    A review of neuroleptic malignant syndrome : incidence and features in Malta

    This paper describes Malta’s first sample of Neuroleptic Malignant Syndrome (NMS) and reviews the current literature. A retrospective sample of all diagnosed cases of NMS was reviewed using Pope’s (1986) criteria. Twelve cases were identified yielding an incidence of 0.67%. The range of associated risk factors and complications agreed with other reports. There were also two cases of uncontrolled diabetes. Treatments commonly used were Bromocriptine and Levodopa. There were no deaths due to NMS and no recurrence on re-exposure. The sample is too small to draw any statistically significant conclusions, however, the results are mostly in line with those obtained from larger samples. Malta’s incidence is towards the lower end of the reported range of 0.02% to 3.23%, but higher than that reported in centres trying to recognise NMS early and reduce risk factors. This suggests that Malta could benefit from trying to adopt such measures. Given Malta’s small size, it would be relatively easy to disseminate such information. This study, despite its comprehensive sample, failed to reveal any other reported cases. Nor has it been demonstrated that diabetics were at higher risk of developing NMS or its complications.     Factors invoved in onset and recovery from postnatal depression

    Background: A wide variety of psychosocial variables have been implicated in the onset and recovery from postnatal depression. A number of these factors were examined on a representative sample of pregnant Maltese women attending St Luke's Hospital. Method: A random sample of 239 pregnant women were interviewed at booking using a detailed sociodemographic history, the Revised Version of the Clinical Interview Schedule (CIS-R) and Maltese translation of the Edinburgh Postnatal Depression Scale (EPDS). The CIS-R was again administered over the phone at 36 weeks and the EPDS sent by post. At eight weeks postpartum, the CIS-R, modified version of the Social Maladjustment Schedule and the EPDS were again administered to 95.8% of women. Results: Onset of depression in the postpartum was not predicted by depressive symptomatology during pregnancy, marital status, level of education, planning of pregnancy, woman's, partner's or family's reaction to pregnancy and marital adjustment. However being primiparous and employed during pregnancy were significantly associated with postnatal depression. In contrast, depressed women who remained depressed postnatally had a past personal psychiatry history and significantly higher scores on the CISR recorded at 36 weeks gestation. Limitation: The number of women examined in the study yielded a small number of depressed women, for which the results are limited in value. The sample is that of Maltese women booking in at the antenatal clinic, thus excluding women who present late, close to delivery date. The follow up period was limited to eight weeks postpartum only, excluding those who develop depressive episodes after 8 weeks. Conclusions: Some cases of postpartum depression may be traced back to pregnancy while others do not seem to be predictable during pregnancy further contributing evidence to the view that the prenatal period may be a separate entity from the postnatal period with regards to depressive illness.peer-reviewe

    At a meeting of the Association Committee in Luxembourg on April 25th 1972, the Maltese delegation expressed their dissatisfaction with the functioning and results of the EEC/Malta Association Agreement which commenced in April 1971. The delegation stated that although Malta had obtained substantial tariff concessions in the industrial sector, she was 'unable to take advantage of them'. The raison d'etre of the Agreement was to cosolidate and extend the economic and commercial relations, between the EEC and Malta, by the harmonious development of trade. This was made possible under the terms of Article 238 of the Treaty of Rome. 1 It was hoped that the Agreement would result in an extension of Malta's home market for industrial exports which would absorb the existing spare capacity within the industrial sector. It was also seen as a natural extension of European integration, consolidating the political and economic interests involved.peer-reviewe

    Optimal parametrizations of adiabatic paths

    The parametrization of adiabatic paths is optimal when tunneling is minimized. Hamiltonian evolutions do not have unique optimizers. However, dephasing Lindblad evolutions do. The optimizers are simply characterized by an Euler-Lagrange equation and have a constant tunneling rate along the path irrespective of the gap. Application to quantum search algorithms recovers the Grover result for appropriate scaling of the dephasing.     High-quality ion beams by irradiating a nano-structured target with a petawatt laser pulse

    We present a novel laser based ion acceleration scheme, where a petawatt circularly polarized laser pulse is shot on an ultra-thin (nano-scale) double-layer target. Our scheme allows the production of high-quality light ion beams with both energy and angular dispersion controllable by the target properties. We show that extraction of all electrons from the target by radiation pressure can lead to a very effective two step acceleration process for light ions if the target is designed correctly. Relativistic protons should be obtainable with pulse powers of a few petawatt. Careful analytical modeling yields estimates for characteristic beam parameters and requirements on the laser pulse quality, in excellent agreement with one and two-dimensional Particle-in Cell simulations.Comment: 18 pages, 7 figures, accepted in New.     Reinventing residual reserves in the sea: Are we favouring ease of establishment over need for protection?

As systems of marine protected areas (MPAs) expand globally, there is a risk that new MPAs will be biased toward places that are remote or unpromising for extractive activities, and hence follow the trend of terrestrial protected areas in being 'residual' to commercial uses. Such locations typically provide little protection to the species and ecosystems that are most exposed to threatening processes. There are strong political motivations to establish residual reserves that minimize costs and conflicts with users of natural resources. These motivations will likely remain in place as long as success continues to be measured in terms of area (km2) protected. The global pattern of MPAs was reviewed and appears to be residual, supported by a rapid growth of large, remote MPAs. The extent to which MPAs in Australia are residual nationally and also regionally within the Great Barrier Reef (GBR) Marine Park was also examined. Nationally, the recently announced Australian Commonwealth marine reserves were found to be strongly residual, making almost no difference to 'business as usual' for most ocean uses. Underlying this result was the imperative to minimize costs, but without the spatial constraints of explicit quantitative objectives for representing bioregions or the range of ecological features in highly protected zones. In contrast, the 2004 rezoning of the GBR was exemplary, and the potential for residual protection was limited by applying a systematic set of planning principles, such as representing a minimum percentage of finely subdivided bioregions. Nonetheless, even at this scale, protection was uneven between bioregions. Within-bioregion heterogeneity might have led to no-take zones being established in areas unsuitable for trawling with a risk that species assemblages differ between areas protected and areas left available for trawling. A simple four-step framework of questions for planners and policy makers is proposed to help reverse the emerging residual tendency of MPAs and maximize their effectiveness for conservation. This involves checks on the least-cost approach to establishing MPAs in order to avoid perverse outcomes

    Calcium-phosphate derived from mineralized algae for bone tissue engineering applications

    In this work, several routes are described towards obtaining pure inorganic phases derived from Coralline officinallis red algae. The scanning electron microscopy studies have shown that it becomes possible not only to eliminate the undesired organic phase, but also to preserve or tailor the red algae typical microporosity. X-ray diffraction analysis was used to investigate the phase content of the red algae before and after performing the different treatment routes. Hydroxyapatite nanocrystallites were obtained after converting the coralline calcium carbonate skeleton by means of combining thermal and chemical routes. These results were confirmed by Fourier transform infra-red spectroscopic analysis. The processing routes herein described are very promising in order to design bioceramics of algae origin that might find useful applications as bone fillers and tissue engineering scaffolds

    Drinking by sea snakes from oceanic freshwater lenses at first rainfall ending seasonal drought

    Acquisition of fresh water (FW) is problematic for FW-dependent animals living in marine environments that are distant from sources of FW associated with land. Knowledge of how marine vertebrates respond to oceanic rainfall, and indeed the drinking responses of vertebrates generally following drought, is extremely scant. The Yellow-bellied Sea Snake (Hydrophis platurus) is the only pelagic species of squamate reptile and ranges across the Indo-Pacific oceans, having one of the largest geographic distributions of any vertebrate species. It requires FW and dehydrates at sea during periods of drought. Here we report drinking behaviors of sea snakes precisely at the transition from dry to wet season when rainfall first impacted the ocean following 6 months of seasonal drought. We show that the percentage of sea snakes that voluntarily drank FW in the laboratory when captured over eight successive days decreased from 80% to 13% before and after rainfall commenced, respectively. The percentage of snakes that drank immediately following capture exhibited a significant linear decline as the earliest rains of the wet season continued. Drinking by snakes indicates thirst related to dehydration, and thus thirsty snakes must have dehydrated during the previous six months of drought. Hence, the progressive decline in percentage of thirsty snakes indicates they were drinking from FW lenses associated with the first rainfall events of the wet season. These data reinforce the importance of accessing oceanic FW from precipitation, with implications for survival and distribution of pelagic populations that might be subjected to intensifying drought related to climate change
