    Impact of asymptomatic genital tract infections on in vitro Fertilization (IVF) outcome

    BACKGROUND: Infertility is estimated to affect approximately 9-30% of reproductive-aged couples. Several conditions involving one or both partners may contribute to infertility. The aim of this study is to evaluate the role of asymptomatic genital tract infections in the outcome of In Vitro Fertilization (IVF) in couples with infertility. METHODS: A total of 285 infertile couples were enrolled in the study. Vaginal/endocervical swabs and semen samples were collected and subjected to microbiological analysis. Spermiograms were carried out on semen specimens, and lactobacilli were quantified in vaginal swabs. Data were associated with IVF results and analysed by using non parametric tests and multivariate analysis. RESULTS: Microbiological analysis showed that 46.3% of couples presented with an asymptomatic genital tract infection. Spermiogram results showed a significantly diminished motility of sperm cells in samples positive to microbiological testing compared to negative specimens. Enterococcus faecalis was the most prevalent species (11.6%) in positive semen samples and was found to negatively affect both sperm morphology (p = 0.026) and motility (p = 0.003). Analysis of genital swabs from females showed that the presence of E. faecalis (p<0.0001), Escherichia coli (p = 0.0123), Streptococcus agalactiae (p<0.0001), and Gardnerella vaginalis (p = 0.0003) was significantly associated to reduced levels of vaginal lactobacilli. Association of microbiological data with IVF outcome showed that 85.7% of IVF+ couples was microbiologically negative, while IVF was successful in just 7.5% of couples infected with E. faecalis and/or U. urealyticum and/or M. hominis (p = 0.02). CONCLUSIONS: The results show the negative impact of E. faecalis on sperm quality and the association of definite bacterial pathogens with reduced levels of vaginal lactobacilli. The presence of E. faecalis and/or U. urealyticum and/or M. hominis in genital samples of infertile couples is predictive for a negative outcome of IVF

    Environmental complaint insights through text mining based on the driver, pressure, state, impact, and response (DPSIR) framework. Evidence from an Italian environmental agency

    Individuals, local communities, environmental associations, private organizations, and public representatives and bodies may all be aggrieved by environmental problems concerning poor air quality, illegal waste disposal, water contamination, and general pollution. Environmental complaints represent the expressions of dissatisfaction with these issues. As the time-consuming of managing a large number of complaints, text mining may be useful for automatically extracting information on stakeholder priorities and concerns. The paper used text mining and semantic network analysis to crawl relevant keywords about environmental complaints from two online complaint submission systems: online claim submission system of Regional Agency for Prevention, Environment and Energy (Arpae) (“Contact Arpae”); and Arpae's internal platform for environmental pollution (“Environmental incident reporting portal”) in the Emilia-Romagna Region, Italy. We evaluated the total of 2477 records and classified this information based on the claim topic (air pollution, water pollution, noise pollution, waste, odor, soil, weather-climate, sea-coast, and electromagnetic radiation) and geographical distribution. Then, this paper used natural language processing to extract keywords from the dataset, and classified keywords ranking higher in Term Frequency-Inverse Document Frequency (TF-IDF) based on the driver, pressure, state, impact, and response (DPSIR) framework. This study provided a systemic approach to understanding the interaction between people and environment in different geographical contexts and builds sustainable and healthy communities. The results showed that most complaints are from the public and associated with air pollution and odor. Factories (particularly foundries and ceramic industries) and farms are identified as the drivers of environmental issues. Citizen believed that environmental issues mainly affect human well-being. Moreover, the keywords of “odor”, “report”, “request”, “presence”, “municipality”, and “hours” were the most influential and meaningful concepts, as demonstrated by their high degree and betweenness centrality values. Keywords connecting odor (classified as impacts) and air pollution (classified as state) were the most important (such as “odor-burnt plastic” and “odor-acrid”). Complainants perceived odor annoyance as a primary environmental concern, possibly related to two main drivers: “odor-factory” and “odors-farms”. The proposed approach has several theoretical and practical implications: text mining may quickly and efficiently address citizen needs, providing the basis toward automating (even partially) the complaint process; and the DPSIR framework might support the planning and organization of information and the identification of stakeholder concerns and priorities, as well as metrics and indicators for their assessment. Therefore, integration of the DPSIR framework with the text mining of environmental complaints might generate a comprehensive environmental knowledge base as a prerequisite for a wider exploitation of analysis to support decision-making processes and environmental management activities

    Method for inducing experimental pneumococcal meningitis in outbred mice

    BACKGROUND: Streptococcus pneumoniae is the leading cause of bacterial meningitis. Pneumococcal meningitis is associated with the highest mortality among bacterial meningitis and it may also lead to neurological sequelae despite the use of antibiotic therapy. Experimental animal models of pneumococcal meningitis are important to study the pathogenesis of meningitis, the host immune response induced after infection, and the efficacy of novel drugs and vaccines. RESULTS: In the present work, we describe in detail a simple, reproducible and efficient method to induce pneumococcal meningitis in outbred mice by using the intracranial subarachnoidal route of infection. Bacteria were injected into the subarachnoid space through a soft point located 3.5 mm rostral from the bregma. The model was tested with several doses of pneumococci of three capsular serotypes (2, 3 and 4), and mice survival was recorded. Lethal doses killing 50 % of animals infected with type 2, 3 and 4 S. pneumoniae were 3.2 × 10, 2.9 × 10 and 1.9 × 10(2 )colony forming units, respectively. Characterisation of the disease caused by the type 4 strain showed that in moribund mice systemic dissemination of pneumococci to blood and spleen occurred. Histological analysis of the brain of animals infected with type 4 S. pneumoniae proved the induction of meningitis closely resembling the disease in humans. CONCLUSIONS: The proposed method for inducing pneumococcal meningitis in outbred mice is easy-to-perform, fast, cost-effective, and reproducible, irrespective of the serotype of pneumococci used

    Multivariate characterisation of Italian monovarietal red wines using MIR spectroscopy

    Aim: The aim of this study was to investigate the application of mid-infrared (MIR) spectroscopy combined with multivariate analysis, to provide a rapid screening tool for discriminating among different Italian monovarietal red wines based on the relationship between grape variety and wine composition in particular phenolic compounds. Methods and results: The MIR spectra (from 4000 to 700 cm\u20121) of 110 monovarietal Italian red wines, vintage 2016, were collected and evaluated by selected multivariate data analyses, including principal component analysis (PCA), linear discriminant analysis (DA), support vector machine (SVM), and soft intelligent modelling of class analogy (SIMCA). Samples were collected directly from companies across different regions of Italy and included 11 grape varieties: Sangiovese, Nebbiolo, Aglianico, Nerello Mascalese, Primitivo, Raboso, Cannonau, Teroldego, Sagrantino, Montepulciano and Corvina. PCA showed five wavelengths that mainly contributed to the PC1, including a much-closed peak at 1043 cm\u20121, which correspond to the C\u2013O stretch absorption bands that are important regions for glycerol, whereas the ethanol peaks at around 1085 cm\u20121. The band at 877 cm\u20121 are related to the C\u2013C stretching vibration of organic molecules, whereas the asymmetric stretching for C\u2013O in the aromatic \u2013OH group of polyphenols is within spectral regions from 1050 to 1165 cm\u20121. In particular, the (1175)\u20131100\u20131060 cm\u20121 vibrational bands are combination bands, involving C\u2013O stretching and O\u2013H deformation of phenolic rings. The 1166\u20131168 cm\u20121 peak is attributable to inplane bending deformations of C\u2013H and C\u2013O groups of polyphenols, respectively, for which polymerisation may cause a slight peak shift due to the formation of H-bridges. The best result was obtained with the SVM, which achieved an overall correct classification for up to 72.2% of the training set, and 44.4% for the validation set of wines, respectively. The Sangiovese wines (n=19) were split into two sub-groups (Sang-Romagna, n=12 and Sang-Tuscany, n=7) considering the indeterminacy of its origins, which is disputed between Romagna and Tuscany. Although the classification of three grape varieties was problematic (Nerello Mascalese, Raboso and Primitivo), the remaining wines were almost correctly assigned to their actual classes. Conclusions: MIR spectroscopy coupled with chemometrics represents an interesting approach for the classification of monovarietal Italian red wines, which is important in quality control and authenticity monitoring. Significance and impact of the study: Authenticity is a main issue in winemaking in terms of quality evaluation and adulteration, in particular for origin certified/protected wines, for which the added marketing value is related to the link of grape variety with the area of origin.     Inhibition of matrix metalloproteinases attenuates brain damage in experimental meningococcal meningitis

    Background: Approximately 7% of survivors from meningococcal meningitis (MM) suffer from neurological sequelae due to brain damage in the course of meningitis. The present study focuses on the role of matrix metalloproteinases (MMPs) in a novel mouse model of MM-induced brain damage. Methods: The model is based on intracisternal infection of BALB/c mice with a serogroup C Neisseria meningitidis strain. Mice were infected with meningococci and randomised for treatment with the MMP inhibitor batimastat (BB-94) or vehicle. Animal survival, brain injury and host-response biomarkers were assessed 48 h after meningococcal challenge. Results: Mice that received BB-94 presented significantly diminished MMP-9 levels (p < 0.01), intracerebral bleeding (p < 0.01), and blood brain barrier (BBB) breakdown (p < 0.05) in comparison with untreated animals. In mice suffering from MM, the amount of MMP- 9 measured by zymography significantly correlated with both intracerebral haemorrhage (p < 0.01) and BBB disruption (p < 0.05). Conclusions: MMPs significantly contribute to brain damage associated with experimental MM. Inhibition of MMPs reduces intracranial complications in mice suffering from MM, representing a potential adjuvant strategy in MM post-infection sequelae

    Diagnostic and Therapeutic Approach to Children and Adolescents with Obstructive Sleep Apnea Syndrome (OSA): Recommendations in Emilia-Romagna Region, Italy

    Obstructive sleep apnoea syndrome (OSA) in paediatrics is a rather frequent pathology caused by pathophysiological alterations leading to partial and prolonged obstruction (hypoventilation) and/or intermittent partial (hypopnoea) or complete (apnoea) obstruction of the upper airways. Paediatric OSA is characterised by daytime and night-time symptoms. Unfortunately, there are few data on shared diagnostic-therapeutic pathways that address OSA with a multidisciplinary approach in paediatric age. This document summarizes recommendations from the Emilia-Romagna Region, Italy, developed in order to provide the most appropriate tools for a multidisciplinary approach in the diagnosis, treatment and care of paediatric patients with OSA. The multidisciplinary group of experts distinguished two different 'step' pathways, depending on the age group considered (i.e., under or over two years). In most cases, these pathways can be carried out by the primary care paediatrician, who represents the first filter for approaching the problem. For this reason, it is essential that the primary care paediatrician receives adequate training on how to formulate the diagnostic suspicion of OSA and on what criteria to use to select patients to be sent to the hospital centre. The relationship between the paediatrician of the patient and her/his parents must see a synergy of behaviour between the various players in order to avoid uncertainty about the diagnostic and therapeutic decisions as well as the follow-up phase. The definition and evaluation of the organizational process and outcome indicators of the developed flow-chart, and the impact of its implementation will remain fundamental

    The secretion and maturation of prosaposin and procathepsin D are blocked in embryonic neural progenitor cells

    The notion that prosaposin (Prosap) is likely involved in brain development and regeneration led us to explore its expression in stem/progenitor neural cells and its fate after cell differentiation. The expression of procathepsin-cathepsin D (proCath-Cath D), an endoprotease that plays an important role in the processing and sorting of Prosap, has been concomitantly examined. Our data evidenced that in embryonic human neural progenitor cells (eHNPCs) intact and high molecular weight intermediate forms of Prosap and intermediate forms of Cath D accumulated inside the cells, while the formation of saposins and mature Cath D was impaired. Furthermore, neither Prosap nor proCath D were secreted from eHNPCs. The block of the processing and secretion shared by Prosap and proCath D was overcome during the course of differentiation of eHNPCs into a mixed population of astrocytes and neuronal cells. Upon differentiation, large amounts of Prosap and proCath D were secreted from the cells, while saposins and mature Cath D were produced inside the cells. The dramatic accumulation of Prosap (an antiapoptotic factor) and reduction of mature Cath D (a proapoptotic factor) in the undifferentiated eHNPCs most likely play a role in the molecular mechanisms regulating the resistance to apoptotic signals of these cells and might represent a critically important issue in HNPCs biology. © 2008 Elsevier B.V.     NF-YA splice variants have different roles on muscle differentiation

    The heterotrimeric CCAAT-binding factor NF-Y controls the expression of a multitude of genes involved in cell cycle progression. NF-YA is present in two alternatively spliced isoforms, NF-YAs and NF-YAl, differing in 28 aminoacids in the N-terminal Q-rich activation domain. NF-YAs has been identified as a regulator of stemness and proliferation in mouse embryonic cells (mESCs) and human hematopoietic stem cells (hHSCs), whereas the role of NF-YAl is not clear. In the muscle system, NF-YA expression is observed in proliferating cells, but barely detectable in terminally differentiated cells in vitro and adult skeletal muscle in vivo. Here, we show that NF-YA inactivation in mouse myoblasts impairs both proliferation and differentiation. The overexpression of the two NF-YA isoforms differentially affects myoblasts fate: NF-YAs enhance cell proliferation, while NF-YAl boosts differentiation. The molecular mechanisms were investigated by expression profilings, detailing the opposite programs of the two isoforms. Bioinformatic analysis of the regulated promoters failed to detect a significant presence of CCAAT boxes in the regulated genes. NF-YAl activates directly Mef2D, Six genes, and p57kip2 (Cdkn1c), and indirectly the myogenic regulatory factors (MRFs). Specifically, Cdkn1c activation is induced by NF-Y binding to its CCAAT promoter and by reducing the expression of the lncRNA Kcnq1ot1, a negative regulator of Cdkn1c transcription. Overall, our results indicate that NF-YA alternative splicing is an influential muscle cell determinant, through direct regulation of selected cell cycle blocking genes, and, directly and indirectly, of muscle-specific transcription factors

    The impact of the competence quorum sensing system on Streptococcus pneumoniae biofilms varies depending on the experimental model

    Background: Different models for biofilm in Streptococcus pneumoniae have been described in literature. To permit comparison of experimental data, we characterised the impact of the pneumococcal quorum-sensing competence system on biofilm formation in three models. For this scope, we used two microtiter and one continuous culture biofilm system. Results: In both microtiter models the competence system influences stability and structure of biofilm in the late attachment phase and synthetic competence stimulating peptide (CSP) restored wild type phenotypes in the comC mutants unable to produce the peptide. Early attachment of single cells to well bottoms was found for both systems to be competence independent, while later phases, including microcolony formation correlated to an intact competence system. The continuous culture biofilm model was not affected by mutations in the competence locus, but deletion of capsule had a significant impact in this model. Conclusions: Since biofilm remains a largely uncharacterised multi-parameter phenotype it appears to be advisable to exploit more than one model in order to draw conclusion of possible relevance of specific genotypes on pneumococcal physiology.Claudia Trappetti, Luciana Gualdi, Lorenzo Di Meola, Prashant Jain, Cindy C Korir, Paul Edmonds, Francesco Iannelli, Susanna Ricci, Gianni Pozzi and Marco R Oggion
