    Diminishing Incontinence in Long-Term Care using Electronic Health Records

    Urinary incontinence affects up to 70% of residents living in a long-term care facility and can affect their quality of life. Specifically, urinary incontinence has a direct impact on older adults in regards to self-esteem, pressure ulcer development, falls, urinary tract infections, and psychosocial wellbeing. The goal of this quality improvement pilot project was to determine if an electronic health record (EHR) assessment tool could help older adults remain continent longer and assist in maintaining an independent lifestyle. Orem\u27s self-care deficit theory and social cognitive theory were used to determine how the electronic health record incontinence template could be used to monitor residents for incontinence and affect the incidence of incontinence. Out of 25 residents, 13 met the requirements for inclusion in the pilot study. Quantitative data were collected and documented in the EHR for 4 weeks and compared to the immediate 4 week period post-implementation of the EHR template. Descriptive analyses of pre- and post-implementation EHR assessments showed there were no EHR assessments completed pre-implementation and 2 residents out of 13 had EHR assessments completed post-implementation. The available data suggested that the EHR template, if edited, could be effective for tracking incontinence. The template needed to address bladder incontinence only rather than bowel and bladder. Feedback from nursing staff indicated that a future study should be conducted over a longer period than 4 weeks to see if results would remain consistent. Nurses working in the long term care environment would benefit from reading this project. This study contributes to social change as evidenced by the residents who remained continent longer by having individual toileting plans partially developed by the template; therefore, they remained a viable part of the community

    Inequality in action: entitlement and estimated time of arrival request in emergency calls

    This research report is submitted to the Faculty of Humanities, University of the Witwatersrand, Johannesburg, in partial fulfilment of the degree of Masters of Arts in Research Psychology by Dissertation and Coursework.In this study an inductive approach is taken to analysing emergency calls from a government operated emergency medicine call centre in the Western Cape, South Africa. This ethnomethodologically informed study’s analytic approach is that of conversation analysis and membership categorisation analysis. During analysis specific attention is paid to the ending sequence of calls and in particular asymmetries in displays of entitlement in the context of estimated time of arrival (ETA) requests in the emergency calls. I answer two research questions; i) how do callers display entitlement through ETA requests in emergency calls? and ii) how are social asymmetries displayed in ETA requests in emergency calls? Nine excerpts are analysed in detail to demonstrate systematic interactional machinery at play. Asymmetries in entitlement are found to be collaborative accomplishments of the participants in the calls and may serve as a mechanism for reproducing material inequalities in everyday interaction. For developing nations, where there is severe material inequality, inequality can be understood to be an institutional structure that not only affects the material existence of members of society but the everyday interactions society members engage in

    Les mouvements passifs: un traitement de choix chez la clientèle blessée médullaire?

    Travail d'intégration présenté à la Faculté de médecine en vue de l’obtention du grade de maîtrise professionnelle en physiothérapieIntroduction/problématique : Chez la clientèle blessée médullaire, les mouvements passifs (MP) sont utilisés d’emblée en physiothérapie pour réduire les contractures, diminuer la spasticité et contrôler la douleur. Cependant, il n'existe actuellement aucun consensus en ce qui concerne l'efficacité, les paramètres d'application ainsi que les sites d'administration prioritaires de cette modalité. Objectif: À la demande de l'hôpital Sacré-Coeur, un guide destiné aux proches aidants sera réalisé expliquant les méthodes d'application adéquates des MP auprès de la clientèle blessée médullaire. Méthodologie : Recension des écrits à l’aide de bases de données reconnues. Résultats : Les MP à prioriser sont : extension du coude, pro-supination, flexion, abduction et rotation externe d’épaule, extension du poignet et du genou, flexion, extension et abduction de hanche et finalement flexion dorsale de cheville. Les paramètres d'application des MP ne sont pas établis. Selon un consensus, les mouvements doivent être effectués quotidiennement et durant quelques répétitions à chaque articulation. Par contre, les études actuelles ne démontrent pas de résultats significatifs en faveur de l'augmentation des amplitudes articulaires, de la gestion de la douleur et de la diminution de la spasticité par l’application des MP. L’échelle numérique de douleur, la goniométrie et une combinaison d’outils de mesure évaluant la spasticité peuvent être utilisées pour mesurer ces différentes variables. Conclusion : Il est recommandé d’utiliser les MP en complémentarité avec d’autres modalités. De plus amples études devront être effectuées au sujet des MP afin de clarifier les paramètres d’utilisation, leur efficacité ainsi que les outils de mesure à utiliser
