    Improving dialysis adherence for high risk patients using automated messaging: Proof of concept

    AbstractComorbidities and socioeconomic barriers often limit patient adherence and self-management with hemodialysis. Missed sessions, often associated with communication barriers, can result in emergency dialysis and avoidable hospitalizations. This proof of concept study explored using a novel digital-messaging platform, EpxDialysis, to improve patient-to-dialysis center communication via widely available text messaging and telephone technology. A randomized controlled trial was conducted through Washington University-affiliated hemodialysis centers involving ESRD patients with poor attendance, defined as missing 2–6 sessions over the preceding 12 weeks. A cross-over study design evaluated appointment adherence between intervention and control groups. Comparing nonadherence rates eight weeks prior to enrollment, median appointment adherence after using the system increased by 75%, and median number of unintended hospitalization days fell by 31%. A conservative cost-benefit analysis of EpxDialysis demonstrates a 1:36 savings ratio from appointment adherence. EpxDialysis is a low-risk, cost-effective, intervention for increasing hemodialysis adherence in high-risk patients, especially at centers caring for vulnerable and low-income patients.</jats:p

    Improved iron-tolerance in recycled aluminum alloys via direct strip casting process

    Recycled aluminum alloys are pivotal for sustainable manufacturing, offering strength, durability, and environmental advantages. However, the presence of iron (Fe) impurities poses a major challenge, undermining their properties and recyclability. Conventional manufacturing processes result in coarse Fe-rich intermetallic compounds that limit the tolerance of Fe content and negatively influence performance of advanced aluminum alloys. To address this, rapid solidification techniques like direct strip casting have been explored. In this work, a detailed study of the strip cast microstructure was conducted by scanning electron microscopy, electron backscattered diffraction and atom probe tomography. Our results reveal that alloys produced by DSC exhibit significantly refined microstructures and are free from coarse Fe-rich intermetallics, thereby retaining the majority of Fe in solid solution. These findings indicate that strip casting significantly enhances Fe-tolerance in aluminum alloys, making it an attractive process for future aluminum recycling, with implications for sustainable high-performance applications

    Thermal gravity, black holes and cosmological entropy

    Taking seriously the interpretation of black hole entropy as the logarithm of the number of microstates, we argue that thermal gravitons may undergo a phase transition to a kind of black hole condensate. The phase transition proceeds via nucleation of black holes at a rate governed by a saddlepoint configuration whose free energy is of order the inverse temperature in Planck units. Whether the universe remains in a low entropy state as opposed to the high entropy black hole condensate depends sensitively on its thermal history. Our results may clarify an old observation of Penrose regarding the very low entropy state of the universe.Comment: 5 pages, 2 figures, RevTex. v4: to appear in Phys. Rev.

    The Red-Naped Sapsucker in Nebraska

    The Red-naped Sapsucker (Sphyrapicus nuchalis) was for many years accorded subspecific status within Yellow-bellied Sapsucker (Sphyapicus varius), but the American Ornithologists’ Union recently revised this opinion, and now regards nuchalis as a “good” species (Auk 102:680). Red-naped Sapsucker can be added to the Nebraska (species) list on the basis of three specimens in the collection of the University of Nebraska State Museum (UNSM). The breeding range of Red-naped Sapsucker includes the Rocky Mountains east of the Cascades from southwest Canada south to central Arizona and it winters in much of northern Mexico, north as far as the southern part of its breeding range (The NOU Checklist of North American Birds, Sixth Edition, 1983)
