    A Narrative Review of Health-Seeking Behaviour in Malaysia: Factors and Emerging Themes

    This article provides a critical assessment of research on health-seeking behaviour in Malaysia. As the topic is both medically and anthropologically relevant, this narrative review aims to collate and synthesise anthropological and medical studies conducted in Malaysia on health-seeking behaviour among different populations in the country. The specific goal of this narrative review is to identify the methodology employed in such studies, compile a list of medical conditions for which health-seeking behaviour has been researched, identify health-seeking barriers or determinants across studies, and highlight emerging trends in related research. Multiple databases were used to search for published materials on this topic by Malaysian researchers, such as PubMed, Scopus, and My Jurnal. A total of 39 studies were included in this review. 29 studies utilised quantitative designs, while the remaining studies utilised mixed-method and qualitative designs. Non-communicable diseases were studied more frequently than communicable diseases. The majority of reviewed studies were conducted in Peninsular Malaysia, with very few studies focusing on the populations of Sabah and Sarawak in East Malaysia. While all of the studies highlight the multidimensional factors that influence health-seeking behaviour in the context of Malaysia, the utilisation of technology for accessing health-related information and COVID-19-related studies has been identified as an emerging trend in the literature on health-seeking behaviour in this countr

    MENTAL HEALTH RESEARCH: ENGAGING COMMUNITY FOR RESEARCH IMPACT

    This panel discussion was one of the sessions in International Multidisciplinary Prospective Research in Education and Social Sciences (IMPRESS21) conference that took place on 7-9 September 2021. This discussion was about community involvement in mental health research and the panel members were Professor Mohamed Hatta Shaharom, a senior medical practitioner and Professor of Psychiatry. Next panellist, Dr. Kahar Abdulla is an International Coordinator at European Alliance against Depression who is passionate in increasing mental health awareness in the community. Final panellist, Mr. Gan Chun Hong is a lecturer and clinical psychologist whose work focuses on the area of rehabilitation clinical psychology. The aim of this discussion is to illustrate the role of community in mental health management, research and in post-graduate training.     The prevalence of internet gaming disorders and the associated psychosocial risk factors among adolescents in Malaysian secondary schools

    Internet Gaming Disorder (IGD) has been placed under the conditions for further study segment in DSM-5. The purpose of the current study was to develop a preliminary psychosocial model as a reference for providing appropriate intervention, particularly for adolescents with IGD. A total of 5290 adolescents from secondary schools in seven states in Malaysia were recruited by using proportionate random sampling. A standardized set of validated questionnaires such as DASS-21, BIS-11, and IGDS9-SF were distributed to participants that fulfilled the inclusion criteria. The prevalence of IGD among Malaysian adolescents was 3.5%. The bivariate analysis summarized that factors such as stress, impulsivity, gender, time spent using the internet, and relationship with parents and friends; have statistically significant associations with IGD. The logistic regression model revealed that adolescents with IGD were 9 times more likely to experience extremely severe stress (p < 0.001). Several psychosocial factors were associated strongly with IGD in the current study, however, mental health shows the most significant issues among adolescents with IGD. Immediate intervention through a psychological approach to internet gaming is needed from parents, schools, and also respective stakeholders. IGD may become one of the addictions diseases that cause deterioration in many aspects of an adolescent’s future life without serious intervention

    Prevalence and factors associated with intimate partner violence among postpartum women attending government primary health care clinics in Malaysia

    Intimate partner violence (IPV) is an important gender-based, social, and public health problem that affects women worldwide, including women who are pregnant or have recently given birth. Studies have shown that violence against women often increases during pregnancy and the postpartum period. This study aims to examine lifetime and past-year prevalence of IPVamong postpartum women in Malaysia, and to determine the socio-demographic as well as husband’s/partner’s behavioral factors associated with IPV exposure. This is a nationwide, cross-sectional and clinic-based study involving a total of 5727 women at 6 to 16 weeks postpartum, who attended randomly selected government health clinics between July to November 2016. Face-to-face interviews were conducted by trained female enumerators based on a pre-validated structured questionnaire, using mobile devices as data collection tools. Chisquared tests and multivariable logistic regressions were used to investigate selected factors associated with IPVexposure. The lifetime and past-year prevalence of any form of IPV among postpartum women were 4.94% (95% CI [3.81,6.39]) and 2.42% (95% CI [1.74,3.35]) respectively, with the highest prevalence being emotional violence, followed by physical and sexual violence. Multivariable analysis showed that husband’s/partner’s behaviors, such as frequent alcohol use, drug use, fighting habits and controlling behaviour were significantly associated with both lifetime and past-year IPV (all p < 0.001 for past-year IPV). These findings suggest that prevention and intervention strategies for IPV should consider the prevention of substance use and reducing controlling behaviors by husband/partner, as well as raising awareness to build healthy relationships through education

Among the psychological predictors studied are illness perception, health locus of control, and self-efficacy. Quality of life, anxiety and depression and demographic variables are among the predictors included in the study. Two series of studies were conducted to answer the research question formulated for each study. Study 1 aims to determine the psychological predictor of health outcomes among Malaysian hypertensive patient. This Study 1 examines the differences in physical and medical indices as well as psychological predictor across three periods of tune (baseline, after intervention complete and at six month upon completion of the initial intervention) between intervention and waiting list control group. Study 2 explored the determinant of successful control of hypertension among Malaysian patients. In order to adequately answer the research question posted by both studies, both quantitative and qualitative modes of study were selected and patients were recruited from two selected community health centers located in Malaysia. The finding of the Study 1 highlight the physical and medical indices (weight, BMI, SBP, DBP, creatinine level and urea) of the intervention group did significantly change (P<.05) over three times period with a medium to large change seen at Time to Time3 with effect size ranged between -.41 to -.85. Total cholesterol and HDL-C associated with Chance subscales of MHLC Form C (p<.05); and creatinine change with Chance subscales of MHLC Form C (p<.05). However, this finding has unable to provide answer to the question of why some patients undergone an intervention and do well while others do not still remain a question. The findings of the Study 2 highlight the similarities and differences of successful and unsuccessful control of hypertension participant's view on the determinant of successful controlled of hypertension from Malaysian patients perspective. The content analysis conducted on the six interviews revealed both successful and unsuccessful control of hypertension participants shared similar views on the three aspects (i.e. knowledge of condition; factors motivate lifestyle change or barrier to change; and view on current intervention programme) covered during interview. But, both group had slightly different view about control and lifestyle change and it's due to insufficient knowledge of the aspect discuss during interview or some participant felt satisfied with their condition and they feel it is sufficient for them to ensure that they are cure or some felt not ready to commit to any change. Another possible reason for these differences because they just had been few months diagnosed with hypertension. In conclusion, the findings of this research project highlight the importance of further studying the contributing factors which create a gap or discrepancy among those hypertensive patients who has been attending intervention programme. This discrepancy may comes in various form which either from the patient themselves or health care provider or the environment. The study outcome also highlighted the importance for the health care provider to discuss the actual fact about the patient's condition every time patient's attending follow-up clinic. The implication of this practice will limit the patient with whatever knowledge they have on their illness without perceiving illness control should be practiced as a whole. In other words, all the control measures include compliance to medication, controlling diet, regular exercising, and live stressful lifestyle should be perceived as inter-related and not to be selected based on any preference as to ensure positive health outcome.
