Physiologic and Symptomatic Responses to Low-Level Substances in Individuals with and without Chemical Sensitivities: A Randomized Controlled Blinded Pilot Booth Study
We conducted a pilot study using a randomized, single-blind, placebo-controlled exposure among 10 individuals with and 7 without reported chemical sensitivities in a dedicated testing chamber. Objectives of the study were to explore the length of the adaptation period to obtain stable readings, evaluate responses to different substances, and measure the level and type of symptomatic and physiologic reactions to low-level exposures. Reported and observed symptoms, electrodermal response, heart rate, skin temperature, surface electromyogram, respiratory rate, contrast sensitivity, and the Brown-Peterson cognitive test were used and compared between cases and controls and between test substances (glue, body wash solution, dryer sheet) and control substances (unscented shampoo and clean air). Subjects with chemical sensitivities (cases) took longer to adapt to baseline protocols than did controls. After adaptation, despite small study numbers, cases displayed statistically significant responses (all measures, p < 0.02) in tonic electrodermal response to test substances compared with controls and compared with the control substance. Symptoms were also higher in cases than in controls for the body wash solution (p = 0.05) and dryer sheets (p = 0.02). Test–retest showed good agreement for both symptoms and tonic electrodermal responses (McNemar’s test, p = 0.32 and p = 0.33, respectively). Outside of skin conductance, other measures had no consistent patterns between test and control substances and between cases and controls. This study shows the importance of using an adaptation period in testing individuals with reported chemical sensitivities and, despite small numbers, raises questions about underlying mechanisms and level of reactivity to low-level chemical exposures in sensitive individuals
The role of a pseudocapsula in thymic epithelial tumors: outcome and correlation with established prognostic parameters. Results of a 20-year single centre retrospective analysis
<p>Abstract</p> <p>Background</p> <p>Treatment of thymoma is often based on observation of only a few patients. Surgical resection is considered to be the most important step. Role of a pseudocapsula for surgery, its clinical significance and outcome compared with established prognostic parameters is discussed which has not been reported so far.</p> <p>Methods</p> <p>84 patients with thymoma underwent resection and analysis was carried out for clinical features, prognostic factors and long-term survival.</p> <p>Results</p> <p>Fifteen patients were classified in WHO subgroup A, 21 in AB, 29 in B and 19 patients in C. Forty two patients were classified in Masaoka stage I, 19 stage II, 9 stage III and 14 stage IV. Encapsulated thymoma was seen in 40, incomplete or missing capsula in 44 patients. In 71 complete resections, local recurrence was 5%. 5-year survival was 88.1%. Thymomas with pseudocapsula showed a significant better survival (94.9% vs. 61.1%, respectively) (p = 0.001) and was correlated with the absence of nodal or distant metastasis (p = 0.04 and 0.001, respectively). Presence of pseudocapsula as well as the Masaoka and WHO classification, and R-status were of prognostic significance. R-status and Masaoka stage appeared to be of independent prognostic significance in multivariate analysis.</p> <p>Conclusion</p> <p>Intraoperative presence of an encapsulated tumor is a good technical marker for the surgeon to evaluate resectability and estimate prognosis. Although the presence of a capsula is of strong significance in the univariate analysis, it failed in the multivariate analysis due to its correlation with clinical Masaoka stage. Masaoka stage has a stronger relevance than WHO classification to determinate long-term outcome.</p
Sensitivity to electricity – Temporal changes in Austria
<p>Abstract</p> <p>Background</p> <p>An increasing number of persons suffer from non-specific health symptoms such as headache, sleep disturbances, difficulties in concentrating and more. In lack of a medical explanation, more and more persons take refuge to the assumption that they were electromagnetic hypersensitive (EHS) and electromagnetic pollution causes their problems. The discussion whether electromagnetic fields (EMF) could cause such adverse health effects is still ongoing.</p> <p>Methods</p> <p>Based on the Austrian inhabitants a statistical cross-sample of the general population with regard to age, gender and federal state had been investigated to assess the actual situation and potential temporal changes in comparison with a former study of 1994. In a telephone survey a total number of 526 persons were included.</p> <p>Results</p> <p>This study showed an actual EHS prevalence of 3.5% compared with 2% estimated in 1994. About 70% of the sample believed that electromagnetic pollution could be a risk factor for health. More than 30% declared to at least some degree to be concerned about their well-being near mobile phone base stations or power lines. However, only 10% were actively looking for specific information. Media triggered EHS hypothesis in 24% of the cases.</p> <p>Conclusion</p> <p>The results show that concerns about EMF did not decrease with time in spite of scientific studies and health risk assessments concluding that a causal relationship of EMF below recommended reference levels and non-specific health symptoms would be implausible.</p
The association between renal function and structural parameters: a pig study
<p>Abstract</p> <p>Background</p> <p>The objective was to investigate the association between renal structural parameters and renal function. The structural parameters were renal cortical volume, total renal volume, number of glomeruli, and total glomerular volume, and renal function was expressed by the single kidney GFR (skGFR). Investigations were performed using both healthy and chronically diseased kidneys. We investigated which of the structural parameters showed the best correlation to renal function and evaluated the possibility of predicting the renal function from structural parameters.</p> <p>Methods</p> <p>Twenty-four pigs, twelve with healthy kidneys and twelve with diseased kidneys, underwent skGFR measurements. Nephrectomies were performed and structural parameters were estimated using stereological procedures. The correlation between the structural parameters and skGFR was analysed by Pearson's correlation test. The prediction of skGFR from structural parameters was analysed by a linear regression test.</p> <p>Results</p> <p>In general, we demonstrated a good correlation between structural parameters and skGFR. When all kidneys were evaluated together Pearson's correlation coefficient between skGFR and any stereological parameter was above 0.60 and highly significant (p < 0.001), and with r-values ranging from 0.62 regarding number of glomeruli, to 0.78 regarding cortical volume. The best correlation was found between cortical volume and skGFR. Prediction of single kidney GFR from any structural parameter showed to be quite imprecise.</p> <p>Conclusion</p> <p>The observed correlations between structural parameters and renal function suggest that these parameters may potentially be useful as surrogate markers of the renal function. At present, however, precise prediction of renal function based on a single structural parameter seems hard to obtain.</p
Primary chemotherapy with adriamycin, cisplatin, vincristine and cyclophosphamide in locally advanced thymomas: a single institution experience
From 1990 to 1997, 16 consecutive patients with stage III and IVa invasive thymoma were treated in a single institution with primary chemotherapy consisting in adriamycin (40 mg m–2), cisplatin (50 mg m–2) administered intravenously on day 1, vincristine (0.6 mg m–2) on day 2 and cyclophosphamide (700 mg m–2) on day 4 (ADOC). The courses were repeated every 3 weeks. The aim was to evaluate the impact of this cytotoxic regimen with respect to response rate, per cent of patients radically resected, time to progression and overall survival. Two complete responses (one clinical and one pathological) and 11 partial responses were observed (overall response rate 81.2%); two patients had stable disease and one progressed. Toxicity was mild as only two patients developed grade III/IV neutropenia and one patient grade III nausea/vomiting. Nine patients were radically resected (five out of ten with stage III, and four out of six with stage IVa). Median time to progression and overall survival was 33.2 and 47.5 months respectively. Three patients were alive and disease free after more than 5 years. The ADOC scheme is highly active and manageable in the treatment of locally advanced thymoma. As a preoperative approach it should be offered to patients not amenable to surgery or to those surgically resectable but with a great deal of morbidity. © 1999 Cancer Research Campaig
Instantaneous and time-averaged flow fields of multiple vortices in the tip region of a ducted propulsor
The instantaneous and time-averaged flow fields in the tip region of a ducted marine propulsor are examined. In this flow, a primary tip-leakage vortex interacts with a secondary, co-rotating trailing edge vortex and other co- and counter-rotating vorticity found in the blade wake. Planar particle imaging velocimetry (PIV) is used to examine the flow in a plane approximately perpendicular to the mean axis of the primary vortex. An identification procedure is used to characterize multiple regions of compact vorticity in the flow fields as series of Gaussian vortices. Significant differences are found between the vortex properties from the time-averaged flow fields and the average vortex properties identified in the instantaneous flow fields. Variability in the vortical flow field results from spatial wandering of the vortices, correlated fluctuations of the vortex strength and core size, and both correlated and uncorrelated fluctuations in the relative positions of the vortices. This variability leads to pseudo-turbulent velocity fluctuations. Corrections for some of this variability are performed on the instantaneous flow fields. The resulting processed flow fields reveal a significant increase in flow variability in a region relatively far downstream of the blade trailing edge, a phenomenon that is masked through the process of simple averaging. This increased flow variability is also accompanied by the inception of discrete vortex cavitation bubbles, which is an unexpected result, since the mean flow pressures in the region of inception are much higher than the vapor pressure of the liquid. Power analysis, sample size, and assessment of statistical assumptions—improving the evidential value of lighting research
The reporting of accurate and appropriate conclusions is an essential aspect of scientific research, and failure in this endeavor can threaten the progress of cumulative knowledge. This is highlighted by the current reproducibility crisis, and this crisis disproportionately affects fields that use behavioral research methods, as in much lighting research. A sample of general and topic-specific lighting research papers was reviewed for information about sample sizes and statistical reporting. This highlighted that lighting research is generally underpowered and, given median sample sizes, is unlikely to be able to reveal small effects. Lighting research most commonly uses parametric statistical tests, but assessment of test assumptions is rarely carried out. This risks the inappropriate use of statistical tests, potentially leading to type I and type II errors. Lighting research papers also rarely report measures of effect size, and this can hamper cumulative science and power analyses required to determine appropriate sample sizes for future research studies. Addressing the issues raised in this article related to sample sizes, statistical test assumptions, and reporting of effect sizes can improve the evidential value of lighting research
Reactive Oxygen Species Production by Forward and Reverse Electron Fluxes in the Mitochondrial Respiratory Chain
Reactive oxygen species (ROS) produced in the mitochondrial respiratory chain (RC) are primary signals that modulate cellular adaptation to environment, and are also destructive factors that damage cells under the conditions of hypoxia/reoxygenation relevant for various systemic diseases or transplantation. The important role of ROS in cell survival requires detailed investigation of mechanism and determinants of ROS production. To perform such an investigation we extended our rule-based model of complex III in order to account for electron transport in the whole RC coupled to proton translocation, transmembrane electrochemical potential generation, TCA cycle reactions, and substrate transport to mitochondria. It fits respiratory electron fluxes measured in rat brain mitochondria fueled by succinate or pyruvate and malate, and the dynamics of NAD+ reduction by reverse electron transport from succinate through complex I. The fitting of measured characteristics gave an insight into the mechanism of underlying processes governing the formation of free radicals that can transfer an unpaired electron to oxygen-producing superoxide and thus can initiate the generation of ROS. Our analysis revealed an association of ROS production with levels of specific radicals of individual electron transporters and their combinations in species of complexes I and III. It was found that the phenomenon of bistability, revealed previously as a property of complex III, remains valid for the whole RC. The conditions for switching to a state with a high content of free radicals in complex III were predicted based on theoretical analysis and were confirmed experimentally. Selection on Alleles Affecting Human Longevity and Late-Life Disease: The Example of Apolipoprotein E
It is often claimed that genes affecting health in old age, such as cardiovascular and Alzheimer diseases, are beyond the reach of natural selection. We show in a simulation study based on known genetic (apolipoprotein E) and non-genetic risk factors (gender, diet, smoking, alcohol, exercise) that, because there is a statistical distribution of ages at which these genes exert their influence on morbidity and mortality, the effects of selection are in fact non-negligible. A gradual increase with each generation of the ε2 and ε3 alleles of the gene at the expense of the ε4 allele was predicted from the model. The ε2 allele frequency was found to increase slightly more rapidly than that for ε3, although there was no statistically significant difference between the two. Our result may explain the recent evolutionary history of the epsilon 2, 3 and 4 alleles of the apolipoprotein E gene and has wider relevance for genes affecting human longevity
