    Management and Complications of Arnold Chiari Hydrocephalus at Tertiary Health Care Center

    Objective: to investigate the role of CT and MRI in diagnosis of Arnold chiari malformation of hydrocephalus and its treatment with VP shunt along with complications. Study Design: Prospective study Place and duration: Department of Neurosurgery, DG Khan Medical College from May 4, 2018 to May 4, 2019. Methodology: Fifty patients of congenital hydrocephalus who were not treated previously were selected. Detailed history about disease and clinical examination of patients was performed. Follow ups were done at neurosurgery OPD. SPSS software for data analysis was used and mean ± SD, frequency and percentages were calculated for variables. P value ≤0.05 was considered as significant. Results: Treatment in case of congenital hydrocephalus as VP shunt, intra-aneurysmal coiling, excision of meningomyelocele with VP shunt and suboccipital craniectomy + upper cervical laminectomy (scucl) were observed as 10%, 6%, 8% and 8% respectively.  Twelve percent of patients were not treated. Conclusion: CT and MRI are the main diagnostic tools for diagnosis of Arnold chiari malformation and VP shunt is the treatment of choice.     The local wisdom values of the Acehnese have proven to be used as a mechanism in conflict resolution. The problem of conflict is not only the social reality of the people of Aceh, but it has become a problem for the world community. Many conflicts occur in communities that require deep knowledge and broad insight into adat so that they can be resolved through deliberation and consensus. Knowledge of local wisdom and insight into customary-based conflict resolution is not evenly distributed among gampong apparatus, therefore this community service aims to strengthen the role of local wisdom-based conflict resolution to create peace in Gampong Meunasah Mesjid, Muara Dua sub-district, Lhokseumawe City. The method used is strengthening and capacity building. Stages of implementation, problem identification, program implementation in the form of training. So that it is hoped that capacity and knowledge development will be achieved, improvement of the social and cultural values of the community. The benefits and impacts of this training are that the community will gain a strengthened understanding of customary values and local wisdom that can be used as a means of conflict resolution in the national legal system; strengthening understanding of customary values and local wisdom according to Islamic law; the establishment of partnerships, friendship and cooperation between the campus and the community in maintaining peace in the community; preservation and improvement of values and the role of conflict resolution in sociological and cultural contexts in society

S(5) and S(7) rings are formed due to intra­molecular N—H⋯Cl and N—H⋯O hydrogen bonds, respectively. In the crystal, N—H⋯O hydrogen bonds link the mol­ecules into C(8) chains along the a axis. Further C—H⋯O and N—H⋯O hydrogen bonds link these chains in pairs, forming a polymeric network

    An area-optimized N-bit multiplication technique using N/2-bit multiplication algorithm

    A unique design for an optimized N-bit multiplier is proposed and implemented which utilizes a modified divide-and-conquer technique. The conventional technique requires four N/2-bit multipliers to perform N-bit multiplication, whereas the proposed design uses only one multiplier module in hardware to perform the functionality of four modules. It uses Dadda algorithm in its multiplier module. It has been implemented using Verilog HDL, and a good accuracy of results was observed in simulations which effectively verify its functionality. Design was also synthesized on various FPGAs including Spartan 3E, Virtex-5 and Virtex-7. Performance summary, after place and route, showed that the proposed approach significantly reduces hardware utilization. Furthermore, the proposed design is almost 75% more efficient in terms of resources utilization and operating frequency as compared to the conventional design

    This study has described how the pattern of political communication between government institutions and its impact on public confidence in development in Aceh Province, Indonesia. This study focuses more on the analysis of communication patterns between the executive (ie: Governor of Aceh Province) and the legislative (DPR) in carrying out their respective duties and functions in the context of development in Aceh Province, Indonesia. This study uses a descriptive qualitative approach aimed at describing the pattern of political communication between government institutions and analyzing the level of trust of the Acehnese people. The benefit of this study is to provide a formula for good and effective political communication with elites at various levels of institutions from the executive to the legislature. Based on the results of the study, it can be concluded that the patterns of political communication between government institutions, namely the executive and legislative institutions, are personal communication patterns, informal communication patterns, family communication patterns, party or organizational communication patterns. All types of communication patterns are carried out between parties in carrying out interest lobbying and development policies in the Aceh Province, Indonesia. This type of communication pattern, based on the results of research, has been carried out by both parties in running government institutions in Aceh, namely the Aceh Government and the Aceh Legislature

    The Implementation of Meunasah-based Sharia in Aceh: A Social Capital and Islamic Law Perspective

    This research examined meunasah as the social capital in implementing Islamic law in Aceh. It aimed to explain the social capital of meunasah concerning its role and functions in supporting the Islamic law implementation in Aceh. This research has its root in studying the sociology of law, emphasizing the empirical legal analysis within the society. The researchers used social capital, which consisted of cognitive and structural aspects as a theoretical framework, and interviews, literature study, and observation, as the data collection methods. This study found that meunasah had a central position in Acehnese society as a center for religious activities, a place to learn the Qur’an, social activities, and as a customary institution. Since time immemorial, meunasah has played a central role as a center for community activities at the gampong level. Even though meunasah had transformed itself into a mosque in urban areas, its function and role remained as a place for internalizing the Islamic law principles, as a medium for socializing Islamic law, and as a center for religious, social activities. This study argues that meunasah can still be the ‘glue’ of social networks with its religious, social, and customary values as a cognitive, social capital. On another side, imuem meunasah can be considered a structural social capital. Meunasah, as part of social institutions, can support social order and order, which is a social function of law. Without the support of meunasah as a cognitive social capital, the researchers argue that Islamic law will be difficult to be internalized in society

    Role of early contrast enhanced CT scan in severity prediction of acute pancreatitis

    Abstract Severe pancreatitis occurs in approximately 15-25% of patients with acute pancreatitis. The objective of our study was to compare the CTSeverity Index (CTSI) with a clinical score (BISAP score) to predict severity of acute pancreatitis. Forty-eight consecutive patients with acutepancreatitis who underwent contrast enhanced CT scan within 72 hours of presentation were included. Results of our study showed that both CTSI and BISAP score were reliable predictors of mortality (p value = 0.019 and \u3c0.001 respectively) and need for mechanical ventilation (p value = .002 and .006 respectively). Positive predictive value of CTSI to predict recovery without intervention was 91.4% as compared to 78% for that of BISAP score. Receiver Operating Characteristics (ROC) Curves showed CT scan was superior to BISAP Score in predicting need of percutaneous or surgical intervention. Early CT scan may be utilized for prediction of clinical course of patients with acute pancreatitis

    Cloning and expression of hepatitis B surface gene in E. coli

    Background: Hepatitis B virus (HBV) is among the smallest DNA viruses resulting in ~800,000 deaths each year. Pakistan is considered a country affected by HBV. In Pakistan, the most dominant genotype is D. HBV is an enveloped virus of 3.2 kb. The study's goal was to express hepatitis B surface antigen in a bacterial host to produce a recombinant protein.Method: Blood samples were collected in EDTA coated vacutainer from patients after their consent. DNA was extracted from serum through the phenol-chloroform method; Hepatitis B surface gene was cloned in TA cloning vector, subclone in pET 28a expression vector. An expression vector containing the Surface gene was then transformed into a competent bacterial host BL21 and inducted with IPTG at 0.1-0.2mM concentration for expression. The expressed proteins (soluble and pellet form) were analyzed on SDS PAGE.Results: Hepatitis B Surface gene of 681bp after PCR were detected under UV light then successfully cloned and subcloned in pET 28 expression vector. The restricted fragment indicating the gene of interest was 681bp when analyzed on 1.2% Agarose gel under UV light. The required protein of 25kDa was obtained in soluble form when detected on 12% SDS PAGE after staining with Coomassie Blue dye.Conclusion: Hepatitis B surface gene was successfully expressed in both insoluble and pellet forms using E.coli. The expression of surface protein needs to maximize through optimizing conditions to be used as potent candidate for vaccine production to prevent hepatitis B infection.Keywords: Hepatitis B virus; Surface gene; Cloning vector; pET expression vector
