    Spectral degeneracy and escape dynamics for intermittent maps with a hole

    We study intermittent maps from the point of view of metastability. Small neighbourhoods of an intermittent fixed point and their complements form pairs of almost-invariant sets. Treating the small neighbourhood as a hole, we first show that the absolutely continuous conditional invariant measures (ACCIMs) converge to the ACIM as the length of the small neighbourhood shrinks to zero. We then quantify how the escape dynamics from these almost-invariant sets are connected with the second eigenfunctions of Perron-Frobenius (transfer) operators when a small perturbation is applied near the intermittent fixed point. In particular, we describe precisely the scaling of the second eigenvalue with the perturbation size, provide upper and lower bounds, and demonstrate L1L^1 convergence of the positive part of the second eigenfunction to the ACIM as the perturbation goes to zero. This perturbation and associated eigenvalue scalings and convergence results are all compatible with Ulam's method and provide a formal explanation for the numerical behaviour of Ulam's method in this nonuniformly hyperbolic setting.     The heat transfer modelling for bone metastatic lesion minimization using two different cement types

    Bone tumors grow when cells divide without any control, forming a tissue mass. Bone tumors could be benign or malignant, and primary or metastatic due to systemic cancer cells dissemination. They destroy bone and lead to pathological fractures. The main objective of this work is to study the thermal effect induced by the bone cement polymerization, in the bone metastatic tumor minimization. To assess the clinical effect, it is important to test this methodology before its application and obtain sustained results. In this work, a numerical model was developed to predict the temperature distribution produced by cement polymerization. Thus, distinct tests were produced for different two cements types and amounts introduced in a cortical and spongy bone metastatic lesion, with or without an intramedullary titanium nail. The bone cement was introduced to fill in a metastatic lytic lesion area, which the main objective is playing a promising role for bone tumor necrosis due to thermal effects and biomechanical stabilization for function and pain relief.info:eu-repo/semantics/publishedVersio

    The Effect of Regional Analgesia on Vascular Tone in Hip Arthroplasty Patients

    Background: While it is assumed that neuraxial analgesia and pain management may beneficially influence perioperative hemodynamics, few studies provided data quantifying such effects and none have assessed the potential contribution of the addition of a nerve block. Questions/purposes: This clinical trial compared the visual analog scale (VAS) scores and measurement of arterial tone using augmentation index of patients who received combined spinal-epidural (CSE) only to patients who received both CSE and lumbar plexus block. Methods: After obtaining written consent, 92 patients undergoing total hip arthroplasty were randomized to receive either CSE or CSE with lumbar plexus block (LPB). Perioperative pain and arterial tone were measured using VAS scores and augmentation index (AI) respectively, at baseline and at various times postoperatively. Results: After the exclusion of 2 patients, 44 patients received CSE alone and 46 patients received CSE and LPB. Patient demographics and perioperative characteristics were similar in both groups. AI continuously decreased after placement of a CSE with or without LBP, beyond full resolution of neuraxial and peripheral blockade. Although the LPB group demonstrated a statistically significant reduction of VAS pain scores in the postanesthesia care unit (PACU; P \u3c 0.05), overall, the addition of a LPB did not significantly reduce the AI when compared to the control group. Conclusion: The addition of a LPB provided better pain control in the PACU but did not reduce the AI, compared to the control group. We conclude that the addition of a LPB may have limited ability to affect arterial tone in the presence of a continuous infusion of epidural analgesics. In summary, the addition of a LPB in patients undergoing total hip arthroplasty is clinically effective and provided better pain control, especially in the immediate postoperative period. The continuous decrease on the AI in both groups beyond the full resolution of the neuroaxial and LPB will require further studies

    Numerical thermal study in bone tumor lesion

    With the evolution of science and new diagnostic technologies, it was possible to observe a continuous improvement in the treatments in general and in the aid of the patients' quality of life. Malignant tumors can be primary or secondary (metastases), with abnormal growth of cells able to invade other types of tissues and organs through systemic dissemination. Sarcomas are rare primary malignancies formed from mesenchymal tissue and often located at the extremities. In this work, the main objective is to evaluate the minimization of the evolution of bone tumor lesion through the injection of bone cement, filling in the space of the lytic tumor lesion. This methodology allows to verify at the adjacent cement – bone tissue interface, an increase in temperature that can control the local growth of bone metastasis. Different computational models, obtained by medical image processing, will be carried out for two analyses (patient younger than 70 years and older than 70 years). The computational model allows a transient thermal analysis using the finite element method. The temperature results may determine the thermal necrosis effect in the bone tumor lesion.
