Methods The aim of this study—“EQUIP” (Enhancing Quitline Utilization among In-Patients)—is to determine the effectiveness, and cost-effectiveness, of warm handoff versus fax referral for linking hospitalized smokers with tobacco quitlines. This study employs a two-arm, individually randomized design. It is set in two large Kansas hospitals that have dedicated tobacco treatment interventionists on staff. At each site, smokers who wish to remain abstinent after discharge will be randomly assigned to groups. For patients in the fax group, staff will provide standard in-hospital intervention and will fax-refer patients to the state tobacco quitline for counseling post-discharge. For patients in the warm handoff group, staff will provide brief in-hospital intervention and immediate warm handoff: staff will call the state quitline, notify them that a warm handoff inpatient from Kansas is on the line, then transfer the call to the patients’ mobile or bedside hospital phone for quitline enrollment and an initial counseling session. Following the quitline session, hospital staff provides a brief check-back visit. Outcome measures will be assessed at 1, 6, and 12 months post enrollment. Costs are measured to support cost-effectiveness analyses. We hypothesize that warm handoff, compared to fax referral, will improve care transitions for tobacco treatment, enroll more participants in quitline services, and lead to higher quit rates. We also hypothesize that warm handoff will be more cost-effective from a societal perspective. Discussion If successful, this project offers a low-cost solution for more efficiently linking millions of hospitalized smokers with effective outpatient treatment—smokers that might otherwise be lost in the transition to outpatient care.     There is ample evidence that synthesis of HIV-1 proviral DNA from the viral RNA genome during reverse transcription requires host factors. However, only a few cellular proteins have been described in detail that affect reverse transcription and interact with reverse transcriptase (RT). HIV-1 integrase is an RT binding protein and a number of IN-binding proteins including INI1, components of the Sin3a complex, and Gemin2 affect reverse transcription. In addition, recent studies implicate the cellular proteins HuR, AKAP149, and DNA topoisomerase I in reverse transcription through an interaction with RT. In this review we will consider interactions of reverse transcription complex with viral and cellular factors and how they affect the reverse transcription process

    COVID 19:Seroprevalence and vaccine responses in UK dental care professionals

    Dental care professionals (DCPs) are thought to be at enhanced risk of occupational exposure to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). However, robust data to support this from large-scale seroepidemiological studies are lacking. We report a longitudinal seroprevalence analysis of antibodies to SARS-CoV-2 spike glycoprotein, with baseline sampling prior to large-scale practice reopening in July 2020 and follow-up postimplementation of new public health guidance on infection prevention control (IPC) and enhanced personal protective equipment (PPE). In total, 1,507 West Midlands DCPs were recruited into this study in June 2020. Baseline seroprevalence was determined using a combined IgGAM enzyme-linked immunosorbent assay and the cohort followed longitudinally for 6 mo until January/February 2021 through the second wave of the coronavirus disease 2019 pandemic in the United Kingdom and vaccination commencement. Baseline seroprevalence was 16.3%, compared to estimates in the regional population of 6% to 7%. Seropositivity was retained in over 70% of participants at 3- and 6-mo follow-up and conferred a 75% reduced risk of infection. Nonwhite ethnicity and living in areas of greater deprivation were associated with increased baseline seroprevalence. During follow-up, no polymerase chain reaction–proven infections occurred in individuals with a baseline anti–SARS-CoV-2 IgG level greater than 147.6 IU/ml with respect to the World Health Organization international standard 20-136. After vaccination, antibody responses were more rapid and of higher magnitude in those individuals who were seropositive at baseline. Natural infection with SARS-CoV-2 prior to enhanced PPE was significantly higher in DCPs than the regional population. Natural infection leads to a serological response that remains detectable in over 70% of individuals 6 mo after initial sampling and 9 mo from the peak of the first wave of the pandemic. This response is associated with protection from future infection. Even if serological responses wane, a single dose of the Pfizer-BioNTech 162b vaccine is associated with an antibody response indicative of immunological memory

    Surveillance of climate-sensitive zoonotic diseases: Leptospirosis at livestock slaughterhouses in three regions of Uganda

    Leptospirosis is an important bacterial zoonosis worldwide and is disproportionately associated with low-income settings and with extreme weather events due to climate change. Transmission to humans often occurs when infected rodents and domestic animals contaminate the environment via urine as the bacteria preferentially colonise kidneys. Surveillance of leptospirosis at slaughterhouses can therefore be useful in providing information on vast areas of a country and screening for diseases that are not considered during animal inspections. We determined the prevalence of Leptospira bacteria in the kidneys of 2,030 livestock kidney samples (820 cattle, 761 pigs, 335 goats, 114 sheep), and 117 small mammals by realtime PCR in a cross-sectional survey of slaughter facilities in three regions in Uganda. We extracted DNA and performed real-time polymerase chain reaction (PCR) tests targeting the lipL32 gene for pathogenic leptospires. Positive samples with cycle threshold values below 38 were further characterised using single locus sequence typing (SLST) to determine likely genomospecies. PCR products were sequenced by Eurofins Genomics (Ebersberg, Germany) and identification of genomospecies was done using the basic local alignment search tool (BLAST). Multi-locus sequencing typing (MLST) was performed on selected SLST-positive samples to determine sequence types (ST) and likely serogroups. The allelic profiles were analysed using Bionumerics software and the sequence types were determined using the PubMLST database. An overall prevalence of pathogenic leptospires of 2.58 % (95 % confidence intervals [CI]: 1.89–3.42) was observed, with sheep having the highest prevalence (6.12 % CI: 2.69–12.89), followed by cattle (4.25 %, 95 % CI: 2.91–5.98), goats (2.08 %, 95 % CI=0.91–4.38), and pigs (0.46 %, 95 % CI = 0.12–1.31) in decreasing order. The genomospecies L. borgpetersenii (11 in cattle and one in goat), L. kirschneri (five in cattle and four in sheep) and L. interrogans (one in a pig) were determined. Preliminary MLST results on one sample reveal L. kirschneri ST62 which is related to serogroup Grippotyphosa. Surveillance of important zoonoses using slaughterhouses as sentinels has the potential to offer essential information on the epidemiology of important zoonotic diseases in Uganda

    Climate change and anthropogenic intervention impact on the hydrologic anomalies in a semi-arid area : lower Zab river basin, Iraq

    Climate change impact, drought phenomena and anthropogenic stress are of increasing apprehension for water resource managers and strategists, particularly in arid regions. The current study proposes a generic methodology to evaluate the potential impact of such changes at a basin scale. The Lower Zab River Basin located in the north of Iraq has been selected for illustration purposes. The method has been developed through evaluating changes during normal hydrological years to separate the effects of climate change and estimate the hydrologic abnormalities utilising Indicators of Hydrologic Alteration. The meteorological parameters were perturbed by applying adequate delta perturbation climatic scenarios. Thereafter, a calibrated rainfall-runoff model was used for streamflow simulations. Findings proved that climate change has a more extensive impact on the hydrological characteristics of the streamflow than anthropogenic intervention (i.e. the construction of a large dam in the catchment). The isolated baseflow is more sensitive to the precipitation variations than to the variations of the potential evapotranspiration. The current hydrological anomalies are expected to continue. This comprehensive basin study demonstrates how climate change impact, anthropogenic intervention as well as hydro-climatic drought and hydrological anomalies can be evaluated with a new methodology

    Finding outlier light-curves in catalogs of periodic variable stars

    We present a methodology to discover outliers in catalogs of periodic light-curves. We use cross-correlation as measure of ``similarity'' between two individual light-curves and then classify light-curves with lowest average ``similarity'' as outliers. We performed the analysis on catalogs of variable stars of known type from the MACHO and OGLE projects and established that our method correctly identifies light-curves that do not belong to those catalogs as outliers. We show how our method can scale to large datasets that will be available in the near future such as those anticipated from Pan-STARRS and LSST.Comment: 16 pages, 24 figure
