This survey aimed to describe the floristic composition and structure of the epiphytic community occurring in a terra firme forest in the city of Coari, Brazil, in the Amazon region. Data collection was performed with a 1.5 ha plot method, with which upland, slope and lowland habitats were sampled. All angiosperm epiphytes and their host plants (diameter at breast height > 10 cm) were sampled. We recorded 3.528 individuals in 13 families, 48 genera and 164 species. Araceae was the most prevalent family with regard to the importance value and stood out in all related parameters, followed by Bromeliaceae, Cyclanthaceae and Orchidaceae. The species with the highest epiphytic importance values were Guzmania lingulata (L.) Mez. and Philodendron linnaei Kunth. The predominant life form was hemiepiphytic. Estimated floristic diversity was 3.2 (H'). The studied epiphytic community was distributed among 727 host plants belonging to 40 families, 123 genera and 324 species. One individual of Guarea convergens T.D. Penn. was the host with the highest richness and abundance of epiphytes. Stems/trunks of host plants were the most colonized segments, and the most favorable habitat for epiphytism was the lowlands, where 84.1% of species and 48.2% of epiphytic specimens were observed

Background Shared decision-making (SDM) is often considered the ideal for decision-making in oncology. Views of specific groups such as ethnic minorities have seldom been considered in its development. Aim In this study we seek to assess in oncology if there is a need for adaptation of the current SDM model to ethnic minorities and to formulate possible adjustments. Design This study is embedded in empirical bioethics, an interdisciplinary approach integrating empirical data with ethical reasoning to formulate normative conclusions regarding a practice. For the empirical social scientific part, a cross-sectional qualitative study will be conducted; for the ethical reflection the Reflective Equilibrium will be used to develop a coherent view on the application of SDM among ethnic minorities in oncology. Method Semi-structured interviews combined with visual methods (timelines and relational maps) will be held with healthcare professionals (HCPs), ethnic minority patients, and their relatives to identify values steering the behavior of these actors in SDM. In addition, focus groups (FGs) will be held with ethnic minority community members to identify value structures at the group level. Respondents will be recruited through organizations with access to ethnic minorities and collaborating hospitals. Data will be analyzed using a reflexive thematic analysis through the lens of Schwartz's value theory. The results of the empirical phase will be included in the RE to formulate possible adjustments of the SDM model, if needed. Discussion The integration of empirical data with ethical reflection is an innovative method in decision-making. This method enables a systematic and profound assessment of the need for adaptation of SDM and the formulation of theoretically and empirically based suggestions for adaptations of the model. Findings of this study may enrich the SDM model.Analysis and support of clinical decision makin

Onset of hypertension during pregnancy is associated with long-term worse blood pressure control and adverse cardiac remodeling

Up to 20% of women with hypertensive pregnancy disorders might persist with chronic hypertension. This study compared clinical and echocardiographic features between women whose hypertension began as hypertensive pregnancy disorders (PH group) and women whose diagnosis of hypertension did not occur during pregnancy (NPH group). Fifty PH and 100 NPH women were cross-sectionally evaluated by clinical, laboratory, and echocardiography analysis, and the groups were matched by duration of hypertension. PH exhibited lower age (46.6 ± 1.4 vs. 65.3 ± 1.1 years; P < .001), but higher systolic (159.8 ± 3.9 vs. 148.0 ± 2.5 mm Hg; P = .009) and diastolic (97.1 ± 2.4 vs. 80.9 ± 1.3 mm Hg; P < .001) blood pressure than NPH, although used more antihypertensive classes (3.4 ± 0.2 vs. 2.6 ± 0.1; P < .001). Furthermore, PH showed higher left ventricular wall thickness and increased prevalence of concentric hypertrophy than NPH after adjusting for age and blood pressure. In conclusion, this study showed that PH may exhibit worse blood pressure control and adverse left ventricular remodeling compared with NPH.Up to 20% of women with hypertensive pregnancy disorders might persist with chronic hypertension.
