    Prevalence and determinants of child undernutrition and stunting in semiarid region of Brazil

    OBJETIVO : Analisar tendências na prevalência e determinantes da desnutrição em crianças na região semiárida do Brasil. MÉTODOS : Foram analisados dados de duas pesquisas transversais domiciliares de base populacional que utilizaram a mesma metodologia. A amostragem por conglomerados foi utilizada para coletar os dados de 8.000 famílias, do estado do Ceará, Nordeste do Brasil, para os anos de 1987 e 2007. A desnutrição aguda foi calculada como peso/idade < -2 desvios padrão; nanismo como altura/idade < -2 desvios padrão; e emaciação como peso/altura < -2 desvios padrão. Os dados sobre os determinantes biológicos e sociodemográficos foram analisados por meio de análises multivariadas com base em um modelo teórico hierarquizado. RESULTADOS : Amostras de 4.513 e 1.533 crianças menores de três anos de idade, em 1987 e 2007, respectivamente, foram incluídas nas análises. A prevalência de desnutrição aguda foi reduzida em 60,0%, passando de 12,6% em 1987, para 4,7% em 2007, enquanto a prevalência de nanismo foi reduzida em 50,0%, passando de 27,0% em 1987 para 13,0% em 2007. A prevalência de emaciação teve pouca alteração no período. Em 1987, as características socioeconômicas e biológicas (renda familiar, escolaridade da mãe, disponibilidade de latrina e água potável, consulta médica e hospitalização da criança, idade, sexo e peso ao nascer) foram fatores significativamente associados à desnutrição, ao nanismo e à emaciação. Em 2007, os determinantes da desnutrição ficaram restritos às características biológicas (idade, sexo e peso ao nascer). Apenas uma característica socioeconômica, a disponibilidade de latrina, permaneceu significantemente associada ao nanismo. CONCLUSÕES : O desenvolvimento socioeconômico, além de intervenções de saúde, parecem ter efetivamente contribuído para a melhoria do estado nutricional das crianças. Peso ao nascer, especialmente o peso extremamente baixo (< 1.500 g), aparece como o fator de risco mais importante para a desnutrição na primeira infância.OBJECTIVE : To analyze the evolution in the prevalence and determinants of malnutrition in children in the semiarid region of Brazil. The prevalence of acute malnutrition was reduced by 60.0%, from 12.6% in 1987 to 4.7% in 2007, while prevalence of stunting was reduced by 50.0%, from 27.0% in 1987 to 13.0% in 2007. Prevalence of wasting changed little in the period. In 1987, socioeconomic and biological characteristics (family income, mother’s education, toilet and tap water availability, children’s medical consultation and hospitalization, age, sex and birth weight) were significantly associated with undernutrition, stunting and wasting. In 2007, the determinants of malnutrition were restricted to biological characteristics (age, sex and birth weight). Only one socioeconomic characteristic, toilet availability, remained associated with stunting. CONCLUSIONS : Socioeconomic development, along with health interventions, may have contributed to improvements in children’s nutritional status.     Burns are frequent accidents and represent the second cause of death in childhood. The aim of this study was to identify the profile of accidents with children burned by hot liquids. This was a descriptive study conducted with 62 parents of interned children from a Burn Treatment Center in Fortaleza, Ceara, during the period of January to June, 2002. The studied variables were: age at the time of the accident, the sex, the substance that caused the burn and specific characteristics of the burn. As a result, there was a prevalence of the age group 1 to 2 years old with 30 (48.4%) admissions; 34 (54.8%) were male. Hot water was responsible for 23 (37.1%) cases and the second degree burns prevailed in 62 (62%) cases. Regarding to affected body parts, it was found that the torso was burned in 49 (28.8%) children. At the time of the thermal accident, 51 (82.3%) children were in their mothers’ company; after the incident, 36 (59.6%) children were immediately taken to the hospital. Concerning educational guidelines on prevention of burns, 48 (77.4%) parents reported not having received them from the health team. It was concluded that the profile of accidents with children burned by hot liquids at this Reference Center consists of children in first childhood, male, mainly with second degree lesions in the torso, which occurred while in their mother’s companyEste trabalho objetivou identificar o perfil dos acidentes em crianças queimadas por líquidos aquecidos. Estudo descritivo, realizado com 62 pais de crianças internadas em um Centro de Tratamento para Queimados, em Fortaleza, Ceará, no período de janeiro a junho de 2002. As variáveis estudadas foram: a idade do acidente, sexo, substância causadora da queimadura, e ascaracteristicas específicas da queimadura. Como resultado, houve prevalecencia o grupo de 1 a 2 anos com 30 (48,4%) internamentos, 34 (54,8%) eram do sexo masculino; a água quente foi responsável por 23 (37,1%) casos e predominaram as lesões de 2º grau com 62 (62%) ocorrências. No que se refere às regiões corporais atingidas, identificou-se que o tronco se apresentou queimado em 49 (28,8%) crianças. No momento do acidente térmico, 51 (82,3%) crianças estavam acompanhadas pela mãe; após o incidente, 36 (59,6%) crianças foram levadas imediatamente ao hospital. Quanto às orientações educativas na prevenção de queimaduras duarante o periodo de internamento, 48 (77,4%) pais referiram que não as receberam pela equipe de saúde. Concluiu-se que o perfil do acidente infantil com líquidos aquecidos neste Centro de Referência consiste de crianças na primeira infância, do sexo masculino, principalmente com lesões de segundo grau em tronco, as quais ocorreram enquanto na companhia de suas mães

    Spontaneous and induced abortion trends and determinants in the Northeast semiarid region of Brazil: a transversal series

    Abstract Objectives: this study intends to estimate the rates, associated factors and trends of selfreported abortion rates in the northeast of Brazil. Methods: series of population-based surveys realized in Ceará, northeast of Brazil, one of the poorest states in the country. A sample of about 27,000 women of reproductive age was used. Abortion was assessed according to women´s information and rates were calculated using official population estimates. The trends and the association among socioeconomic and reproductive factors were studied using regressive models. Results: a trend for reduction in rates was identified. For induced abortion, the determinants were: not having a partner, condom in the last sexual intercourse, first child up to 25years old (AOR= 5.21; ACI: 2.9 – 9.34) and having less than 13years old at first sexual intercourse (AOR= 5.88; ACI: 3.29 – 10.51). For spontaneous abortion were: having studied less than 8 years, knowledge and use of morning-after pill (AOR= 26.44; ACI: 17.9 – 39.05) and not having any children (AOR= 3.43). Conclusions: rates may have been low due to self-reporting. Young age and knowledge about contraceptive methods were associated to both kinds of abortion, while education level along with spontaneous and marital status with induced. Programs to reduce abortion rates should focus on single younger women with low education

    Metodologia das Pesquisas Populacionais de Saúde Materno-Infantil: uma série transversal realizada no Estado do Ceará de 1987 a 2007

    Objectivo: descrever os métodos utilizados nas Pesquisas de Saúde Materno-Infantil (PESMIC) no Ceará, durante 20 anos. Métodos: a série de inquéritos realizados no período de 20 anos foi analisada nos seguintes aspectos: a) Estudos transversais em série de base populacional, com amostra representativa de 8000 famílias; b) Utilizou-se questionários e medições antropométricas, estudando-se níveis e causas de mortalidade infantil, prevalência de desnutrição da mãe e da criança, duração da amamentação, cobertura vacinal, prevalência e manejo da diarreia e acesso aos serviços de saúde materno-infantil; c) Obteve-se como resultado as variáveis desfechos (principal mortalidade infantil) e diversos determinantes materno-infantis e socioeconómicos. Resultados: o índice de resposta foi de quase 100% nos cinco inquéritos realizados (1986-2007), e ocorreram importantes mudanças nos indicadores sociodemográficos e de saúde; redução de 81% da taxa de mortalidade infantil, aumento de 43% da taxa de amamentação; índice de imunização de 95%. Observou-se aumento de 243% do índice de sobrepeso. Foram entrevistados no total somado dos cinco estudos: 177.132 residentes, sendo 47.508 mulheres e 13.049 crianças. Conclusões: recomenda-se a continuidade desses estudos utilizando-se a mesma metodologia, possibilitando analises de variações temporais

    The relation of cash transfer programs and food insecurity among families with preschool children living in semiarid climates in Brazil

    <div><p>Abstract Background Food insecurity has important effects on human health, particularly in children’s. It continues to increase, with an estimated prevalence of 14.9% in the USA and 35% in Brazil. There have been few studies on the effect of cash transfer programs (CTPs) on the prevalence of food security in Brazil. Objective Evaluate the association between cash transfer programs and reductions in inequity and food insecurity. Method Population-based cross-sectional study in the state of Ceará, Northeast Brazil, with a sample of 8.000 households. Ceará is one of the poorest states. The state population of 8.5 million inhabitants, social security benefits and government grants, “ Bolsa Família”, have become the most stable source of income. The main outcomes measures were food insecurity and CTP participation. Multivariate logistic models were constructed to assess the association between participation in CTPs and food security. Results Participation in CTPs was found to be independently related to the prevalence of food security (APR 2.29 95% CI 1.57-3.33), as are education level, residential setting, and children’s nutritional status. Conclusions CTPs and investment in education are initiatives that might be used to reduce food insecurity.</p></div
