Estudio de la deleción espontánea de los exones 45–55 en el gen DMD y su aplicación como modelo de terapia génica. Análisis de factores condicionantes del fenotipo y estudio del modelo celular
La deleción de los exones 45 al 55 en el gen DMD (del45-55) se ha postulado como un interesante
modelo terapéutico, mediante el que se podrían beneficiar hasta 60% de pacientes con distrofia muscular de
Duchenne. Sin embargo, ésta se expresa con una gran variabilidad clínica, observando sujetos asintomáticos,
mientras que otros presentan debilidad muscular, así como alteraciones cardiacas severas. El objetivo de la
presente tesis doctoral es llevar a cabo un profundo análisis de la del45-55 para tratar de desvelar los factores que
promueven
su presentación de manera asintomática, de forma que pueda replicarse como modelo de terapia para la distrofia
muscular de Duchenne.
Metodología
Para ello, se coordinó un estudio transversal multicéntrico, para identificar al máximo y realizar una evaluación
clínica y funcional de sujetos con la del45-55. Además, se diseñó un panel de genes de secuenciación masiva para
analizar los casos índice y determinar los puntos de rotura en los intrones 44 y 55 del gen DMD; así como la
presencia de polimorfismos en genes modificadores de DMD y de mutaciones concurrentes. Se llevó a cabo
análisis de estadística bayesiana estudios de correlación. También se realizó estudios de haplotipos, así como
de expresión de distrofina en nueve biopsias musculares.
Como prueba de concepto se procedió a reproducir del45-55 específica encontrada en un subgrupo de sujetos
analizados; en una línea de mioblastos DMD, mediante el sistema de edición CRISPR-Cas9.
Resultados
La serie recogida consta de un total de 57 sujetos, 23 índice que expresan fenotipo tipo Becker (28%), cardiopatía
aislada (19%) y signos asintomáticos (53%). Se detectaron alteraciones cognitivas en el 90% de los pacientes
pediátricos. Los casos índice se agruparon en únicamente 10 deleciones con puntos de rotura específicos, debido 5
eventos fundadores (uno recurrente). El subgrupo D5 se asoció estadísticamente con una mayor severidad, no
encontrando alteraciones promovidas por sus puntos de rotura. Todas las biopsias analizadas presentaban una
expresión de distrofina superior al 67%. Entre los factores modificadores en trans explorados, encontramos que la
ausencia de un determinado haplotipo en LTBP4 se asociaba con la presencia de cardiopatía.
La introducción de la del45-55 en mioblastos DMD mediante CRIPSR-Cas9 es capaz de recuperar la expresión de
distrofina y mejorar los defectos de diferenciación que presentaba esta línea, sin ocasionar modificaciones no
deseadas en el genoma.
Conclusión
Se confirmó que la del45-55 segrega con una gran proporción de fenotipos asintomáticos o benignos, pero también
existe la presencia de formas severas, de cardiopatía aislada y alteraciones cognitivas. Aunque identificamos una
influencia de un determinado punto de rotura y del haplotipo en LTBP4 con la severidad, deben existir otros
mecanismos responsables de la variabilidad. Aún así la del45-55 podría suponer un potencial modelo terapéutic

In recent algorithmic family simulates different biological processes observed in Nature in order to efficiently address complex optimization problems. In the last years the number of bio-inspired optimization approaches in literature has grown considerably, reaching unprecedented levels that dark the future prospects of this field of research. This paper addresses this problem by proposing two comprehensive, principle-based taxonomies that allow researchers to organize existing and future algorithmic developments into well-defined categories, considering two different criteria: the source of inspiration and the behavior of each algorithm. Using these taxonomies we review more than three hundred publications dealing with nature- inspired and bio-inspired algorithms, and proposals falling within each of these categories are examined, leading to a critical summary of design trends and similarities between them, and the identification of the most similar classical algorithm for each reviewed paper. From our analysis we conclude that a poor relationship is often found between the natural inspiration of an algorithm and its behavior. Furthermore, similarities in terms of behavior between different algorithms are greater than what is claimed in their public disclosure: specifically, we show that more than one-third of the reviewed bio-inspired solvers are versions of classical algorithms. Grounded on the conclusions of our critical analysis, we give several recommendations and points of improvement for better methodological practices in this active and growing research field

La buena implementación de la ciudad sostenible ha sido el objetivo del Congreso Iberoamericano CIUDAD SOSTENIBLE 2019. La presente publicación recoge las actas del Congreso, impulsado por el Departamento de Composición Arquitectónica de la Universidad Politècnica de València y la Red Hispanoamericana para la fundamentación y práctica de la ciudad sostenible, con la colaboración de ONU-HÁBITAT, la agencia de la ONU para la ciudad sostenible.
Desde los Objetivos de Desarrollo Sostenible y, en particular, desde la Nueva Agenda Urbana,debe seguirse una profundización académica y profesional para la mejor consecución técnica e interdisciplinar de la ciudad sostenible. Impact of an intradialysis virtual-reality-based exercise program on healthcare resources expenditure: a micro-costing analysis
[EN] Background Engagement in exercise by haemodialysis (HD) patients has been shown to generate benefits both in terms of improved functional capacity and in the health-related quality of life. The use of non-immersive virtual reality (VR) games represents a new format for the implementation of intradialysis exercise. Some studies have shown that engaging in exercise for 6 months reduces the consumption of antihypertensive drugs and decreases the time spent admitted to hospital among individuals receiving HD treatments. The objective of this study was to evaluate changes in the consumption of healthcare resources and micro-costing for patients on HD who completed a VR exercise program. Materials and methods Design: This study is a secondary analysis of a clinical trial. The participants performed an intradialysis exercise program with non-immersive virtual reality for 3 months. The variables were recorded in two periods: 12 months before and 12 months after the start of the exercise program. Results The micro-costing analysis showed a significant decrease in the mean cost, in euros, for the consumption of laboratory tests - 330 (95% CI:[- 533, - 126];p = 0.003), outpatient visits - 351 ([- 566, - 135];p = 0.003), and radiology tests - 111 ([- 209, - 10];p = 0.03) in the 12 months after the implementation of the exercise program relative to the 12 months prior to its start. Conclusion The implementation of intradialysis exercise programs decreased the expenditure of some healthcare resources. Preclinical characterization of antagomiR-218 as a potential treatment for myotonic dystrophy
Myotonic dystrophy type 1 (DM1) is a rare neuromuscular disease caused by expansion of unstable CTG repeats in a non-coding region of the DMPK gene. CUG expansions in mutant DMPK transcripts sequester MBNL1 proteins in ribonuclear foci. Depletion of this protein is a primary contributor to disease symptoms such as muscle weakness and atrophy and myotonia, yet upregulation of endogenous MBNL1 levels may compensate for this sequestration. Having previously demonstrated that antisense oligonucleotides against miR-218 boost MBNL1 expression and rescue phenotypes in disease models, here we provide preclinical characterization of an antagomiR-218 molecule using the HSALR mouse model and patient-derived myotubes. In HSALR, antagomiR-218 reached 40-60 pM 2weeks after injection, rescued molecular and functional phenotypes in a dose- and time-dependent manner, and showed a good toxicity profile after a single subcutaneous administration. In muscle tissue, antagomiR rescued the normal subcellular distribution of Mbnl1 and did not alter the proportion of myonuclei containing CUG foci. In patient-derived cells, antagomiR-218 improved defective fusion and differentiation and rescued up to 34% of the gene expression alterations found in the transcriptome of patient cells. Importantly, miR-218 was found to be upregulated in DM1 muscle biopsies, pinpointing this microRNA (miRNA) as a relevant therapeutic target.This work was funded by research grants from Instituto de Salud Carlos III, including funds from FEDER, to M.P.-A. and B.L. Intensificación en el desarrollo de competencias en asignaturas de Tratamiento de Aguas a través del análisis de diagramas de flujo de instalaciones mediante el empleo de técnicas de gamificación y estrategias de trabajo colaborativo
El objetivo principal del presente proyecto de innovación docente es el de subsanar una problemática detectada en los últimos
años en las asignaturas de Tratamiento y Tecnología de Aguas del Grado en Ciencias Ambientales, Ingeniería Sanitaria en la
Construcción Civil (Especialidad Construcciones Civiles) del Grado en Ingeniería Civil, y Tecnologías para el Tratamiento de
Aguas del Grado en Ingeniería Química. Los estudiantes de las asignaturas indicadas anteriormente presentan serias
dificultades para implementar, interpretar y analizar diagramas de flujo de instalaciones de tratamiento de aguas, ya sean de
potabilización, desalación, depuración y/o reutilización. Este aspecto es uno de los de mayor relevancia dentro del temario de
dichas asignaturas y se traduce en tasas de rendimiento y de éxito relativamente bajas. La aplicación de la innovación docente
desarrollada en el presente proyecto pretende solventar esta deficiencia de los estudiantes en estas asignaturas de tratamiento
de aguas mediante estrategias de trabajo colaborativo e introduciendo la evaluación por pares. Asimismo, esta metodología
permitirá potenciar el desarrollo de diferentes competencias de las asignaturas en cuestión
Dystrophinopathy Phenotypes and Modifying Factors in Exon 45-55 Deletion
Duchenne muscular dystrophy (DMD) exon 45-55 deletion (del45-55) has been postulated as a model that could treat up to 60% of DMD patients, but the associated clinical variability and complications require clarification. We aimed to understand the phenotypes and potential modifying factors of this dystrophinopathy subset. This cross-sectional, multicenter cohort study applied clinical and functional evaluation. Next generation sequencing was employed to identify intronic breakpoints and their impact on the Dp140 promotor, intronic long noncoding RNA, and regulatory splicing sequences. DMD modifiers (SPP1, LTBP4, ACTN3) and concomitant mutations were also assessed. Haplotypes were built using DMD single nucleotide polymorphisms. Dystrophin expression was evaluated via immunostaining, Western blotting, reverse transcription polymerase chain reaction (PCR), and droplet digital PCR in 9 muscle biopsies. The series comprised 57 subjects (23 index) expressing Becker phenotype (28%), isolated cardiopathy (19%), and asymptomatic features (53%). Cognitive impairment occurred in 90% of children. Patients were classified according to 10 distinct index-case breakpoints; 4 of them were recurrent due to founder events. A specific breakpoint (D5) was associated with severity, but no significant effect was appreciated due to the changes in intronic sequences. All biopsies showed dystrophin expression of >67% and traces of alternative del45-57 transcript that were not deemed pathogenically relevant. Only the LTBP4 haplotype appeared associated the presence of cardiopathy among the explored extragenic factors. We confirmed that del45-55 segregates a high proportion of benign phenotypes, severe cases, and isolated cardiac and cognitive presentations. Although some influence of the intronic breakpoint position and the LTBP4 modifier may exist, the pathomechanisms responsible for the phenotypic variability remain largely unresolved.
