    Análisis de la eficiencia hospitalaria de Canarias

    Es de vital importancia estudiar la eficiencia en el sector sanitario público y teniendo en cuenta la situación actual que reclaman los hospitales con recursos suficientes y gestionados eficientemente. El objetivo principal de este trabajo es analizar la eficiencia de los hospitales del Sistema Canario de Salud para el último año disponible en las estadísticas, 2017, mediante un modelo de frontera no paramétrico (DEA) en la modalidad BBC de rendimientos variables a escala con orientación al input y aportar, además, recomendaciones de mejora en los planteamientos de gestión a los hospitales. Los resultados obtenidos muestran que el Servicio Canario de Salud en el sector privado es altamente eficiente, mientras que el sector público tiene una eficiencia media, debido a un exceso de todos los inputs, que han de disminuir, gestionándolos más eficientemente para lograr un nivel óptimo de eficiencia técnica o global, especialmente los relacionados con el personal.It is of vital importance to study efficiency in the public health sector and taking into account the current situation that demands hospitals with sufficient resources and managed efficiently. The main objective of this work is to analyse the efficiency of the hospitals in the Canary Islands Health System for the last year available in the statistics, 2017, using a non-parametric frontier model (DEA) in the BBC modality of variable scale performance with an-input orientation, and also to provide recommendations for improvements in the management approaches to the hospitals. The results obtained show that the Canary Islands Health Service in the private sector is highly efficient, while the public sector has average efficiency, due to an excess of all inputs, which must be reduced, managing them more efficiently to achieve an optimum level of technical or overall efficiency, especially those related to personnel

    LC3 subfamily in cardiolipin-mediated mitophagy: a comparison of the LC3A, LC3B and LC3C homologs

    Externalization of the phospholipid cardiolipin (CL) to the outer mitochondrial membrane has been proposed to act as a mitophagy trigger. CL would act as a signal for binding the LC3 macroauto-phagy/autophagy proteins. As yet, the behavior of the LC3-subfamily members has not been directly compared in a detailed way. In the present contribution, an analysis of LC3A, LC3B and LC3C interaction with CL-containing model membranes, and of their ability to translocate to mitochondria, is described. Binding of LC3A to CL was stronger than that of LC3B; both proteins showed a similar ability to colocalize with mitochondria upon induction of CL externalization in SH-SY5Y cells. Besides, the double silencing of LC3A and LC3B proteins was seen to decrease CCCP-induced mitophagy. Residues 14 and 18 located in the N-terminal region of LC3A were shown to be important for its recognition of damaged mitochondria during rotenone- or CCCP-induced mitophagy. Moreover, the in vitro results suggested a possible role of LC3A, but not of LC3B, in oxidized-CL recognition as a counterweight to excessive apoptosis activation. In the case of LC3C, even if this protein showed a stronger CL binding than LC3B or LC3A, the interaction was less specific, and colocalization of LC3C with mitochondria was not rotenone dependent. These results suggest that, at variance with LC3A, LC3C does not participate in cargo recognition during CL-mediated-mitophagy.     Lipids in Mitochondrial Macroautophagy: Phase Behavior of Bilayers Containing Cardiolipin and Ceramide

    Cardiolipin (CL) is a key lipid for damaged mitochondrial recognition by the LC3/GABARAP human autophagy proteins. The role of ceramide (Cer) in this process is unclear, but CL and Cer have been proposed to coexist in mitochondria under certain conditions. Varela et al. showed that in model membranes composed of egg sphingomyelin (eSM), dioleoyl phosphatidylethanolamine (DOPE), and CL, the addition of Cer enhanced the binding of LC3/GABARAP proteins to bilayers. Cer gave rise to lateral phase separation of Cer-rich rigid domains but protein binding took place mainly in the fluid continuous phase. In the present study, a biophysical analysis of bilayers composed of eSM, DOPE, CL, and/or Cer was attempted to understand the relevance of this lipid coexistence. Bilayers were studied by differential scanning calorimetry, confocal fluorescence microscopy, and atomic force microscopy. Upon the addition of CL and Cer, one continuous phase and two segregated ones were formed. In bilayers with egg phosphatidylcholine instead of eSM, in which the binding of LC3/GABARAP proteins hardly increased with Cer in the former study, a single segregated phase was formed.     HIV testing policies for migrants and ethnic minorities in EU/EFTA Member States.

    In the context of an European Centre for Disease Prevention and Control (ECDC) research project, our objective was to describe current recommendations regarding HIV testing and counselling targeting migrants and ethnic minorities in the European Union/European Economic Area/European Free Trade Association (EU/EEA/EFTA) Member States. An on-line survey was conducted among 31 EU/EEA/EFTA Member States. The survey inquired on the existence of specific HIV testing and counselling recommendations or policies for migrants and/or ethnic minorities and the year of their publication. Additionally, we performed a review of national recommendations, guidelines or any other policy documents retrieved from an Internet search through the different countries' competent bodies. Twenty-nine (94%) country representatives responded the survey, and 28 documents from 27 countries were identified. National guidelines on HIV testing are heterogeneous and tailored, according to the epidemiological situation. Twenty-two countries identify migrants and four countries identify ethnic minorities as particularly vulnerable to HIV. Sixteen countries explicitly recommend offering an HIV test to migrants/ethnic minorities. Guidelines especially target people originating from HIV endemic countries, and benefits of HIV early detection are highlighted. HIV testing is not mandatory in any country, but some countries overtly facilitate this practice. Benefits of HIV testing in migrants and ethnic minorities, at both individual and community levels are recognized by many countries.     Effect of ATG12–ATG5-ATG16L1 autophagy E3-like complex on the ability of LC3/GABARAP proteins to induce vesicle tethering and fusion

    In macroautophagy, the autophagosome (AP) engulfs portions of cytoplasm to allow their lysosomal degradation. AP formation in humans requires the concerted action of the ATG12 and LC3/GABARAP conjugation systems. The ATG12–ATG5-ATG16L1 or E3-like complex (E3 for short) acts as a ubiquitin-like E3 enzyme, promoting LC3/GABARAP proteins anchoring to the AP membrane. Their role in the AP expansion process is still unclear, in part because there are no studies comparing six LC3/GABARAP family member roles under the same conditions, and also because the full human E3 was only recently available. In the present study, the lipidation of six members of the LC3/GABARAP family has been reconstituted in the presence and absence of E3, and the mechanisms by which E3 and LC3/GABARAP proteins participate in vesicle tethering and fusion have been investigated. In the absence of E3, GABARAP and GABARAPL1 showed the highest activities. Differences found within LC3/GABARAP proteins suggest the existence of a lipidation threshold, lower for the GABARAP subfamily, as a requisite for tethering and inter-vesicular lipid mixing. E3 increases and speeds up lipidation and LC3/GABARAP-promoted tethering. However, E3 hampers LC3/GABARAP capacity to induce inter-vesicular lipid mixing or subsequent fusion, presumably through the formation of a rigid scaffold on the vesicle surface.     Search for same-sign top-quark pair production at √s=7 TeV and limits on flavour changing neutral currents in the top sector.

    An inclusive search for same-sign top-quark pair production in pp collisions at √s = 7TeV is performed using a data sample recorded with the CMS detector in 2010, corresponding to an integrated luminosity of 35 pb-1. This analysis is motivated by recent studies of pp ̅ →tt ̅ reporting mass-dependent forward-backward asymmetries larger than expected from the standard model. These asymmetries could be due to Flavor Changing Neutral Currents (FCNC) in the top sector induced by t-channel exchange of a massive neutral vector boson (Z’). Models with such a Z' also predict enhancement of same-sign top-pair production in pp or pp ̅ collisions. Limits are set as a function of the Z’ mass and its couplings to u and t quarks.     Study of exclusive two-photon production of W+W− in pp collisions at s√=7 TeV and constraints on anomalous quartic gauge couplings

    A search for exclusive or quasi-exclusive W+W− production by photon-photon interactions, pp → p(*)W+W−p(*), at s√=7 TeV is reported using data collected by the CMS detector with an integrated luminosity of 5.05 fb−1. Events are selected by requiring a μ ±e∓ vertex with no additional associated charged tracks and dilepton transverse momentum p T(μ ±e∓) > 30 GeV. Two events passing all selection requirements are observed in the data, compared to a standard model expectation of 2.2 ± 0.4 signal events with 0.84 ± 0.15 background. The tail of the dilepton p T distribution is studied for deviations from the standard model. No events are observed with p T  > 100 GeV. Model-independent upper limits are computed and compared to predictions involving anomalous quartic gauge couplings. The limits on the parameters aW0,C/Λ2 with a dipole form factor and an energy cutoff Λcutoff = 500 GeV are of the order of 10−4.We congratulate our colleagues in the CERN accelerator departments for the excellent performance of the LHC and thank the technical and administrative staffs at CERN and at other CMS institutes for their contributions to the success of the CMS effort.     Search for the standard model Higgs boson produced in association with a top-quark pair in pp collisions at the LHC

    A search for the standard model Higgs boson produced in association with a top-quark pair is presented using data samples corresponding to an integrated luminosity of 5.0 fb?1 (5.1 fb?1 ) collected in pp collisions at the center-of-mass energy of 7 TeV (8 TeV). Events are considered where the top-quark pair decays to either one lepton+jets (tt ? `?qq 0bb) or dileptons (tt ? ` +?`??bb), ` being an electron or a muon. The search is optimized for the decay mode H ? bb. The largest background to the ttH signal is top-quark pair production with additional jets. Artificial neural networks are used to discriminate between signal and background events. Combining the results from the 7 TeV and 8 TeV samples, the observed (expected) limit on the cross section for Higgs boson production in association with top-quark pairs for a Higgs boson mass of 125 GeV is 5.8 (5.2) times the standard model expectation

    Search for supersymmetry in hadronic final states with missing transverse energy using the variables a T and b-quark multiplicity in pp collisions at root s = 8 TeV

    An inclusive search for supersymmetric processes that produce final states with jets and missing transverse energy is performed in pp collisions at a centre-of-mass energy of 8 TeV. The data sample corresponds to an integrated luminosity of 11.7 fb-1 collected by the CMS experiment at the LHC. In this search, a dimensionless kinematic variable, ? T, is used to discriminate between events with genuine and misreconstructed missing transverse energy. The search is based on an examination of the number of reconstructed jets per event, the scalar sum of transverse energies of these jets, and the number of these jets identified as originating from bottom quarks. No significant excess of events over the standard model expectation is found. Exclusion limits are set in the parameter space of simplified models, with a special emphasis on both compressed-spectrum scenarios and direct or gluino-induced production of third-generation squarks. For the case of gluino-mediated squark production, gluino masses up to 950-1125 GeV are excluded depending on the assumed model. For the direct pair-production of squarks, masses up to 450 GeV are excluded for a single light first- or second-generation squark, increasing to 600 GeV for bottom squarks

    Search for physics beyond the standard model in events with T leptons, jets, and large transverse momentum imbalance in pp collisions at root s = 7 TeV

    A search for physics beyond the standard model is performed with events having one or more hadronically decaying T leptons, highly energetic jets, and large transverse momentum imbalance. The data sample corresponds to an integrated luminosity of 4.98 fb-1 of proton-proton collisions at root s=7 TeV collected with the CMS detector at the LHC in 2011. The number of observed events is consistent with predictions for standard model processes. Lower limits on the mass of the gluino in supersymmetric models are determined
