    Ohvira syndrome with left radial hemimelia: a rare association

    Ohvira syndrome is rare complex anomaly consisting of uterus didelphys, unilateral ipsilateral obstructed hemivagina and ipsilateral renal agenesis. It typically presents with dysmenorrhea or pelvic pain shortly after menarche due to collection of secretions in the uterus. Reporting a case of 32 years unmarried nulligravida lady with complaints of pain abdomen since 20 days with history of similar complaints 15 years bac. Patient was on Inj. DMPA 2 years back. On examination upper limb skeletal deformity-left radial hemimelia present with a mass of 14-16 weeks gravid uterus on per abdomen examination. USG (February 2020), was suggestive of uterus didelphys bicollis with collection of 108 cc noted in the right uterine cavity with right kidney agenesis with obscured right ovary. Total abdominal hysterectomy with left salphingo-oopherectomy was done. Specimen features were suggestive of uterus didelphys bicornis bicollis with right uterus hematometra with right cervix blind with right sided blind vagina with features suggestive of Ohvira syndrome

    Evaluation of fungal pathogens, Fusarium semitectum Berk. and Ravenel and Hirsutella thompsonii Fisher against red spider mite, Tetranychus neocaledoni-cus (Andre) of okra under laboratory and greenhouse conditions

    Efficacy of fungal pathogens, Fusarium semitectum Berk. and Ravenel and Hirsutella thompsonii Fisher was evaluated against red spider mite, Tetranychus neocaledonicus (Andre) of okra under laboratory and greenhouse conditions. Among the treatments evaluated under laboratory conditions, the highest mortality of 78.20 and 88.34 percent adults and immatures, respectively was recorded at 2.10 x 109 spores per ml of F. semitectum, whereas it was 82.05 and 92.31 per cent adults and immatures, respectively in case of H. thompsonii at 4.60 x 108 spores per ml. Among the twelve treatments evaluated under greenhouse conditions, H. thompsonii 4.60 x 108 spores per ml, F. semitectum 2.10 x 109 spores per ml + H. thompsonii 4.60 x 108 spores per ml, H. thompsonii 4.60 x 108 spores per ml + dicofol 0.025% and F. semitectum 2.10 x 109 spores per ml + H. thompsonii 4.60 x 108 spores per ml + dicofol 0.025% were found to be on par with each other with a cumulative percent mortality of 81.20, 81.48, 81.21 and 82.59 percent, respectively. By testing the field efficacy of F. semitectum and H. thompsonii against T. neocaledonicus, these fungal pathogens can be used as potential biocontrol agents for the sustainable management of mites on okra. Hence the present study evaluates the potentiality of F. semitectum for the management of T. neocaledonicus by comparing with the well-known acaricidal fungus, H. thompsonii

    Multifactorial Aspects Influencing Non-Alcoholic Fatty Liver Disease (Nafld)

    Nonalcoholic fatty liver disease (NAFLD) is a growing public health concern, with a prevalence of up to 25% worldwide. While once considered a benign condition, NAFLD is now recognized as a major cause of chronic liver disease, liver failure, and hepatocellular carcinoma. The pathogenesis of NAFLD is multifactorial and involves a complex interplay between genetic, environmental, and metabolic factors. In this review, we provide an overview of the multifactorial aspects of NAFLD, including genetic predisposition, insulin resistance, dyslipidemia, gut microbiota, dietary factors, and physical inactivity. We also discuss the role of inflammation, oxidative stress, and hepatic steatosis in the progression of NAFLD to nonalcoholic steatohepatitis (NASH), cirrhosis, and hepatocellular carcinoma. Finally, we review the current and emerging therapies for NAFLD and NASH, including lifestyle modifications, pharmacological interventions, and surgical approaches. The multifactorial nature of NAFLD requires a comprehensive approach to diagnosis, treatment, and prevention, with a focus on addressing the underlying metabolic and environmental factors that contribute to its development and progression

    Prevalence & antibiogram of Pseudomonas aeruginosa at S.S.G. Hospital, Baroda, Gujarat, India

    Background: Pseudomonas aeruginosa is a gram negative bacterium that continues to be a major cause of opportunistic nosocomial infections, causing around 9-10% of hospital infections. It is hard to treat because of intrinsic resistance of the species and its ability to further resistance to multiple groups including β-lactams, aminoglycosides and fluoroquinolones. Aims: This study was undertaken to determine the prevalence of Pseudomonas and their susceptibility pattern at S.S.G. Hospital, BARODA. Materials and Methods: Between March 2015 to May 2015, 150strains of P. aeruginosa were isolated from different clinical specimens. The samples were selected on the basis of their growth on Mac Conkey and nutrient agar medium with oxidase positive. Colonies were subjected to biochemical tests to identify species. Antimicrobial susceptibility of all the isolates was performed by disc diffusion (Kirby –Bauer) method according to CLSIs guidelines. Results: Majority of isolates of P. aeruginosa were obtained from specimens of blood, pus, wound, sputum, tracheal aspirates, pleural fluid, ICD fluid, bile fluid.The prevalence of pathogen was 4.15% and 98% pathogens were sensitive Piperacillin+Tazobactum followed by Meropenem (93.33%), Levofloxacin (92.66%) , Ceftazidime (82%), Cefoperazone(81.33%), Piperacillin (80.66%), Amikacin(56%), Gentamicin(54.66%). Conclusion: The results confirmed the occurrence of drug resistant strains of P. aeruginosa. Meropenem, Levofloxacin and Piperacillin+Tazobactumwere found to be the most effective antimicrobial drugs. It is rational treatment regimens prescription by the physicians to limit the further spread of antimicrobial resistance among the P. aeruginosa strains

    “A Clinical Study Of Feto-Maternal Outcome In Relation To One Or More Than One Loops Of Cord Around Neck”

    Nuchal cord, one or more loops is a common finding at delivery, but its clinical relevance is not entirely clear. Different results have been obtained from several studies that have analysed deliveries with nuchal cords1-5. Midwives have been taught, for over 200 years, that when the head is born in a vertex presentation, they should feel for the cord around the neck6, and, if found, loop it over the head, slip it over the shoulder or severe the cord. If the cord is loose, baby can be delivered normally7,8. If the cord is too tight to go over infant’s head, it can be slipped over the infant’s shoulder and deliver the body through the cord, the cord can then be unwrapped from around the baby after birth. If the cord is too tight to slip back over the shoulder, one may use the somersault manoeuvre to deliver the baby9,10. The cord can also be clamped and cut to allow for vaginal delivery if other methods of nuchal cord management are not feasible

    Loss of N-WASP drives early progression in an Apc model of intestinal tumourigenesis

    N‐WASP (WASL) is a widely expressed cytoskeletal signalling and scaffold protein also implicated in regulation of Wnt signalling and homeostatic maintenance of skin epithelial architecture. N‐WASP mediates invasion of cancer cells in vitro and its depletion reduces invasion and metastatic dissemination of breast cancer. Given this role in cancer invasion and universal expression in the gastrointestinal tract, we explored a role for N‐WASP in the initiation and progression of colorectal cancer. While deletion of N‐wasp is not detectably harmful in the murine intestinal tract, numbers of Paneth cells increased, indicating potential changes in the stem cell niche and migration up the crypt‐villus axis was enhanced. Loss of N‐wasp promoted adenoma formation in an adenomatous polyposis coli (Apc) deletion model of intestinal tumourigenesis. Thus, we establish a tumour suppressive role of N‐WASP in early intestinal carcinogenesis despite its later pro‐invasive role in other cancers. Our study highlights that while the actin cytoskeletal machinery promotes invasion of cancer cells, it also maintains normal epithelial tissue function and thus may have tumour suppressive roles in pre‐neoplastic tissues

    BCAA catabolism in brown fat controls energy homeostasis through SLC25A44.

    Branched-chain amino acid (BCAA; valine, leucine and isoleucine) supplementation is often beneficial to energy expenditure; however, increased circulating levels of BCAA are linked to obesity and diabetes. The mechanisms of this paradox remain unclear. Here we report that, on cold exposure, brown adipose tissue (BAT) actively utilizes BCAA in the mitochondria for thermogenesis and promotes systemic BCAA clearance in mice and humans. In turn, a BAT-specific defect in BCAA catabolism attenuates systemic BCAA clearance, BAT fuel oxidation and thermogenesis, leading to diet-induced obesity and glucose intolerance. Mechanistically, active BCAA catabolism in BAT is mediated by SLC25A44, which transports BCAAs into mitochondria. Our results suggest that BAT serves as a key metabolic filter that controls BCAA clearance via SLC25A44, thereby contributing to the improvement of metabolic health

    Cosmic Evolution Early Release Science (CEERS) survey: The colour evolution of galaxies in the distant Universe

    The wavelength-coverage and sensitivity of JWST now enables us to probe the rest-frame UV - optical spectral energy distributions (SEDs) of galaxies at high-redshift (z>4z>4). From these SEDs it is, in principle, through SED fitting possible to infer key physical properties, including stellar masses, star formation rates, and dust attenuation. These in turn can be compared with the predictions of galaxy formation simulations allowing us to validate and refine the incorporated physics. However, the inference of physical properties, particularly from photometry alone, can lead to large uncertainties and potential biases. Instead, it is now possible, and common, for simulations to be \emph{forward-modelled} to yield synthetic observations that can be compared directly to real observations. In this work, we measure the JWST broadband fluxes and colours of a robust sample of 5<z<105<z<10 galaxies using the Cosmic Evolution Early Release Science (CEERS) Survey. We then analyse predictions from a variety of models using the same methodology and compare the NIRCam/F277W magnitude distribution and NIRCam colours with observations. We find that the predicted and observed magnitude distributions are similar, at least at 5858 the distributions differ somewhat, though our observed sample size is small and thus susceptible to statistical fluctuations. Likewise, the predicted and observed colour evolution show broad agreement, at least at 5<z<85<z<8. There is however some disagreement between the observed and modelled strength of the strong line contribution. In particular all the models fails to reproduce the F410M-F444W colour at z>8z>8, though, again, the sample size is small here.Comment: 11 pages, 10 figures, submitted to MNRA

    Social determinants of health rather than race impact health-related quality of life in 10-year-old children born extremely preterm

    Background: Reducing healthcare disparities among children is extremely important given the potential impact of these disparities on long-term health-related quality of life (HRQL). Race and parental socioeconomic status (SES) are associated with child health-related quality of life (HRQL), but these associations have not been studied in infants born Extremely Preterm (EP), a population at increased risk for physical, cognitive, and psychosocial impairments. Achieving health equity for infants born EP across their life course requires identifying the impact of racism and SES on HRQL. Objective: To evaluate the association between self-reported maternal race, SES factors, and HRQL among 10-year-old children born EP. Design/ Methods: Participants were identified from an ongoing multicenter prospective longitudinal study of Extremely Low Gestational Age Newborns (ELGAN Study), born between 2002-2004, and evaluated at 10 years of age using the Pediatric Quality of Life (QoL) Inventory completed by their parent or guardian, assessing physical, emotional, social, school, and total (composite) QoL domains. Multivariable regression models were used to evaluate the relationship between QoL scores and self-identified maternal race, adjusting for SES factors (education level, marital status, and public insurance). Results: Of 1198 study participants who were alive at 10 years of age, 863 (72.0%) were evaluated at 10 years of age. Differences in mean 10-year QoL scores across racial groups were observed and were significant on univariate analysis. However, these associations attenuated when adjusted for mother’s marital status, public insurance status, and education status. Comparing children with English as primary language spoken at home versus any other language revealed a significant difference only in school QoL, in which non-English language was associated with more favorable school QoL scores. Conclusions: Among 10-year-old children born EP, differences in parent-reported QoL were associated with maternal SES factors but not with race. Our results suggest that interventions designed to improve mothers’ SES may enhance QoL of children born EP. Furthermore, these results underscore that race is a social construct, rather than a biological variable, as we work towards greater equity in care provision
