    Relevance of culturally-appropriate approaches in health promotion: a look at Igbo philosophies in dealing with eye care challenges in Nigeria

    Reverence for the culture and beliefs of people most times kindles their interest in a unique way in programmes that they may likely tend to adopt, scorn or attack. In recent times, there have been fatal attacks on health workers in different parts of the world due to misunderstanding of their mission. These health workers have been constantly molested and killed while carrying out their duties. The misunderstandings stem from the fact that these people’s cultural belief was not put into consideration while planning the health programme for that population. Human beings are not chemical elements that could give the same kind of reaction given same environment and conditions, but they have consciences and choices that could be influenced by a number of factors. Hence, this study was an attempt to demonstrate the usefulness of a culturally-appropriate health promotion approach in an adult Nigerian population. This was population-based qualitative study that used face-to-face semi-structured interviews to collect data from three sets of participants [Service users n=28; doctors n=8; and policy makers n=3]. The transcribed interviews provided six important themes among which are: the desire for health education, the need to think outside the box, and the Igbo philosophies. These themes were analysed using interpretative phenomenological approach framework to provide rich information about this population. Understanding the culture and beliefs of the target population could help to shape and plan effective health promotion programmes within a given community.Keywords: Culturally-appropriate approach, Health promotion, Igbo philosophies, eyecare, Nigeri

    Genome-wide and Mendelian randomisation studies of liver MRI yield insights into the pathogenesis of steatohepatitis

    Background A non-invasive method to grade the severity of steatohepatitis and liver fibrosis is magnetic resonance imaging (MRI) based corrected T1 (cT1). We aimed to identify genetic variants influencing liver cT1 and use genetics to understand mechanisms underlying liver fibroinflammatory disease and its link with other metabolic traits and diseases. Methods First, we performed a genome-wide association study (GWAS) in 14,440 Europeans in UK Biobank with liver cT1 measures. Second, we explored the effects of the cT1 variants on liver blood tests, and a range of metabolic traits and diseases. Third, we used Mendelian randomisation to test the causal effects of 24 predominantly metabolic traits on liver cT1 measures. Results We identified six independent genetic variants associated with liver cT1 that reached GWAS significance threshold (p<5x10-8). Four of the variants (rs75935921 in SLC30A10, rs13107325 in SLC39A8, rs58542926 in TM6SF2, rs738409 in PNPLA3) were also associated with elevated transaminases and had variable effects on liver fat and other metabolic traits. Insulin resistance, type 2 diabetes, non-alcoholic fatty liver and BMI were causally associated with elevated cT1 whilst favourable adiposity (instrumented by variants associated with higher adiposity but lower risk of cardiometabolic disease and lower liver fat) was found to be protective. Conclusion The association between two metal ion transporters and cT1 indicates an important new mechanism in steatohepatitis. Future studies are needed to determine whether interventions targeting the identified transporters might prevent liver disease in at risk individuals

    A baseline survey of dissolved methane in aquifers in Great Britain

    Interest in dissolved methane (CH4) concentrations in aquifers in England, Scotland and Wales (‘Great Britain’ or GB) has grown concurrently with interest in the exploitation of unconventional gas sources (UGS). Experience, mainly from North America, has shown the importance of a pre-production baseline against which changes possibly due to UGS extraction can be compared. The British Geological Survey, aided by water utilities, private users and regulators, has compiled a unique dataset for CH4 in groundwaters of GB. This focuses principally on areas where UGS exploration is considered more likely, as indicated by the underlying geology. All the main water supply aquifers (Principal aquifers) were targeted, plus Secondary aquifers where locally important. The average dissolved CH4 concentration across GB in the aquifers sampled was 45 μg/l. Out of a total of 343 sites, 96% showed dissolved CH4 concentrations b100 μg/l, 80% b10 μg/l, and 43% b 1 μg/l. No site had a CH4 concentration above the US Department of the Interior suggested risk action level of 10,000 μg/l. While most sites were sampled only once, a sub set was monitored quarterly to determine the magnitude of seasonal or other variations. Generally these variations were minor, with 84% of sites showing variations within the range 0.5–37 μg/l, but some aquifers where the porosity was primarily fracture-related showed larger changes (0.5–264 μg/l). This may have been due to the nature of sampling at these sites which, unlike the others, did not have installed pumps. Since the regulatory compliance monitoring attending UGS operations will include the measurement of parameters such as dissolved CH4, it is essential that sampling methods are tested to ensure that reliable and comparable datasets can be obtained

    Associations between blood sex steroid concentrations and risk of major adverse cardiovascular events in healthy older women in Australia: a prospective cohort substudy of the ASPREE trial

    Background: Blood testosterone concentrations in women decline during the reproductive years and reach a nadir in the seventh decade, after which concentrations increase and are restored to those of reproductive-aged women early in the eighth decade. We aimed to establish the association between the concentration of testosterone in the blood and risk of major adverse cardiovascular events (MACE) and all-cause mortality in healthy older women. Methods: SHOW was a prospective cohort substudy of the longitudinal randomised ASPREE trial. Eligible participants were women aged at least 70 years from Australia with unimpaired cognition, no previous MACE, and a life expectancy of at least 5 years. Participants who were receiving hormonal or steroid therapy were ineligible for inclusion. We measured serum concentrations of sex steroids with liquid chromatography–tandem mass spectrometry and of SHBG with immunoassay. We compared lower concentrations of sex hormones with higher concentrations using four quartiles. Primary endpoints were risk of MACE and all-cause mortality, the associations of which with sex steroid concentrations were assessed using Cox proportional hazards regression that included age, body-mass index, smoking status, alcohol consumption, diabetes, hypertension, dyslipidaemia, impaired renal function, and treatment allocation in the ASPREE trial (aspirin vs placebo). ASPREE is registered with ClinicalTrials.gov, NCT01038583. Findings: Of the 9180 women recruited to the ASPREE trial between March 10, 2010, and Dec 31 2014, 6358 participants provided sufficient biobank samples at baseline and 5535 were included in the final analysis. Median age at entry was 74·0 years (IQR 71·7–77·7). During a median 4·4 years of follow-up (24 553 person-years), 144 (2·6%) women had a first MACE (incidence 5·9 per 1000 person-years). During a median 4·6 years of follow-up (3·8–5·6), 200 women died (7·9 per 1000 person-years). In the fully adjusted models, higher concentrations of testosterone were associated with a lower incidence of MACE (quartile 4 vs quartile 1: hazard ratio 0·57 [95% CI 0·36–0·91]; p=0·02), as were higher concentrations of DHEA (quartile 4 vs quartile 1: 0·61 [0·38–0·97]; p=0·04). For oestrone, a lower risk of MACE was seen for concentrations in quartile 2 only, compared with quartile 1 (0·55 [0·33–0·92]; p=0·02). In fully adjusted models, no association was seen between SHBG and MACE, or between any hormone or SHBG and all-cause mortality. Interpretation: Blood concentrations of testosterone and DHEA above the lowest quartile in older women were associated with a reduced risk of a first-ever MACE. Given that the physiological effects of DHEA are mediated through its steroid metabolites, if the current findings were to be replicated, trials investigating testosterone therapy for the primary prevention of ischaemic cardiovascular disease events in older women would be warranted.     Spectral Subtraction of Robot Motion Noise for Improved Event Detection in Tactile Acceleration Signals

    New robots for teleoperation and autonomous manipulation are increasingly being equipped with high-bandwidth accelerometers for measuring the transient vibrational cues that occur during con- tact with objects. Unfortunately, the robot\u27s own internal mechanisms often generate significant high-frequency accelerations, which we term ego-vibrations. This paper presents an approach to characterizing and removing these signals from acceleration measurements. We adapt the audio processing technique of spectral subtraction over short time windows to remove the noise that is estimated to occur at the robot\u27s present joint velocities. Implementation for the wrist roll and gripper joints on a Willow Garage PR2 robot demonstrates that spectral subtraction significantly increases signal-to-noise ratio, which should improve vibrotactile event detection in both teleoperation and autonomous robotics

    The Approach to Ergodicity in Monte Carlo Simulations

    The approach to the ergodic limit in Monte Carlo simulations is studied using both analytic and numerical methods. With the help of a stochastic model, a metric is defined that enables the examination of a simulation in both the ergodic and non-ergodic regimes. In the non-ergodic regime, the model implies how the simulation is expected to approach ergodic behavior analytically, and the analytically inferred decay law of the metric allows the monitoring of the onset of ergodic behavior. The metric is related to previously defined measures developed for molecular dynamics simulations, and the metric enables the comparison of the relative efficiencies of different Monte Carlo schemes. Applications to Lennard-Jones 13-particle clusters are shown to match the model for Metropolis, J-walking and parallel tempering based approaches. The relative efficiencies of these three Monte Carlo approaches are compared, and the decay law is shown to be useful in determining needed high temperature parameters in parallel tempering and J-walking studies of atomic clusters.Comment: 17 Pages, 7 Figure

We explore the possibility that these slow moving dark matter particles are small mirror matter dust particles originating from our solar system.     Tidal Dwarf Galaxies at Intermediate Redshifts

    We present the first attempt at measuring the production rate of tidal dwarf galaxies (TDGs) and estimating their contribution to the overall dwarf population. Using HST/ACS deep imaging data from GOODS and GEMS surveys in conjunction with photometric redshifts from COMBO-17 survey, we performed a morphological analysis for a sample of merging/interacting galaxies in the Extended Chandra Deep Field South and identified tidal dwarf candidates in the rest-frame optical bands. We estimated a production rate about 1.4 {\times} 10^{-5} per Gyr per comoving volume for long-lived TDGs with stellar mass 3 {\times} 10^{8-9} solar mass at 0.5<z<1.1. Together with galaxy merger rates and TDG survival rate from the literature, our results suggest that only a marginal fraction (less than 10%) of dwarf galaxies in the local universe could be tidally-originated. TDGs in our sample are on average bluer than their host galaxies in the optical. Stellar population modelling of optical to near-infrared spectral energy distributions (SEDs) for two TDGs favors a burst component with age 400/200 Myr and stellar mass 40%/26% of the total, indicating that a young stellar population newly formed in TDGs.     This paper is devoted to investigate the cold plasma wave properties. The analysis has been restricted to the neighborhood of the pair production region of the Kerr magnetosphere. The Fourier analyzed general relativistic magnetohydrodynamical equations are dealt under special circumstances and dispersion relations are obtained. We find the xx-component of the complex wave vector numerically. The corresponding components of the propagation vector, attenuation vector, phase and group velocities are shown in graphs.     Tunneling of quantum rotobreathers

    We analyze the quantum properties of a system consisting of two nonlinearly coupled pendula. This non-integrable system exhibits two different symmetries: a permutational symmetry (permutation of the pendula) and another one related to the reversal of the total momentum of the system. Each of these symmetries is responsible for the existence of two kinds of quasi-degenerated states. At sufficiently high energy, pairs of symmetry-related states glue together to form quadruplets. We show that, starting from the anti-continuous limit, particular quadruplets allow us to construct quantum states whose properties are very similar to those of classical rotobreathers. By diagonalizing numerically the quantum Hamiltonian, we investigate their properties and show that such states are able to store the main part of the total energy on one of the pendula. Contrary to the classical situation, the coupling between pendula necessarily introduces a periodic exchange of energy between them with a frequency which is proportional to the energy splitting between quasi-degenerated states related to the permutation symmetry. This splitting may remain very small as the coupling strength increases and is a decreasing function of the pair energy. The energy may be therefore stored in one pendulum during a time period very long as compared to the inverse of the internal rotobreather frequency.Comment: 20 pages, 11 figures, REVTeX4 styl
