    Clinical outcomes of dental implants in patients with and without history of periodontitis: A 20-year prospective study.

    AIM To present the 20-year clinical outcomes of tissue-level implants in partially edentulous patients previously treated for periodontitis and in periodontally healthy patients (PHP). MATERIAL AND METHODS The original population consisted of 149 partially edentulous patients consecutively enrolled in a private specialist practice and divided into three groups: PHP, moderately periodontally compromised patients (mPCP) and severely PCP (sPCP). After successful completion of periodontal/implant therapy, patients were enrolled in an individualized supportive periodontal care (SPC) programme. RESULTS Eighty-four patients rehabilitated with 172 implants reached the 20-year examination. During the observation time, 12 implants were removed (i.e., 11 due to biological complications and 1 due to implant fracture), leading to an overall implant survival rate of 93% (i.e., 94.9% for PHP, 91.8% for mPCP and 93.1% for sPCP [p = .29]). At 20 years, PCP compliant with SPC did not present with significantly higher odds of implant loss compared with PHP compliant with SPC (p > .05). Conversely, PCP not compliant with SPC experienced implant loss with odds ratio of 14.59 (1.30-164.29, p = .03). CONCLUSIONS Tissue-level implants, placed after comprehensive periodontal therapy and SPC, yield favourable long-term results. However, patients with a history of periodontitis and non-compliant with SPC are at higher risk of biological complications and implant loss

    Long-term treatment outcomes of single maxillary buccal peri-implant soft tissue dehiscences: A 10-year prospective study.

    INTRODUCTION To evaluate the 10-year clinical outcomes following surgical treatment of shallow isolated peri-implant soft-tissue dehiscences (PSTD) at single tissue level dental implants. METHODS The baseline population included 16 patients (16 implants) displaying an isolated peri-implant maxillary buccal soft-tissue dehiscence. The recipient bed was prepared with a minimally-invasive split-thickness flap limited to the buccal aspect to stabilize the tuberosity connective tissue graft (CTG) onto the periosteum. At the end of treatment, patients were enrolled in an individualized supportive peri-implant care (SPC) program. The aesthetic outcome was evaluated on photographs by three clinicians using a visual analog scale (VAS). RESULTS SPC during the 10-years proceeded uneventfully in all patients. A total of 12 patients completed the 10-year examination, as 3 patients dropped-out and 1 implant was lost. Complete PSTD coverage was obtained at 7 implant sites (i.e., 58%) while the mean PSTD coverage amounted to 89.6% ± 17.1% without statistically significant differences between 1 and 10 years (p > 0.05). Stable peri-implant parameters (i.e., PD and BoP) and full-mouth scores (i.e., FMPS, FMBS) were recorded throughout the observation period (p > 0.05). The aesthetic improvements obtained in the short-term were maintained up to 10 years. CONCLUSION Within their limits, the present results indicate that the proposed surgical technique is a simple and reliable treatment option for the treatment of single maxillary buccal PSTDs in selected cases with positive results up to 10 years in patients under regular SPC (NCT04983758-this clinical trial was not registered prior to participant recruitment)

    Phenotypic frailty is characterized by a progressive decline in physical functioning. During ageing, morphological and functional alterations involve the brain, and chief theories involve ox-idative stress, free radical accumulation, and reduced antioxidant defenses as the most implicated mechanisms. From boosting the immune system to fighting senescence, medicinal mushrooms have been found to have a number of health and longevity benefits. Among them, Hericium erinaceus (He) has been demonstrated to display a variety of physiological effects, including anti-aging properties. Thus, He represents an attractive natural source for developing novel medicines and functional foods, based on the identification of its active ingredients and metabolites. Particularly, H. erinaceus primordium (He2) extract contains a high amount of Ergothioneine (ERGO), the longevity vitamin. Herein, we revealed the preventive effect of ERGO-rich He2 extract in a preclinical model, focusing on locomotor decline during ageing monitored through spontaneous behavioral test. This effect was accompanied by a significant decrease in some oxidative stress markers (NOS2, COX2) paralleled by an increase in P53, showed in cerebellar cortex cells and fibres by immunohistochemistry. In summary, we demonstrated the neuro-protective and preventive effects of He2 extract during aging, probably due to its peculiarly high ERGO content

    Integrated plan for the sustainable management of the Lagoon of Marano and Grado

    The Lagoon of Marano and Grado has been experiencing some remarkable environmental impacts, which have affected the high-value natural ecosystem and consequently influenced relevant socio-economic aspects. One of the main problems relates to the diffuse contamination of soil and groundwater in the mainland, as well as sediments in rivers and in the lagoon itself. The need to ensure safe navigation conditions in the lagoon and in the Corno River serving Porto Nogaro harbour is another critical emergency. Other environmental issues of concern are: loss of morphological diversity, watershed basins’ pollution loads and protection of lagoon habitats. Due to the emergency situation, a Special Commissioner has been appointed by the national government to proceed with the resolution of the most critical problems. A specific “Operational Plan of interventions” was therefore elaborated in 2009. Approaching problems and solutions in an integrated way, the Plan represents one of the tools for the sustainable development of the lagoon. It strongly supports the adoption of adaptive management, foreseeing the periodical updating and improvement of objectives, strategies and interventions. The Plan also identifies the need for coordination among institutions and involvement of stakeholders as key elements for the implementation of adaptive management

    Building climate resilience through nature-based solutions in Europe: A review of enabling knowledge, finance and governance frameworks

    The European Union (EU) has firmly positioned itself as a global leader in promoting and implementing nature-based solutions (NBS). The recently released EU Biodiversity Strategy for 2030, Strategy on Adaptation to Climate Change, and Forest Strategy - all representing key pillars of the ambitious European Green Deal (EGD) - rely on NBS to both preserve and restore ecosystem integrity and increase climate resilience. Although research and policy in Europe have advanced the conceptualization and operationalization of NBS, a much wider adoption is needed to reach the ambitious goals of the EGD and fulfil its vision of transforming into a sustainable, climate-neutral, climate resilient, fair, and prosperous EU by 2050. In this paper, we review recent EU-supported research, policy, and practices to identify critical dimensions that still need to be addressed for greater uptake of NBS. While recognising the multiple societal challenges NBS can target, we build on the key messages from the ‘5th European Climate Change Adaptation conference ECCA 2021′ and focus our analysis on NBS for climate change adaptation and disaster risk reduction. We screen a wide range of NBS cases across the EU and identify-three core challenges to implementation: the lack of a comprehensive evidence base on the effectiveness of NBS to address targeted challenges; the need for a greater involvement of the private sector in financing NBS; and opportunities for enhancing stakeholder engagement in the successful design and implementation of NBS. We take these challenges as the starting point for a broader reflection and critical discussion on the role of i) knowledge, i) finance, including investments in NBS and divestments from nature-negative projects, and iii) governance and policy frameworks to enable the uptake of NBS. We conclude by identifying options for the EU to foster the uptake of NBS in research, policy and practice

    Flood Protection in Venice under Conditions of Sea-Level Rise: An Analysis of Institutional and Technical Measures

    It is widely acknowledged that in times of climate change loss of coastal resources and risk for human life can be minimized by implementing adaptation strategies. Such strategies need to encompass a balanced mix of non-structural (institutional) and structural (technical) measures based on sound scientific knowledge. This article discusses measures carried out to protect the city of Venice, Italy from flooding (locally known as “high water”), and reflects on their ability to anticipate a possible acceleration of sea-level rise as induced by climate change. It is based on scientific literature, legislative and policy documents of key institutions, reports and documents of organizations working on Venice issues, newspaper articles, and interviews. Our analysis shows that the synergic action of the hydraulic defense infrastructure under construction is in principle adequate to withstand a broad range of sea-level rise scenarios for the next 100 years. However, when the goal is to use these investments effectively major changes in the existing institutional arrangements will be required in the years to come. The Venice findings point out the difficulties and yet the importance of identifying and implementing both non-structural and structural measures to adapt to climate change
