    Antiviral Treatment for COVID-19: A Systematic Review

    Objetivo: En diciembre de 2019 se informó de un nuevo tipo de coronavirus que afecta a los diferentes sistemas de salud del mundo. Hemos realizado una revisión sistemática de los posibles antivirales estudiados que podrían ser útiles en esta catástrofe de salud pública. Fuentes de datos: Se realizó una estrategia de búsqueda con términos MESH en PubMed, Web of Science y Scopus. Además, se revisaron los ECA publicados en Clinicaltrials.gov. Se realizaron búsquedas en las bases de datos entre abril y junio de 2020. Selección de estudios: Seleccionamos todos los ensayos controlados aleatorios que evaluaban los efectos de los antivirales y se incluyeron 5 estudios de una base de datos de investigación de 280 artículos recopilados. Después de eliminar los artículos duplicados, se seleccionaron 43 para su revisión. Finalmente, 5 artículos fueron elegibles para revisión de texto completo y se incluyeron en el artículo. Resultados: Los datos de los ensayos controlados aleatorios actuales no mostraron una mejoría clínica en términos de mortalidad, necesidad de soporte de oxígeno o necesidad de intubación en pacientes que usaron antivirales versus aquellos que no lo hicieron. No se demostró mejoría clínica. Se observó que existe dificultad para calcular la mejoría clínica, esta gran diferencia hace que los estudios elegibles sean difíciles de comparar. Conclusión: estos predictores, sin embargo, necesitan más trabajo para validar la confiabilidad. Se necesitan más ensayos clínicos con antivirales para observar una relación entre la mejoría clínica o la mortalidad por SARS-CoV-19.Aim: In December 2019, there were reports of a new type of coronavirus that affects the different health systems of the world. We have carried out a systematic review of the possible antivirals studied that could be useful in this public health catastrophe. Data Sources: A search strategy with MESH terms was performed in PubMed, Web of Science, and Scopus. Also, RCTs published in clinicaltrials.gov were reviewed. The databases were searched between April and June 2020. Study Selection: We selected all Randomized Controlled Trials evaluating the effects of antivirals and 5 studies were included from a research database of 280 articles collected between. After removing duplicated articles, 43 were selected for review. Finally, 5 articles were eligible for full-text review and included in the article. Results: Current randomized controlled trial data showed no clinical improvement in terms of mortality, need for oxygen support or need for intubation in patients who used antivirals versus those who did not. No clinical improvement was demonstrated. It was observed that there is difficulty in calculating clinical improvement, this large difference makes the eligible studies difficult to compare. Conclusion: These predictors, however, need further work to validate reliability. More clinical trials involving antivirals are needed to observe a relationship between clinical improvement or mortality from SARS-CoV-19.Tesi

    Adapting for the COVID-19 pandemic in Ecuador, a characterization of hospital strategies and patients

    The World Health Organization (WHO) declared coronavirus disease-2019 (COVID-19) a global pandemic on 11 March 2020. In Ecuador, the first case of COVID-19 was recorded on 29 February 2020. Despite efforts to control its spread, SARS-CoV-2 overran the Ecuadorian public health system, which became one of the most affected in Latin America on 24 April 2020. The Hospital General del Sur de Quito (HGSQ) had to transition from a general to a specific COVID-19 health center in a short period of time to fulfill the health demand from patients with respiratory afflictions. Here, we summarized the implementations applied in the HGSQ to become a COVID-19 exclusive hospital, including the rearrangement of hospital rooms and a triage strategy based on a severity score calculated through an artificial intelligence (AI)-assisted chest computed tomography (CT). Moreover, we present clinical, epidemiological, and laboratory data from 75 laboratory tested COVID-19 patients, which represent the first outbreak of Quito city. The majority of patients were male with a median age of 50 years. We found differences in laboratory parameters between intensive care unit (ICU) and non-ICU cases considering C-reactive protein, lactate dehydrogenase, and lymphocytes. Sensitivity and specificity of the AI-assisted chest CT were 21.4% and 66.7%, respectively, when considering a score >70%; regardless, this system became a cornerstone of hospital triage due to the lack of RT-PCR testing and timely results. If health workers act as vectors of SARS-CoV-2 at their domiciles, they can seed outbreaks that might put 1,879,047 people at risk of infection within 15 km around the hospital. Despite our limited sample size, the information presented can be used as a local example that might aid future responses in low and middle-income countries facing respiratory transmitted epidemics

    En el siguiente trabajo se presentan una serie de reflexiones como resultados de experiencias significativas y específicas suscitadas alrededor de los procesos de formación profesional en el diplomado sobre abordaje psicosocial de los contextos de violencia que desarrolla la universidad nacional abierta y a distancia, el cual permitió que los educando generaran el reconocimiento de la imagen y la narrativa como instrumento para identificar, visibilizar y concientizar sobre variables psicosociales que afectan los territorios que han sido marcados por la violencia en Colombia. Permitiéndoles presentar un planteamiento estratégico de acciones y actividades que respondan a las necesidades psicosociales de afrontamiento, reparación, reconciliación, dignificación y acciones sin daño sobre las poblaciones afectadas por el desplazamiento forzado en los contextos de violencia tan marcados en el País.The following work presents a series of conclusions as a result of specific experience raised around the professional training processes in the diploma course on psychosocial approach to the contexts of violence developed by the Universidad Nacional Abierta y a distancia, which allowed the students to develop the recognition of the image and the narrative as an instrument to identify, make visible and raise awareness about psychosocial variables in the territories that have been marked by violence in Colombia. Allowing them to present a strategic approach to actions and activities that respond to the psychosocial needs of coping, reparation, reconciliation, dignification and actions without harm on the populations affected by forced displacement in the contexts of violence so marked in the Country

    CD32 is expressed on cells with transcriptionally active HIV but does not enrich for HIV DNA in resting T cells

    The persistence of HIV reservoirs, including latently infected, resting CD4+ T cells, is the major obstacle to cure HIV infection. CD32a expression was recently reported to mark CD4+ T cells harboring a replication-competent HIV reservoir during antiretroviral therapy (ART) suppression. We aimed to determine whether CD32 expression marks HIV latently or transcriptionally active infected CD4+ T cells. Using peripheral blood and lymphoid tissue of ART-treated HIV+ or SIV+ subjects, we found that most of the circulating memory CD32+ CD4+ T cells expressed markers of activation, including CD69, HLA-DR, CD25, CD38, and Ki67, and bore a TH2 phenotype as defined by CXCR3, CCR4, and CCR6. CD32 expression did not selectively enrich for HIV- or SIV-infected CD4+ T cells in peripheral blood or lymphoid tissue; isolated CD32+ resting CD4+ T cells accounted for less than 3% of the total HIV DNA in CD4+ T cells. Cell-associated HIV DNA and RNA loads in CD4+ T cells positively correlated with the frequency of CD32+ CD69+ CD4+ T cells but not with CD32 expression on resting CD4+ T cells. Using RNA fluorescence in situ hybridization, CD32 coexpression with HIV RNA or p24 was detected after in vitro HIV infection (peripheral blood mononuclear cell and tissue) and in vivo within lymph node tissue from HIV-infected individuals. Together, these results indicate that CD32 is not a marker of resting CD4+ T cells or of enriched HIV DNA–positive cells after ART; rather, CD32 is predominately expressed on a subset of activated CD4+ T cells enriched for transcriptionally active HIV after long-term ART

    Weaker HLA footprints on HIV in the unique and highly genetically admixed host population of Mexico

    HIV circumvents HLA class I-restricted CD8+ T-cell responses through selection of escape mutations that leave characteristic mutational “footprints,” also known as HLA-associated polymorphisms (HAPs), on HIV sequences at the population level. While many HLA footprints are universal across HIV subtypes and human populations, others can be region specific as a result of the unique immunogenetic background of each host population. Using a published probabilistic phylogenetically informed model, we compared HAPs in HIV Gag and Pol (PR-RT) in 1,612 subtype B-infected, antiretroviral treatment-naive individuals from Mexico and 1,641 individuals from Canada/United States. A total of 252 HLA class I allele subtypes were represented, including 140 observed in both cohorts, 67 unique to Mexico, and 45 unique to Canada/United States. At the predefined statistical threshold of a q value of <0.2, 358 HAPs (201 in Gag, 157 in PR-RT) were identified in Mexico, while 905 (534 in Gag and 371 in PR-RT) were identified in Canada/United States. HAPs identified in Mexico included both canonical HLA-associated escape pathways and novel associations, in particular with HLA alleles enriched in Amerindian and mestizo populations. Remarkably, HLA footprints on HIV in Mexico were not only fewer but also, on average, significantly weaker than those in Canada/United States, although some exceptions were noted. Moreover, exploratory analyses suggested that the weaker HLA footprint on HIV in Mexico may be due, at least in part, to weaker and/or less reproducible HLA-mediated immune pressures on HIV in this population. The implications of these differences for natural and vaccine-induced anti-HIV immunity merit further investigation

    Differential immunodominance hierarchy OF CD8+ T cell responses in HLA-B*27:05 AND B*27:02-mediated control of HIV-1 infection

    The well-characterized association between HLA-B*27:05 and protection against HIV disease progression has been linked to immunodominant HLA-B*27:05-restricted CD8+ T-cell responses toward the conserved Gag KK10 (residues 263 to 272) and polymerase (Pol) KY9 (residues 901 to 909) epitopes. We studied the impact of the 3 amino acid differences between HLA-B*27:05 and the closely related HLA-B*27:02 on the HIV-specific CD8+ T-cell response hierarchy and on immune control of HIV. Genetic epidemiological data indicate that both HLA-B*27:02 and HLA-B*27:05 are associated with slower disease progression and lower viral loads. The effect of HLA-B*27:02 appeared to be consistently stronger than that of HLA-B*27:05. In contrast to HLA-B*27:05, the immunodominant HIV-specific HLA-B*27:02-restricted CD8+ T-cell response is to a Nef epitope (residues 142 to 150 [VW9]), with Pol KY9 subdominant and Gag KK10 further subdominant. This selection was driven by structural differences in the F pocket, mediated by a polymorphism between these two HLA alleles at position 81. Analysis of autologous virus sequences showed that in HLA-B*27:02-positive subjects, all three of these CD8+ T-cell responses impose selection pressure on the virus, whereas in HLA-B*27:05-positive subjects, there is no Nef VW9-mediated selection pressure. These studies demonstrate that HLA-B*27:02 mediates protection against HIV disease progression that is at least as strong as or stronger than that mediated by HLA-B*27:05. In combination with the protective Gag KK10 and Pol KY9 CD8+ T-cell responses that dominate HIV-specific CD8+ T-cell activity in HLA-B*27:05-positive subjects, a Nef VW9-specific response is additionally present and immunodominant in HLA-B*27:02-positive subjects, mediated through a polymorphism at residue 81 in the F pocket, that contributes to selection pressure against HIV

    Inside the Outbreak of the 2009 Influenza A (H1N1)v Virus in Mexico

    Influenza viruses pose a threat to human health because of their potential to cause global disease. Between mid March and mid April a pandemic influenza A virus emerged in Mexico. This report details 202 cases of infection of humans with the 2009 influenza A virus (H1N1)v which occurred in Mexico City as well as the spread of the virus throughout the entire country.From May 1st to May 5th nasopharyngeal swabs, derived from 751 patients, were collected at 220 outpatient clinics and 28 hospitals distributed throughout Mexico City. Analysis of samples using real time RT-PCR revealed that 202 patients out of the 751 subjects (26.9%) were confirmed to be infected with the new virus. All confirmed cases of human infection with the strain influenza (H1N1)v suffered respiratory symptoms. The greatest number of confirmed cases during the outbreak of the 2009 influenza A (H1N1)v were seen in neighbourhoods on the northeast side of Mexico City including Iztapalapa, Gustavo A. Madero, Iztacalco, and Tlahuac which are the most populated areas in Mexico City. Using these data, together with data reported by the Mexican Secretariat of Health (MSH) to date, we plot the course of influenza (H1N1)v activity throughout Mexico.Our data, which is backed up by MSH data, show that the greatest numbers of the 2009 influenza A (H1N1) cases were seen in the most populated areas. We speculate on conditions in Mexico which may have sparked this flu pandemic, the first in 41 years. We accept the hypothesis that high population density and a mass gathering which took in Iztapalapa contributed to the rapid spread of the disease which developed in three peaks of activity throughout the Country

    Phase I study to determine the safety, tolerability and immunostimulatory activity of thalidomide analogue CC-5013 in patients with metastatic malignant melanoma and other advanced cancers

    We assessed the safety, tolerability and efficacy of the immunomodulatory drug, CC-5013 (REVIMID(TM)), in the treatment of patients with metastatic malignant melanoma and other advanced cancers. A total of 20 heavily pretreated patients received a dose-escalating regimen of oral CC-5013. Maximal tolerated dose, toxicity and clinical responses were evaluated and analysis of peripheral T-cell surface markers and serum for cytokines and proangiogenic factors were performed. CC-5013 was well tolerated. In all, 87% of adverse effects were classified as grade 1 or grade 2 according to Common Toxicity Criteria and there were no serious adverse events attributable to CC-5013 treatment. Six patients failed to complete the study, three because of disease progression, two withdrew consent and one was entered inappropriately and withdrawn from the study. The remaining 14 patients completed treatment without dose reduction, with one patient achieving partial remission. Evidence of T-cell activation was indicated by significantly increased serum levels of sIL-2 receptor, granulocyte- macrophage colony-stimulating factor, interleukin-12 (IL-12), tumour necrosis factor-alpha and IL-8 in nine patients from whom serum was available. However, levels of proangiogenic factors vascular endothelial growth factor and basic foetal growth factor were not consistently affected, This study demonstrates the safety, tolerability and suggests the clinical activity of CC-5013 in the treatment of refractory malignant melanoma. Furthermore, this is the first report demonstrating T-cell stimulatory activity of this class of compound in patients with advanced cancer
