THE USE FGSI AS PREDICTIVE OF UROLOGICAL SIRS PATIENTS

Objective: To demonstrate the usefulness of Fournier’s Gangrene Severity Index (FGSI) score for differentiating the outcome between Systemic Inflammatory Response Syndrome (SIRS) patients with upper and lower urological abnormalities. Material & Method:A retrospective study of case records from year 2009-2012 of SIRS patients with urological abnormalities at Saiful Anwar General Hospital (SAGH) Malang was carried out. The data were collected from the Medical Record Division in SAGH Malang. SIRS was clinicaly diagnosed based on medical history, physical examination and laboratory findings. SIRS without urological abnormalities were excluded from the analysis. The FGSI, which was developed to assign a numerical score that describes the severity of disease, was used in our study. This index presents patients vital signs (temperature, heart and respiratory rates) and metabolic parameters (sodium, potassium, creatinine, and bicarbonate levels, hematocrit, and white blood cell count). Patients with SIRS and urological abnormalities were divided according to upper tract and lower tract urological abnormalities. Total FGSI score was classified as mild (0-8), moderate (9-17), severe (> 17). The data were assessed according to whether the patient survived or died.Results:75 of the 203 evaluated patients died. From those 75 patients, 67% were male and 33% were female, 75% with upper urological abnormalities and 25% with lower abnormalities (p < 0,05). From those 203 patients; the results were analyzed with binary logistical regression and Spearman correlation analysis using SPSS 15 softwarewith 95% confidence interval (CI). There is significant relationship between FGSI and outcome of the patient with upper urological abnormalities and lower urological abnormalities, with correlation coefficient more high in relationship between FGSI and outcome of the patient with upper urological abnormalities (0,4 vs 0,1). Conclusion: FGSI is simple and objective outcome predictor to differentiate survival between SIRS patients with upper urological abnormalities and lower urological abnormalities. EVALUASI KINERJA OPERASIONAL ARMADA BARU PERUM DAMRI UBK SEMARANG ( TRAYEK BANYUMANIK – JOHAR ) (Operational Performance Evaluation of New Fleet Perum Damri UBK Semarang (Banyumanik – Johar Route ))

Pengembangan dan pembangunan kota Semarang salah satunya terwujud dengan adanya pemusatan dan penyebaran lokasi kegiatan masyarakat. Perum Damri yang bergerak dibidang angkutan umum memfasilitasi pergerakan mobilisasi penduduk untuk mendapatkan akses ke lokasi – lokasi tersebut dengan menggunakan armada bus besar. Pada akhir tahun 2004, Perum Damri mulai meremajakan armadanya dengan bus sedang untuk dapat memenuhi efisiensi dan efektivitas pergerakan. Penelitian terhadap kinerja operasional armada baru berupa bus sedang pada trayek Perumnas Banyumanik – Pasar Johar dimaksudkan untuk memberikan informasi kepada Perum Damri sebagai acuan untuk memperbaiki atau meningkatkan efisiensi dan efektifitas untuk berkembang sejalan dengan kebutuhan masyarakat akan jasa angkutan kota di Semarang. Metode penelitian ini adalah penelitian lapangan dengan mengumpulkan data yang berhubungan dengan parameter transportasi jalan meliputi kecepatan perjalanan, selang waktu, waktu singgah, faktor muat, tingkat ketersediaan, waktu sirkulasi, dan utilisasi. Dari hasil analisa diperoleh bahwa kinerja operasional angkutan bus sedang pada rute Banyumanik – Johar adalah sebagai berikut : • Kecepatan (21,23 km/jam), faktor muat (170,45%), waktu sirkulasi (101,5 menit), dan kelayakan (1,22) merupakan parameter yang terpenuhi. • Selang waktu (9,36 menit), Tingkat ketersediaan (76,47%), utilitas kendaraan (219,1 km), dan waktu singgah (12,68 menit) merupakan parameter yang belum terpenuhi. Dari hasil penelitian dapat disimpulkan bahwa kinerja operasional bus Damri trayek Perumnas Banyumanik – Pasar Johar cukup baik dengan beberapa kekurangan tersebut di atas Untuk meningkatkan pelayanan kepada masyarakat maka disarankan Perum Damri menambah armada bus sebanyak 2 unit dengan mempertahankan tarif awal ( Umum : Rp 1500,- ; Pelajar : Rp 1000,- ) atau menambah armada busnya dengan disertai kenaikan tarif, menyusun kembali jadual tiba dan pemberangkatan serta perlu adanya kedisiplinan dari kru bus Damri dalam pengoperasianny

EFFECTIVITY OF LI-ESWT FOR ERECTILE DYSFUNCTION MANAGEMENT BASED ON EHS AND IIEF-5

Objective: This study was conducted to determine the effectiveness of LI-ESWT as therapeutic modality in ED patients. Material & Methods: Erectile Hardness Scales (EHS) and International Index of Erectile Function (IIEF-5) scores was measured in 22 patients. The observation was performed before therapy started, each therapy session, and each follow-up session. Therapy sessions were carried out once a month for 8 months and follow-up was done once a month for 8 months. Results: There was a significant increase in IIEF-5 scores since the 5th LI-ESWT therapy session (P<0.05, Wilcoxon Test). Also, there’s a significant increase in EHS score in 1 month after LI-ESWT therapy sessions was completed (P=0.05, Wilcoxon Test). Conclusion: There is a significant improvement in the EHS and IIEF-5 score before and after treatment using LI-ESWT. LI-ESWT seems to be a promising future therapy for ED

ABSTRAK Objective: Our case demonstrates effective tuberculosis-related Addison's disease care with laparoscopic adrenalectomy and anti-tuberculosis medication. Case(s) Presentation: A 43-year-old patient complained of left flank pain for two months before admission, unexplained weight loss, general weakness, decreased appetite, and without long-term cough. The physical examination resulted in hyperpigmentation on the fingers of both hands. MRI revealed an enlarged left adrenal gland and the blood test showed elevated cortisol to 1668 mmol/L. Three-port left laparoscopic transperitoneal adrenalectomy was used. Histopathology results showed epitheloid histiocytes that formed granulomas with caseous necrosis and Datia Langerhans cells referring to tuberculosis; no malignancy was found from the same sample. Discussion: Addison's illness is the primary cause of tuberculosis. Due to the widespread use of anti-TB medications, the incidence of adrenal tuberculosis has been on the decline recently; nonetheless, in developing nations, tuberculosis is still the predominant cause of Addison's disease. Conclusion: Surgery and standard treatment for extrapulmonary tuberculosis were adequate to diagnose and alleviate the symptoms of cortisol disturbance caused by Addison’s disease. Proapoptosis Effect of Root of

This study aimed to predict the proapoptosis effect of E. longifolia active compounds on prostate cancer by in silico analysis. Protein data such as BCL-2 (GI: 2506216), Caspase 3 (GI: 6978605), Caspase 8(GI: 11560103), data quassinoid (ID: 5459060 and chantin (ID: 97176) were collected from GenBank of NCBI. Protein BCL-2 collected from NCBI compare with Protein Data Bank (PDB) and UNIPROT. The docking process was carried out using software HEX 8.0. to compute the binding affinity between ligands (active compounds of Pasak Bumi) and protein target. The interaction between quassinoid and chantin was strongest and stable against caspase-9, indicating that the active ingredient in E. longifolia triggered caspase-9 activity after activation of BH3 domains in Bcl-2 in prostate cancer. The low energy binding between quassinoid and chantin with caspase-3 indicates the interaction between the active ingredients is not strong with caspase-3. E. longifolia active ingredients that are potentially used in the treatment of prostate cancer are quassinoid and chantin by inducing apoptotic mechanisms via both extrinsic and intrinsic pathways. The combination of active ingredients of E. longifolia that is quassinoid and chantin can be used as a strategy of prostate cancer therapy both through extrinsic and intrinsic pathways
