    家族介護者が家族の集いに参加する意味

    本研究は,在宅認知症高齢者の家族介護者が,家族の会の主催する集いに参加することの意味を質的因子探索的な分析方法を用いて明らかにすることを目的とした.毎月1回実施される集いの参加者を対象とし,1事例毎に分析を行い,カテゴリー化を行った.また,1事例毎から導き出された計6事例の分析結果を更に統合し,カテゴリーの抽出を行った.統合分析の結果,《共通体験から得られる共感》《吐露できる安心》《自分の介護の振り返り》《新たな介護観の獲得に向けた示唆》の4つのカテゴリーが抽出された.ピアカウンセリングとも言える集いに参加することの意味を検討した結果,家族介護者にとっての集いの意味は重要で多岐にわたることが分かった.集いへの参加は,介護者及び認知症高齢者双方のQOL向上につながると考えられる.The purpose of this study was to search for the meaning of participation in the meeting of family caregiver of the dementia elderly. This study was a qualitative research, and the subjects were 6 family caregivers. 4 categories for the meaning of participation in the meeting were identified : Sympathy obtained from common experience, Safety that can be revealed, Looking back on my nursing, Suggestion in acquisition of idea of new nursing. The clarification of the meaning of participation in the meeting of family caregiver with dementia elderly could contribute to the construction of a renewed family caregiver of the dementia elderly image and the further understanding of the family caregiver in the areas of medicine and nursing education

    High purity NaI(Tl) scintillator to search for dark matter

    A high purity and large volume NaI(Tl) scintillator was developed to search for cosmic dark matter. The required densities of radioactive impurities (RIs) such as U-chain, Th-chain are less than a few ppt to establish high sensitivity to dark matter. The impurity of RIs were effectively reduced by selecting raw materials of crucible and by performing chemical reduction of lead ion in NaI raw powder. The impurity of 226^{226}Ra was reduced less than 100 μ\muBq/kg in NaI(Tl) crystal. It should be remarked that the impurity of 210^{210}Pb, which is difficult to reduce, is effectively reduced by chemical processing of NaI raw powder down to less than 30 μ\muBq/kg. The expected sensitivity to cosmic dark matter by using 250 kg of the high purity and large volume NaI(Tl) scintillator (PICO-LON; Pure Inorganic Crystal Observatory for LOw-background Neutr(al)ino) is 7×\times1045^{-45} cm2^{2} for 50 GeV/c2/c^{2} WIMPs.Comment: 6 pages, 2 Figures, Proceedings of International Symposium on Radiation Detectors and Their Uses (ISRD2016).     In the course of our studies on the heterologous expression of giant biosynthetic genes, we discovered a novel cryptic biosynthetic gene cluster in Streptomyces rochei IFO12908. During our efforts to express biosynthetic genes using the host SUKA strain derived from Streptomyces avermitilis, a novel polyene macrolactam compound designated as JBIR-156 was produced. We report herein the cloning and heterologous expression of the JBIR-156 biosynthetic gene cluster, and the isolation, structure determination, and cytotoxic activity of this novel compound

    We report a patient in Japan infected with Cryptococcus gattii genotype VGIIa who had no recent history of travel to disease-endemic areas. This strain was identical to the Vancouver Island outbreak strain R265. Our results suggest that this virulent strain has spread to regions outside North America

    Interaction of triosephosphate isomerase from the cell surface of Staphylococcus aureus and α-(1→3)-mannooligosaccharides derived from glucuronoxylomannan of Cryptococcus neoformans

    The glycolytic enzyme triosephosphate isomerase (TPI; EC 5.3.1.1) of Staphylococcus aureus is a candidate adhesion molecule for the interaction between the bacterium and the fungal pathogen Cryptococcus neoformans. TPI may recognize the mannan backbone of glucuronoxylomannan (GXM) of C. neoformans. We purified TPI from extracts of S. aureus surface proteins to investigate its binding by surface plasmon resonance analysis. The immobilized TPI reacted with GXM in a dose-dependent manner. Furthermore, the interactions between staphylococcal TPI and α-(1→3)-mannooligosaccharides derived from GXM were examined. The oligosaccharides exhibited binding with TPI; however, monomeric mannose did not. Differences in the slopes of the sensorgrams were observed between oligosaccharides with an even number of residues versus those with an odd number. A heterogeneous ligand-parallel reaction model revealed the existence of at least two binding sites on TPI. The enzymic activities of TPI were inhibited in a dose-dependent manner by α-(1→3)-mannooligosaccharides larger than triose. The binding of TPI and α-(1→3)-mannotriose near the substrate-binding site was predicted in silico (AutoDock 3.05). An oligosaccharide of size equal to or greater than triose could bind to the site, affecting enzymic activities. Moreover, affinities were indicated, especially for biose and tetraose, to another binding pocket, which would not affect enzymic activity. These data suggest a novel role for TPI, in addition to glycolysis, on the surface of S. aureus

    高齢化社会の到来に伴い一言で老人といっても幅広い年代が対象となり、一律に老人では説明できない。実際看護をしていると90歳をすぎた老人はその年代迄にはない、穏やかさ、焦りのなさ、人生を達観しているような感じを受けることが多い。そこで90歳以上の老人にみられるイメージや心理面の特徴を明らかにし看護実践の一助としたいと考えた。方法は80歳以上の入院患者50名に身体、生活、家族面からみた現状、他者から見たイメージ、主観的幸福感を調査した。その結果、90歳以上は80歳代にくらべ看護者に肯定的イメージに受け取られる傾向にあった。主観的幸福感を示すモラール得点の総合点では差はなかったが得点する内容に差が見られ、80歳は積極的な生き方で得点しているものが多く90歳代は現状に満足している点で得点している者が多かった。看護者からみたイメージと本人の主観的幸福感は両年代とも肯定的イメージで相関した。The population of the aged is currently increasing with advanced medical science and technology. The aged have become to keep their lives long. In this paper, We especially focused on their psychological characteristics of elderly people of 90 and older to offer high quality of nursing care for them. We interviewed 50 patients of 80 years and older about a subjective well-being, using a questionaire based on Philadelphia Geriatric Center Morale Scale. And also we asked 50 nurses, who took care of them exclusively, a questinaire about their images of the aged. We report as follows : 1. The nurses estimated that their images of the second group (range : 90 years old ~) were better than the first group (range : 80~89 years old). 2. In subjective well-being which Philadelphia Geriatric Center Morale Scale showed, above-mentioned both groups were similar in total scores. However, each item was marked differently by them. 3. In both groups, their subjective well-being correlated with good images for them

    BRCAness Predicts Resistance to Taxane-Containing Regimens in Triple Negative Breast Cancer During Neoadjuvant Chemotherapy

    AbstractBackgroundTo provide optimal treatment of heterogeneous triple negative breast cancer (TNBC), we need biomarkers that can predict the chemotherapy response.Patients and MethodsWe retrospectively investigated BRCAness in 73 patients with breast cancer who had been treated with taxane- and/or anthracycline-based neoadjuvant chemotherapy (NAC). Using multiplex, ligation-dependent probe amplification on formalin-fixed core needle biopsy (CNB) specimens before NAC and surgical specimens after NAC. BRCAness status was assessed with the assessor unaware of the clinical information.ResultsWe obtained 45 CNB and 60 surgical specimens from the 73 patients. Of the 45 CNB specimens, 17 had BRCAness (38.6% of all subtypes). Of the 23 TNBC CNB specimens, 14 had BRCAness (61% of TNBC cases). The clinical response rates were significantly lower for BRCAness than for non-BRCAness tumors, both for all tumors (58.8% vs. 89.3%, P = .03) and for TNBC (50% vs. 100%, P = .02). All tumors that progressed with taxane therapy had BRCAness. Of the patients with TNBC, those with non-BRCAness cancer had pathologic complete responses significantly more often than did those with BRCAness tumors (77.8% vs. 14.3%, P = .007). After NAC, the clinical response rates were significant lower for BRCAness than for non-BRCAness tumors in all subtypes (P = .002) and in TNBC cases (P = .008). After a median follow-up of 26.4 months, 6 patients—all with BRCAness—had developed recurrence. Patients with BRCAness had shorter progression-free survival than did those with non- BRCAness (P = .049).ConclusionIdentifying BRCAness can help predict the response to taxane, and changing regimens for BRCAness TNBC might improve patient survival. A larger prospective study is needed to further clarify this issue
