    Emergency engineering reconstruction mode based on the perspective of professional donations

    IntroductionIn the construction of emergency engineering, the effective participation of organizations such as government and construction enterprises can improve engineering emergency services to emergency adaptive, which faces the challenge of the enthusiasm of enterprises’ emergency participation. This study proposed a new diversified social governance mode for public emergency facility construction.MethodsThis paper empirically analyzes the performance of stock market returns before and after construction enterprises’ participation in emergency engineering.ResultsAgainst the backdrop of COVID-19, the analysis based on the sample data of 141 listed companies found that both construction enterprises directly involved in emergency hospital construction and non-state-owned enterprises involved in donations have higher excess return rates. In contrast, social concern has a positive mediating effect between emergency donations and stock market returns. The study suggests that stock market returns from donation behavior and reputation capital become incentives for firms to actively participate in emergency donations, providing a behavioral basis for professional donations by construction firms.DiscussionBased on the above assumptions, this paper proposes the organizational model of emergency engineering construction and the “Engineering Community” relationship based on the “Engineering Multi-governance” theory.     Detecting fake news and disinformation using artificial intelligence and machine learning to avoid supply chain disruptions

    Fake news and disinformation (FNaD) are increasingly being circulated through various online and social networking platforms, causing widespread disruptions and influencing decision-making perceptions. Despite the growing importance of detecting fake news in politics, relatively limited research efforts have been made to develop artificial intelligence (AI) and machine learning (ML) oriented FNaD detection models suited to minimize supply chain disruptions (SCDs). Using a combination of AI and ML, and case studies based on data collected from Indonesia, Malaysia, and Pakistan, we developed a FNaD detection model aimed at preventing SCDs. This model based on multiple data sources has shown evidence of its effectiveness in managerial decision-making. Our study further contributes to the supply chain and AI-ML literature, provides practical insights, and points to future research directions.© The Author(s) 2022.     Prophylaxis of Surgical Site Infection in Cranial Surgery with Vancomycin Powder Application into Wound

    Objective: To see the effect of application of Vancomycin powder directly into the subgaleal space in reducing the postoperative surgical site infections.Materials and Methods: All the patients who underwent consecutive elective craniotomies from April 2017 to May 2018 Dept of Neurosurgery, szabmu, PIMS, Islamabad. The control group received the standard routine prophylaxis according to the hospital protocols, whereas the treatment group, in addition to the standard prophylaxis, received Vancomycin powder in the surgical wound in addition to the standard routine prophylaxis. Results: 182 patients were enrolled in the study, 91 allocated to each the control and treatment group (Vancomycin). Six patients were lost to follow up. There were 90 patients in the control group and 86 patients in the treatment (Vancomycin) group. Both the groups were almost statistically similar. In the control group, 34.09% (n = 60) were male and 17.04% (n = 30) were female. In the treatment group, 29.54% (n = 52) were male and 19.31% (n = 34) were female. The overall rate of surgical site infection (SSI) was 3.97% (7 out of 176 cases). A statistically significant difference found in infection rate between the treatment group, 0% (0 out of 86 cases) and the control group, 7.77% (7 out of 90 cases) with the p value of 0.002. Conclusions: The use of topical Vancomycin powder in surgical wounds may significantly reduce the incidence of infection in patients undergoing elective craniotomies. It is a promising means of preventing devastating and harmful postoperative wound infections

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: Multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P \u3c 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (β coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    A Low Profile Penta-band Antenna for Portable Devices

    Recent years have seen a rapid growth of portable wireless communication systems. Limited form factor and operation at multiple frequencies of these devices require novel solutions of efficient embedded antennas. It has increased the demand of microstrip patch antennas due to their inherent properties of being low profile, simple design, small size and ease of fabrication and integration. Miniaturisation requirements have seen rise of multiband patch antennas. This paper presents the design and analysis of a novel multiband microstrip patch antenna. The antenna consists of a rectangular slot with two E-shaped stubs on both of its sides. An inverted T-shaped stub is present on the upper side of the slot while an I-shaped stub is there on the bottom side. A T-shaped feeding line feeds the antenna. The slot, stubs and feed collectively produce five frequency bands centred at 1.5 GHz, 2.2 GHz, 3.1 GHz, 4.2 GHz and 5.3 GHz for LTE/4G/5G, WiBro/WiMax, Satcomm and WLAN applications. The antenna offers small size, good impedance bandwidth and high gain at all operating frequencies
