    An agonist-antagonist pitch production model

    Prosody is a phenomenon that is crucial for numerous fields of speech research, accenting the importance of having a robust prosody model. A class of intonation models based on the physiology of pitch pro- duction are especially attractive for their inherent multilingual support. These models rely on an accurate model of muscle activation. Tradi- tionally they have used the 2nd order spring-damper-mass (SDM) mus- cle model. However, recent research has shown that the SDM model is not sufficient for adequate modelling of the muscle dynamics. The 3rd order Hill type model offers a more accurate representation of mus- cle dynamics, but it has been shown to be underdamped when using physiologically plausible muscle parameters. In this paper we propose an agonist-antagonist pitch production (A2P2) model that both validates and gives insight behind the improved results of using higher-order crit- ically damped system models in intonation modelling

Nevertheless, the impact of traveling over relatively short distances (across 3 or fewer time zones) on diurnal patterning of typical physiological response patterns has yet to be studied in a large, epidemiological sample. Design: The current research focuses on 764 middle-aged men comparing variations in diurnal cortisol regulation based on number of time zones traveled eastward or westward the day before.     Genome Sequence Analyses of Pseudomonas savastanoi pv. glycinea and Subtractive Hybridization-Based Comparative Genomics with Nine Pseudomonads

    Bacterial blight, caused by Pseudomonas savastanoi pv. glycinea (Psg), is a common disease of soybean. In an effort to compare a current field isolate with one isolated in the early 1960s, the genomes of two Psg strains, race 4 and B076, were sequenced using 454 pyrosequencing. The genomes of both Psg strains share more than 4,900 highly conserved genes, indicating very low genetic diversity between Psg genomes. Though conserved, genome rearrangements and recombination events occur commonly within the two Psg genomes. When compared to each other, 437 and 163 specific genes were identified in B076 and race 4, respectively. Most specific genes are plasmid-borne, indicating that acquisition and maintenance of plasmids may represent a major mechanism to change the genetic composition of the genome and even acquire new virulence factors. Type three secretion gene clusters of Psg strains are near identical with that of P. savastanoi pv. phaseolicola (Pph) strain 1448A and they shared 20 common effector genes. Furthermore, the coronatine biosynthetic cluster is present on a large plasmid in strain B076, but not in race 4. In silico subtractive hybridization-based comparative genomic analyses with nine sequenced phytopathogenic pseudomonads identified dozens of specific islands (SIs), and revealed that the genomes of Psg strains are more similar to those belonging to the same genomospecies such as Pph 1448A than to other phytopathogenic pseudomonads. The number of highly conserved genes (core genome) among them decreased dramatically when more genomes were included in the subtraction, suggesting the diversification of pseudomonads, and further indicating the genome heterogeneity among pseudomonads. However, the number of specific genes did not change significantly, suggesting these genes are indeed specific in Psg genomes. These results reinforce the idea of a species complex of P. syringae and support the reclassification of P. syringae into different species

    Global Retinoblastoma Presentation and Analysis by National Income Level

    Importance: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. Objectives: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. Design, Setting, and Participants: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. Main Outcomes and Measures: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. Results: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- A nd middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Advanced disease at diagnosis was more common in LMICs even after adjusting for age (odds ratio for low-income countries vs upper-middle-income countries and HICs, 17.92 [95% CI, 12.94-24.80], and for lower-middle-income countries vs upper-middle-income countries and HICs, 5.74 [95% CI, 4.30-7.68]). Conclusions and Relevance: This study is estimated to have included more than half of all new retinoblastoma cases worldwide in 2017. Children from LMICs, where the main global retinoblastoma burden lies, presented at an older age with more advanced disease and demonstrated a smaller proportion of familial history of retinoblastoma, likely because many do not reach a childbearing age. Given that retinoblastoma is curable, these data are concerning and mandate intervention at national and international levels. Further studies are needed to investigate factors, other than age at presentation, that may be associated with advanced disease in LMICs

    COMPARATIVE ASSESSMENT OF LARGE ARTERY ELASTICITY IN PATIENTS WITH STABLE ANGINA RECEIVING METABOLIC THERAPY

    Aim. To assess the effects of metabolic therapy, administered together with standard antianginal treatment, on large artery elasticity and endotheliumdependent vasodilation in patients with stable angina and subsequent heart failure. Material and methods. The study included 74 patients with coronary heart disease (CHD), stable angina of II–III Functional Class (FC), and FC II–III chronic heart failure (CHF). In the main group, standard antianginal treatment was combined with metabolic therapy (trimetazidine, trimethylhydrazinium propionate, and isosorbide dinitrate) for 4 months; the control group received standard treatment only. Results. The four-month metabolic therapy failed to substantially improve the pulse wave velocity (PWV) in elastic arteries. In muscular arteries, PWV significantly decreased during the treatment with isosorbide dinitrate, which negatively affected endothelium-dependent vasodilation of radial arteries. However, trimetazidine treatment was associated with a significant improvement in endothelial function, as well as with a significant reduction in anginal FC and a substantial improvement in exercise capacity. Trimethylhydrazinium propionate therapy was linked to non-significant positive dynamics of these parameters. Conclusion. In stable angina patients, the combination of trimetazidine and standard antianginal therapy was associated with a significant improvement in endotheliumdependentvasodilation of muscular arteries. Consideringits positiveimpact on endothelial function and the absence of direct effects on arterial tonus in stable angina, trimetazidine therapy was more effective than isosorbide dinitrate treatment
