Concentrations of leptin and C-reactive protein in serum and follicular fluid during assisted reproductive cycles
BACKGROUND: There are only a few studies that have investigated inflammatory processes during ovarian hyperstimulation, with contradictory results especially concerning outcome. The aim of the study was to investigate the inflammatory markers C-reactive protein and leptin in serum and follicular fluid and to correlate these with the outcome. METHODS: One hundred and sixty-two gonadotrophin stimulated cycles were evaluated. Serum concentrations of leptin and C-reactive protein were measured at the initiation of stimulation, on the day of hCG administration or the day before, and on the day of oocyte retrieval. They were also determined in the follicular fluid. RESULTS: Serum leptin and C-reactive protein levels increased significantly during stimulation until the day of oocyte pick up, but following different patterns. After stimulation, they correlated with each other in serum and follicular fluid, but not with estradiol or progesterone concentration, embryo quality, or the pregnancy rate. Molecular imaging: novel tools in visualizing rheumatoid arthritis
Molecular imaging is a rapidly emerging field in biomedical research, aiming at the visualization, characterization and quantification of molecular and cellular processes non-invasively within intact living organisms. To sense biological processes such as gene expression, angiogenesis, apoptosis or cell trafficking in vivo, imaging reporter agents that interact specifically with molecular targets and appropriate imaging systems are currently under development. In rheumatoid arthritis, these novel tools will be used to evaluate physiological and pathophysiological processes, to facilitate diagnosis and monitor therapeutic regimens, to enable reliable prognosis and to support the development of new therapies. In this review, we summarize the basic principles of molecular imaging, such as the development of molecular imaging agents, the actual capabilities of different imaging modalities and the most recent advances in molecular imaging, demonstrating the potential of this technology. With regard to their applicability in rheumatic diseases, we discuss potential molecular targets, current experimental approaches and the future prospects for molecular imaging in rheumatoid arthriti
Analysis of margin classification systems for assessing the risk of local recurrence after soft tissue sarcoma resection
Purpose:
To compare the ability of margin classification systems to determine local recurrence (LR) risk after soft tissue sarcoma (STS) resection.
Methods:
Two thousand two hundred seventeen patients with nonmetastatic extremity and truncal STS treated with surgical resection and multidisciplinary consideration of perioperative radiotherapy were retrospectively reviewed. Margins were coded by residual tumor (R) classification (in which microscopic tumor at inked margin defines R1), the R+1mm classification (in which microscopic tumor within 1 mm of ink defines R1), and the Toronto Margin Context Classification (TMCC; in which positive margins are separated into planned close but positive at critical structures, positive after whoops re-excision, and inadvertent positive margins). Multivariate competing risk regression models were created.
Results:
By R classification, LR rates at 10-year follow-up were 8%, 21%, and 44% in R0, R1, and R2, respectively. R+1mm classification resulted in increased R1 margins (726 v 278, P < .001), but led to decreased LR for R1 margins without changing R0 LR; for R0, the 10-year LR rate was 8% (range, 7% to 10%); for R1, the 10-year LR rate was 12% (10% to 15%) . The TMCC also showed various LR rates among its tiers (P < .001). LR rates for positive margins on critical structures were not different from R0 at 10 years (11% v 8%, P = .18), whereas inadvertent positive margins had high LR (5-year, 28% [95% CI, 19% to 37%]; 10-year, 35% [95% CI, 25% to 46%]; P < .001).
Conclusion:
The R classification identified three distinct risk levels for LR in STS. An R+1mm classification reduced LR differences between R1 and R0, suggesting that a negative but < 1-mm margin may be adequate with multidisciplinary treatment. The TMCC provides additional stratification of positive margins that may aid in surgical planning and patient education
The hysteroscopic view of infertility: the mid-secretory endometrium and treatment success towards pregnancy
The purpose of this study was the analysis of a correlation, in infertile patients, between the quality of the endometrium based on its vascularisation and the chances of conception. Hysteroscopy was carried out to determine the quality of the endometrial surface using the Sakumoto-Masamoto classification ("good” vs. "poor” endometrium) in the secretory phase of the menstrual cycle. The results were set in relation to the outcome of the subsequent infertility treatment, i.e. the establishment of a pregnancy within the study period (4years). In 108 (67%) of the 162 followed-up patients, the endometrium was endoscopically classified as "good”, while in 54 (33%) the result was "poor”. The overall pregnancy rate was 37% (60 patients); 47 of all pregnancies (78%) occurred in women with a "good” endometrium while 13 (22%) had a "poor” classification. This positive association between the establishment of a pregnancy in the follow-up and a "good" classification of the endometrial vasculature in the group with a "good" endometrium was significant (P = 0.0165, Fisher's exact test). The inter-rater reliability of the diagnosis of surgical site infection in the context of a clinical trial.
ObjectivesThe diagnosis of surgical site infection following endoprosthetic reconstruction for bone tumours is frequently a subjective diagnosis. Large clinical trials use blinded Central Adjudication Committees (CACs) to minimise the variability and bias associated with assessing a clinical outcome. The aim of this study was to determine the level of inter-rater and intra-rater agreement in the diagnosis of surgical site infection in the context of a clinical trial.Materials and methodsThe Prophylactic Antibiotic Regimens in Tumour Surgery (PARITY) trial CAC adjudicated 29 non-PARITY cases of lower extremity endoprosthetic reconstruction. The CAC members classified each case according to the Centers for Disease Control (CDC) criteria for surgical site infection (superficial, deep, or organ space). Combinatorial analysis was used to calculate the smallest CAC panel size required to maximise agreement. A final meeting was held to establish a consensus.ResultsFull or near consensus was reached in 20 of the 29 cases. The Fleiss kappa value was calculated as 0.44 (95% confidence interval (CI) 0.35 to 0.53), or moderate agreement. The greatest statistical agreement was observed in the outcome of no infection, 0.61 (95% CI 0.49 to 0.72, substantial agreement). Panelists reached a full consensus in 12 of 29 cases and near consensus in five of 29 cases when CDC criteria were used (superficial, deep or organ space). A stable maximum Fleiss kappa of 0.46 (95% CI 0.50 to 0.35) at CAC sizes greater than three members was obtained.ConclusionsThere is substantial agreement among the members of the PARITY CAC regarding the presence or absence of surgical site infection. Agreement on the level of infection, however, is more challenging. Additional clinical information routinely collected by the prospective PARITY trial may improve the discriminatory capacity of the CAC in the parent study for the diagnosis of infection.Cite this article: J. On the influence of tree size on the climate–growth relationship of New Zealand kauri (Agathis australis): insights from annual, monthly and daily growth patterns
Many tree-ring-based climate reconstructions are based on the assumption that the climate reaction of trees is independent of their size. Here, we test this assumption for New Zealand kauri (Agathis australis), one of the longest tree ring-based proxies for the El Niño-Southern Oscillation (ENSO). The most recent kauri chronology contains a large amount of archaeological material, e.g. timber for which the original tree size is often unknown. We analyzed the climate–growth relationship of different-sized kauri in a pristine forest using different temporal scales, i.e. annually, monthly and daily data on tree growth and climate conditions. Trees of different life stages exhibited approximately the same seasonal growth peaks during austral spring (October and November). The dormancy period overlaps with the period where weekly air temperature maxima are below ca. 17–18 °C, and where the corresponding daily minima are below ca. 8 °C. However, both correlation functions between annual growth and seasonal climate as well as Kalman filter regressions between daily growth and climate conditions suggest an influence of tree size on the climate–growth relationship for kauri. Smaller trees (DBH < 40 cm) contain weaker climate signals than larger trees. Therefore, the precautionary stripping of near-pith material (first 20 cm) from kauri chronologies may result in more uniform responses to climate forcing and thus enhance the reliability of long-term climate reconstructions

The best book on Great Plains sports is H. G. Bissinger\u27s Friday Night Lights (1990). That classic has spawned a critically acclaimed television series and numerous awards. FNL not only told the story of a football season at Odessa Permian High School in urban West Texas; it also asked and answered some very big questions that concerned high school athletic corruption, coaching pressures, cheerleader/ football player interaction, school integration, local community politics, treatment of players of color, Texas\u27s new rules prohibiting playing with failing grades, pressures on teachers, drugs, player abuse, and on and on. It remains a beautifully written and crafted expose.
Our Boys is not Friday Night Lights. The location and subject are comparable: a season with a Great Plains football community with a winning tradition. Smith Center, Kansas, county seat of Smith County situated in northwest Kansas near the Nebraska border, is a small community of 1,931 hardy Kansans. Note that Odessa, Texas, has a population of 90,000 plus-a significant difference, but the passions in Odessa and Smith Center seem quite similar. Many of the Smith County residents are farmers and make use of the hotly contested Republican River waters for irrigation. Farmers, whom Ag schools term producers, produce grains and beef for regional and national markets; Smith Center High School produces football players, occasionally for Kansas State University and more often for regional four-year colleges

Mari Sandoz and Her 1956 Fifty-Year Predictions
Wintertime 1956 in New York City for Mari Sandoz was a time of reassessment. She had been thinking about a commitment she made, and it was time to meet it. She had agreed to compose predictions about American life for the next fifty years (from 1957 to 2007) that along with at least 57 others would be placed in a time capsule and stored in the cornerstone of the building that housed KETV in downtown Omaha.
Sandoz typed up her predictions on her typewriter in her relatively new apartment and entitled the five double-spaced pages December, 2006 A.D. and sent it off. The time capsule was to be opened and shared with the public in the next century in 2007 and without much fanfare, the capsule was dug out and made available. Of course, Sandoz kept copies of her predictions, and they can be found today in the Sandoz Archives at Chadron State College.
