Multiple congenital bilateral trigger digits in a 2-year-old child: case report

    Get PDF
Trigger finger is a rare condition in children. In this paper, we report on a 2-year-old boy with multiple congenital bilateral trigger digits. The patient had no history of perinatal trauma, viral or bacterial infections, or metabolic disorders. The patient was treated with physiotherapy for one year. At the one-year follow-up, the boy presented with six trigger fingers (3 on the right hand, 3 on the left hand). Neither thumb was involved. The six trigger fingers were treated surgically: first, the right-hand trigger fingers and, six months later, those of the left hand. After each operation, a 4-week brace in extension was applied to the operated hand. The symptoms were completely resolved after surgical treatment. Many authors have recommended surgical release for the treatment of trigger finger in children; empirical treatment with physiotherapy may be an option when symptoms present or appear at an older age

Content: Beamhouse area is an important step in leather technology, either in the final quality of the leather or in the contribution to the contamination of effluents. In unhairing process, the use of enzymes as sulphide assistants can reduce sulfhidric gas emissions to reach permitted levels for health. The characterization of enzymatic extracts allows controlling the proteolysis mechanism so that its action does not attack the reticular structure. A new fungal enzyme extracts were applied on submerged bovine skin in the soaking and unhairing steps. Fungal strains were isolated from alkaline soils of coast of Buenos Aires province and enzymatic extracts (EE) were obtained by submerged culture with bovine hair waste from hair-saving unhairing process as inducer in the production of keratinolytic enzymes. This activity was determined by quantitative test and the most productive strains were selected: Clonostachys rosea (CR), Emericellopsis minima (EM), Paecilomyces lilacinus (PL), Penicillium sp (Psp), Fusarium oxysporum (FO), Acremonium sp (A) and an unidentified filamentous fungus strain with sterile yellow mycelium (SYM). EE were tested at laboratory scale on submerged bovine skin with biocide 0,2% w/w wet skin, anionic 0,1% and non-ionic 0,5% tensioactives in soaking and unhairing respectively and incubated 48 h at 37°C in shaking thermostatic bath. Controls were tested in the same conditions without EE. Morphological changes in the skin were observed by scanning electron microscopy (SEM). Pieces of bovine skin were fixed, post-fixed in formaldehyde 4% and dehydrated in alcohol solutions, treated by critical point drying, metalized and analyzed in SEM. It was observed with FO, EM and A extracts empty hair follicles, absence of epidermis, papillary layer of dermis exposed, hairs enclosed by detached hair follicle sheaths and collagen fibers without characteristic patterns. With PL and SYM EE some hairs were detached while Psp and CR EE did not produce changes. In control samples epidermis and hair without modification were observed. In the quantitative test, keratinolytic activities increased in the following order: A<PL<Psp<CR<FO<SYM. Submerged culture with keratin as inductor produced keratinolytic enzymes useful for unhairing step. Fusarium oxysporum extract showed the greatest effect on the skin, thus the relationship between keratinolytic activity and depilatory effect was found. However, it is necessary to find the optimal conditions to avoid the damage of collagen and enable its application as a sustainable technology. Wickerhamomyces anomalus is pectinolytic yeast isolated from citrus fruits peels in the province of Misiones, Argentine. In the present work, enzymes produced by this yeast strain were characterized, and polygalacturonase physicochemical properties were determined in order to evaluate the application of the supernatant in the maceration of potato tissues. W. anomalus was able to produce PGin liquid mediumcontaining glucose and citrus pectin, whosemode of actionwas mainly of endo type. The supernatant did not exhibit esterase or lyase activity. No others enzymes, capable of hydrolyzing cell wall polymers, such as cellulases and xylanases, were detected. PGshowedmaximal activity atpH4.5 and at temperature range between 40∘Cand50∘C. It was stable in the pH range from3.0 to 6.0 and up to 50∘Cat optimumpH.The enzymatic extractmacerated potato tissues efficiently.Volume of single cells increased with the agitation speed. The results observed make the enzymatic extract produced by W. anomalus appropriate for future application in food industry, mainly for the production of fruit nectars or mashed of vegetables such as potato or cassava, of regional interest in the province of Misiones, Argentine.Fil: Martos, María Alicia. We report the clinical and radiographic medium-term results obtained for 20 patients (24 fingers) treated surgically for post-traumatic malunion of the proximal phalanx of the finger. In all cases we performed a corrective osteoclasia or osteotomy at the site of malunion, followed by miniplate and screw fixation or by screw fixation only. The corrective osteoclasia was performed when malalignment was addressed within six weeks after injury. Two patients who had two fractures underwent additional surgery (tenolysis and/or capsulolysis) to improve function and ROM. At the final follow-up, at a mean of 24 months after corrective surgery, good or excellent clinical and radiographic results were obtained in all the patients. The pseudoclaw deformity disappeared in all cases in which a volar angulation deformity was present. An average improvement of about 30% in the range of motion of the MP and PIP joints was observed; only 4 patients complained of mild pain at the maximum degrees of articular excursion of the MP and PIP joints. All the patients presented an improvement in grip strength. The mean DASH score in our series was 5 points. In two of the four cases treated by an intra-articular corrective osteotomy, mild radiographic signs of osteoarthritis at the MP joint were present. The data for this study confirm that "in situ" osteotomy stabilized by miniplates and/or screws is an effective procedure to correct post-traumatic malunions of the proximal phalanges of the fingers

Present work employs the QSAR formalism to predict the ED50 anticonvulsant activity of ringed-enaminones, in order to apply these relationships for the prediction of unknown open-chain compounds containing the same types of functional groups in their molecular structure. Two different modeling approaches are applied with the purpose of comparing the consistency of our results: (a) the search of molecular descriptors via multivariable linear regressions; and (b) the calculation of flexible descriptors with the CORAL (CORrelation And Logic) program. Among the results found, we propose some potent candidate open-chain enaminones having ED50 values lower than 10 mg·kg-1 for corresponding pharmacological studies. These compounds are classified as Class 1 and Class 2 according to the Anticonvulsant Selection Project.Instituto de Investigaciones Fisicoquímicas Teóricas y AplicadasFacultad de Ciencias Exacta
