    Detecting short-term change and variation in health-related quality of life: within- and between-person factor structure of the SF-36 health survey.

    BackgroundA major goal of much aging-related research and geriatric medicine is to identify early changes in health and functioning before serious limitations develop. To this end, regular collection of patient-reported outcome measure (PROMs) in a clinical setting may be useful to identify and monitor these changes. However, existing PROMs were not designed for repeated administration and are more commonly used as one-time screening tools; as such, their ability to detect variation and measurement properties when administered repeatedly remain unknown. In this study we evaluated the potential of the RAND SF-36 Health Survey as a repeated-use PROM by examining its measurement properties when modified for administration over multiple occasions.MethodsTo distinguish between-person (i.e., average) from within-person (i.e., occasion) levels, the SF-36 Health Survey was completed by a sample of older adults (N = 122, M age  = 66.28 years) daily for seven consecutive days. Multilevel confirmatory factor analysis (CFA) was employed to investigate the factor structure at both levels for two- and eight-factor solutions.ResultsMultilevel CFA models revealed that the correlated eight-factor solution provided better model fit than the two-factor solution at both the between-person and within-person levels. Overall model fit for the SF-36 Health Survey administered daily was not substantially different from standard survey administration, though both were below optimal levels as reported in the literature. However, individual subscales did demonstrate good reliability.ConclusionsMany of the subscales of the modified SF-36 for repeated daily assessment were found to be sufficiently reliable for use in repeated measurement designs incorporating PROMs, though the overall scale may not be optimal. We encourage future work to investigate the utility of the subscales in specific contexts, as well as the measurement properties of other existing PROMs when administered in a repeated measures design. The development and integration of new measures for this purpose may ultimately be necessary

    Implicit Extended Kalman Filter for Optical Terrain Relative Navigation Using Delayed Measurements

    The exploration of celestial bodies such as the Moon, Mars, or even smaller ones such as comets and asteroids, is the next frontier of space exploration. One of the most interesting and attractive purposes from the scientific point of view in this field, is the capability for a spacecraft to land on such bodies. Monocular cameras are widely adopted to perform this task due to their low cost and system complexity. Nevertheless, image-based algorithms for motion estimation range across different scales of complexities and computational loads. In this paper, a method to perform relative (or local) terrain navigation using frame-to-frame features correspondences and altimeter measurements is presented. The proposed image-based approach relies on the implementation of the implicit extended Kalman filter, which works using nonlinear dynamic models and corrections from measurements that are implicit functions of the state variables. In particular, here, the epipolar constraint, which is a geometric relationship between the feature point position vectors and the camera translation vector, is employed as the implicit measurement fused with altimeter updates. In realistic applications, the image processing routines require a certain amount of time to be executed. For this reason, the presented navigation system entails a fast cycle using altimeter measurements and a slow cycle with image-based updates. Moreover, the intrinsic delay of the feature matching execution is taken into account using a modified extrapolation method

    Facilitating Sustainable Career Development in Fragility: A Psycho-Linguistic Intervention for Employability of Individuals with Fragile Literacy Skills

    The brief report examines the burgeoning interest in sustainable career development by discussing the role of employability of individuals with fragile literacy skills, i.e., second-language learners (L2), and individuals with developmental dyslexia (DD). Considering sustainable career development as the umbrella of practices facilitating individuals flourishing, we aim to present an integrated flexible intervention to promote the employability of individuals with fragile literacy skills (DD and L2) that combines both the promotion of linguistic skills and psychological resources. Using an experimental research design, we tested our training intervention on language skills (reading and writing) coupled with psychological training intended to enhance psychological resources and psychological capital. A total of 38 individuals with DD took part in our examination (n = 22 in the experimental group) and n = 26 L2 (n = 11 in the experimental group). We measured employability skills and psychological capital pre and post our interventions both in the control and in the experimental groups. Our result show that the intervention led to a significant improvement in L2 and DD. We discuss our results and implications for research and practice

    Importance of modelling decisions on estimating trajectories of depressive symptoms and co-morbid conditions in older adults: Longitudinal studies from ten European countries

    BackgroundInternational comparisons of trajectories of depressive symptoms in older adults are scarce and longitudinal associations with co-morbid conditions not fully understood.ObjectiveTo compare trajectories of depressive symptoms from participants living in 10 European Countries and identify ages at which the associations of co-morbid conditions with these trajectories become more relevant.MethodsLatent growth curve models were fitted to depressive symptoms scores from participants of the Survey of Health and Retirement in Europe (SHARE) initiative (combined n = 21,253) and co-morbid conditions modelled as time varying covariates. To identify the ages at which the association between co-morbid conditions and depressive symptoms was significant the Johnson-Neyman (JN) technique was used.ResultsThe shape of depressive symptoms trajectories varied between countries, and was highly dependent on modelling decisions. The association between the average number of co-morbidities reported over time and depressive symptoms was consistent and positive across countries and ages.ConclusionInternational differences in ageing-related trajectories of depressive symptoms emerged. The longitudinal association of co-morbid conditions with trajectories of depressive symptoms was found, but the results overall suggest that modelling decisions could greatly influence the outcomes, and should thus be interpreted with caution

    Psychopathology in Young People With Intellectual Disability

    Context Comorbid severe mental health problems complicating intellectual disability are a common and costly public health problem. Although these problems are known to begin in early childhood, little is known of how they evolve over time or whether they continue into adulthood. Objective To study the course of psychopathology in a representative population of children and adolescents with intellectual disability. Design, Setting, and Participants The participants of the Australian Child to Adult Development Study, an epidemiological cohort of 578 children and adolescents recruited in 1991 from health, education, and family agencies that provided services to children with intellectual disability aged 5 to 19.5 years in 6 rural and urban census regions in Australia, were followed up for 14 years with 4 time waves of data collection. Data were obtained from 507 participants, with 84% of wave 1 (1991-1992) participants being followed up at wave 4 (2002-2003). Main Outcome Measures The Developmental Behaviour Checklist (DBC), a validated measure of psychopathology in young people with intellectual disability, completed by parents or other caregivers. Changes over time in the Total Behaviour Problem Score and 5 subscale scores of the DBC scores were modeled using growth curve analysis. Results High initial levels of behavioral and emotional disturbance decreased only slowly over time, remaining high into young adulthood, declining by 1.05 per year on the DBC Total Behaviour Problem Score. Overall severity of psychopathology was similar across mild to severe ranges of intellectual disability (with mean Total Behavior Problem Scores of approximately 44). Psychopathology decreased more in boys than girls over time (boys starting with scores 2.61 points higher at baseline and ending with scores 2.57 points lower at wave 4), and more so in participants with mild intellectual disability compared with those with severe or profound intellectual disability who diverged from having scores 0.53 points lower at study commencement increasing to a difference of 6.98 points below severely affected children by wave 4. This trend was observed in each of the subscales, except the social-relating disturbance subscale, which increased over time. Prevalence of participants meeting criteria for major psychopathology or definite psychiatric disorder decreased from 41% at wave 1 to 31% at wave 4. Few of the participants (10%) with psychopathology received mental health interventions during the study period. Conclusion These results provide evidence that the problem of psychopathology comorbid with intellectual disability is both substantial and persistent and suggest the need for effective mental health interventions
