    Expanding Paramedicine in the Community (EPIC): study protocol for a randomized controlled trial.

    BackgroundThe incidence of chronic diseases, including diabetes mellitus (DM), heart failure (HF) and chronic obstructive pulmonary disease (COPD) is on the rise. The existing health care system must evolve to meet the growing needs of patients with these chronic diseases and reduce the strain on both acute care and hospital-based health care resources. Paramedics are an allied health care resource consisting of highly-trained practitioners who are comfortable working independently and in collaboration with other resources in the out-of-hospital setting. Expanding the paramedic's scope of practice to include community-based care may decrease the utilization of acute care and hospital-based health care resources by patients with chronic disease.Methods/designThis will be a pragmatic, randomized controlled trial comparing a community paramedic intervention to standard of care for patients with one of three chronic diseases. The objective of the trial is to determine whether community paramedics conducting regular home visits, including health assessments and evidence-based treatments, in partnership with primary care physicians and other community based resources, will decrease the rate of hospitalization and emergency department use for patients with DM, HF and COPD. The primary outcome measure will be the rate of hospitalization at one year. Secondary outcomes will include measures of health system utilization, overall health status, and cost-effectiveness of the intervention over the same time period. Outcome measures will be assessed using both Poisson regression and negative binomial regression analyses to assess the primary outcome.DiscussionThe results of this study will be used to inform decisions around the implementation of community paramedic programs. If successful in preventing hospitalizations, it has the ability to be scaled up to other regions, both nationally and internationally. The methods described in this paper will serve as a basis for future work related to this study.Trial registrationClinicalTrials.gov: NCT02034045. Date: 9 January 2014

    Single-Unit Responses Selective for Whole Faces in the Human Amygdala

    The human amygdala is critical for social cognition from faces, as borne out by impairments in recognizing facial emotion following amygdala lesions and differential activation of the amygdala by faces. Single-unit recordings in the primate amygdala have documented responses selective for faces, their identity, or emotional expression, yet how the amygdala represents face information remains unknown. Does it encode specific features of faces that are particularly critical for recognizing emotions (such as the eyes), or does it encode the whole face, a level of representation that might be the proximal substrate for subsequent social cognition? We investigated this question by recording from over 200 single neurons in the amygdalae of seven neurosurgical patients with implanted depth electrodes. We found that approximately half of all neurons responded to faces or parts of faces. Approximately 20% of all neurons responded selectively only to the whole face. Although responding most to whole faces, these neurons paradoxically responded more when only a small part of the face was shown compared to when almost the entire face was shown. We suggest that the human amygdala plays a predominant role in representing global information about faces, possibly achieved through inhibition between individual facial features

    SNOT-22 in a Control Population

    AIM: To assess SNOT-22 and its subscales in a non-rhinosinusitis UK-wide population.  METHODOLOGY/PRINCIPLE: This analysis uses data from the 'Chronic Rhinosinusitis Epidemiology Study' (CRES) which recruited from 30 centres across the UK, and the Socioeconomic Cost of ChrOnic Rhinosinusitis study' (SocCoR); 250 volunteers without CRS were recruited as part of these studies. Study-specific questionnaires including demographics, socioeconomic factors and past medical history as well as SNOT-22 and SF-36 were distributed. The control (non-CRS) population had no self-reported nasal problems in the past, no chronic conditions undergoing active treatment and no hospital admissions in the preceding 12 months.  RESULTS: The mean SNOT-22 total score overall was 12.0. The mean was 10.2 for males with a median of 6.5, and a mean of 13.2 for females with a median of 9. Females scored significantly more highly than males on the sleep/fatigue and facial domains.  CONCLUSIONS: Our data demonstrate differences in SNOT-22 amongst males and females.     Family Medicine needs assessment: Studying the clinical work of general practitioners in Ethiopia

    Background and Objective: Some universities in sub-Saharan Africa have initiated Family Medicine (FM) residency programs. This study was conducted by FM colleagues at Addis Ababa University (AAU) in Ethiopia and the University of Toronto, Canada to inform the FM residency curriculum at AAU. It was designed to determine the clinical problems that family physicians in Ethiopia may encounter.Methods: We used a mixed methods approach: Modified time-motion study and brief interviews. We observed 46 general practitioners (GPs) across ten sites in Ethiopia. Trained observers recorded time-motion data while GPs conducted their daily work. This data was supplemented by brief interviews with the GPs.Findings: Clinical encounters occupied 82% of GP work. The common symptoms were digestive-abdominal pain (21% visits), respiratory-cough (16%), and general-fever and chills (16%). The common diagnoses were infectious (22% visits), genitourinary (12%), circulatory (10%), and endocrine (10%). Challenges identified were lack of clinical resources (57% of GPs), difficulties in communication (48%) and excessive workload (33%). Most common requests were for information technology (78%) and HIV (46%) training.Conclusion: The profile of common symptoms and diagnoses indicated the competencies family physicians in the regions should have. This information will be used to develop an appropriate FM curriculum at AAU

    Mesoscopic Cooperative Emission From a Disordered System

    We study theoretically the cooperative light emission from a system of N≫1N\gg 1 classical oscillators confined within a volume with spatial scale, LL, much smaller than the radiation wavelength, λ0=2πc/ω0\lambda_0=2\pi c/\omega_0. We assume that the oscillators frequencies are randomly distributed around a central frequency, ω0\omega_0, with some characteristic width, Ω≪ω0\Omega\ll\omega_0. In the absence of disorder, that is Ω=0\Omega=0, the cooperative emission spectrum is composed of a narrow subradiant peak superimposed on a wide superradiant band. When Ω≠0\Omega\neq 0, we demonstrate that if NN is large enough, the subradiant peak is not simply broadened by the disorder but rather splits into a system of random narrow peaks. We estimate the spectral width of these peaks as a function of N,L,ΩN, L, \Omega, and λ0\lambda_0. We also estimate the amplitude of this mesoscopic structure in the emission spectrum.Comment: 25 pages including 6 figure
