    Cameos of compassion: exploring compassionate care using secondary analysis of digital patients’ stories.

    Patient stories have been identified as a powerful tool to improve quality of care. Healthtalk.org is a digital resource (specific health-related website) presenting patients’ experiences of illness and healthcare through trigger films, videos and articles. Data have been generated from narrative interviews conducted by experienced researchers, based at the Health Experiences Research Group (HERG), University of Oxford. Our project explored the potential use of secondary analysis of digital sources as a methodological innovation to develop as a tool for teaching compassion to nursing students. For that, a purposive sample of transcripts from the HERG archive were selected for secondary analysis. Patients expressed both positive and negative experiences of care. Positive themes included: continuity of care and attentiveness to the fundamentals of care. Negative themes were related to poor quality of care; ignoring patient and family needs; and not being available for patients and family when needed. We concluded that secondary analysis of narrative interviews provides a powerful resource for identifying positive and negative patient experiences for learning and teaching. These can be designed into a digital toolkit and used as a learning and teaching resource to develop staff and students’ reflexivity in relation to the values and leadership behaviours associated with compassionate care and positive practice

    The LABOCA survey of the extended Chandra Deep Field South : two modes of star formation in active galactic nucleus hosts?

    We study the co-existence of star formation and active galactic nucleus (AGN) activity in Chandra X-ray-selected AGN by analyzing stacked 870 μm submillimeter emission from a deep and wide map of the Extended Chandra Deep Field South (ECDFS), obtained with the LABOCA instrument at the APEX telescope. The total X-ray sample of 895 sources with median redshift z ~ 1 drawn from the combined (E)CDFS X-ray catalogs is detected at >11σ significance at a mean submillimeter flux of 0.49 ± 0.04 mJy, corresponding to a typical star formation rate (SFR) around 30 M sun yr-1 for a T = 35 K, β = 1.5 graybody far-infrared spectral energy distribution. The good signal-to-noise ratio permits stacking analyses for major subgroups, splitting the sample by redshift, intrinsic luminosity, and AGN obscuration properties. We observe a trend of SFR increasing with redshift. An increase of SFR with AGN luminosity is indicated at the highest L 2-10 keV >~ 1044 erg s-1 luminosities only. Increasing trends with X-ray obscuration as expected in some AGN evolutionary scenarios are not observed for the bulk of the X-ray AGN sample but may be present for the highest intrinsic luminosity objects with L 2-10 keV >~ 1044 erg s-1. This behavior suggests a transition between two modes in the co-existence of AGN activity and star formation. For the bulk of the sample, the X-ray luminosity and obscuration of the AGN are not intimately linked to the global SFR of their hosts. The hosts are likely massive and forming stars secularly, at rates similar to the pervasive star formation seen in massive galaxies without an AGN at similar redshifts. In these systems, star formation is not linked to a specific state of the AGN and the period of moderately luminous AGN activity may not highlight a major evolutionary transition of the galaxy. The change indicated toward more intense star formation, and a more pronounced increase in SFRs between unobscured and obscured AGN reported in the literature at highest (L 2-10 keV >~ 1044 erg s-1) luminosities suggests that these luminous AGNs follow an evolutionary path on which obscured AGN activity and intense star formation are linked, possibly via merging. Comparison to local hard X-ray-selected AGN supports this interpretation. SFRs in the hosts of moderate luminosity AGN at z ~ 1 are an order of magnitude higher than at z ~ 0, following the increase in the non-AGN massive galaxy population. At high AGN luminosities, hosts on the evolutionary link/merger path emerge from this secular level of star formation

    Understanding and responding to COVID-19 in Wales: protocol for a privacy-protecting data platform for enhanced epidemiology and evaluation of interventions

    INTRODUCTION: The emergence of the novel respiratory SARS-CoV-2 and subsequent COVID-19 pandemic have required rapid assimilation of population-level data to understand and control the spread of infection in the general and vulnerable populations. Rapid analyses are needed to inform policy development and target interventions to at-risk groups to prevent serious health outcomes. We aim to provide an accessible research platform to determine demographic, socioeconomic and clinical risk factors for infection, morbidity and mortality of COVID-19, to measure the impact of COVID-19 on healthcare utilisation and long-term health, and to enable the evaluation of natural experiments of policy interventions. METHODS AND ANALYSIS: Two privacy-protecting population-level cohorts have been created and derived from multisourced demographic and healthcare data. The C20 cohort consists of 3.2 million people in Wales on the 1 January 2020 with follow-up until 31 May 2020. The complete cohort dataset will be updated monthly with some individual datasets available daily. The C16 cohort consists of 3 million people in Wales on the 1 January 2016 with follow-up to 31 December 2019. C16 is designed as a counterfactual cohort to provide contextual comparative population data on disease, health service utilisation and mortality. Study outcomes will: (a) characterise the epidemiology of COVID-19, (b) assess socioeconomic and demographic influences on infection and outcomes, (c) measure the impact of COVID-19 on short -term and longer-term population outcomes and (d) undertake studies on the transmission and spatial spread of infection. ETHICS AND DISSEMINATION: The Secure Anonymised Information Linkage-independent Information Governance Review Panel has approved this study. The study findings will be presented to policy groups, public meetings, national and international conferences, and published in peer-reviewed journals

    Representational predicaments for employees: Their impact on perceptions of supervisors\u27 individualized consideration and on employee job satisfaction

    A representational predicament for a subordinate vis-à-vis his or her immediate superior involves perceptual incongruence with the superior about the subordinate\u27s work or work context, with unfavourable implications for the employee. An instrument to measure the incidence of two types of representational predicament, being neglected and negative slanting, was developed and then validated through an initial survey of 327 employees. A subsequent substantive survey with a fresh sample of 330 employees largely supported a conceptual model linking being neglected and negative slanting to perceptions of low individualized consideration by superiors and to low overall job satisfaction. The respondents in both surveys were all Hong Kong Chinese. Two case examples drawn from qualitative interviews illustrate and support the conceptual model. Based on the research findings, we recommend some practical exercises to use in training interventions with leaders and subordinates. © 2013 Copyright Taylor and Francis Group, LLC

    Patient satisfaction and non-UK educated nurses: a cross-sectional observational study of English National Health Service Hospitals

    Objectives: to examine whether patient satisfaction with nursing care in National Health Service (NHS) hospitals in England is associated with the proportion of non-UK educated nurses providing care.Design: cross-sectional analysis using data from the 2010 NHS Adult Inpatient Survey merged with data from nurse and hospital administrator surveys. Logistic regression models with corrections for clustering were used to determine whether the proportions of non-UK educated nurses were significantly related to patient satisfaction before and after taking account of other hospital, nursing and patient characteristics.Setting: 31 English NHS trusts.Participants: 12?506 patients 16?years of age and older with at least one overnight stay that completed a satisfaction survey; 2962 bedside care nurses who completed a nurse survey; and 31 NHS trusts.Main outcome measure: patient satisfaction.Results: the percentage of non-UK educated nurses providing bedside hospital care, which ranged from 1% to 52% of nurses, was significantly associated with patient satisfaction. After controlling for potential confounding factors, each 10-point increase in the percentage of non-UK educated nurses diminished the odds of patients reporting good or excellent care by 12% (OR=0.88), and decreased the odds of patients agreeing that they always had confidence and trust in nurses by 13% (OR=0.87). Other indicators of patient satisfaction also revealed lower satisfaction in hospitals with higher percentages of non-UK educated nurses.Conclusions: use of non-UK educated nurses in English NHS hospitals is associated with lower patient satisfaction.
