Resistance studies of erythromycin and rifampin for Rhodococcus equi over a 10-year period
This study sought to determine whether an increase in resistance of Rhodococcus equi to the antibiotics rifampin and erythromycin occurred over a 10-year period. This was carried out by the use of E test strips for rifampin and erythromycin to determine the MIC (minimum inhibitory concentration) values of Rhodococcus equi to this combination of antibiotics
Comparative transcriptome analysis by RNAseq of necrotic enteritis <i>Clostridium perfringens</i> during <i>in vivo</i> colonization and <i>in vitro</i> conditions
BACKGROUND: Necrotic enteritis (NE) caused by netB-positive type A Clostridium perfringens is an important bacterial disease of poultry. Through its complex regulatory system, C. perfringens orchestrates the expression of a collection of toxins and extracellular enzymes that are crucial for the development of the disease; environmental conditions play an important role in their regulation. In this study, and for the first time, global transcriptomic analysis was performed on ligated intestinal loops in chickens colonized with a netB-positive C. perfringens strain, as well as the same strain propagated in vitro under various nutritional and environmental conditions. RESULTS: Analysis of the respective pathogen transcriptomes revealed up to 673 genes that were significantly expressed in vivo. Gene expression profiles in vivo were most similar to those of C. perfringens grown in nutritionally-deprived conditions. CONCLUSIONS: Taken together, our results suggest a bacterial transcriptome responses to the early stages of adaptation, and colonization of, the chicken intestine. Our work also reveals how netB-positive C. perfringens reacts to different environmental conditions including those in the chicken intestine. Common carp (Cyprinus carpio L.) alters its feeding niche in response to changing food resources: direct observations in simulated ponds
We used customized fish tanks as model fish ponds to observe grazing, swimming, and conspecific social behavior of common carp (Cyprinus carpio) under variable food-resource conditions to assess alterations in feeding niche. Different food and feeding situations were created by using only pond water or pond water plus pond bottom sediment or pond water plus pond bottom sediment and artificial feeding. All tanks were fertilized twice, prior to stocking and 2 weeks later after starting the experiment to stimulate natural food production. Common carp preferred artificial feed over benthic macroinvertebrates, followed by zooplankton. Common carp did not prefer any group of phytoplankton in any treatment. Common carp was mainly benthic in habitat choice, feeding on benthic macroinvertebrates when only plankton and benthic macroinvertebrates were available in the system. In the absence of benthic macroinvertebrates, their feeding niche shifted from near the bottom of the tanks to the water column where they spent 85% of the total time and fed principally on zooplankton. Common carp readily switched to artificial feed when available, which led to better growth. Common carp preferred to graze individually. Behavioral observations of common carp in tanks yielded new information that assists our understanding of their ecological niche. This knowledge could be potentially used to further the development of common carp aquaculture
Rhodococcus equi venous catheter infection: a case report and review of the literature
<p>Abstract</p> <p>Introduction</p> <p><it>Rhodococcus equi </it>is an animal pathogen that was initially isolated from horses and is being increasingly reported as a cause of infection in humans with impaired cellular immunity. However, this pathogen is underestimated as a challenging antagonist and is frequently considered to be a mere contaminant despite the potential for life-threatening infections. Most case reports have occurred in immunocompromised patients who have received organ transplants (for example kidney, heart, bone marrow) or those with human immunodeficiency virus infection. Infections often manifest as pulmonary involvement or soft tissue abscesses. Bacteremia related to <it>R. equi </it>infections of tunneled central venous catheters has rarely been described.</p> <p>Case presentation</p> <p>We report the case of a 63-year-old non-transplant recipient, non-HIV infected Caucasian woman with endometrial carcinoma who developed recurrent bloodstream infections and septic shock due to <it>R. equi </it>and ultimately required the removal of her port catheter, a subcutaneous implantable central venous catheter. We also review the medical literature related to human infections with <it>R. equi</it>.</p> <p>Conclusion</p> <p><it>R. equi </it>should be considered a serious pathogen, not a contaminant, particularly in an immunocompromised patient who presents with a central venous catheter-related bloodstream infection. Counseling patients with central venous catheters who participate in activities involving exposure to domesticated animals is recommended.</p
A survey of Salmonella spp and Campylobacter spp in dairy goat faeces and bulk tank milk in the Murcia region of Spain
This study was designed to investigate the occurrence of Salmonella spp and Campylobacter spp in faeces samples from 222 healthy Murciano-Granadina dairy goats reared on 12 farms in Spain and in samples of bulk tank milk from 11 of those herds. Neither Salmonella spp nor Campylobacter spp were isolated from any of the samples. Our results suggest that, under the management practices applied to this breed in Spain, Murciano-Granadina goats are not likely to be a significant reservoir for these food-borne pathogens
The challenges faced in the design, conduct and analysis of surgical randomised controlled trials
Randomised evaluations of surgical interventions are rare; some interventions have been widely
adopted without rigorous evaluation. Unlike other medical areas, the randomised controlled trial
(RCT) design has not become the default study design for the evaluation of surgical interventions.
Surgical trials are difficult to successfully undertake and pose particular practical and methodological challenges. However, RCTs have played a role in the assessment of surgical innovations and there is scope and need for greater use. This article will consider the design, conduct and analysis of an RCT of a surgical intervention. The issues will be reviewed under three
headings: the timing of the evaluation, defining the research question and trial design issues.
Recommendations on the conduct of future surgical RCTs are made. Collaboration between

Biophysical Basis for Three Distinct Dynamical Mechanisms of Action Potential Initiation
Transduction of graded synaptic input into trains of all-or-none action
potentials (spikes) is a crucial step in neural coding. Hodgkin identified three
classes of neurons with qualitatively different analog-to-digital transduction
properties. Despite widespread use of this classification scheme, a
generalizable explanation of its biophysical basis has not been described. We
recorded from spinal sensory neurons representing each class and reproduced
their transduction properties in a minimal model. With phase plane and
bifurcation analysis, each class of excitability was shown to derive from
distinct spike initiating dynamics. Excitability could be converted between all
three classes by varying single parameters; moreover, several parameters, when
varied one at a time, had functionally equivalent effects on excitability. From
this, we conclude that the spike-initiating dynamics associated with each of
Hodgkin's classes represent different outcomes in a nonlinear
competition between oppositely directed, kinetically mismatched currents. Class
1 excitability occurs through a saddle node on invariant circle bifurcation when
net current at perithreshold potentials is inward (depolarizing) at steady
state. Class 2 excitability occurs through a Hopf bifurcation when, despite net
current being outward (hyperpolarizing) at steady state, spike initiation occurs
because inward current activates faster than outward current. Class 3
excitability occurs through a quasi-separatrix crossing when fast-activating
inward current overpowers slow-activating outward current during a stimulus
transient, although slow-activating outward current dominates during constant
stimulation. Experiments confirmed that different classes of spinal lamina I
neurons express the subthreshold currents predicted by our simulations and,
further, that those currents are necessary for the excitability in each cell
class. Thus, our results demonstrate that all three classes of excitability
arise from a continuum in the direction and magnitude of subthreshold currents.
Through detailed analysis of the spike-initiating process, we have explained a
fundamental link between biophysical properties and qualitative differences in
how neurons encode sensory input
The risk factors for unexplained antepartum stillbirths in Scotland, 1994 to 2003
Objective:
To determine the factors contributing to unexplained antepartum stillbirth in Scotland.
Study Design:
A 10-year birth database in Scotland was used to compare the unexplained antepartum stillbirth with other birth outcomes. The sample unit was a pregnant mother with a gestational age of 20 weeks and above and with a fetal birth weight of 200 g and above.
Result:
Maternal age of 35 years and above, lower deprivation category, inaccessible area of residence, maternal smoking, maternal height of <160 cm and gestational age of above 39 weeks were significantly associated with unexplained antepartum stillbirth. In multivariable analysis only maternal age (adjusted odds ratio (OR): 1.8, confidence interval (CI): 1.1 to 3.0, P=0.02), smoking during pregnancy (adjusted OR: 2.0, CI: 1.1 to 3.5, P=0.02), and maternal height (adjusted OR: 1.4, CI: 1.1 to 1.8, P=0.01), remain significant. Screening of pregnancies based on these three risk factors had 4.2% sensitivity and 99.4% specificity. The prevalence of stillbirth for this population was 0.2%. A positive predictive value of only 1.2% implies that only 1 in 83 women with these three risk factors will have antepartum stillbirth. The remaining 82 will suffer needless anxiety and potentially diagnostic procedures.
Conclusion:
Advanced maternal age, maternal smoking, and shorter maternal height were associated risk for unexplained antepartum stillbirth but screening based on these factors would be of limited value
High grade angiosarcoma arising in fibroadenoma
Primary angiosarcoma of the breast is a rare tumour that account for fewer than 0.05% of all malignant mammary tumours. Angiosarcoma may have an perfidious clinical onset. Radiologic findings are often nonspecific and may appear completely normal in one-third of cases with primary angiosarcoma. The prognosis is usually poor because of the high rates of local recurrence and early development of metastases. Aggressive surgical resection is the mainstay of treatment. The role of adjuvant therapy has not yet been well established
