Valproic acid restricts mast cell activation by Listeria monocytogenes.
Mast cells (MC) play a central role in the early containment of bacterial infections, such as that caused by Listeria monocytogenes (L.m). The mechanisms of MC activation induced by L.m infection are well known, so it is possible to evaluate whether they are susceptible to targeting and modulation by different drugs. Recent evidence indicates that valproic acid (VPA) inhibits the immune response which favors L.m pathogenesis in vivo. Herein, we examined the immunomodulatory effect of VPA on L.m-mediated MC activation. To this end, bone marrow-derived mast cells (BMMC) were pre-incubated with VPA and then stimulated with L.m. We found that VPA reduced MC degranulation and cytokine release induced by L.m. MC activation during L.m infection relies on Toll-Like Receptor 2 (TLR2) engagement, however VPA treatment did not affect MC TLR2 cell surface expression. Moreover, VPA was able to decrease MC activation by the classic TLR2 ligands, peptidoglycan and lipopeptide Pam3CSK4. VPA also reduced cytokine production in response to Listeriolysin O (LLO), which activates MC by a TLR2-independent mechanism. In addition, VPA decreased the activation of critical events on MC signaling cascades, such as the increase on intracellular Ca2+ and phosphorylation of p38, ERK1/2 and -p65 subunit of NF-κB. Altogether, our data demonstrate that VPA affects key cell signaling events that regulate MC activation following L.m infection. These results indicate that VPA can modulate the functional activity of different immune cells that participate in the control of L.m infection
The histone deacetylase inhibitor valproic acid attenuates phospholipase Cγ2 and IgE-mediated mast cell activation.
Mast cell activation through the high-affinity IgE receptor (FcεRI) plays a central role in allergic reactions. FcεRI-mediated activation triggers multiple signaling pathways leading to degranulation and synthesis of different inflammatory mediators. IgE-mediated mast cell activation can be modulated by different molecules, including several drugs. Herein, we investigated the immunomodulatory activity of the histone deacetylase inhibitor valproic acid (VPA) on IgE-mediated mast cell activation. To this end, bone marrow-derived mast cells (BMMC) were sensitized with IgE and treated with VPA followed by FcεRI cross-linking. The results indicated that VPA reduced mast cell IgE-dependent degranulation and cytokine release. VPA also induced a significant reduction in the cell surface expression of FcεRI and CD117, but not other mast cell surface molecules. Interestingly, VPA treatment inhibited the phosphorylation of PLCγ2, a key signaling molecule involved in IgE-mediated degranulation and cytokine secretion. However, VPA did not affect the phosphorylation of other key components of the FcεRI signaling pathway, such as Syk, Akt, ERK1/2, or p38. Altogether, our data demonstrate that VPA affects PLCγ2 phosphorylation, which in turn decreases IgE-mediated mast cell activation. These results suggest that VPA might be a key modulator of allergic reactions and might be a promising therapeutic candidate

This chapter presents a review of the situation concerning the various geomorphological hazards in the country, including some information about existing programmes for research, control and mitigation. With its great variety of climatic, geological and morphodynamic environments, Spain is subject to every kind of natural hazard: tsunamis, floods, volcanism, and mass movements. The whole of the territory is prone to some kind of geomorphological hazard but it is in the eastern and southern coastal strips that the risks are greatest. One of the main problems for the mitigation of geomorphological hazards in Spain is the lack of an appropriate regulatory framework for the incorporation of natural hazard assessments into land-use planning and management at the macro-, meso- and micro-planning levels. The coverage of hazard mapping is still far from complete or adequate, and much work remains to be done. There has been considerable diversity in the methods used for risk assessment and for the cartographic representation of natural hazards. An urgent need is to establish common, accepted methodologies and criteria, based on indicators defined as clearly as possible, and to standardize map legends and scales for different planning levels. Information programmes for the general public also need to be considerably expanded
Metabolic control in a nationally representative diabetic elderly sample in Costa Rica: patients at community health centers vs. patients at other health care settings
<p>Abstract</p> <p>Background</p> <p>Costa Rica, like other developing countries, is experiencing an increasing burden of chronic conditions such as diabetes mellitus (DM), especially among its elderly population. This article has two goals: (1) to assess the level of metabolic control among the diabetic population age ≥ 60 years old in Costa Rica, and (2) to test whether diabetic elderly patients of community health centers differ from patients in other health care settings in terms of the level of metabolic control.</p> <p>Methods</p> <p>Data come from the project CRELES, a nationally representative study of people aged 60 and over in Costa Rica. This article analyzes a subsample of 542 participants in CRELES with self-reported diagnosis of diabetes mellitus. Odds ratios of poor levels of metabolic control at different health care settings are computed using logistic regressions.</p> <p>Results</p> <p>Lack of metabolic control among elderly diabetic population in Costa Rica is described as follows: 37% have glycated hemoglobin ≥ 7%; 78% have systolic blood pressure ≥ 130 mmHg; 66% have diastolic blood pressure ≥ 80 mmHg; 48% have triglycerides ≥ 150 mg/dl; 78% have LDL ≥ 100 mg/dl; 70% have HDL ≤ 40 mg/dl. Elevated levels of triglycerides and LDL were higher in patients of community health centers than in patients of other clinical settings. There were no statistical differences in the other metabolic control indicators across health care settings.</p> <p>Conclusion</p> <p>Levels of metabolic control among elderly population with DM in Costa Rica are not that different from those observed in industrialized countries. Elevated levels of triglycerides and LDL at community health centers may indicate problems of dyslipidemia treatment among diabetic patients; these problems are not observed in other health care settings. The Costa Rican health care system should address this problem, given that community health centers constitute a means of democratizing access to primary health care to underserved and poor areas.</p
First Latin American clinical practice guidelines for the treatment of systemic lupus erythematosus: Latin American Group for the Study of Lupus (GLADEL, Grupo Latino Americano de Estudio del Lupus)-Pan-American League of Associations of Rheumatology (PANLAR)
Systemic lupus erythematosus (SLE), a complex and heterogeneous autoimmune disease, represents a significant challenge for both diagnosis and treatment. Patients with SLE in Latin America face special problems that should be considered when therapeutic guidelines are developed. The objective of the study is to develop clinical practice guidelines for Latin American patients with lupus. Two independent teams (rheumatologists with experience in lupus management and methodologists) had an initial meeting in Panama City, Panama, in April 2016. They selected a list of questions for the clinical problems most commonly seen in Latin American patients with SLE. These were addressed with the best available evidence and summarised in a standardised format following the Grading of Recommendations Assessment, Development and Evaluation approach. All preliminary findings were discussed in a second face-to-face meeting in Washington, DC, in November 2016. As a result, nine organ/system sections are presented with the main findings; an 'overarching' treatment approach was added. Special emphasis was made on regional implementation issues. Best pharmacologic options were examined for musculoskeletal, mucocutaneous, kidney, cardiac, pulmonary, neuropsychiatric, haematological manifestations and the antiphospholipid syndrome. The roles of main therapeutic options (ie, glucocorticoids, antimalarials, immunosuppressant agents, therapeutic plasma exchange, belimumab, rituximab, abatacept, low-dose aspirin and anticoagulants) were summarised in each section. In all cases, benefits and harms, certainty of the evidence, values and preferences, feasibility, acceptability and equity issues were considered to produce a recommendation with special focus on ethnic and socioeconomic aspects. Microenvironment M1/M2 macrophages and tumoral progression vary within C57BL/6 mice from same substrain in prostate cancer model
Abstract Cancer mice models are critical for immune-oncology research; they provide conditions to explore tumor immunoenviroment aiming to advance knowledge and treatment development. Often, research groups breed their own mice colonies. To assess the effect of C57BL/6 mice breeding nuclei in prostate cancer development and intratumoral macrophage populations, an isotransplantation experiment was performed. C57BL/6J mice from two breeding nuclei (nA and nB) were employed for prostate adenocarcinoma TRAMP-C1 cell implantation; tumor growth period and intratumoral macrophage profile were measured. BL/6nB mice (54%) showed tumor implantation after 69-day growth period while BL/6nA implantation reached 100% across tumor growth period (28 days). No difference in total macrophage populations was observed between groups within several tumoral regions; significantly higher M2 macrophage profile was observed in tumor microenvironments from both mice groups. Nevertheless, BL/6nB tumors showed around twice the population of M1 profile (11–27%) than BL6nA (4–15%) and less non-polarized macrophages. The M1:M2 average ratio was 1:8 for group A and 1:4 for B. Our results demonstrate different tumor progression and intratumoral macrophage populations among mice from the same substrain. Data obtained in this study shows the relevance of animal source renewal for better control of murine cancer model variables
Pregnant Women Infected with Zika Virus Show Higher Viral Load and Immunoregulatory Cytokines Profile with CXCL10 Increase
Background: Zika virus (ZIKV) infection during pregnancy usually shows only mild symptoms and is frequently subclinical. However, it can be vertically transmitted to the fetus, causing microcephaly and other congenital defects. During pregnancy, the immune environment modifications can alter the response to viruses in general and ZIKV in particular. Objective: To describe the role of pregnancy in the systemic pro- and anti-inflammatory response during symptomatic ZIKV infection. Materials and Methods: A multiplex assay was used to measure 25 cytokines, chemokines, and receptors in 110 serum samples from pregnant and nonpregnant women with and without ZIKV infection with and without symptoms. Samples were collected through an epidemiological surveillance system. Results: Samples from pregnant women with ZIKV infection showed a higher viral load but had similar profiles of inflammatory markers as compared with nonpregnant infected women, except for CXCL10 that was higher in infected pregnant women. Notably, the presence of ZIKV in pregnancy favored a regulatory profile by significantly increasing anti-inflammatory cytokines such as interleukin (IL)-10, receptors IL-1RA, and IL-2R, but only those pro-inflammatory cytokines such as IL-6, interferon (IFN)-α, IFN-γ and IL-17 that are essential for the antiviral response. Interestingly, there were no differences between symptomatic and weakly symptomatic ZIKV-infected groups. Conclusion: Our results revealed a systemic anti-inflammatory cytokine and chemokine profile that could participate in the control of the virus. The anti-inflammatory response in pregnant women infected with ZIKA was characterized by high CXCL10, a cytokine that has been correlated with congenital malformations
Epilepsia extratemporal. Diagnóstico neuropatológico y evolución clínica postquirúrgica. Programa Nacional de Cirugía de Epilepsia, 2002-2017
In centers dedicated to epilepsy surgery, about 20% of surgical
interventions involve extratemporal epilepsy and the majority
of cases correspond to the frontal lobe. The objective was to
evaluate the neuropathological findings and the post-surgical
clinical evolution in patients with drug-resistant extratemporal
epilepsy treated in the National Epilepsy Surgery Program. From
the database (Pathological Anatomy) of patients with drugresistant focal epilepsy, operated between 2002 and 2017, cases
with extratemporal epilepsy were selected. All surgeries were
guided by electrocorticography. Of the 64 patients studied, 17
were extratemporal. The average age at the time of surgery was
24.7 years In 66.66%, lesions were detected in imaging studies.
Histological diagnoses were focal cortical dysplasia type I (4), IIa
(1) and IIb (5); pilocytic astrocytoma (1); cavernous angioma (1);
ganglioglioma (1); meningioangliomatosis (1); polymicrogyria (1)
and descriptive (2). In the last evaluation, 8 patients continued with
epileptic seizures (47.05%), 6 were without seizures (35.29%)
and 3 did not attend the consultation. 26.56% of the patients
corresponded with extra-temporal epilepsy, a result similar to that
described in the literature. Neuropathological diagnoses are similar
to those found in temporal lobe epilepsy with a predominant focal
cortical dysplasia. Although the post-surgical outcome is inferior to
temporary resections, surgery offers a benefit to these patients by
reducing the frequency of seizuresEn los centros dedicados a la cirugía de la epilepsia, alrededor del
20% de las intervenciones quirúrgicas involucran a las epilepsias
extratemporales y la mayoría de los casos corresponden al lóbulo
frontal. El objetivo fue evaluar los hallazgos neuropatológicos y
la evolución clínica postquirúrgica en los pacientes con epilepsias
extratemporales farmacorresistente atendidos en el Programa
Nacional de Cirugía de Epilepsia. De la base de datos (Anatomía
Patológica) de pacientes con epilepsia focal farmacorresistente,
operados entre 2002 y 2017. Se seleccionaron los casos con
epilepsias extratemporales, todas las cirugías se guiaron por
electrocorticografía. De los 64 pacientes estudiados, 17 fueron
extratemporales. La edad promedio en el momento de la cirugía
fue de 24,7 años. En el 66,66% se detectó lesión en los estudios
imagenológicos. Los diagnósticos histológicos fueron displasia
cortical focal tipo I (4), IIa (1) y IIb (5); astrocitoma pilocítico (1);
angioma cavernoso (1); ganglioglioma (1); meningioangliomatosis
(1); polimicrogiria (1) y descriptivo (2). En la última evaluación
realizada, 8 pacientes (47,05%) continuaban con crisis epilépticas,
6 (35,29%) estaban sin crisis y 3 no asistieron a consulta. El
26,56% de los pacientes se correspondió con una epilepsia
extratemporal, resultado similar a lo descrito en la literatura. Los
diagnósticos neuropatológicos son similares a los encontrados
en la epilepsia del lóbulo temporal con predomino de la displasia
cortical focal. A pesar de que el resultado postquirúrgico es inferior
a las resecciones temporales, la cirugía ofrece un beneficio a estos
pacientes al reducir la frecuencia de las crisi
