chronic Lymphocytic Leukemia Based on Exome-Sequencing Data

    Chronic lymphocytic leukemia (CLL) is the most frequent lymphoproliferative syndrome in Western countries, and it is characterized by recurrent large genomic rearrangements. During the last decades, array techniques have expanded our knowledge about CLL's karyotypic aberrations. The advent of large sequencing databases expanded our knowledge cancer genomics to an unprecedented resolution and enabled the detection of small-scale structural aberrations in the cancer genome. In this study, we have performed exome-sequencing-based copy number aberration (CNA) and loss of heterozygosity (LOH) analysis in order to detect new recurrent structural aberrations. We describe 54 recurrent focal CNAs enriched in cancer-related pathways, and their association with gene expression and clinical evolution. Furthermore, we discovered recurrent large copy number neutral LOH events affecting key driver genes, and we recapitulate most of the large CNAs that characterize the CLL genome. These results provide "proof-of-concept" evidence supporting the existence of new genes involved in the pathogenesis of CLL
